RESUMO

La incidencia de masas anexiales diagnosticadas durante el embarazo varía según las diferentes series consultadas; sin embargo, masas de tamaño superior a 6 cm suponen casos excepcionales en la literatura. El manejo de este tipo de pacientes supone un reto para el equipo multidisciplinario, ya que se debe tener en cuenta el riesgo de cáncer de ovario en este tipo de pacientes. Se trata de una gestante de 34 años a la que se le diagnostica una masa anexial gigante durante el control ecográfico del primer trimestre. Ante la ausencia de sufrimiento fetal y teniendo en cuenta las características radiológicas de la lesión, se decide mantener una actitud expectante a la espera de un parto vía vaginal y poder realizar una cirugía electiva para la exéresis de la lesión. Tras el parto, la paciente sufre una anemización progresiva, por lo que se decide priorizar la intervención quirúrgica. Se llevó a cabo una exéresis de la masa mediante un abordaje mínimamente invasivo. Tanto la paciente como el recién nacido, tuvieron una evolución favorable. La paciente fue dada de alta tres días después de la cirugía(AU)


The incidence of adnexal masses diagnosed in pregnancy varies by the different consulted series; however, over 6cm long masses represent exceptional cases in literature. The management of this type of patients is a challenge for the multidisciplinary team since the risk of ovarian cancer should be taken into consideration in this type of patients. This is a 34 years-old pregnant patient who was diagnosed with giant adnexal mass during the echographic control of the first trimester. Due to the lack of fetal distress and the radiological characteristics of the lesion, it was decided to keep surveillance to wait for the vaginal delivery and to perform an elective surgery to remove the lesion. After the childbirth, the patient suffered a process of anemia, so it was decided to prioritize surgery. The mass was excised through a minimally invasive approach. Both the mother and the newborn had favorable recovery and the patient was discharged from hospital three days after surgery(AU)

RESUMO

Objective To analyze the related factors of ovario - cystic rupture during laparoscopic surgery of benign ovary tumor and to investigate the prognosis. Methods 110 patients who received laparoscopic surgery for ovary tumor were retrospectively analyzed. Results A total of 68 benign ovarian cyst was successfully removed with- out intra-operative rupture , while 42 were intra-operative ruptured. Mean surgical time was significantly longer in rupture group. The incidence of ovarian cystic rupture was related to the removal of left-side tumor and cyst and pel- vic adhesions,whereas no correlation was observed with patients'ages,size of cyst, gravidity,and parity,volume of bleeding,post surgical menstruation alteration,abdominal pain,and cyst rupture. Conclusion Skill,experience in laparoscopic practices and the usage of laparoscopic bag help to avoid cystic rupture. When rupture occurs, timely cleanness with saline can decrease post-surgery complications and does not affect subsequent pregnancy.

RESUMO

O artigo apresenta uma revisão sobre o manejo de tumores malignos de células germinativas do ovário, com especial atenção para o tratamento cirúrgico e adjuvante. Adicionalmente, discute a repercussão desta conduta no futuro reprodutivo e endocrinológico das pacientes.


This article reviews ovary germ cells malignant tumors management with special attention to surgical and adjuvant treatments. In addition, the consequence of these procedures on patient's reproduction and endocrinology are discussed.

RESUMO

Objective To investigate the feasibility,reliability,and complications of laparoscopic second-look exploration for ovary epithelial cancer.Methods The operation was performed under general anaesthesia.The 4-port laparoscopy was carried out.Physiological saline was used to wash the abdominal and pelvic cavity for cytological examination.Then the exploration of abdominal and pelvic cavity was performed,adhesions were dissected,and multiple punch biopsies(at more than 20 sites) were conducted in the abdominal and pelvic cavity.Results All the operations were completed successfully.The operative time was 61.3?16.7 min,and the hemorrhage volume was 98.7?32.1 ml.Positive findings were encountered in 4 cases(28.6%),3 of which(21.4%) were identified by the naked eye and 1 of which was identified by microscope,and negative findings,10 cases(71.4%).Of the 4 cases of positive findings,the tumor was located at the pelvic peritoneum or the paracolic sulci peritoneum.No abdominal organ injuries occurred,and no conversion to open surgery was needed.Delayed healing of umbilical incision happened in 1 case.Of the 10 cases of negative findings,recurrence of tumor was observed in 1 case(10.0%) at 48 months after operation.Conclusions Laparoscopic second-look exploration for ovary epithelial cancer is feasible,with advantages of minimal invasion,fast recovery,and good patient's tolerance.

RESUMO

Objective To observe the anti-tumor action of Fuzheng Yuliu Tablet(FYT).Methods Mice received subcutaneous injection of S180 into right axilla.Then the tumor-implanted mice were randomized into 6 groups: model group,FYT groups(at the dose of 2.4 and 12.4g/kg respectively),cisplatin group(at the dose of 8?g/d),and cisplatin+FYT groups.The treatment lasted 10 days.The tumor-inhibitory rate and the life-prolongation rate were observed after treatment.The cultured ovarian cancer cell strains of SKOV3 were randomized into 4 groups:model group,two FYT groups and cisplatin group.Apoptosis of SKOV3 was detected by enzyme-linked immunosorbent assay(ELISA)after culturing with the medicine for 48 hours.Results The tumor growth was inhibited to different extends in all of the treatment groups,the decrease of tumor mass weight was obvious in cisplatin group and FYT groups,and the body weight was increased in FYT groups and cisplatin+FYT groups(P

RESUMO

The positive binding activity of lectin, peanut agglutinin (PNA), against the mucinous malignancies of the ovary was studied in order to clarify biologic differences among those lesions using immunoperoxidase method (ABC). A total of 23 cases were included in this study and they were classified as 10 cases of mucinous cystadenocarcinoma, 9 mucinous tumors of borderline malignancy and 4 pseudomyxoma peritonei, histologically. Nine of 10 cystadenocarcinomas and all cases of pseuomyxoma peritonei showed more than moderate degree of positive binding activity (>2+) with PNA in the neoplastic epithelial cells. In the cases of borderline malignancy, only 3 of 9 revealed as much similar binding pattern with PNA as cystadenocarcinoma group, in contrast, minimal degree of positivity (1+) was noted in the remainder. These findings may suggest heterogeneity in the biochemical characteristics among the cases of borderline lesion. And it is proposed that the higher PNA binding cases in ovarian mucinous borderline malignancy require extensive sampling by multiple sections and further careful follow-up study.
