RESUMO

【Objective:】 To understand the true experience and feelings of nurses in the intensive care unit on the clinical experience of moral dilemmas, and to provide strategies for reducing the impact of moral dilemmas. 【Methods:】 Thirteen nurses in the intensive care unit were interviewed face to face by descriptive nature research method. The traditional content analysis method were used to compute the data, and the coding, categorization and topic extraction were carried out cyclically until the data were saturated and the interview was stopped. 【Results:】 The moral dilemma experience of nurses in the intensive care unit had the following three themes: negative psychological experience, negative behavior experience, and life and death cognitive experience. 【Conclusion:】 Nurses in the intensive care unit will have complex negative experiences after experiencing moral dilemmas. This negative experience threatens nurses’ mental health. Nursing managers should pay attention to the impact of moral dilemmas on nurses and formulate effective measures, thereby ensure nursing safety and maintain the healthy development of the nursing team.

【Objective:】 To understand the moral dilemma, coping style of frontline nurses and their correlation during the COVID -19. 【Methods:】 From March to April, 2021, 651 nurses working in designated hospitals for COVID -19 were selected by convenient sampling, and the Chinese version of Moral Distress Scale-Revised (MDS-R) and Chinese version of Coping Style Questionnaire (CSQ) were used for investigation. 【Results:】 Frontline nurses’ MDS-R score was 91.0, "value conflicts" was the moral dilemma with the highest frequency and intensity, more than half (51.0%) of the respondents had considered leaving their position because of moral dilemma. "Problem solving" and "seeking for help" were the major coping strategies adopted by the nurses. Nurses who directly provide medical services for COVID -19 patients, dissatisfied with hospital-related training and adopted negative coping strategies had higher moral dilemmas than other nurses. 【Conclusion:】 The moral dilemma of frontline nurses is at a high level during the period of epidemic prevention and control. Nursing managers should pay attention to guiding and helping nurses to establish positive coping styles and improve their ability to respond to major public health emergencies.

RESUMO

This paper reviewed the research on moral injury among foreign medical staff in the background of the COVID -19 pandemic. It was found that foreign medical staff bore multiple moral pressures and impacts in the face of the epidemic, including the collision between medical practice and utilitarian policy, the inability to meet personal needs, the rapid transformation of medical mode, and a variety of comprehensive factors. Therefore, the moral injury of foreign medical staff is particularly prominent. In order to avoid and reduce the occurrence of moral injury, it is necessary to strengthen the cultivation of moral resilience, provide psychological and social support, and carry out personalized treatment for medical staff.

Moral emotion is a kind of psychological activity that generated when dealing with realistic moral relationship or analyzing human behaviors, expressed as emotional emotions of love and hatred. From the perspective of moral injury research, on the one hand, moral emotion expresses as emotional collapse, which reflects the huge gap between reality and moral cognition, and deviates from values; on the other hand, it shows the typical symptoms of moral injury, such as shame, guilt, anger, apathy and so on. Based on this, the therapy of moral injury should start from solving the ethical conflict of moral injury, and carry out the healing process of "ethical and moral cognitive education, enlightening the thinking of core value issues, encouraging and guiding independent moral practice" in stages according to the different levels of individual moral emotion. As a result, moral emotion can be rational enough to promote healing moral injury.

RESUMO

Moral injury is a kind of trauma symptom caused by the impact of external immoral events on the internal moral value system of individuals or groups. Its origin lies in the deviation between individual’s internal moral schema and external moral situation. As a collection of moral consciousness units with value orientation in the regular activities of individuals in the moral environment, moral schema is composed of periphery and core. Moral schema realizes the domination of moral ego by supervising individual’s speech and behavior, exercising moderate control over individuals based on moral practice, and examining individual behavior with self-guilt as the tool of judgment. Moral schema is selective about what happened and acts as a filter for trauma events. Based on this, individuals can interpret or be subverted by moral events, thus playing a preventive role in moral injury.

