RESUMO

Lower extremity movement is a complex and large range of limb movement. Arterial stents implanted in lower extremity are prone to complex mechanical deformation, so the stent is required to have high comprehensive mechanical properties. In order to evaluate the mechanical property of different stents, in this paper, finite element method was used to simulate and compare the mechanical properties of six nitinol stents (Absolute Pro, Complete SE, Lifestent, Protégé EverFlex, Pulsar-35 and New) under different deformation modes, such as radial compression, axial compression/tension, bending and torsion, and the radial support performance of the stents was verified by experiments. The results showed that the comprehensive performance of New stent was better than other stents. Among which the radial support performance was higher than Absolute Pro and Pulsar-35 stent, the axial support performance was better than Complete SE, Lifestent and Protégé EverFlex stent, the flexibility was superior to Protégé Everflex stent, and the torsion performance was better than Complete SE, Lifestent and Protégé Everflex stent. The TTR2 type radial support force tester was used to test the radial support performance of 6 types, and the finite element analysis results were verified. The mechanical properties of the stent are closely related to the structural size. The result provides a reference for choosing a suitable stent according to the needs of the diseased location in clinical applications.

Introducción: La enfermedad arterial de miembros inferiores es un marcador de riesgo coronario, causa de invalidez y muerte en quienes la padecen, su identificación temprana puede atenuar estos efectos. Objetivo: Identificar la enfermedad arterial de miembros inferiores no diagnosticada a través de Eco-Doppler en pacientes con factores de riesgo aterogénicos. Método: Se realizó un estudio descriptivo de tipo transversal que incluyó a 100 pacientes de 40 años o más, fumadores, diabéticos y/o hipertensos, sin diagnóstico de enfermedad arterial de miembros inferiores, a quienes se le realizó Eco-Doppler de miembro inferior. Resultados: Se identificaron lesiones compatibles con EAMI en 69 por ciento de los estudiados, cuya edad media fue de 64,81 ± 10,12 años, y discreto predominio del sexo masculino. Las arterias más afectadas fueron la tibial posterior y la pedia con 43 por ciento y 39 por ciento respectivamente, las medidas de asociación mostraron OR (IC 95 por ciento) de 4,15 para la diabetes mellitus, 1,63 para el tabaquismo seguido de la hipertensión arterial con 0,27. Conclusiones: Seis de cada diez pacientes presentaron lesiones ateroscleróticas identificables por Eco-Doppler, predominaron las del sector tibial posterior y pedio en fumadores y diabéticos, estos últimos tuvieron cuatro veces más riesgo de padecer la enfermedad(AU)


Introduction: Arterial disease of the lower limbs is a marker of coronary risk, causing disability and death in those who suffer from it. Early detection can mitigate these effects. Objective: To identify undiagnosed lower limb arterial disease through Echo-Doppler in patients with atherogenic risk factors. Method: A descriptive, cross-sectional study was carried out in 100 patients aged 40 years or older, smoking habits history, and diabetic and / or hypertensive patients, with no diagnosis of arterial disease in the lower limbs, who underwent Eco-Doppler of the lower limb. Results: EAMI compatible lesions were identified in 69 percent of those studied, whose mean age was 64.81 ± 10.12 years, and a discrete male predominance. The most affected arteries were the posterior tibial and pediatric arteries in 43 percent and 39 percent respectively, the association measures showed OR (95 percent CI) of 4.15 for diabetes mellitus, 1.63 for smoking followed by arterial hypertension with 0.27. Conclusions: Six out of ten patients showed atherosclerotic lesions identifiable by Echo-Doppler, those of the posterior tibial sector and pedium predominated in smokers and diabetics, the latter had four times the risk of suffering from the disease(AU)

Objective To observe the curative effect of catheter thrombolysis in the treatment of subacute lower limb arterial occlusion.Methods Eighty-five patients with subacute lower limb arterial occlusion (98 limbs) and treated with catheter thrombolysis were enrolled and the removal of thrombus and patency of vessels were evaluated by angiography.Then,balloon dilatation or stent implantation was performed according to the angiographic findings after thrombolysis. Results After catheter thrombolysis, the degree and length of occlusion were significantly improved (P < 0.01). To judge the curative effect: significantly effective in 5 limbs (5.1%), effective in 85 limbs (86.7%), ineffective in 8 limbs(8.2%),total effective rate was 91.8%.Catheter thrombolysis combined with balloon dilatation and stent implantation were performed according the vascular occlusion status and all the occluded segments were open, the postoperative angiography showed no distal vascular occlusion. No bleeding occurred. Conclusions Catheter thrombolysis is effective and safe in the treatment of lower extremity arterial occlusion in subacute stage.Endovascular treatment combined with endovascular treatment can achieve good results.

