RESUMO

Orbit is a common site for vascular lesions in all age groups, with venous malformations being the most common type. Orbital varices are a rare subset of venous malformation which is a vascular hamartoma that communicate with normal orbital veins. The mainstay of diagnosis and differentiation from other vascular and neoplastic lesions of orbit is demonstration of change in size of lesion on dynamic maneuvers. In this article, we present a case of left orbital venous varix which showed enlargement on Valsalva maneuver. The patient was evaluated using prone MRI technique to demonstrate increase in the size of lesion. Multiple asymptomatic developmental venous anomalies were detected on brain screening, which is a known association.

Objective To investigate the effectiveness of treatment of primary orbital varix via venous embolization therapy approach. Methods From January 2007 to January 2015,the clinical data of 12 patients with primary orbital varix were analyzed retrospectively. All the micro-catheters were implanted via the inferior petrosal sinus approach. The microcoils and Onyx18 were used to embolize the primary orbital varix. Four patients were embolized with micro-coils only, three were embolized with Onyx, and five were embolized with microcoil + Onyx. Results After successful catheterization, the lesions were totally embolized in 12 patients. The symptoms of postural exophthalmos disappeared and the pain was relieved,the depressed symptom of eyeball disappeared in 10 cases, and two patients were relieved partially ( single material embolization) . Nine patients were followed up for 6 to 24 months. The orbital DSA,MRI or CT re-examination was performed. The thrombosis of orbital varices within the lesions was observed and no cavity was found. One of the patients suffered from limited lateral eyeball abduction. Another three were lost to follow up. Conclusion The embolization treatment of primary orbital varix is safe, effective, and convenient via inferior petrosal sinus approach.

RESUMO

We report a case of the primary deep orbital venous varix treated by endovascular coil embolization procedure by transfemoral catheterization. This method of treatment has the advantage of image‑guided localization of the pathology, real‑time management and confirmation of the success of the procedure in the sitting.

El enoftalmos es un motivo de consulta poco frecuente en la práctica médica. La mayor parte de los casos se debe a un aumento de la cavidad ósea orbitaria de origen traumático. Presentamos el caso de una mujer de 63 años que consultó por enoftalmos progresivo de su ojo izquierdo, de doce años de evolución. El examen oftalmológico reveló la presencia de un marcado enoftalmos del lado izquierdo, que mejoraba notablemente con maniobras de Valsalva. Una tomografía computarizada y una resonancia magnética de la órbita permitieron diagnosticar una várice orbitaria no complicada. Las várices orbitarias son infrecuentes y por lo general se manifiestan por episodios de exoftalmos intermitente. Sin embargo, en raras ocasiones, la distensión y colapso repetidos pueden llevar a un enoftalmos progresivo por atrofia de la grasa orbitaria.


The enophthalmos is a rare cause of consultation in medical practice. The majority of cases are due to orbital space enlargement of traumatic origin.We report the case of a 63 year old patient with progressive enophthalmos of the left eye, with twelve years of evolution. Ophthalmologic examination revealed the presence of a marked enophthalmos on the left side, which improved significantly with Valsalva maneuvers. A CT scan and an MRI of the orbit allowed diagnosing uncomplicated orbital varices. Orbital varices are rare and usually manifest by episodes of intermittent exophthalmos. However, on rare occasions, repeated distension and collapse can lead to progressive enophthalmos by orbital fat atrophy.

RESUMO

Orbital varix is a relatively rare disease that causes exophthalmos due to increase of cerebral venous pressure. We experienced a forty-two-year-old male patient, who complained of intermittent exophthalmos and blurred vision. The diagnosis was based on clinical manifestation and various radiologic studies including computed tomography(CT), venography, and magnetic resonance imaging(MRI). MRI is a useful and safe diagnostic study through which we could observe the orbital varix and assess the relationship between the varix and the orbital structure. In this case, the varix is huge and attached to superior rectus muscle. There being many surgical risks, he has been followed up without surgical treatment and educated to modify the life pattern for prevention of increase of carotid venous presure.
