RESUMO

Abstract Objective: To estimate the direct costs of treating excess body weight in children and adolescents attending a public children's hospital. Methods: This study analyzed the costs of the disease within the Brazilian Unified Health System (SUS) for 2,221 patients with excess body weight using a microcosting approach. The costs included operational expenses, consultations, and laboratory and imaging tests obtained from medical records for the period from 2009 to 2019. Healthcare expenses were obtained from the Table of Procedures, Medications, Orthoses/Prostheses, and Special Materials of SUS and from the hospital's finance department. Results: Medical consultations accounted for 50.6% (R$703,503.00) of the total cost (R $1,388,449.40) of treatment over the period investigated. The cost of treating excess body weight was 11.8 times higher for children aged 5-18 years compared to children aged 2-5 years over the same period. Additionally, the cost of treating obesity was approximately 4.0 and 6.3 times higher than the cost of treating overweight children aged 2-5 and 5-18 years, respectively. Conclusion: The average annual cost of treating excess body weight was R$138,845.00. Weight status and age influenced the cost of treating this disease, with higher costs being observed for individuals with obesity and children over 5 years of age. Additionally, the important deficit in reimbursement by SUS and the small number of other health professionals highlight the need for restructuring this treatment model to ensure its effectiveness, including a substantial increase in government investment.

RESUMO

Obesity is a global Non-Communicable Chronic Disease (NCD) associated with various comorbidities and a high mortality rate. This scenario has increasingly affected the female population, leading to a rise in prevalence and related health issues. Therefore, the present study aimed to assess the health-related quality of life of women with overweight or obesity and symptoms of COVID-19 using a multi-professional intervention model. This research was conducted as a parallel group and repeated measures pragmatic trial, in which 28 participants aged between 25 and 65 were allocated into two groups: experimental (intervention group) and control (non-intervention participants). The Body Mass Index (BMI) was (30.5 ± 5.45 kg/m²) in the Experimental Group, and the Control Group was (31 ± 8.2 kg/m2). The 12-Item Health Survey (SF-12) questionnaire was applied to assess the quality of life in the physical and mental domains of COVID-19 survivors with different symptom severities (mild, moderate, severe) compared to the control group. At the end of the program, 28 participants finished the study (15 from the experimental group and 13 from the control group). The results indicated a significant improvement in the mental health domain only in the experimental group after the intervention period (p 0.05). Considering these findings, multi-professional actions emerge as a crucial component for enhancing the quality of life, particularly within mental health, during the 16-week intervention period.

SUMMARY: This article has two aims: (a) first aim was to determine what is the most applicable and the simplest alternative for recommended BMI categories for underweight, overweight and obesity related to IOTF references, from the practical standpoint; (b) second aim was to determine the prevalence of the nutritional status in Montenegro on this representative sample of school children aged 9-13 years and compare them with peers from relevant and similar studies from both the local region and globally. A total sample of 1478 healthy children from Montenegro participated in this study divided into two sub-samples of 732 girls and 746 boys. According to the IOTF body mass index (BMI) reference values were used through ROC curve analysis to evaluate potential alternatives for estimation of the nutritional status of this sample of children. Only WHtR did not show significant age-related differences in the case of both genders. Considering the nutritional status of children from this study it has been found that boys have a considerably higher prevalence of being overweight (22.7 % vs. 16.4 %) and obese (7.5 % vs. 3.3 %) compared to girls. On the other hand, girls were more prevalent to be underweight (10.5 % vs. 7.5 %). WHtR seems like the best alternative for the estimation of obesity and being overweight due to simplicity and the equipment needed.