The previous moral injury theory implied the premise of taking individual as the research object, while there were few studies on collective moral injury. Collective moral injury is defined as a series of consequences of violating widely accepted moral beliefs rather than simply adding up symptoms of injured individuals within a group. From the America "Floyd Case", this paper analyzed the functional elements and generating mechanism of collective moral injury, that was, the diversification of trauma sources events could appear alone or interweave, collective moral resilience showed the characteristics of social integrity and time accumulation, the interaction between collective trauma sources and collective moral resilience determined whether collective moral injury occurs. As the We-Media become popular, collective feature of moral injury is more evident, which presents two new features that the Internet platform becomes a hub for trauma events and the information Internet recorded consolidates collective moral resilience. To deal with collective moral injury, it was needed to respond to social concerns and guidance moral public opinion, respect the original endogenous order and inherit the traditional culture, satisfy reasonable moral needs and enlighten moral sense of people as well.

RESUMO

Infertile women experience complex biological and social experiences in the process of receiving IVF. From the perspective of females undergoing IVF, it is helpful to analyze the ethical and social problems that IVF may face more intuitively and concretely. In this study, 213 women who were undergoing IVF were randomly selected to carry out a questionnaire survey. The results showed that the choice of IVF for infertile women was driven by multiple pressures from family and society, not entirely on their own; They are not aware of the technology, but have a positive attitude, and are eager to be fully informed in the treatment process. In addition, the treatment brings great economic pressure to women who were undergoing IVF and has no significant impact on the relationship between husband and wife. Only by paying attention to the above problems and solving them can we benefit more infertile women and help them realize their reproductive wishes.

The application of face recognition technology is gradually expanding to the medical field. It has been initially used in the medical diagnosis of endocrine diseases and genetic syndrome. This technology is expected to be used for the screening of genetic syndrome and endocrine diseases, shortening the delay period of disease diagnosis and helping the staging of endocrine diseases. However, this technology also has some moral risks, such as the risk of personal information security disclosure, the challenge to the future of mankind and the division of moral responsibility. This paper reflected on the dilemma of moral responsibility in the application of face recognition and medical diagnosis, and explored the two basic problems of "who is the subject of moral responsibility" and "the specific division of moral responsibility of different moral subjects" in face recognition and medical diagnosis. Finally, some suggestions on the moral responsibility in face recognition and medical diagnosis are put forward. The first is to determine the role of face recognition and medical diagnosis as an auxiliary category, and doctors are still the main medical subject; the second is to build the responsibility ethics mechanism and laws and regulations, the establishment of the responsibility system of face recognition and medical diagnosis is inseparable from the joint action of law and ethics.

Psychological research results confirmed that the negative impact of shame on self often causes psychological diseases, and shame does not always related to ethical value. People with serious sense of shame will lead to negative evaluation and overall negation to themselves, and the sense of shame accompanied by great pain will cause great harm to individuals. To realize the sublimation of shame, the ethical orientation of its self cognitive schema and evaluation method must be changed, from the orientation of "transforming self and reaching perfection" to the position of "caring for self and accepting imperfect self" . To tap the positive value of shame, individuals must avoid hurting themselves and then care for themselves. "The principle of not harming life" and "do not denying people’s ethical subject status" are the bottom line of the ethical value of shame.

In March 2020, the outflow of age limited videos from ICU in Spain inspired us to rethink whether there is age discrimination in the allocation of scarce medical resources. This paper frist reflected on the problem of age discrimination caused by this phenomenon from four moral intuitions: the sacred view of life, the quality of life and values, public health ethics and Chinese culture, and then examined whether it is illegal from the legal level, finally pointed out the negative impact on the society, and put forward that taking age as the standard for the allocation of scarce medical resources is not suitable for China’s national conditions.

Human-animal chimera technology is increasingly applied to large animals. It is highly possible to cultivate human organs in pigs for transplantation in the future. Chimeric animals contain more and more human materials and are present in their brains and reproductive systems, which may humanize them and give them a higher moral status and even dignity. It raises an important ethical question: should such chimeric animals be created? If so, how they should be treated. It is not convincing to refute the research of human-animal chimera with the arguments of destroying natural order, unnatural and moral chaos. But the non-identity view holds that scientific research makes the lives of chimeric animals more valuable and that they are not actually harmed because their only other options do not exist. The answers to these ethical questions can resolve the ethical risks arising from the research of human-animal chimera.

