RESUMO

Objective To investigate the status quo of oral nursing in the emergency department of top three hospitals in Shanxi province, and to analyze the existing problems so as to provide the basis and reference for improving the clinical operation standard. Methods In the form of WeChat questionnaire, a questionnaire survey was conducted to investigate 190 nurses who met the requirements in emergency departments of 6 top three hospitals in Shanxi province. Results All the 6 hospitals surveyed did not incorporate oral care into the nurse examination program; 28.42%(73/190) of the subjects had oral assessments of endotracheal intubation according to their clinical experience;chlorhexidine (70.00%,133/190) and physiological saline (65.79%,125/190) were the most commonly used oral care solutions in clinic;48.42%(92/190) of nurses performed this operation for 6-10 min;91.58%(114/190) of nurses believed that oral care played an important role in the recovery of these patients. Conclusions The oral care and oral care of the patients in the emergency department are mostly clinical experience, so it is urgent to establish a unified standard of implementation on the basis of evidence. The professional knowledge of oral nursing of the patients undergoing oral trachea intubation is low, so it is suggested that the training of emergency nurses be strengthened and the operation should be included in the assessment of nursing operations.

RESUMO

Objective To study the effect of blind oral trachea cannula on haemodynamics and stress reaction of patient.Methods Fifty patients with general anesthesia were selected and randomly di-vided into blind oral trachea cannula group( T group) and endotracheal intubation group( C group) , 25 ca-ses for each group.Patients'mean arterial pressure( MAP) and heart rate( HR) were recorded in two groups at the following time points:before induction ( T0 ) , before intubation ( T1 ) ,3 min after intubation ( T2 ) , 60min after intubation(T3),The blood sugar and blood serum corticosteroid(CORT)were recorded at T0 and T3 points, The intubation frequency、time of completing intubation and adverse reactions such as pharyngo-dynia and cerchnus were recorded.Results The the MAP and HR in T group [ ( 88.6 ±11.0 ) mmHg, (70.9 ±9.8)min, respectively]were markedly lower than that in C group[(101.8 ±10.4)mmHg, (79.7 ±14.4)min, respectively]at T2( P <0.05).The blood sugar and CORT in T group[(6.2 ±1.4)mmol/L, (380.1 ±111.3)nmol/L, respectively] were markedly lower than that in C group[(7.5 ±1.3)mmol/L, (450.8 ±129.5)nmol/L, respectively]at T3( P <0.05).There were no significant difference in com-plication.Conclusions The blind oral trachea cannula have smaller effect on haemodynamics and stress reaction of patient, then have few side effect.

Objective To study the efficacy of the electrolyzed oxidizing water (EOW) in the oral irrigation of patients with oral trachea cannula. Methods 60 patients with oral trachea cannula were ran-domly divided into the experimental group and the control group, the experimental group received oral irri-gation with EOW,while the control group used normal saline. The oral irrigation efficacy was compared be-tween the two groups. Results In the experimental group,the bacterial clearance rate was 95.21% ,while the control one's was 37.19%, the stomatodysodia rate of the experimental group was 3.33%, the control one's was 86.60%, the oral fungus infection rate of the experimental group was 0% , the control one's was 43.33%, the incidence of dental ulcer of the experimental group was 10.00%, the control one's was 3.33%. Significant difference existed in bacterial clearance rate and the stomatedysodia rate between the two groups. Conclusions Oral irrigation with EOW in oral trachea cannula patients can decrease or con-trol the field planting of bacterium in pharynx oralis and stomatodysodia, and prevent pulmonary infection.
