BACKGROUND: Cervical spondylotic myelopathy is a common degenerative disease of the cervical spine in spinal surgery. Surgical decompression is the most effective method to prevent the further development of the disease. However, patients are often accompanied by residual neurological symptoms such as sensory and motor dysfunction after surgery. Currently, the drug treatment for postoperative neurological recovery of patients is still unclear clinically. OBJECTIVE: To investigate the effect of mecobalamine combined with mouse nerve growth factor on the recovery of nerve function after cervical spondylotic myelopathy surgery. METHODS: A total of 82 cases of cervical spondylotic myelopathy from June 2017 to September 2018 in the Affiliated Hospital of Xuzhou Medical University were enrolled in this study. All the patients were randomly divided into observation group and control group with 41 cases in each. Mouse nerve growth factor injection was applied to the control group, and mecobalamine injection was added to the research group. All the patients were treated for 2 weeks. The symptoms and signs in both groups were respectively recorded before and after the treatment, and postoperative residual neurological symptoms were evaluated by Neck Disability Index (NDI) scores and Japanese Orthopedic Association scores. RESULTS AND CONCLUSION: At 1 month, 3 months, 6 months and 1 year after the operation, the NDI scores of the two groups were lower than those before the treatment, and the JOA scores were both getting higher than those before the treatment. The NDI scores in the observation group at 6 months and 1 year after the operation were significantly lower than those in the control group, and as well, the Japanese Orthopedic Association scores were significantly higher than those in the control group. These findings indicate that the combination of mecobalamine and mouse nerve growth factor is helpful to the recovery of nerve function in patients after cervical spondylotic myelopathy surgery, and the effect is better than that of mouse nerve growth factor alone.

OBJECTIVE: To observe the effect of long-snake moxibustion on pain, functional disorder and body constitution in the patients with lumbago of cold-dampness type. METHODS: A total of 90 patients with lumbago of cold-dampness type were randomized into long-snake moxibustion, acupuncture and medication groups, 30 cases in each group. In the long-snake moxibustion group, long-snake moxibustion was exerted on the spinal (between Dazhui[GV14] and Yaoshu[GV2]) with the self-prepared moxa powder formula combined with fresh ginger and moxa wool, once a week, for 4 weeks. Regular acupuncture was given to the acupuncture group, 30 min each time, once a day, 8 times as a treatment course, at the interval of 2 days between the courses. The duration of treatment was 1 month. In the medication group, Diclofenac Sodium was prescribed for oral administration, 25 mg each time, three times a day, and Mecobalamine tablets, 0.5 mg each time, 3 times a day. The duration of medication was 1 month. Before and after treatment, the differences were evaluated in the scores of visual analog scale (VAS), the present pain intensity(PPI), Oswestry disability index (ODI) and the scale of yang-deficiency body constitution among the 3 groups. RESULTS: After the treatment, VAS and PPI score and ODI were significantly reduced in the patients of the 3 groups as compared with those before treatment (all P0.05). The score of the scale of yang-deficiency body constitution in the long-snake moxibustion group was significantly lower than that in the acupuncture group and the medication group (P<0.05). Regarding the clinical effect, the effective rate was 93.33% (28/30) in the long-snake moxibustion group and it was 86.67% (26/30) in the acupuncture group and 73.33% (22/30) in the medication group after the treatment. The effective rate in the long-snake moxibustion group was significantly higher than those in the acupuncture group and the medication group (P<0.05). CONCLUSION: The long-snake moxibustion therapy can achieve significant effect on lumbago of cold-dampness type. This therapy can alleviate pain, relieve the functional disorders and improve the conditions of body constitution in the patients. Hence, it is applicable for the clinical promotion.

@#AIM: To investigate the protective effect of nerve growth factor(NGF)combined with mecobalamine on optic nerve and the influence on matrix metalloproteinase-2(MMP-2), tissue inhibitor of matrix metalloprotease-2(TIMP-2). <p>METHODS: Totally 54 patients(73 eyes)with acute angle closure glaucoma undergoing trabeculectomy was included in the study, and the patients were divided into the control group(30 cases, 35 eyes)and combination group(24 cases, 38 eyes)according to the digital table method. The patients in the control group were treated with mecobalamine, and the patients in combination group were treated with NGF combined with mecobalamine. Visual acuity, intraocular pressure, mean light sensitivity(MS), visual field mean defect(MD), retinal nerve fibre layer(RNFL)thickness, the nipple cup/disc ratio, P100 wave incubation period and amplitude of P100 wave, neuron-specific enolase(NSE), nitric oxide(NO), nitric oxide synthase(NOS), matrix metalloproteinase-2(MMP-2), metalloproteinases tissue inhibiting factor-2(TIMP-2)before and after treatment were observed in the two groups. The adverse reactions during medication were supervised. <p>RESULTS: Compared with those before treatment, visual acuity, MS, and P100 wave amplitudes increased, MD and P100 wave latency decreased(<i>P</i><0.05)after treatment, the change ranges in combination group were greater than those in control group(<i>P</i><0.05). There was no significant change in intraocular pressure, RNFL, the nipple cup/disc ratio before and after treatment in the two groups(<i>P</i>>0.05). Compared with those before treatment, serum NO, NOS, MMP-2 levels in the two groups increased, and the levels of NES, TIMP-2, TIMP-2/MMP-2 decreased(<i>P</i><0.05), but the change degrees in combination group were greater than those in control group(<i>P</i><0.05). <p>CONCLUSION: NGF combined with mecobalamine can improve postoperative visual function of glaucoma, which has certain regulatory effects on MMP-2 and TIMP-2.