Resumo Neste estudo com 333 participantes, investigamos como estereótipos raciais e atratividade facial impactam as avaliações morais na área de saúde. Utilizamos imagens de rostos com diferentes níveis de atratividade em cenários de dilemas morais, realizando análises estatísticas, como ANOVA e ANCOVA, para examinar essas interações complexas. Descobrimos que a atratividade física influencia positivamente as avaliações morais apenas em cenários de baixo conflito moral, não se aplicando a situações de alto conflito ou impessoais. A relação entre a cor da pele autodeclarada e o sexo dos participantes só se confirmou em um cenário específico, destacando a complexidade dessas influências. As hipóteses três e quatro, que sugeriam que as identidades racial e moral dos participantes suprimiriam os efeitos dos estereótipos raciais e da atratividade facial, não foram confirmadas. Entretanto, observou-se que o perfil étnico dos rostos, junto com a motivação dos participantes para controlar preconceitos, influenciou positivamente as avaliações morais. Esses resultados são interpretados à luz de teorias sobre atração interpessoal, julgamento moral e relações intergrupais, fornecendo insights importantes para as complexas dinâmicas que moldam as avaliações morais na área da saúde.


Abstract In this study of 333 participants, we investigated how racial stereotypes and facial attractiveness impact moral evaluations in healthcare. We used pictures of faces with different levels of attractiveness in moral dilemma scenarios, performing statistical analyses such as ANOVA and ANCOVA to examine these complex interactions. We found that physical attractiveness positively influences moral evaluations only in low moral conflict scenarios and does not apply to high conflict or impersonal situations. The relationship between self-declared skin color and participants' gender was only confirmed in a specific scenario, highlighting the complexity of these influences. Hypotheses three and four, which suggested that participants' ethnic and moral identity would suppress the effects of racial stereotypes and facial attractiveness, were not confirmed. However, we observed that the ethnic profile of the faces and the participants' motivation to control prejudice positively influenced moral evaluations. These results are interpreted in light of theories on interpersonal attraction, moral judgment, and intergroup relationships, providing essential insights into the complex dynamics that shape moral evaluations in healthcare.

ABSTRACT Objectives: to understand nurses' experiences of moral distress related to work overload during the COVID-19 pandemic in Brazil. Methods: qualitative research, whose data collection occurred through individual interviews with 19 nurses who worked on the front line of COVID-19 in health services in southeastern Brazil. Data were analyzed using thematic content analysis. Results: work overload proved to be a powerful source of experiences of moral distress due to excessive working hours during vaccination, double working hours, a troubled relationship due to pressure from managers and the population and physical and mental exhaustion, which prevented nurses from act according to their judgment. Final Considerations: nurses' work overload reflects on quality patient care and prevents nurses from acting in accordance with their moral principles, generating moral distress in nurses.


RESUMEN Objetivos: comprender las experiencias de angustia moral de los enfermeros relacionadas con la sobrecarga de trabajo durante la pandemia de COVID-19 en Brasil. Métodos: investigación cualitativa, cuya recolección de datos ocurrió a través de entrevistas individuales con 19 enfermeros que actuaron en la primera línea de COVID-19 en servicios de salud de la región Sudeste de Brasil. Los datos fueron analizados mediante análisis de contenido temático. Resultados: la sobrecarga de trabajo resultó ser una poderosa fuente de experiencias de sufrimiento moral por exceso de jornada durante la vacunación, doble jornada laboral, relaciones conflictivas por presiones de los directivos y la población y agotamiento físico y mental, lo que impidió al enfermero actuar según su criterio. Consideraciones Finales: la sobrecarga de trabajo de las enfermeras se refleja en la calidad de la atención al paciente e impide que las enfermeras actúen de acuerdo con sus principios morales, generando sufrimiento moral en las enfermeras.


