RESUMO

ObjectiveTo explore the clinical efficacy of Huanglian Jiedutang as an adjunctive treatment for acute cerebral infarction complicated with gastric motility disorder. MethodSixty patients with acute cerebral infarction complicated with gastric motility disorder with fire toxin syndrome were randomly divided into a western medicine control group (control group) and a traditional Chinese medicine (TCM) combined treatment group (observation group), with 30 cases in each group. The control group received basic treatment for cerebral infarction and relevant western medical symptomatic treatment based on the patients' gastrointestinal symptoms. The observation group received Huanglian Jiedutang in addition to the treatment provided to the control group. The treatment course was 7 days. Neurological deficit scores and gastrointestinal dysfunction scores were assessed in both groups before treatment and on the 4th and 7th days of treatment. Gastrointestinal electrographic parameters, serum citrulline (CIT), and motilin (MTL) levels were measured in both groups before treatment and on the 7th day of treatment. Clinical efficacy was compared between the two groups. ResultCompared with the baseline in both groups, the neurological deficit scores and gastrointestinal dysfunction scores were significantly reduced on the 4th and 7th days of treatment (P<0.05). The reductions in these scores were more significant on the 7th day compared with those on the 4th day of treatment (P<0.05). On the 4th and 7th days of treatment, the observation group showed a significantly greater reduction in neurological deficit scores and gastrointestinal dysfunction scores compared with the control group (P<0.05). On the 7th day of treatment, compared with the baseline, both groups showed a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. On the 7th day of treatment, compared with the control group, the observation group had a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. After 7 days of treatment, the total effective rate in the observation group was 90.00% (27/30), higher than 76.67% (23/30) in the control group, but the difference was not statistically significant. ConclusionAdjunctive treatment with Huanglian Jiedutang can effectively improve the symptoms of neurological function impairment and gastrointestinal dysfunction in patients with acute cerebral infarction complicated with gastric motility disorder, increase gastric antral and gastric body electric wave amplitudes, improve gastric motility disorder, and increase serum CIT and MTL levels, thereby improving the imbalanced secretion function of the gastrointestinal tract.

Objective:To investigate the curative efficacy of modified Qilang prescription on drug-dependent constipation with Qi and Yin deficiency and the effects on serum vasoactive intestinal peptide (VIP), motilin (MTL), 5-hydroxytryptamine (5-HT), and 5-hydroxytryptamine 4 receptor (5-HT4R). Method:A total of 160 patients diagnosed with drug-dependent constipation were randomly divided into a treatment group (<italic>n</italic>=80, Qilang prescription) and a control group (<italic>n</italic>=80, lactulose oral solution). The treatment lasted for eight weeks. Changes in clinical symptoms, traditional Chinese medicine (TCM) syndrome, and serum VIP, MTL, 5-HT, and 5-HT4R before and after treatment were observed. The clinical efficacies of the two groups were compared. An eight-week follow-up was carried out for the observation of recurrent rate and TCM syndrome. Result:The overall response rate of the treatment group (90.91%) was higher than that (75.00%) of the control group<italic> </italic>(<italic>Z</italic>=-6.514,<italic>P</italic><0.05). There was no significant difference in serum VIP, MTL, 5-HT, and 5-HT4R between the two groups before treatment. After treatment for eight weeks, both groups showed reduced serum VIP level as compared with those before treatment, and the treatment group was inferior to the control group (<italic>P</italic><0.05). The serum MTL levels of the two groups were both higher than those before treatment (<italic>P</italic><0.05), and the treatment group was superior to the control group (<italic>P</italic><0.05). After treatment, the level of 5-HT in the treatment group was higher than that in the control group (<italic>P</italic><0.05). The post-treatment 5-HT4R level in the treatment group slightly increased (<italic>P</italic><0.05), but no significant difference in 5-HT4R levels between the two groups after treatment was observed. During the eight-week follow-up, the recurrence rate in the treatment group was significantly lower than that in the control group at the 2nd and 4th weeks (<italic>P</italic><0.05). There was no significant difference in the recurrence rate between the treatment group [57.14% (40/70)] and the control group [64.81% (35/54)] after eight weeks. Conclusion:Modified Qilang prescription was superior to lactulose in the short- and mid-term efficacy on drug-dependent constipation with Qi and Yin deficiency. No significant difference in the long-term efficacy was observed. The underlying therapeutic mechanism might be related to the regulation of serum VIP, MTL, 5-HT, and 5-HT4R levels.

