SUMMARY: The human soleus muscle has attracted attention in the fields of sport science, rehabilitation medicine, etc. for improving exercise performance in training, preventing injuries, and rehabilitation. The sagittal tendon plate of the soleus muscle is particularly important in rehabilitation. Few studies, however, have evaluated the shape of the sagittal tendon plate in the human soleus muscle in detail and attempted to classify its variations based on its morphology. In this study, we conducted a detailed analysis of the morphology of the sagittal tendon plates in soleus muscle specimens from Japanese cadavers and constructed a morphology-based classification system and evaluated their frequencies of occurrence. First, the specimens were divided into those with a sagittal tendon plate that was visible on the anterior surface (pennate muscle group) and those without (non-pennate muscle group). Next, based on the "number" and "breadth" of the sagittal tendon plates, the pennate muscle group specimens were further classified into four classes: Class I (one tendon, thin), Class II (one tendon, slightly broad), Class III (one tendon, very broad), and Class IV (two tendons, thin). Subsequently, the specimens were further divided into three types based on the position where the sagittal tendon plate joined the Achilles tendon: median tendon type, lateral tendon type, and medial tendon type (a total 13 divisions). When 458 Japanese soleus muscle specimens were classified into these divisions, the occurrence frequencies of Class I-IV were 80.57 %, 4.59 %, 5.46 %, and 1.09 %, respectively. In Class I, the median tendon type was more frequent than the lateral and medial tendon types, accounting for 48.47 % overall. The classification types of the sagittal tendon plate and their respective occurrence frequencies shown in this study are expected to serve as fundamental data in implementing rehabilitation of soleus muscle.


El músculo sóleo humano ha atraído la atención de la ciencia del deporte, la medicina de rehabilitación, etc. para mejorar el rendimiento del ejercicio en el entrenamiento, prevenir las lesiones y rehabilitación. La lámina tendinosa sagital del músculo sóleo es particularmente importante en la rehabilitación. Sin embargo, pocos estudios han evaluado en detalle la forma de la placa lámina sagital en el músculo sóleo humano y han intentado clasificar sus variaciones en función de su morfología. Realizamos un análisis detallado de la morfología de las láminas de los tendones sagitales en muestras de músculo sóleo de cadáveres japoneses y construimos un sistema de clasificación basado en la morfología y, además, evaluamos su frecuencia de aparición. Los especímenes se dividieron en aquellos con una lámina de tendón sagital que era visible en la superficie anterior (grupo muscular pennado) y aquellos sin (grupo muscular no pennado). A continuación, según el "número" y el "ancho" de las láminas de los tendones sagitales, las muestras del grupo de músculos pennados se clasificaron en cuatro clases: Clase I (un tendón, delgado), Clase II (un tendón, ligeramente ancho), Clase III (un tendón, muy ancho) y Clase IV (dos tendones delgados). Posteriormente, las muestras se dividieron en tres tipos, según la posición donde la lámina del tendón sagital se unía al tendón calcáneo: tipo de tendón mediano, tipo de tendón lateral y tipo de tendón medial (un total de 13 divisiones). En estas divisiones se clasificaron 458 especímenes de músculo sóleo de indiviuos japoneses, las frecuencias de ocurrencia de Clase I-IV fueron 80,57 %, 4,59 %, 5,46 % y 1,09 %, respectivamente. En la Clase I, el tipo de tendón mediano era más frecuente que los tipos de tendón lateral y medial, representando el 48,47 % del total. Se espera que los tipos de clasificación de la lámina del tendón sagital y sus respectivas frecuencias de aparición, que se reportan en este estudio, sirvan como datos fundamentales para implementar la rehabilitación del músculo sóleo.