Obese individuals face greater complications in contracting SARS-CoV-2 due to their chronic proinflammatory state and reduced immune response, related to excess adipose tissue. Virus interaction with ACE2 receptors and renal ectopic lipid retention are key issues in this context. Objective. To analyze the specific features of obesity that increase susceptibility to severe COVID-19 symptoms, from articles published between 2020 and 2022, and to promote future research. Methodology. A systematic review of original articles between 2020 and 2022 was conducted using key terms and Boolean operators in databases such as PubMed, Scopus, ProQuest, etc. Non-original studies were excluded to obtain more specific research. Results. Of 180 articles found, 42 were selected. Among these, it was highlighted that obese patients, especially elderly men, presented severe complications. However, young people with severe obesity and people with low weight also showed a higher risk of mortality. Decreased lung function, low vitamin D levels, and altered ACE2 were implicated in the severity of infection. Obesity-associated hyperglycemia increased the risk of ICU admission and mechanical ventilation, while insulin resistance worsened prognosis. Conclusion. Obesity emerges as an important risk factor for severity and mortality due to COVID-19, pointing to the need for specific attention to this group of patients and further research in the area.

Based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement (PRISMA), a systematic review with metanalysis was conducted to identify and summarize the effects of school-based physical activity interventions that sought to control and / or reduce blood pressure (systolic and diastolic) in children and / or adolescents with overweight and / or obesity. In September 2022, potential studies were searched in five electronic databases (Pubmed, Scielo, Scopus, Sportdiscus, and Web of Science) and in reference lists. Randomized controlled trials conducted in schools with interventions involving physical activity and assessment of systolic and diastolic blood pressure in children and adolescents aged 6 to 19 years with overweight and / or obesity were con-sidered for synthesis. The risk of bias was assessed using an adapted version of the Effective Public Health Practice Project tool (EPHPP). Metanalysis was developed from the random model. Four studies were included. For systolic blood pressure, a summary effect of -0.10 (95% CI: -0.39; 0.19; I2 = 0%) was observed. For diastolic pressure, the metanalysis indicated -0.33 (95% CI: -0.62; -0.04; I2 = 11%). Considering the promising effects on diastolic blood pressure, we suggest the develop-ment of more school-based interventions based on physical activity practice for overweight and / or obese populations, which may also add environmental elements, longer duration, multicomponent approaches, and parent / guardian involvement to their strategies.

ObjectiveTo explore the influence of excessive weight gain during pregnancy in pre-pregnancy overweight and obese women on pregnancy outcomes and neonatal conditions, and to provide scientific evidence for formulating weight management strategies before and during pregnancy and prevent adverse pregnancy outcomes. MethodsClinical data of 2 172 parturients collected from a community in Huangpu District from 2017 to 2021 were retrospectively analyzed, and they were divided into pre-pregnancy overweight and obesity group (n=530), normal pre-pregnancy weight group(n=937), and underweight pre-pregnancy group(n=705) according to maternal precursor body mass index (BMI). Based on their weight gain during pregnancy, the parturient were divided into moderate gestational weight gain (MGWG) group and excessive gestational weight gain (EGWG) group. Meanwhile, the pregnancy and neonatal outcomes such as postpartum hemorrhage, puerperal infection, placental abruption, premature rupture of membranes, mode of delivery, premature birth, stillbirth, fetal distress, admission to the intensive care unit (ICU), macrosomia, and Apgar score, were recorded. Then the differences in pregnancy and neonatal outcomes between groups were compared. The effects of pre-pregnancy BMI and gestational weight gain on pregnancy outcomes and neonatal conditions was retrospectively analyzed. ResultsThe pre-pregnancy overweight and obese group had higher proportions of placental abruption, premature rupture of membranes, cesarean section, premature birth, fetal distress, and macrosomia compared to the normal pre-pregnancy weight group and the underweight pre-pregnancy group, with Apgar scores lower than the normal pre-pregnancy weight group and the underweight pre-pregnancy group (all P<0.05). The EGWG group had higher proportions of postpartum hemorrhage, placental abruption, premature rupture of membranes, cesarean section, premature birth, fetal distress, admission to the ICU, and macrosomia than the MGWG group (all P<0.05). In the pre-pregnancy overweight and obese group, the EGWG group had higher proportions of placental abruption, premature rupture of membranes, premature birth, fetal distress, admission to the ICU, and macrosomia than the MGWG group, with lower Apgar scores than the MGWG group (all P<0.05). In the normal pre-pregnancy weight group, the EGWG group had higher proportions of placental abruption, premature rupture of membranes, premature birth, fetal distress, admission to the ICU, and macrosomia than the MGWG group (all P<0.05). In the pre-pregnancy overweight and obese group, the EGWG group had higher proportions of premature rupture of membranes, cesarean section, premature birth, fetal distress, and macrosomia than the EGWG group in the normal pre-pregnancy weight group(all P<0.05). Logistic regression analysis showed that EGWG in pre-pregnancy overweight and obese women was a risk factor for placental abruption (OR=2.971, 95%CI: 1.098‒8.042), premature rupture of membranes (OR=4.662, 95%CI: 2.798‒7.770), cesarean delivery (OR=1.375,95%CI: 1.260‒2.541), premature birth (OR=4.249, 95%CI: 2.384‒7.573), fetal distress (OR=3.238, 95%CI: 1.589‒6.598), admission to the ICU (OR=3.010, 95%CI: 1.265‒7.164), and macrosomia (OR=5.437, 95%CI: 3.392‒8.716) (all P<0.05). ConclusionExcessive gestational weight gain in pre-pregnancy overweight and obese women is a risk factors for placental abruption, premature rupture of membranes, cesarean section, premature birth, fetal distress, admission to the ICU, and macrosomia.