RESUMO Objetivos: compreender vivências de sofrimento moral de enfermeiros relacionadas à sobrecarga de trabalho durante a pandemia de COVID-19 no Brasil. Métodos: pesquisa qualitativa, cuja coleta de dados ocorreu através de entrevistas individuais com 19 enfermeiros que atuaram na linha de frente da COVID-19 em serviços de saúde da região Sudeste do Brasil. Os dados foram analisados mediante análise temática de conteúdo. Resultados: a sobrecarga de trabalho mostrou-se potente fonte para vivências de sofrimento moral devido à jornada de trabalho excessiva na vacinação, à dupla jornada de trabalho, à relação conturbada por pressão de gestores e população e ao esgotamento físico e mental, os quais impediam o enfermeiro de agir conforme o seu julgamento. Considerações Finais: a sobrecarga de trabalho dos enfermeiros reflete na assistência de qualidade ao paciente, e impede que os enfermeiros atuem conforme os seus princípios morais, gerando sofrimento moral nos enfermeiros.

En la modernidad, la reflexión moral ha estado influida por orientaciones fundamentadas en la razón y en el sentimiento. Dos ejemplares de estas orientaciones son la Ilustración y el Romanticismo, respectivamente. El desarrollo de estos movimientos culturales e intelectuales está asociado a las demandas por los derechos políticos y sociales, junto a la insistencia en la importancia de la solidaridad y la hermandad en el desarrollo de las comunidades. Este artículo de reflexión presenta una perspectiva interpretativa del concepto de religión de Max Scheler La reflexión está enfocada en las raíces románticas de la propuesta Scheleriana, derivadas de la discusión moderna entre el Racionalismo y el Romanticismo. Adicionalmente, se presenta el contexto político y social al que responde la conceptualización de Scheler: Como resultado de esta reflexión, se concluye que el concepto de religión de Scheler reivindica la importancia de los valores y el compromiso moral frente a las crisis políticas que sufrió Europa a inicios del siglo XX.


In modernity moral reflection has been influenced by orientations grounded in reason and in feeling. Two exemplars of these orientations are the Enlightenment and Romanticism, respectively The development of these cultural and intellectual movements is associated with demands for social and political rights, together with the insistence in the importance of solidarity and brotherhood in the development of communities. This reflection paper presents an interpretative perspective of Max Scheler's concept of religion. The reflection is focused on the romantic roots of the Schelerian proposal, derived from the modern discussion between Rationalism and Romanticism. Additionally the political and social context to which Scheler's conceptualization responds is presented. As a result of this reflection, it is concluded that Scheler's concept of religion claims the importance of values and moral commitment against Europe's political crises at the beginning of the 20th century.


Na modernidade, a reflexão moral foi influenciada por orientações baseadas na razão e no sentimento. Dois exemplos dessas orientações são o Iluminismo e o Romantismo, respectivamente. O desenvolvimento desses movimentos culturais e intelectuais está associado a demandas por direitos políticos e sociais, juntamente com a insistência na importância da solidariedade e da fraternidade no desenvolvimento das comunidades. Este artigo de reflexão apresenta uma perspectiva interpretativa sobre o conceito de religião de Max Scheler O foco da reflexão são as raízes românticas da abordagem scheleriana, derivadas da discussão moderna entre racionalismo e romantismo. Além disso, é apresentado o contexto político e social ao qual a conceitualização de Scheler responde. Como resultado dessa reflexão, conclui-se que o conceito de religião de Scheler reivindica a importância dos valores e do compromisso moral diante das crises políticas sofridas pela Europa no início do século XX.

This study aimed to examine the relationship between the burnout of physicians and nurses and their moral sensitivity during the pandemic. This was a descriptive and correlational study. This study was carried out with physicians and nurses who were continuing to work actively during the pandemic. Snowboard sampling method was utilized. The total mean score of the participants on the MSQ was 90.78±19.10. The total mean score of the COVID-19 Burnout Scale was 34.52±9.65. A statistically weak and significant correlation was found between the COVID-19 Burnout Scale and the MSQ total score, as well as the "benefit" and "conflict" sub-dimensions. It was determined that as MSQ total scores, "benefit" and "conflict" scores decreased in healthcare workers, COVID-19 burnout scores increased. In our study, the moral sensitivity of the participants was found to be moderate, and their burnout levels were found to be high. Although there was a weak relationship, it was observed that burnout levels increased as moral sensitivity levels increased. It is crucial that we learn from pandemic experiences and transfer this knowledge to future generations. In particular, we must assimilate these important lessons into training in order to develop and protect the moral sensitivity of healthcare professionals.


