RESUMO

OBJECTIVE To investigate and control the risk factors about nosocomial fungal infection in cerebral apoplectic patients. METHODS To study retrospectively 3027 hospitalized patients with cerebral apoplexia in our hospital from Sep 2002 to Sep 2005. RESULTS The incidence of nosocomial fungal infection was 3.24% in all(3 027 cases).Nosocomial fungal infection occurred mainly in lower respiratory tract(67.2%) and oral cavity(21.1%),the main pathogens of nosocomial fungal infection were Candida spp(96.7%).Developments of nosocomial fungal infections were closely related with aging,time of antibiotics therapy,coma,bed-days,diabetes mellitus,difficulties in swallowing,and hypoalbuminemia. CONCLUSIONS The rate of nosocomial fungal infections is higher in cerebral apoplectic patients with aging coma,long bed-days,diabetes mellitus,difficulties in swallowing,hypoalbuminemia,and receiving broad-spectrum antimicrobial agents therapy.Candida spp are the main pathogens of nosocomial fungal infections.We must take some measures to control the risk factors mentioned above and prevent nosocomial fungal infections.

RESUMO

OBJECTIVE To investigate the clinical status about fungal infections in hospital and the types,distribution status and risk factors of the fungi.The cause of nosocomial fungal infection is discussed so as to explore its prophylactic and treatment measures.METHODS Retrospective analysis method was used to analyze 495 patients with fungal infection in People′s Hospital of Shaoxing from Jan 1999 to Dec 2003.RESULTS The major causes of nosocomial fungal infection were found including usage of broadspectrum antibiotics,immunosuppressors and other chemotherapeutics and invasive operations.CONCLUSIONS Antibiotics should be used according to the national standard strictly to prevent dysbacteriosis.Sterile operation and disinfection technique should be carried out strictly to prevent external infections.The primary diseases and malnutrition should be treated actively so as to improve body′s resistibility.These important measures can be helpful against nosocomial fungal infection.

RESUMO

Objective To study the preventive effect of small-dose fluconazol adiministered in low frequency on nosocomial fungal infectoin.Methods The condition of nosocomial fungal infection among 200 patients in pediatric intensive care unit (PICU)were observed,who were suffered with serious infection or basic diseases and underwent broad-spectrum or ultra-broad spectrum antibiotic and steroid hormones treatment.These patients were divided imto treatment and control group.And the patients in treatment group received fluconazol[5 mg/(kg?time)once every 2 days,total 3 times,after that,twice 1 week till improved] to prevent fungal infection .The control group were treated with fluconazol [6-10 mg/(kg?time),once everyday for 10-14 days] after fungal infection.Results The incidence rate of nosocomial fungal infectoin of control group was 58.3%(56 out of 96 cases,which were 44 cases of mouth,5 cases of respiratory tract ,5 cases of digestive tract and 2 cases of urethra ) and that of treatment group was 1.9%(2 out of 104 cases, which were 2 cases of mouth).In control group,37 cases were cured,17 cases improved and 2 cases were not effective.Mouth fungal infection in treatment group was gently and cured with 1 or 2 times of local treatment .The treatment group didn′t occured liver function damage or aggravation.Conclusion For PICU patients, adiministration of small-dose flucomazol in low frequency is an effective and relatively safety method to prevent against nosocomial fungal infectoin.