SUMMARY: The study purposed to examine the morphometry and morphology of crista galli in cone beam computed tomography (CBCT) and apply a new analysis, supervised Machine Learning techniques to find the answers to research questions "Can sex be determined with crista galli morphometric measurements?" or "How effective are the crista galli morphometric measurements in determining sex?". Crista galli dimensions including anteroposterior, superoinferior, and laterolateral were measured and carried out on 200 healthy adult subjects (98 females; 102 males) aged between 18-79 years. Also, crista galli was classified with two methods called morphological types and Keros classification. In this study, the Chi-square test, Student's t-test, and Oneway ANOVA were performed. Additionally, Machine Learning techniques were applied. The means of the CGH, CGW, and CGL were found as 14.96 mm; 3.96 mm, and 12.76 mm in males, respectively. The same values were as 13.54 mm; 3.51 mm and 11.59±1.61 mm in females, respectively. The CG morphometric measurements of males were higher than those of females. There was a significant difference between sexes in terms of morphological classification type. Also, when the sex assignment of JRip was analyzed, out of 102 male instances 62 of them were correctly predicted, and for 98 female instances, 70 of them were correctly predicted according to their CG measurements. The JRip found the following classification rule for the given dataset: "if CGH<=14.4 then sex is female, otherwise sex is male". The accuracy of this rule is not high, but it gives an idea about the relationship between CG measurements and sex. Although the issue that CG morphometric measurements can be used in sex determination is still controversial, it was concluded in the analysis that CG morphometric measurements can be used in sex determination. Also, Machine Learning Techniques give an idea about the relationship between CG measurements and sex.


En el estudio se propuso examinar la morfometría y la morfología de la crista galli del hueso etmoides usando tomografía computarizada de haz cónico (CBCT) y aplicar un nuevo análisis, técnicas de aprendizaje automático supervisado para encontrar las respuestas a las preguntas de investigación "¿Se puede determinar el sexo con mediciones morfométricas de la crista galli?" o "¿Qué tan efectivas son las medidas morfométricas de la crista galli para determinar el sexo?". Las dimensiones de la crista galli, incluidas los diámetros anteroposterior, superoinferior y laterolateral, se midieron y realizaron en 200 sujetos adultos sanos (98 mujeres; 102 hombres) con edades comprendidas entre los 18 y los 79 años. La crista galli se clasificó con dos métodos llamados tipos morfológicos y clasificación de Keros. En este estudio, se realizaron la prueba de Chicuadrado, la prueba t de Student y ANOVA de una vía. Adicionalmente, se aplicaron técnicas de Machine Learning. Las medias de CGH, CGW y CGL se encontraron en 14,96 mm; 3,96 mm y 12,76 mm en hombres, respectivamente. Los mismos valores fueron 13,54 mm; 3,51 mm y 11,59 ± 1,61 mm en mujeres, respectivamente. Las medidas morfométricas del CG de los hombress fueron más altas que las de las mujeres. Hubo una diferencia significativa entre sexos en cuanto al tipo de clasificación morfológica. Además, cuando se analizó la asignación de sexo de JRip, de 102 instancias masculinas, 62 de ellas se predijeron correctamente, y de 98 instancias femeninas, 70 de ellas se predijeron correctamente de acuerdo con las mediciones de CG. El JRip encontró la siguiente regla de clasificación para el conjunto de datos dado: "si CGH<=14.4, por tanto el sexo es femenino, de lo contrario, el sexo es masculino". La precisión de esta regla no es alta, pero da una idea de la relación entre las medidas del CG y el sexo. Aunque la pregunta si las medidas morfométricas CG se pueden usar en la determinación del sexo sigue aún siendo controvertida. Se concluyó en el análisis que las medidas morfométricas CG se pueden usar en la determinación del sexo. Además, las técnicas de aprendizaje automático dan una idea de la relación entre las medidas de CG y el sexo.