Objective To observe the clinical effect of acupuncture combined with ganglioside and mecobalamine injections on idiopathic facial palsy (IFP).Methods A total of 120 IFP patients were divided into a ganglioside group,a mecobalamine group and a consociation group,each of 40.All were given routine drug and acupuncture treatment,while the ganglioside and mecoblamine groups were additionally provided with the corresponding injections of ganglioside and mecobalamine,respectively.The treatment was administered once daily for a total of 14 days.All the groups were evaluated using the House-Brackmann facial nerve grading system before and after the interventions of 14 days ended.The compound muscle action potential (CMAP) wave amplitude and R1 incubation,the motor nerve conduction velocity (MCV) and the sensory nerve conduction velocity (SCV) of the affected facial nerves were observed and compared before and after the 14 days of treatment.Results The total effectiveness rate of the consociation group (95.0%) was significantly higher than that of the ganglioside (90.0%) or the mecobalamine group (87.5%).After the treatment the CMAP wave amplitude had improved significantly and the R1 latency had shortened significantly in the consociation group compared to the other 2 groups.The average MCV and SCV of the consociation group had also improved significantly compared with the other two groups.Conclusion Acupuncture combined with ganglioside and mecobalamine injections is an effective therapy for idiopathic facial palsy.It can improve the patient's clinical symptoms,shorten the CMAP wave amplitude and R1 latency on the affected side and improve MCV and SCV.

Objective To evaluate the efficacy of ginkgo biloba extract injection combined mecobalamine treatment of diabetic peripheral neuropathy.Methods CNKI, CBM, VIP, Wanfang, PubMed, Cochrane library and Google academic search and other databases were searched, the RCT related ginkgo biloba extract injection combined mecobalamine treatment of diabetic peripheral neuropathy were collected.Meta analysis, drawing forest map and funnel plot by using Rev.Man 5 software.Results A total of 21 RCTs involving 1745 patients were obtained, 905 cases in experimental group and 840 cases in control group.Meta analysis showed that the total effective rate of the experimental group was better than that of control group ( RR=1.40, 95%CI:1.31-1.48), the conduction velocity of motor nerve median nerve(MD=5.39, 95%CI:3.90-6.87), common nerve(MD =5.33, 95%CI:3.82-6.83), sensory nerve median nerve (MD =4.31, 95%CI: 2.89-5.74)and common nerve (MD =4.10, 95%CI: 2.67-5.54)in experimental group were better than those of control group, the difference was statistically significant (P<0.05).Conclusion Ginkgo biloba extract injection plus cobalt-amine peripheral neuropathy in patients with diabetes treatment RCT design flaws, affecting the results of the authenticity and reliability; ginkgo biloba extract injection combined mecobalamine can improve the clinical efficacy and improve nerve conduction speed;due to less number of included studies, research quality is not uniform, still need to carry out more high quality randomized controlled trials to further validate the results.

RESUMO

Objective To compare the curative effect of Epalrestat and mecobalamine .Methods Epalrestat to treat 48 pa-tionts in DPN and mecobalamine to treat 42,measuring blood sugar ,blood pressure, blood fat and body mass index (BMI) prior and post treatment ,and measuring the MCV and SCV of nervus medianus ,nervus peronaeus connunis and nervus tibialis with EMG .Re-sults The symptom of the two sets have all been improved after the treatment ,and the effective power of Epalrestat and mecobalamine is 92.7% and 80.5% respectively.mean while there is improvement in MCV and SCV of nervus medianus ,nervus peronaeus connunis and nervus tibialis,and is more obvious in the set of Epalrestat ( P <0.01).In the whole process of the treat of the two sets ,no one appear to have adverse reactions .Conclusions Epalrestat has significant curative effect with less adverse reactions , and deserves to be spreaded in clinic.

Objective To observe the effect of treating DPN with xuesaitong tablet and mecobalamine. Methods 112 patients with DPN were randomly recruited into a treatment group (60 cases) and a control group (52 cases). The control group was treated with mecobalamine, 0.5 mg. Rid., while the treatment group was treated with Xuesaitong tablet, 50mg tid.,On the basis of the control group. Both groups were treated for successive three months. Results The total effective rate was 78.33% and 57.70% in the treatment group and the control group respectively, showing a statistical difference (t= 1.9804, P=0.0271). Conclusion The combined therapy of xuesaitong tablet and mecobalamine was more effective on DPN than mecobalamine used only.