RESUMO

Objective:To compare the resting energy expenditure(REE)characteristics among young men with different body mass indexes(BMI).Methods:Thirty young men[average age was(26.93± 4.16)years]were enrolled in this study. They underwent resting metabolism tests in the Department of Sports Medicine of Peking University Third Hospital from December 2017 to June 2021.The resting meta-bolic rate(RMR)was measured by indirect calorimetry, the body composition was measured by bioresis-tance antibody component analyzer. The REE characteristics were analyzed, and 11 predictive equations were used to estimate RMR and compared with the measured value. The differences were analyzed by paired t-test and intra-class correlation coefficient(ICC).Results:The RMR of the overall 30 young men was(1 960.17±463.11)kcal/d(1 kcal=4.186 8 kJ).Including(1 744.33±249.62)kcal/d in those with normal BMI,which was significantly lower than that in those who were overweight or obese[(2 104.06±520.32)kcal/d,P<0.01],but the weight-corrected RMR in those with normal BMI was significantly higher than that in those who were overweight or obese[(24.02±2.61)kcal/(kg·d)vs.(19.98±4.38)kcal/(kg·d),P<0.01].The RMR was significantly and positively correlated with body weight, adiposity, lean body mass, body surface area, and extracellular fluid in the subjects with diffe-rent BMI(all P<0.05).The predicted values of the 11 prediction equations were not in good agreement with the measured values(all ICC<0.75),with relatively high agreement between the pre-dicted and measured values of the World Health Organization(WHO)equation in overweight obese young men(ICC=0.547,P<0.01).Conclusion:There were significant differences in RMR among young men with different BMI,and the RMR after weight correction should be considered for those who were overweight or obese. The consistency between the predicted values of different prediction equations and the actual measured values of RMR was relatively poor, and it is recommended to accurately measure RMR by indirect calorimetry. For overweight or obese young men,the WHO prediction equation can be considered to calculate RMR,but it is necessary to establish an RMR prediction equation applicable to different BMI populations.