Purpose: To study the intraoperative complications and postoperative clinical outcomes in different types of posterior polar cataract (PPC) following phacoemulsification, based on morphological classification. Methods: All consecutive patients with PPC who underwent phacoemulsification during the study period from 2016 to 2019 were included and sub?grouped based on the morphological characteristics according to Daljit Singh’s classification. Intraoperative complications such as posterior capsular rupture (PCR), vitreous loss, nucleus drop, and Postoperative best?corrected visual acuity (BCVA) at day 1 and day 30 were documented. Results: A total of 388 eyes of 380 patients were included. Eighty nine (22.9%) eyes belonged to type 1, 135 (34.8%) belonged to type 2, 8 (2.1%) belonged to type 3, and 156 (40.2%) belonged to type 4. Thirty?five (9.3%) eyes had intraoperative PCR, with vitreous loss in 21 (60%) eyes, and nucleus/cortex drop in 5 (1.3%) eyes. Six (75%) eyes of type 3, 14 (10.04%) eyes of type 2, 12 (17.7%) eyes of type 4, and 3 (3.4%) eyes of type 1 PPC had PCR. PCR occurred more during the emulsification of the nucleus (18 cases, 51.4%). A significant correlation was seen between intraoperative PCR and type of PPC with a higher incidence in type 3 (P < 0.001). BCVA was found to be significantly worse on day 1 among patients with PCR compared to those who did not and improved well by day 30. Conclusion: PPC morphology is significantly co?related with the occurrence of PCR, emphasizing the need for careful grading of posterior polar cataracts in predicting the risk of intraoperative complications.

Objective To investigate the predictive values of preoperative radiological features-intratumoral arteries and tumoral morphological classification on microvascular invasion (MVI) and on prognosis in patients with hepatocellular carcinoma (HCC).Methods A total of 220 consecutive HCC patients who underwent curative hepatectomy at Nanjing Drum Tower Hospital from January 2008 to December 2014 were retrospectively analyzed.The predictive values of preoperative radiological features and clinical data on MVI were analyzed by the univariate analysis and multivariate logistic regression methods.The prognosis of HCC patients was analyzed by the Kaplan-Meier survival analysis and the Cox proportional hazards models.Results Univariate analysis and multivariate logistic regression showed intratumoral arteries and tumoral morphological classification using preoperative CT[called the radiological predictorsof microvascular invasion (RPMVI)] to be independent predictors of MVI.The AUROC for RPMVI inpredicting MVI was O.830 (95% CI,O.769 ~ 0.891,P <0.05).The Cox multivariate analysis identified Child-Pugh grading,tumor size > 5 cm,RPMVI,MVI and non-anatomical liver resection to be independent risk factorsof overall survival (OS),while tumor size > 5 cm,RPMVI,MVI and non-anatomical liver resection to be independent risk factor sofre currence-free survival (RFS).The 1-,3-,and 5-year OS rates were 83.3%,61.7%,and 40.1% in patients with RPMVI and 97.1%,76.5%,and 69.6% in patients without RPMVI (P <0.05),respectively.The 1-,3-,and 5-year RFS rates were 61.9%,36.9%,and 28.4% in patients with RPMVI and 81.6%,61.9%,and 52.2% in patients without RPMVI (P < 0.05),respectively.Conclusions RPMVI is a novel radiological marker that accurately predicted histological MVI in HCC patients preoperatively.Similar to MVI,RPMVI was found to be an independent risk factor for prognosisin HCC patients,and it may provide the important information for surgical treatment planning in HCC patients.

Se realiza un estudio del desempeño de los modelos ocultos de Márkov (HMM) en la clasificación morfológica supervisada de eritrocitos en muestras de sangre periférica de pacientes con anemia drepanocítica. Los contornos se representan de forma novedosa considerando las diferencias angulares en la curvatura de los puntos del mismo. El entrenamiento de cada modelo se realiza tanto con la descripción normal de los contornos como con la representación de la rotación de los mismos, para garantizar una mayor estabilidad en los parámetros estimados. Se desarrolla un proceso de validación cruzada de 5x1 para estimación del error. Se obtienen las medidas de sensibilidad, precisión y especificidad de la clasificación. Los mejores resultados en cuanto a sensibilidad se obtienen al clasificar eritrocitos pertenecientes a dos clases: normales (96 por ciento) y elongados (99 por ciento). Al considerar además una clase de eritrocitos con otras deformaciones los mejores resultados se obtienen realizando el entrenamiento de los modelos con la rotación de todos los contornos, que alcanzó sensibilidades de normales (94 por ciento), elongados (82 por ciento) y con otras deformaciones (76 por ciento)(AU)