Este estudio pretendía examinar la relación entre el burnout de médicos y enfermeras y su sensibilidad moral durante la pandemia. Se trató de un estudio descriptivo y correlacional, y se llevó a cabo con médicos y enfermeras que seguían trabajando activamente durante la pandemia. Se utilizó el método de muestreo de Snowboard. La puntuación media total de los participantes en el MSQ fue de 90,78±19,10. La puntuación media total de la Escala de Burnout COVID-19 fue de 34,52±9,65. Se encontró una correlación estadísticamente débil y significativa entre la Escala de Burnout COVID-19 y la puntuación total del MSQ, así como las subdimensiones "beneficio" y "conflicto". Se determinó que, a medida que disminuían las puntuaciones totales del MSQ y las puntuaciones de "beneficio" y "conflicto" en los trabajadores sanitarios, aumentaban las puntuaciones de burnout de la COVID-19. En nuestro estudio, se observó que la sensibilidad moral de los participantes era moderada y que sus niveles de burnout eran elevados. Aunque existía una relación débil, se observó que los niveles de burnout aumentaban a medida que lo hacían los de sensibilidad moral. Es crucial que aprendamos de las experiencias pandémicas y transmitamos estos conocimientos a las generaciones futuras. En particular, debemos asimilar estas importantes lecciones en la formación para desarrollar y proteger la sensibilidad moral de los profesionales sanitarios.


Esse estudo objetivou examinar a relação entre o burnout de médicos e enfermeiras e sua sensibilidade moral durante a pandemia. Esse era um estudo descritivo e correlacional. Esse estudo foi realizado com médicos e enfermeiras que continuaram trabalhando ativamente durante a pandemia. O método da amostragem em snowboard foi utilizado. A média total dos participantes no MSQ foi 90,78±19,10. A média total na Escala de Burnout pela COVID-19 foi 34,52±9,65. Uma correlação fraca e significante foi encontrada entre os escores totais na Escalas de Burnout pela COVID-19 e MSQ, assim como nas subdimensões "benefício" e "conflito". Determinou-se que como os escores totais na MSQ e os escores "benefício" e "conflito" diminuiram em trabalhadores de cuidados à saúde, os escores de burnout pela COVID-19 aumentaram. Em nosso estudo, a sensibilidade moral dos participantes foi considerada ser moderada e seus níveis de burnout considerados serem altos. Embora tenha havido uma correlação fraca, foi observado que os níveis de burnout aumentaram quando os níveis de sensibilidade moral aumentaram. É essencial que aprendamos das experiências com a pandemia e passemos esse conhecimento para as futuras gerações. Em particular, nós devemos assimilar essas importantes lições em treinamentos, de forma a desenvolver e proteger a sensibilidade moral de profissionais de cuidados à saúde.

This work aims to characterize religious and secular moralities in the West from a critical-diachronic point of view, seeking to identify spaces that cover moral relativism in solving complex bioethics issues. Concerns such as the destination of surplus embryos, for instance, usually raise polarized positionings without consensual solutions. With the presuppositions introduced on western culture through several events, mainly the principle of laicity, the view of morality widened. This work is reflexive, meta-ethical, with an interdisciplinary approach. It is a national and international literature review regarding the main points on religious and secular morality. We concluded that the present moral overview certainly includes moral relativism based on essential morality, one which can be represented by moral strangers and moral friends.


Este trabajo pretende caracterizar las moralidades religiosa y laica en Occidente desde un punto de vista crítico--diacrónico, buscando identificar espacios que abarquen el relativismo moral en la resolución de cuestiones complejas de bioética. Preocupaciones como el destino de los embriones sobrantes, por ejemplo, suelen suscitar posiciones polarizadas, sin soluciones consensuadas. Con los presupuestos introducidos en la cultura occidental a través de diversos acontecimientos, principalmente el principio de laicidad, se amplió la visión de la moral. Este trabajo es reflexivo, meta-ético, con abordaje interdisciplinario. Se trata de una revisión bibliográfica nacional e internacional sobre los principales puntos de la moral religiosa y laica. Concluimos que el panorama moral actual ciertamente incluye el relativismo moral basado en la moralidad esencial, que puede ser representada por extraños morales y amigos morales.


