RESUMO

Objective: To assess the clinical value of excisional atherectomy with the Turbohawk atherectomy device in treatment of lower limb vascular disease. Methods: A retrospective case study method was adopted by using clinical follow-up data between July 2016 and October 2017 from The First Affiliated Hospital of Xi'an Jiaotong University. There were 15 cases of lower limb vascular disease treated with Turbohawk atherectomy device as the atherectomy group. And 15 cases were selected out 284 ones treated with conventional endovascular therapy during the same period as the control group. Clinical features such as age, gender, clinical symptoms, risk factors for arteriosclerosis, the site and length of lower arterial lesion were considered matched with those of the atherectomy group. Analysis was carried out by comparing stenting rate and remission rate in the two groups, and modification of ABI and vascular patency rate 3 and 6 months after treatment. Results: The stenting rate of atherectomy group was 6.7% (1/15), which was lower than that of the control group (86.7%, 13/15)(P0.05). The patency rate of atherectomy group 6 months after treatment was 93.3% (14/15), which was higher than that of control group 73.3% (11/15)(P<0.05). Conclusion: The use of Turbohawk atherectomy device for treatment of lower limb atherosclerosis disease is a safe and efficient method.

RESUMO

Many factors contribute to a successful total knee arthroplasty, and postoperative coronal lower limb alignment has always been a focus of joint surgeons. Previous researches have suggested that neutral alignment can bring higher prosthesis survival rate and better knee function. However, the theory has been challenged in recent years.In this article, the author introduces the axis, alignment and osteotomy of total knee arthroplasty briefly and reviews the studies on the neutral alignment and kinematic alignment of recent years in order to provide some advice for the clinical operation.

Carbonated water is a fundamental part of many drinks and its effects have been studied in many pathological situations. However, cells and tissue damage as a consequence of carbonated water has not been the subject of extensive research. We assessed the short-term effects of soda on in vitro Hanging-drop culture of myoblasts and ex vivo lower limb of 8-day-old chicken embryo skeletal muscle tissue. Several groups weren designed: a) Control (Con-tyr), b) Carbonated water (Car), c) Coffe (Caf), and d) Cola beverage (Glu). The samples were observed with light microscopy and digital imaging analysis was performed. The ultra-structure of control and treated tissue were observed with electron microscopy. Immunohistochemistry techniques, such as terminal deoxynucleotidyl transferase-mediated dUTP nick- end labeling (TUNEL), TACTS Blue Label (TdT Kits) of R&D Systems were used. The myoblasts monolayers treated with soda showed plenty of eosinophilics elements. The eosinophily corresponds to higher percentage of cell death. The muscular tissue of the low limb treated with carbonated water (Car) showed calcium phosphate and collagen decreases, 53,86% and 82,95% respectively and enlarged nuclei of a higher size, with an evident loss of the parallel arrangement and fragmented nuclei. Compared to control samples, the muscular disorganization was accompanied by a positive reaction of the apoptotic bodies on TACS, also a positive reaction to ApopTacg and another positive reaction for the metalloproteases in the inter fibrillar cartilage matrix. These changes were not significant in Tyrode's solution controls, Coffee and Cola beverage groups. The morphological outcome can be apoptosis, necrosis or a mixed phenotype, suggesting that the carbonated water toxic effect might be related to these cell death processes. Further research, exploring biochemical factors will be required to elucidate necro-apoptotic cell death induced by carbonated water.


El agua carbonatada constituye una parte fundamental en muchas bebidas y su efecto ha sido estudiado en muchas situaciones patológicas. Sin embargo, el daño celular y de tejido como consecuencia del agua carbonatada no ha sido claramente investigado. El presente trabajo evalúa el efecto agudo in vitro de la soda sobre mioblastos obtenidos por cultivos en gota pendiente y el efecto sobre tejido muscular esquelético in vivo del miembro inferior de pollo de 8 días de desarrollo. Cuatro grupos de embriones fueron seleccionados al azar: a) Control (Con-tyr), b) Agua Carbonatada o Soda Club (Car), c) Café (Caf) y d) Bebida de cola (Glu). Las muestras fueron observadas por microscopía de luz. El análisis de imágenes digitales fue realizado. La ultraestructura del tejido control y tratado fue observada con Microscopía Electrónica de Transmisión (MET). La determinación de apoptosis fue realizada a través de TUNEL y l TACS Blue Label. La actividad de metaloproteasa MMP-1 fue ensayada. La población de mioblastos tratados con Soda Club mostró un elevado número de elementos eosinofílicos interpretado como un elevado número de células muertas, a diferencia del control y el grupo tratado con cafeína y bebida de Cola. En el tejido muscular se determinó una reducción de fosfatos de calcio y fibras de colágeno en una proporción de 53,86% y 82,95% respectivamente, acompañada por un desarreglo de las fibras y núcleos fragmentados, con reacción positiva para cuerpos apoptóticos y metaloproteasas en la matriz interfibrilar. Los resultados sugieren que el efecto tóxico del agua carbonatada sobre células y tejido pudiera estar vinculado con procesos combinados de muerte celular como necro-apoptosis. Se sugiere una mayor exploración de los eventos moleculares para dilucidar la combinación de los procesos de muerte celular sugerida en el presente trabajo.