A study of the performance of Hidden Markov Models (HMM) in morphologic supervised classification of erythrocytes in peripheral blood smears of patients with sickle cell disease is realized. Contours are represented in original way considering the angular differences in the curvature of the points of the same. The training of every model comes true with the normal description of the contours and with the representation of the rotation of the same, in order to guarantee a bigger stability in the esteemed parameters. A process of validation crossed of 5x1 for estimate of the error is developed. The measures of sensibility, precision and specificity of classification are obtained. The best results obtain when classifying erythrocytes in two classes, with sensibility values in normal of 96 percent and elongated 99 percent. In the classification of erythrocytes considering the class of other deformations better results obtain accomplishing the training of the models with the rotation of all the contours, that it attained sensibilities of normal (94 percent), elongated (82 percent) and with other deformations (76 percent)(AU)

Los cálculos renales se forman cuando la concentración de los componentes de orina alcanzan un nivel en el cual es posible la cristalización. Aunque las manifestaciones clínicas de todos los cálculos son similares, los mismos difieren en su composición, patogénesis y tratamiento. La clasificación morfoconstitucional de los cálculos urinarios consiste en la aplicación de criterios, de forma mineralógica y morfológica, que sirven para clasificar los cálculos en tipos morfológicos, cada uno con una composición dominante y a los que pueden atribuirse causas probables de la formación del cálculo. Este trabajo, descriptivo y retrospectivo, hace referencia a los resultados obtenidos del análisis de cálculos provenientes de 50 pacientes, realizados en el año 2007, aplicando los criterios de la clasificación morfoconstitucional. Cada paciente recibió su resultado, asegurando la confidencialidad. Los tipos más frecuentes fueron: IIa en núcleo (44,4%), Ia en capas intermedias y superficiales (37,7% y 44,1%). El tipo IVa1 se encuentra en todos los estratos con porcentajes comprendidos entre 13 al 31,1%. Estos tipos morfológicos están relacionados a condiciones de hipercalciuria, hiperoxaluria e infecciones urinarias respectivamente. La presencia de 2 o más tipos morfológicos asociados se dio en 42 pacientes. La asociación de los tipos Ia+IVa1 se presentó en el 45% de los cálculos bifactoriales. Es la primera vez que se aplica el estudio morfológico en Paraguay, sin necesidad de remitir las muestras al exterior para su análisis. La importancia del análisis de los cálculos es encontrar la causa de la litiasis y orientar hacia el tratamiento más apropiado.


Kidney stones are formed when the concentration of urine components reaches such levels that crystallization is induced. Although clinical symptoms of all types of stones are similar, they differ in the composition, pathogenesis and treatment. Morphoconstitutional classification of kidney stones is made by applying mineralogical and morphological criteria, useful to classify them in different morphological types, each one with a dominant composition and a probable etiological cause of the stone formation. This retrospective study describes the results of the analysis of kidney stones from 50 patients made in 2007, applying criteria of morphoconstitutional classification. Each patient received his results respecting confidentiality. The more frequent types were IIa located in the nucleus (44.4%) and Ia in intermediate and superficial strata (37.7% y 44.1%). Type IVa1 was present in all strata with percentages from 13 to 31.1%. These morphological types are related to hypercalciuria, hyperoxaluria and urinary infections respectively. The presence of 2 o more morphological types was observed in 42 patients. The association of Ia+IVa1 was present in 45% of bifactorial kidney stones. This is the first morphological study of kidney stones in Paraguay, without sending samples abroad. The analysis of kidney stones is important not only to find the cause of the lithiasis but also to select the appropriate treatment.