Objective To explore the application value of triggered angiography non-contrast enhanced (TRANCE) technology in diagnosing lower limb arterial occlusive disease.Methods Totally 22 lower limb arterial occlusive disease patients were randomly selected,and then underwent TRANCE and DSA examinations.The arteries from the abdomen to the lower limb were divided into abdominal aorta,common iliac artery,external iliac artery,internal iliac artery,superficial femoral artery,deep femoral artery,popliteal artery,anterior tibial artery,posterior tibial artery and peroneal artery.Totally 337 sections displayed clearly were chosen to go through examinations by TRANCE and DSA.Results Of the 337 sections there were 312 ones with the same stenoses found by TRANCE and DSA,TRANCE found 16 sections with worse stenoses and 9 milder ones than by DSA.There were 153 sections with the same moderate stenoses (≥50%) displayed by TRANCE and DSA;Of the 153 sections,there were 15 ones with worse stenoses and 6 ones with milder stenoses found by TRANCE than by DSA.Kappa value of the two methods was 0.905.Conclusion TRANCE technology is a non-invasive,safe and nonradiative diagnosing method for the lower limb arterial occlusive disease.

Objective To investigate the color Doppler flow imaging ( CDFI ) diagnosis value and related factors analysis of lower limb arterial lesions in patients with type 2 diabetes.Methods 70 cases with type 2 diabetes as observation group , 60 not-type 2 diabetes patients as control group in the same period were selected ,who were giv-en lower limb artery CDFI examination and comparison .Results Lower limb artery blood wall intima-media thickness (IMT) abnormal,atherosclerotic plaques,luminal stenosis,occlusion detection rate of the observation group were 82.86%,60.0%,22.86%,they were significantly higher than 28.33%,16.67%,5.0% of the control group(χ2 =4.38,12.84,7.53,all P<0.05);the lower limb artery plaque location of the observation group often was seen in the femoral artery, posterior tibialis artery and the anterior tibialis , dorsalis pedis artery in 42.86%,10.0%,42.86%, 48.57%,compared with 11.67%,6.67%,3.33%,3.33% of the control group , there were significant differences (χ2 =4.87,12.75,13.97,all P<0.05).In the observation group,the related factors of type 2 diabetes lower limb arterial pathological including age,blood pressure,glycosylated hemoglobin,blood lipid(TC,TG,HDL-C),there were significant differences compared with the control group (t=13.26,5.34,4.00,89.00,6.54,7.54,all P<0.05). Conclusion The CDFI examination is a reliable diagnosis means of patients with type 2 diabetes lower limb artery lesions,which can determine the lesion site ,length,jam extent and vascular anatomical abnormalities ,and has an im-portant clinical value .

Objective To evaluate the correlation between hyperlipidemia and lower limb arterial disease in eldedy diabetic patients with color Doppler uhrasonography.Methods Two hundred and sixty elderly diabetic patients and 206 normal subjects were enrolled in the study.Doppler uhrasonographic manifestations of lower limb arterial disease,blood pressure and blood biochemistry parameters including fasting plasma glucose(FPG),postprandial plasma glucose(PPG),TC,TG,HLD-C,LDL-C,apoprotein B (apoB),very low density lipoprotein cholesterol(VLDL-C),apoprotein Al(apoA1)and fibrinogen(Fb)were compared between the 2 groups.Results (1)The blood biochemistry parameters,including the levels of FPG,PPG,TC,TG,VLDL-C,LDL-C,apoB and Fb were higher in diabetic patients t11an in normal subiects.while HDL-C was lower(all P<0.05).(2)The thickness of intima-media(IMT)in the lower limb arterial disease group was significantly increased and the presence of arterial stenosis and wall plaques more in diabetic patients[IMT in 205 cases(78.85%),atherosclerotic plaque in 241 cases(92.69%),muhivessel disease(more than 2 vessels)in 60 cases(23.08%)and arterial stenosis in 89 cases (34.23%)]than in normal subjects[IMT in 37 cases(17.96%),atherosclerotic plaque in 51 cases (24.76%),muhivessel disease in 27 cases(13.11%)and arterial stenosis in 3 cases(0.01%)],all P<0.01.(3)Regression analysis showed that lower limb arterial disease was correlated with high level of LDLC and low level of HDL-C.Conclusions Arteriosclerotic plaque is one of the early manifestations in elderly diabetic patients and hyperlipidemia may provide the basis of its formation.Doppler uhrasonographic examination of lower limbarterial discloses the early arteriosclerotic changes of lower limb arterial disease in elderly patients and offers the basic data of indication for clinical treatment.