RESUMO

Objective To study the traditional Chinese medicine(TCM)constitution characteristics of overweight/obese patients in Shanghai region and to investigate the correlation of TCM constitution with body composition. Methods Relevant data were collected from the patients with complete information of TCM constitution and human body composition analysis who visited the specialized outpatient clinic of acup-moxibustion catgut embedding therapy in the Department of Endocrinology,Shanghai Tenth People's Hospital from August 2020 to December 2022.The patients were divided into a normal body mass group(BMI<24 kg/m2),an overweight group(24 kg/m2≤BMI<28 kg/m2)and obesity group(BMI≥28 kg/m2),and then the distribution of TCM constitution types in the three groups of patients were analyzed. After that, the correlation between TCM constitution and body composition were explored with multiple regression analysis.Results(1)A total of 315 patients were included,of which 43 patients had normal body mass,85 patients were overweight and 187 patients were obese.(2)The TCM constitution types in descending order of the composition ratio in the normal body mass group and in the overweight group were spleen deficiency constitution, liver stagnation constitution,damp-heat constitution, yang deficiency constitution, and yin deficiency constitution,in the obese group were spleen deficiency constitution, yang deficiency constitution,damp-heat constitution, liver stagnation constitution, and yin deficiency constitution, and in the overweight/obese group were spleen deficiency constitution,damp-heat constitution, yang deficiency constitution, liver stagnation constitution, and yin deficiency constitution. No statistically significant differences of the distribution of TCM constitution types were shown between normal body bass population and overweight/obese population(P>0.05).In both normal body mass population and overweight/obese population, the single body constitution type was common, and biased constitution was rare, and there was no statistically significant difference when comparing between the two groups(P>0.05).(3)The results of multiple regression analysis showed that the basal metabolism of all patients was positively correlated with yang deficiency constitution and was negatively correlated with damp-heat constitution, and the differences were statistically significant(P<0.01).It is indicated that if the score of yang deficiency constitution rose by one point, the basal metabolism would increase by 0.54 kcal, and if the score of damp-heat constitution decreased by one point, the basal metabolism would decrease by 1.005 kcal. Conclusion In Shanghai region, obesity may be the main indication of the variation of the body constitution. In addition to spleen deficiency constitution, the proportions of yang deficiency constitution,damp-heat constitution and liver stagnation constitution are also higher in obese patients. In terms of the correlation between TCM constitution and body composition, basal metabolism is positively correlated with yang deficiency constitution and is negatively correlated with damp-heat constitution. Therefore, for the patients with yang deficiency constitution and damp-heat constitution, the influence of the basal metabolism level on the development of the disease should be taken into account.

RESUMO

BACKGROUND:Resistance exercise increases muscle mass and improves muscle strength, but there is a lack of conclusive evidence on its effects on body composition.Controversial results from randomized controlled trials and multiple Meta-analyses pose a problem for clinical decisions on exercise interventions. OBJECTIVE:Based on the methodological paradigm of umbrella review,to explore the efficacy and safety of resistance exercise in improving the body composition of overweight and obese people. METHODS:We searched PubMed,Cochrane Library,Web of Science, and CNKI for systematic reviews on the effect of resistance exercise in overweight and obese people published from database inception to August 31,2022.Chinese search terms included"anti-resistance,resistance,self-weight,Meta,Meta-analysis,body composition, body fat percentage, fat mass, lean body mass, body mass."English search terms included"resistance training, strength training,weight-lifting strengthening program, body composition, body fat distribution,Meta-analysis,data pooling, overviews, clinical trial, clinical trial overviews."PRISMA was used to evaluate the methodological quality of the included studies, and a GRADE evaluation system was used to grade the quality of evidence. RESULTS AND CONCLUSION:A total of 14 Meta-analyses were included, and GRADE evidence quality evaluation showed that 2 outcome indicators were intermediate,12 were low,and 14 were extremely low.Compared with the control group,23 of the 32 outcome measures showed significant improvement. The intervention effect of resistance exercise on body fat percentage has reached a consensus, but the intervention effects on lean body mass, body mass index and visceral fat still need further confirmation.Resistance exercise is an effective and safe method to improve the body composition of overweight and obese people. The overall quality of the research and the evidence quality of the outcome indicators included in the systematic evaluation are generally low.In the future, the initial state, exercise goal and willingness of the subjects should be fully combined on the basis of clarifying the sample characteristics and refining the scheme design.Through multiple, scientific and personalized exercise prescription designs of water, air and elastic resistance, the safety and compliance of resistance exercise can be improved, providing clear evidence-based support and decision-making basis for resistance exercise intervention.