Objective To study the vascular anatomy for laparoscopic anterior lumbar interbody fusion(LALIF),and to evaluate the feasibility of the surgical approach.Methods Vascular anatomy of 36 adult human bodies were studied by infusing red latex into the arteries.They were classified according to the percentage of the exposed lumbosacral disc in the interiliac triangle.Results The mean height of the aortic bifurcation(AB)was(41.81?13.82)mm,and the height of the confluence of the common iliac vein(CCIV)was(20.22?14.20)mm.The angles of the AB and CCIV were(51.88?12.09)? and(77.08?20.98)? respectively.The width of the median sacral artery(MSA)were(1.57?0.42)mm and(1.33?0.50)mm respectively at the top and bottom of the lumbosacral disc.While the distance from the right iliac vessel(RIV)to MSA at the top and bottom of the lumbosacral disc were(20.83?7.73)mm and(27.60?7.80)mm.The average width of the exposed disc was(36.78?13.06)mm,which accounts for(72.29?25.64)% of that of the disc.According to our morphological classification,22 specimens(61.1%)belong to type Ⅰ(Standard type,being exposed by more than 65%),9(25.0%)type Ⅱ(Narrow type,being exposed by 45% to 65%),and 5(13.9%)type Ⅲ(Barrier type,being exposed by less than 45%).The height of CCIV was negatively,but not significantly,correlate to its angle(r=-0.287,P=0.089).Conclusions In 86.1% of our patients,anterior approach is a safe and effective way for laparoscopic interbody fusion.Moreover,in 61.1% of the patients,the procedure can be carried out without managing the iliac vessels arround the interiliac triangle.

Double outlet right ventricle, a complex of congenital cardiac anomalies in which both great arteries arise wholly or in large part from the morphologic right ventricle presents diverse clinical manifestations according to the morphological characteristics. Eighty cases with DORV were diagnosed and operated at Yonsei Cardiovascular Center from 1988 to 1992. The analysis of the morphological characteristics of this anomaly and comparative study of operative methods and mortality according to the morphological classification led to the following results: 1) The location of VSD varied to be subaortic in 40 cases, subpulmonic in 15, doubly committed in 6 and noncommitted in 19 cases. The great arteries were interrelated in D-malposition in 43 cases and L-malposition in 18 and 26 cases among above mentioned 61 cases revealed the side-by-side relationship. Ten of the cases showed normal position and the rest 9, A-malposition. 2) Pulmonary stenosis was found in 60 cases, and when associated with subpulmonic VSD, occurring less frequently. PDA and ASD secundum were associated in successional order and only 3 cases were verified to carry aortic arch anomalies such as coarctation and interruption, all with subpulmonic VSD. 3) Intraventricular tunnel repair was possible in every case associated with subaortic VSD and the postoperative prognosis was excellent. In comparison, most of the cases associated with other types of VSD in which only palliative surgery or various types of intraventricular repair were performed, the general outcome was poor. But total cavopulmonary connection, one of the physiological corrective methods, showing a better postoperative prognosis is being carefully considered for its broad adoptation in future. In conclusion, double outlet right ventricle, a pathophysiological complex of various anomalies, should be thoroughly evaluated for the morphological characteristics to decide the most appropriate types of operation and for consequent improvement of prognosis.

In 1960, Tskimoto first mentioned the spinal cord lesions caused by compression of posterior longitudinal ligament(OPLL), thereafter, hundreds of cases have been reported. There are, however, some controversial opinions concerning the mechanisms of the formation and development of OPLL and questions why a special ligaments, posterior longitudinal ligaments can be calcified, enlarged and compress the cord. And also, there have been many reports describing various surgical method that could manage OPLL, but the OPLL still has many problems in surgical treatment. Recently, the neurosurgical team at the St. Pauls Hospital, encountered and operated on 7 cases of OPLL at cervical area. After all dignostic tests and examinations were done, the morphological classification, combined other lesions and pathological status were verified and analyzed during treatment. The different surgical methods were taken case by case. The authors would like to share with you our experiences of the surgical methods, indications of operation the results and the progrosis of treatment in OPLL.