Objective To explore the effects of Chinese herbal compound Qinghuayin on the pathological changes of gastric mucosa and interleukin-10 (IL-10) ,nitric oxide (NO) ,gasmn (GAS) and motilin (MTL) in the serum in the animal model of chronic atrophic gastritis (CAG ) in rats .Methods We divided 53 Wistar rats randomly into blank control group (n=8) and CAG model group (n=45) ,and the animal model of CAG in rats was replicated by combination of disease and syndrome .After confirming the sampled rat model was successful built , the other 40 CAG rats in CAG model group were divided into model group ,vitacoenzyme tablet group ,low-dosage TMC group ,medium-dosage TMC group ,and high-dosage TMC group (each group n=8) .With the corresponding drug intervention to different rats for 30 days , the rats were executed . Then their blood was drawn from the abdominal aorta and the gastric tissue was taken to analyze the changes of serum IL-10 ,NO ,GAS and MTL concentrations and gastric mucosa pathology . Results Compared with blank control group , model group had various degrees of gastric mucosa atrophy ; decreased concentrations of serum IL-10 and GAS ; increased NO and MTL ( P<0 .01 ) .Compared with model group,Qinghuayin could improve gastric mucosa pathology in different degrees and increase the concentrations of IL-10 and GAS . Decrease the concentrations of NO and MTL( P<0 .05 or P<0 .01 ) . What's more. The curative effect in high-dosage TMC group was better( P<0. 01 ). Conclusion Chinese herbal compound Qinghuayin can effectively regulate the lopsided expressions of serum IL-10 . NO .GAS and MTL and reverse the pathological and histological changes in the gastric mucosa of CAG rats .

Candida albicans is an important opportunistic fungal pathogen of humans. Phenotypic plasticity is a typical biological feature of C. albicans, which is associated with pathogenicity, host adaptation, and sexual reproduction. Biofilm of C. albicans is a complex community formed by different morphological types of cells (yeast, hyphae and pseudohyphae) and secreted extracellular matrix. C. albicans biofilms are intrinsically resistant to antifungal drugs, the host immune system, and environmental stresses. Biofilm is an important virulence factor and a major clinical challenge. With the development of new technologies in global gene expression profiles and genetic manipulation, the regulatory mechanisms that govern C. albicans biofilm development and drug resistance become more and more clear. Major regulatory mechanisms involve the MAPK and cAMP signaling pathways and transcriptional regulators such as Bcr1 and Tec1. In addition, morphological transitions and sexual reproduction are also involved in the regulation of biofilm development. In this review, we focus on the genetic regulatory mechanisms of biofilm including the roles of cell-wall related proteins, transcription factors, and the MTL locus. In the last section, we also summarize the mechanisms of drug resistance of biofilm in C. albicans.

[Objective] To explore the mechanism of acupuncture at Zusanli point(ST36) in the treatment of functional gastrointestinal disorders(FGIDs). [Methods] A total of 40 Wistar rats were randomized into normal(n=10),model(n=10), Zusanli point(ST36),non-acupuncture point(n=10). FGIDs model was established by exerting the factors of cold-constraint stimulation and hunger disorder synthesis . And the content of rats serum and stomach tissue Gastrin (GAS) and Motilin(MTL) was observed by radioimmuno-assay. [ Results] Compared with the normal group, the content of rats serum and stomach tissue GAS and MTL of the model group was significantly reduced(P<0.01);Compared with the model group, the content of rats serum and stomach tissue GAS and MTL of ST36 group was significantly ascended(P<0.01);Compared with the non-acupuncture point group, the content of rats serum and stomach tissue GAS and MTL of ST36 group had statistically significant differences(P<0.01). [Conclusion] Acupuncture at ST36 had obvious improvement in FGIDs, its mechanism was closely related to GAS and MTL.