Esse trabalha objetiva caracterizar moralidades religiosas e secular no Ocidente desde um ponto de vista crítico--diacrônico, buscando identificar espaços que cubram o relativismo moral ao solucionar temas bioéticos complexos. Preocupações tais como a destinação de embriões excedentes, por exemplo, habitualmente levantam posicionamentos polarizados sem soluções consensuais. Com os pressupostos introduzidos na cultura ocidental através de diversos acontecimentos, principalmente o princípio da laicidade, a visão da moralidade foi ampliada. Esse trabalho é reflexivo, meta-ético, com uma abordagem interdisciplinar. É uma revisão da literatura nacional e internacional a respeito dos pontos principais sobre a moralidade religiosa e secular. Nós concluímos que a atual visão geral da moral certamente inclui o relativismo moral baseado na moralidade essencial, que pode ser representada por estranhos morais e amigos morais.

In August 2018, the Federal Council of Medicine (CFM), the body responsible for the supervision and standardization of medical professional practice in Brazil, published a national code of ethics for medical students (CEMS), which standardizes and suggests behaviors, based on the best moral and ethical values, for medical students, supporting the humanization of medicine and indirectly helping teachers and managers of medical courses. The purpose of the study was to evaluate the dissemination and awareness of scientific publications on the Brazilian CEMS code and consisted of an integrative literature review of all publications related to the Brazilian CEMS published from 2005 to the present. We found 4 studies related to regional and university codes of ethics for medical students published from 2005 to 2022, and regarding the CEMS elaborated by the FCM, we found a single study from its launch in 2018 to 2022. A CEMS has universal importance, facilitating the ethical and human development of future physicians, and despite its importance in the training of physicians, there is a lack of published research on the subject in Brazil.


En agosto de 2018, el Consejo Federal de Medicina (CFM), órgano responsable de la supervisión y normalización de la práctica profesional médica en Brasil, publicó un código nacional de ética para estudiantes de medicina (CEMS), que normaliza y sugiere conductas, basadas en los mejores valores morales y éticos, para los estudiantes de medicina, apoyando la humanización de la medicina y ayudando indirectamente a los profesores y gestores de los cursos de medicina. El propósito del estudio fue evaluar la difusión y el conocimiento de las publicaciones científicas sobre el código de la CEMS brasileña, y consistió en una revisión bibliográfica integradora de las publicaciones relacionadas con la CEMS brasileña publicadas desde 2005 hasta la actualidad. Encontramos 4 estudios relacionados con códigos de ética regionales y universitarios para estudiantes de medicina publicados desde 2005 hasta 2022, y, respecto de los CEMS elaborados por la FCM, encontramos un único estudio desde su lanzamiento en 2018 hasta 2022. Un CEMS tiene importancia universal, facilitando el desarrollo ético y humano de los futuros médicos, y, a pesar de su importancia en la formación de los médicos, falta investigación publicada sobre el tema en Brasil.


Em agosto de 2018, o Conselho Federal de Medicina (CFM), órgão responsável pela fiscalização e normatização do exercício profissional médico no Brasil, publicou um código nacional de ética do estudante de medicina (CEMS), que padroniza e sugere condutas, com base nos melhores valores morais e éticos, para os estudantes de medicina, apoiando a humanização da medicina e auxiliando indiretamente os professores e gestores dos cursos de medicina. O objetivo do estudo foi avaliar a disseminação e a conscientização das publicações científicas sobre o código CEMS brasileiro, e consistiu em uma revisão integrativa da literatura de todas as publicações relacionadas ao CEMS brasileiro publicadas de 2005 até o presente. Encontramos 4 estudos relacionados a códigos de ética regionais e universitários para estudantes de medicina publicados de 2005 a 2022 e, com relação ao CEMS elaborado pela FCM, encontramos um único estudo desde seu lançamento em 2018 até 2022. Um CEMS tem importância universal, facilitando o desenvolvimento ético e humano dos futuros médicos, e, apesar de sua importância na formação dos médicos, há uma carência de pesquisas publicadas sobre o assunto no Brasil.
