RESUMO

Objective To observe the effects of myelotomy on autophagy activation after traumatic spinal cord injury (SCI) in rats. Methods 54 adult female Sprague-Dawley rats were randomly assigned to sham-operated group (SG, n=18), contusion group (CG, n=18) or myelotomy group (MTG, n=18). The T10 SCI model in rats was induced with a New York University (NYU) impactor and myelotomy was performed 24 hours after SCI. They were evaluated with the BBB score 1, 7, 14 days after injury. The expression of mRNA of Beclin-1 and Bcl-2 were detected with real-time quantitative reverse transcriptase polymerase chain reaction (RT-PCR). The formation of autophagosome was investigated under electronic microscope (EM) 3 days after injury. Results BBB score was more in the MTG than in the CG 7 and 14 days after injury (P<0.05), while the expression of Beclin-1 mRNA was less (P<0.05). The expression of Bcl-2 mRNA was more in the MTG than in the CG 3 and 7 days after injury (P<0.05). The expression of Beclin-1 mRNA was negatively correlated with BBB scores (P<0.05). The formation of autophagosome was less in the MTG than in the CG. Conclusion Myelotomy can improve the recovery of motor function in rats after acute traumatic SCI, which may associate with neuroprotection mediated by inhibition of autophagy through the Bcl-2 signaling pathway.

Siringomielia, cavidades quísticas dentro del cordón espinal. Con incidencia baja de 8.4 por 100000 por año, Presentamos nuestra experiencia en tratamiento quirúrgico de 5 casos de siringomielia. Se presenta serie clínica, sin distinción de edad ni sexo, con diagnostico de siringomielia y que cumplan con los criterios quirúrgicos establecidos por el grupo. Estudiamos 5 pacientes, la totalidad de los casos presentaron síndrome disociativo medular por siringomielia, 4 casos sexo femenino, promedio de edad de 40,8 años, rango de seguimiento de 6 años a 6 meses, 5 casos procedentes del estado Mérida, 100% diagnostico por resonancia, 3 caso localización cervical, 1 caso la etiología es hemangioma capilar, a los 5 se les realizo derivación siringosubaracnoidea con mejoría clínica y en el postoperatorio tardío un 20% presento recidiva, manejada conservadoramente.


Syringomyelia, cystic cavities within the spinal cord. With low incidence of 8.4 per 100000 per year, present our experience in surgical treatment of 5 cases of syringomyelia. We present clinical series, regardless of age, sex diagnosed with syringomyelia and meet surgical criteria established by the grupo. We studied 5 patients, all cases had spinal cord syndrome and syringomyelia dissociative 4 cases female, mean age of 40.8 years, range 6-year follow-up to 6 months, 5 cases from the state of Merida, 100% resonance diagnosis, 3 cases cervical lesion, 1 case of capillary hemangioma etiology, to 5 were conducted with clinical improvement siringosubaracnoidea bypass and postoperative period by 20% for 1 case of recurrent, conservatively managed.

The surgical removal of solid deepseated hemangioblastomas remains challenging, because treatment of these lesions is often complicated by severe bleeding associated with the rich vascularity of this tumor, and by severe neural tissue injury associated with the difficulty of en bloc resection, especially when the tumor is located at the cervicomedullary junction. Therefore, preoperative embolization of deepseated solid hemangioblastomas may play an important role in successful surgical removal by reducing major bleeding and neural tissue damage. A 24-year-old woman, 28-weeks pregnant, was admitted to our hospital for the evaluation of quadriparesis, and brain magnetic resonance imaging(MRI) revealed intra-axial mass lesion in the cervicomedullary junction. After delivery, her neurologic symptoms became aggravated, and we decided to operate. Preoperative angiography revealed a hypervascular tumor in the posterior fossa, and embolization of the main feeding artery using gelfoam and microcoil, resulted in marked reduction of tumor vascularity. She underwent a midline suboccipital craniotomy involving the removal of the arch of C-1. The tumor was totally removed through a midline myelotomy, and at her 6-month follow-up she walked independently. We report on the combined use of the preoperative embolization of feeding vessels and subsequent operative resection in a patient with a solid hemangioblastoma at the cervicomedullary junction immediately after delivery.

Direct spinal epidural surface of spinal evoked potentials have been made in 16 adult cats with acute spinal cord injuries produced experimentally in which myelotomy was performed in 8 cats 1 hour after the injury. The remaining 8 animals served as control. The spinal cord injury with 20gm - 20 cm was inflicted over the entire dorsal surface of the spinal cord at L2-3 interspace by Allen's weight-drop method which was improved by placing a curved steel plate anterior to the spinal cord to provide a smooth, hard surface for the receipt of posterior cord impact. Under the operating microscope, a median posterior longitudinal myelotomy approximately 10mm to 15mm in length was made at the injury site 1 hour after the injury. Animals were evaluated electrophysiologically either from preinjury state to postinjury 4 hours(control group) or from preinjury state to post myelotomy 3 hours(myelotomy group). The component waves of the spinal somatosensory evoked potentials(SSEP) of control group disappeared sequentially after the injury in the following order : N4, N3, N2, N1. In myelotomy group, N4 and N3 wave also disappeared, but N1 and N2 wave remained. The latencies of the component waves increased & its amplitudes decreased sequentially after the injury. In myelotomy group, as com-with the control group, post-traumatic latency rate increase as well as amplitude were both reduced.

RESUMO

Primary spinal glioblastoma multiforme is very rare, especially in childhood. 13-year-old boy was admitted to our neurosurgical department because of paraparesis, sensory loss below T10 and sphincter disturbance. Thoracic myelogram showed complete obstruction at T10 level and total laminectomy with partial removal of the intramedullary cord tumor mass was performed under the impression of spinal cord tumor. Postoperatively, his symptoms and signs unchanged. Pathological diagnosis was intramedullary glioblastoma multiforme involving thoracic cord.