<p><b>OBJECTIVE</b>To observe the clinical efficacy of auricular point sticking on prevention and treatment of gastrointestinal complications after gynecological laparoscopic operation of general anesthesia, and to explore whether it is achieved by regulating the secretion of plasma motilin (MTL).</p><p><b>METHODS</b>Sixty patients who received selective gynecological laparoscopy under general anesthesia were randomly assigned into an observation group and a control group, 30 patients in each one. The patients in the observation group were treated with auricular point sticking at each morning and night, 30 min before anesthesia, revival after surgery and 24 h after surgery. The adhesive fabric with vaccaria seeds was pressed at shenmen (TF), wei (CO), benmen (CO), jiaogan (AH) and pizhixia (AT) for 3 to 5 min until the sensation of sourness, distension and numb appeared. The treatment was given for one week. The patients in the control group were treated only with similar adhesive fabric at auricular points at identical time points; each auricular point was pressed for 3 to 5 min. The anus exhaust time, defecating time and borborygmus were recorded; the level of plasma MTL was tested 30 min before anesthesia, 24 h after o-peration and 48 h after operation; the occurrence of nausea and vomiting from the end of operation to the end of treatment were also recorded.</p><p><b>RESULTS</b>Compared with the control group, the occurrence of nausea after operation was reduced in the observation group (<0.05), and the anus exhaust time and defecating time were shortened (both<0.05), and the recovery of borborygmus was improved (<0.05). The levels of MTL 24 h and 48 h after surgery were higher than those before operation in the two groups (all<0.05); The levels of MTL 24 h and 48 h after surgery in the observation group were significantly lower than those in the control group (both<0.05).</p><p><b>CONCLUSIONS</b>The assist of auricular point sticking could reduce the occurrence of nausea-vomiting and accelerate the recovery of gastrointestinal function in gynecological laparoscopic operation under general anesthesia, which is likely to be related with the inhibition on excess secretion of MTL.</p>

Objective To observe the effects of Coptidis Rhizoma on gastrointestinal motility functions, MTL and GAS in spleen-deficiency rats; To explore the dose-effect mechanism of “invigorating stomach” and “spoiling appetite”.Methods Fatigue and troublesome were used to establish exhausted spleen-deficiency models. 60 SD rats were randomly divided into blank control group, model group, positive medicine group, Coptidis Rhizoma high-, medium- and low-dose groups, with 10 rats in each group. Each medication group received relevant medicine for gavage, while the blank control group and model group received normal saline for gavage, once a day for 14 d. The general gastrointestinal motility functions, MTL and GAS of rats in each group were observed.Results Compared with blank control group, the gastric residual rate of model group was significantly higher; small intestinal propulsion rate, MTL, and GAS were significantly lower (P<0.05,P<0.01). Compared with model group, the gastric residual rate in Coptidis Rhizoma low-dose group was significantly lower; small intestinal propulsion rate, MTL and GAS increased significantly (P<0.05,P<0.01); small intestinal propulsion rate and MTL in Coptidis Rhizoma high-dose group were significantly lower (P<0.05).Conclusion High-doseCoptidis Rhizoma leads to “spoiling appetite”, and low-dose Coptidis Rhizoma leads to “invigorating stomach”, which may be related to adjusting gastrointestinal motility directly or adjusting the secretion of gastrointestinal hormones.

Objective To observe the clinical efficacy of electroacupuncture plus umbilical moxibustion in treating gastrointestinal dysfunction after gynecological abdominal operation.Method Totally 112 eligible patients undergone gynecological operation were randomized into group A of 37 cases, group B of 38 cases, and group C of 37 cases. Group A was intervened by electroacupuncture, group B was by umbilical moxibustion, while group C was by electroacupuncture plus umbilical moxibustion. After a treatment course, the symptom scores, restored time of bowel sounds, anal exhaust time, motilin (MTL), gastrin (GAS), and vasoactive intestinal peptide (VIP) contents were observed and compared, and the clinical efficacies were compared between the two groups.Result The symptom scores of the three groups were significantly changed after the intervention (P<0.05). After the treatment, the symptom score of group C was significantly different from that of group A and B (P<0.05). The restored time of bowel sounds and anal exhaust time in group C were significantly different from that of group A and B (P<0.05). The MTL, GAS, and VIP contents of the three groups were significantly changed after the intervention (P<0.05). The MTL, GAS, and VIP contents of group C were markedly different from that of group A and B (P<0.05). The total effective rate was 86.5% in group A, versus 81.6% in group B and 97.3% in group C. The total effective rate of group C was significantly different from that of group A and B (P<0.05).Conclusion Electroacupuncture plus umbilical moxibustion is effective in treating gastrointestinal dysfunction after gynecological abdominal operation.