@#Objective To investigate the characteristics of the dynamic contractions on the surface electromyography (sEMG) single of stroke patients induced from the low limb muscle when exercising passively, exercising initiatively with assistant and against resistance.Methods 24 stroke patients with hemiplegia and 17 normal subjects were tested with sEMG under a dynamic contractions in coxa and knee flexion and extension passively, initiatively with assistant and against resistance. The myoelectric signals were collected and processed by linear time and frequency domain method.Results The values of median frequency (MF) and mean power frequency (MPF) of stroke group were significantly lower, but the value of average EMG (AEMG) was higher ( P<0.001). The values of MF and MPF in activity side were lower than that in non-activity side ( P<0.001). The values of MF and MPF when exercising passively were higher than that when exercising with resistance ( P<0.05). The value of AEMG when exercising with resistance was highest. The values of MF and MPF in the synergist muscle were higher. The values of AEMG in the antagonistic muscle and synergist muscle were higher than that agonist and synergist muscle ( P<0.01). The values of MF and MPF in non-paretic exercising side were higher significantly, but in paretic exercising side and non-paretic silent side were lower. The values of MF and MPF in exercising side from vastus lateralis (VL) were the highest. The values of AEMG in exercising side and non-exercising side from biceps femoris (BF) were the highest. The values of MF and MPF in low limb of stroke group reduced, that in rectus femoris (RF) from paretic side was the lowest; that in BF from non-paretic side was the lowest ( P<0.01). The value of AEMG in low limb of stroke group was high significantly, especially in BF from the low limb of the non-paretic side in stroke patients. The values of AEMG in four group muscles gradually were higher following the higher exercising load, and that in the BF was the highest, and that in vastus medialis (VM) rose significantly.Conclusion The values of MF and MPF of stroke patients with hemiplegia reduce significantly, but the value of AEMG is higher. The values of MF and MPF in exercising side are lower than that in non-exercising side and non-paretic exercising side rising significantly, but that in paretic exercising side and non-paretic silent side reduce significantly. The values of MF and MPF in assistant exercise are higher than that in passive exercise and resistance exercise, but the value of AEMG in resistance exercise is higher than that in assistant exercise and in passive exercise. The values of MF and MPF in synergist muscle rise, but the values of AEMG in antagonist and synergist muscle are higher than that agonist and synergist muscle.

RESUMO

[Objective]To study the therapeutic results of the endovenous laser treatment(EVLT) of varicose veins in lower limbs. [Method]97 cases (111 limbs) were performed with EVLT or EVLT with high ligation of the great saphenous veins according to the clinical manifestations, phlebography and ultrasound. [Results]All patients were followed-up 3~24 months with satisfactory results,without recurrence of varicosity. [Conclution]EVLT is a safe, effective, reliable and minimally invasive treatment for varicosity in lower limbs.

RESUMO

Objective To evaluate the clinical efficacy of transplantation of autologous peripheral blood stem cells(PBSC) for the treatment of toe or heel ulcer and of rest pain in patients with lower limb arterial ischemic diseases.Methods To mobilize patients' own stem cells with G-CSF for 5 days.At the sixth day,PBSC are collected with a blood-cells separator.The PBSC were then intramuscularly injected into ischemic areas of the lower limbs.Results After transplantation,all patients are followed up from 3 to 24 months.The rest pain disappeares in 12 patients,while toe or heel ulcers are cicatrized in 11 cases.However,4 patients lost in following up.ConclusionTransplantation of autologous peripheral blood stem cells is an effective method for the treatment of arterial ischemic disease.
