RESUMO

Objective To investigate the clinical efficacy of electrothermal acupuncture plus joint mobilization in treating knee osteoarthritis.Methods One hundred patients with knee osteoarthritis were randomized to treatment and control groups, 50 cases each. The control group received Maitland's technique and the treatment group, electrothermal acupuncture in addition. The Osteoarthritis Index score was recorded and cytokines (IL-1, IL-6 and TNF-α) contents were measured in the two groups before and after four weeks of treatment. The clinical therapeutic effects were compared between the two groups.Results The total efficacy rate was 91.5% in the treatment group and 80.0% in the control group; there was a statistically significant difference between the two groups (P<0.05). There were statistically significant pre-/post-treatment differences in the WOMAC score and cytokines in the two groups (P<0.01,P<0.05). There were statistically significant post-treatment differences in the WOMAC score and cytokines between the treatment and control groups (P<0.05).Conclusion Electrothermal acupuncture plus joint mobilization is an effective way to treat knee osteoarthritis.

ObjectiveTo investigate the clinical efficacies of ultramicro needle knife plus Maitland technique in treating knee osteoarthritis.MethodEighty patients were randomly allocated to observation and control groups, 40 cases each. The observation group received ultramicro needle knifeplus Maitland technique and the control group, Maitland technique alone. The subjects’ symptoms were scored using the Visual Analogue Scale (VAS) and the Lysholm Knee Score Scale (LKSS) before and after four and six weeks of treatment. The clinical therapeutic effects were evaluated in the two groups.ResultAfter four weeks of treatment, the total efficacy rate was 91.9% in the observation group (37patients) and 76.3% in the control group (38 patients); there was a statistically significant difference between the two groups (P0.05). There were statistically significant differences in the VAS score and the LKSS score between after six and four weeks oftreatment in the control group (P<0.05). No adverse reactions occurred during the whole treatment course.ConclusionUltramicro needle knife plus Maitland technique is significantly superior to Maitland technique alone and can markedly relieve the pain and dysfunction in patients with knee osteoarthritis. It is characterized by short time to produce an effect, a lasting therapeutic effect and high safety.

Introdução: A confiabilidade é uma medida que diz se os resultados são consistentes, estáveis ao longo do tempo, reproduzíveis e seguros. Os estudos de confiabilidade permitem definir a utilidade de um instrumento para um ou vários examinadores, possibilitando maior aplicabilidade clínica. Objetivo: Verificar a confiabilidade intra e interexaminador durante uma simulação da mobilização póstero-anterior do Método Maitland. Método: Participaram 20 voluntários randomizados de ambos os gêneros, com idade entre 20 e 30 anos (média de 21,60±1,18 anos, e mediana de 21 anos) que realizaram o curso de mobilização articular do Método Maitland. A captação dos dados foi realizada por meio de um Relógio Comparador, sendo que cada voluntário realizou 3 séries de 10 mobilizações. Os sujeitos foram orientados a mobilizar em grau 2 conforme a classificação descrita por Maitland. Resultados: A análise dos dados foi realizada pelo cálculo do Coeficiente de Correlação Intra Classe (CCI). Os resultados demonstraram uma baixa confiabilidade intra examinadores para 90% da amostra avaliada (CCI < 0,50) e moderada para 10% da amostra (CCI entre 0,50 e 0,75). Em relação ao Vício, 50% dos voluntários obtiveram um Vício entre -0,5 e 0,5, se aproximando do "ponto alvo" enquanto que os outros 50% se distanciaram do parâmetro estabelecido, mantendo as suas oscilações de 0,5 à 1,5 e -0,5 à -1. A confiabilidade inter examinador também foi verificada com base no Coeficiente de CCI, após a análise das médias finais de cada série dos 20 voluntários, o CCI entre os examinadores encontrado foi de 0,62, sendo este, considerado moderado (0,50 a 0,75). Conclusão: Houve uma confiabilidade intra examinador baixa e inter examinador moderada. Diante do exposto, os resultados indicam que os indivíduos do estudo não possuem um precisão exata da Amplitude de Movimento indicada, no caso, o Grau 2 de mobilização articular proposto por Maitland.

The reliability studies to define the usefulness of an instrument to one or more examiners, allowing for greater clinical applicability Objective: The aim of this study was to verify intra and interexaminer reliability during a postero-anterior mobílization simulation of the Maitland method. Method: Twenty randomized volunteers of genera both articipated of this study, with age between 20 and 30 years old (average to 21,60±1,18 years old and median to 2: years old) that underwent articulate mobilization course of Maitland method. Data's capture was made by a comparctor dock, each volunteer performing 3 series of 10 mobilizations. The volunteers were targeted to mobilize in degree 2 of Maitland classification. Results: Data's analysis was performed by intraclass correlation coefficient (ICC), being considered low reliability when the ICC was minor or equal 0,50, moderate of 0,50 to 0,75 and high above to 0,75. lhe results had demonstrated a low intra examiner reliability for 90% of sample evaluated (ICC < 0,50) and modera" e for 10% of sample (ICC between 0,50 and 0,75). Regarding to the Vicio verification, 50% of volunteers had gotte a Vicio between -0,5 and 0,5, near to the "target" while that the others 50% stay far from established parameter, keeping their oscillations of 0,5 to 1,5 and -0,5 to -1. Reliability interexaminer was also verified based on the ICC, after the ends averages analyze about each series of the 20 volunteers, the ICC found between the examiners was 0,62, being considered moderate (0,50 to 0,75). Conclusions: Intra examiner reliability was low and interexaminer moderate. Front it, the results indicates that the study's individuals do not have a great precision of the movement amplitude tested, in this case the degree 2 to articulate mobilization by Maitland.

RESUMO

@#ObjectiveTo study the effect of comprehensive treatment on lumbar interveretebral disc syndrome.Methods227 patients with lumbar interveretebral disc syndrome were randomized into two groups. Control group(127 patients) only accepted lumbar vertebra traction, while treatment group(100 patients) accepted comprehensive treatment, including lumbar vertebra traction, hot magnet, Maitland manipulation, and Mckeizie back muscles train. ResultsEffect of treatment group was obviously better than control group (P<0.05). Conclusions Comprehensive treatment on lumbar interveretebral disc syndrome can get better effect than simply traction.