Objective To study the excitability gastrointestinal hormone MTL and inhibitory gastrointestinal hormone SEC expression in the intestinal congestion and the effects of Salvia miltior-rhiza pretreatment at different time limits in 45min of rat hepatic ischemia-reperfusion to explore its possible mechanism and significance. Methods 80 rats were randomly divided into 4 groups: normal control group(CO group), sham-operated group(SO group), injury group(IR group), salvia miltior-rhiza pretreatment group(SM group). Non-invasive artery clamp was used to clip then slacken the he-patic pedicle to produce animal model of ischemia-reperfusion. The clamp time was 45 min. The rats in Salvia miltiorrhiza pretreatment group were injected 40 ml/kg saline with 6 g/kg Salvia Miltiorrhiza from caudal vein 30 min before clamping hepatic pedicle. While in the sham-operated group, the porta hepatis was dissected after laparotomy and hepatic pedicle not clamped. The upper jejunums at differ-ent reperfusion time phases (0 h, 3 h, 12 h, 24 h, 72 h) were made into specimens respectively, and then MTL and SEC immunohistochemical measurement were conducted. Results At 0 min of hepatic ischemia-reperfusion digestive tract congestion, the MTL expression of upper jejunum went down,SEC expression of upper jejunum went up. With the reperfusion time extending, the MTL expression also descended to minimum at 24 h, but SEC expression increased to reach the peak at 24 h. The ex-pression of these 2 hormones gradually returned to normal at 72h. The MTL expression of Salvia milt-iorrhiza pretreatment group was higher, SEC expression of SM group were lower than the IR group in each time phase of reperfusion. Conclusion The intestinal congestive injury caused by liver ischemia can down-regulate the excitability of gastrointestinal hormone MTL and up-regulate the gastrointesti-nal inhibitory gastrointestinal hormone SEC expression at reperfusion to inhibit gastrointestinal motili-ty. Salvia miltiorrhiza preconditioning can partly weaken the descending of jejunal MTL and increasing of the jejunal SEC expression, which may contribute to the early recovery of gastrointestinal motility.

The opportunistic fungal pathogen Candida glabrata is the second most common isolate from bloodstream infections worldwide and is naturally less susceptible to the antifungal drug fluconazole than other Candida species. C. glabrata is a haploid yeast that contains three mating-type like loci (MTL), although no sexual cycle has been described. Strains containing both types of mating information at the MTL1 locus are found in clinical isolates, but it is thought that strains containing type a information are more common. Here we investigated if a particular combination of mating type information at each MTLlocus is more prevalent in clinical isolates from hospitalized patients in Mexico and if there is a correlation between mating information and resistance to fluconazole and 5-fluorocytosine. We found that while both types of information at MTL1 are equally represented in a collection of 64 clinical isolates, the vast majority of isolates contain a-type information at MTL2 and α-type at MTL3. We also found no correlation of the particular combination of mating type information at the three MTL loci and resistance to fluconazole.

Objective To study the effect of catharsis method on the levels of plasma MTL and VIP of OOC rats,and investigate the rationality of purgation and lubricant purgation treating OOC. Methods The models of constipation by outlet obstruction were made by constricting the rectum of rats partly, and taking out stitches in vitro to relieve obstruction.Using the Da Cheng Qi Tang (DCQT) and Ma Ren Wan (MRW) as carrieres to object the effect of catharsis method on the levels of plasma MTL and VIP of rats and contrast histopathologie severity of colitis. Results The inflammatory reaction of intestinal mucous was relieved significantly in DCQT group and MRW group, and levels of plasma MTL were raised significantly than normal group(P<0.05). The levels of plasma VIP went down significantly in model group, DCQT group and MRW group than in normal group(P<0.01). Conclusion Purgation and lubricant purgation could relieve the inflammatory reaction of intestinal mucosa of OOC rats, influence the levels of plasm MTL and VIE and promote defecation.But we must consider using them exactly.

Objective To study the effect and mechanism of Xinkai Kujiang method(TCM treatment with the pungent and bitter herbs to regulate the gastrofunctions)on the functional dyspepsia(FD)rats.Methods Twenty-four rats were randomized into control group,model group,and Weikangning(herbal medicine for dyspepsia)group,with 8 in each.After a 7-day adaptive feed,the FD models were made with the clipping tail stimulation.Seven days after modeling,the Weikangning group was given Weikangning Capsule 0.625g/kg by gavage,twice a day,and the other two groups were given normal saline in the same dosage.The medication was appliey in succession for 12 days,and on the thirteenth day,the serum and gastric tissues were collected for detecting the lab indications:plasma motilin(MTL),plasma nitric oxide(NO),gastric tissue MTL,nitric oxide synthase(NOS),and the expression of Cajal interstitial cells(ICC)and gastrointesinal smooth muscle cells(SMC).Results Compared with the control group,the level of plasma MTL in the model group was significantly decreased,the level of NO significantly increased,and the expression of MTL,ICC and SMC positive neurons in the stomach myenteric nerve plexus was significantly weakened,but the expression of NOS positive neurons was significantly strengthened(P
