RESUMO

En las meningoencefalitis de etiología viral se encuentran implicados los paramixovirus (virus de la parotiditis, sarampión y rubeola). Las vacunas combinadas trivalentes han sido utilizadas durante años en muchos países. El objetivo del trabajo es determinar la respuesta poliespecífica de anticuerpos de tipo IgG anti parotiditis, rubeola y sarampión en pacientes pediátricos con meningoencefalitis víricas agudas. Se realizó un estudio retrospectivo en el año 2018 en muestras de pacientes pediátricos con meningoencefalitis viral aguda vacunados con la triple viral, donde se utilizó el índice de anticuerpos específicos anti-parotiditis, anti-rubeola y anti-sarampión para identificar el estado de respuesta inmunológica contra dichos virus en la muestra estudiada, procedentes de la serorraquioteca de LABCEL. Las determinaciones se hicieron por ELISA. Todos los pacientes presentaron respuesta poliespecífica intratecal, disminución significativa del índice de anticuerpos IgG anti-parotiditis con respecto a la edad y al tiempo de respuesta. Se evidencia un acortamiento del tiempo de respuesta de los anticuerpos de tipo IgG específicos anti-parotiditis en relación a la edad de vacunación(AU)


Paramixovirus like mumps, rubella and measles are involved in some viral meningoencephalitis. Triple combined vaccines have been employed for several years in many countries. The aim of this work is to determine the IgG anti mumps, rubella and measles polyspecific response in pediatric patients with acute viral meningoencephalitis. A 2018 retrospective study in pediatric patients with acute viral meningoencephalitis previously vaccinated with the triple viral vaccine MMR was performed to identify the immune response status against mumps, rubella and measles in samples from LABCEL serum and cerebrospinal fluid collection. Quantification of IgG specific antibody was performed by ELISA. Intrathecal polyspecific response was present in all patients. A significant decrement of anti IgG mumps specific antibody index was observed according to age and response time. A shortage of the response time of IgG mumps specific antibodies according to the age of vaccination was demonstrated(AU)

RESUMO

Facial asymmetry is found in patients with or without cosmetic facial alterations. Some patients have facial asymmetry that manifests underlying skeletal problems, while others have only limited soft-tissue facial asymmetry. Orthognathic surgery brings about a dermatic change, as soft tissue covers underlying bones. Limited soft-tissue asymmetry, meanwhile, is difficult to correct. The treatment modalities for the creation or restoration of an esthetically pleasing appearance were autogenous fat grafts, cartilage graft, and silicon injections. A young female patient had right-side facial asymmetry. The clinical assessment involved visual inspection of the face and palpation to differentiate soft tissue and bone. Although the extra-oral examination found facial asymmetry with skin atrophy, the radiographic findings revealed no mandibular atrophy or deviation. She was diagnosed as localized scleroderma with muscle spasm. In conclusion, facial asymmetry patients with skeletal asymmetry can be esthetically satisfied by orthognathic surgery; however, facial atrophy patients with skin or subdermal tissue contraction need treatment by cosmetic dermatological surgery and orthodontic correction.

RESUMO

To evaluate the clinical value of a new self assessment method of pain, so called pain rating score(PRS), we performed the visual analogue scale, verbal rating scale, McGill pain questionnaire, and PRS on 60 patients with complained neuro-musculo-skeletal pain. The results showed that the reliability of PRS was very high. The PRS was a simple, easy, and comprehensive method to assess the pain for its intensity, frequency, duration, and aggravating factors. The PRS was very sensitive for the change of pain. Based on the above results we have concluded that, the new self assessment method of pain, pain rating score(PRS), is a valuable and useful clinical method which is simple, easy to perform and comprehensive.

Skilled and experienced anestheia is of great importance for patients undergoing orthotopic liver transplantation, because of multiple preexisting medical problems in such patients as well as the intraoperative problems of rapid hemodynamic, metabolic, and coagulation changes. In this study, the intraoperative hemodynamic and laboratory data were analyzed in ten dogs that underwent an orthotopic liver transplantation procedure by veno-venous bypass using Biopump. Liver transplantation can be divided into three distinct periods: stage I, or preanhepatic stage, which begins with the induction of anesthesia and continues until cross clamping of portal vein and IVC; stage II, or anhepatic stage, which begins at the anhepatic time and continues until the donor liver is reperfused by the recipients circulating blood; and stage III, or postanhepatic stage, which begins at the time of reperfusion and continues until the end of surgical procedure. The hemodynamic changes at the time of IVC and portal vein cross clamping were decreases in CVP, PCWP, and pulmonary artery pressure in spite of using Biopump. The significant metabolic alternations during anhepatic stage were decrease in blood glucose levels and increase in blood lactate levels. The more significant hemodynamic changes occurred at the time of reperfusion. Systolic pressure decreased suddenly to 58+/-6 mmHg and cardiac output decreased to 1.08+/-0.1l L/min. However heart rate, pulmonary artery pressure, CVP, and PCWP did not change significantly. During stage III, hyperglycemia occurred quite frequently. Significant abnormal coagulation chages could not be found, probably because the dogs were healthy. In conclusion, during anhepatic stage, we have to compensate for alternations of fluid balance. At the time of reperfusion, we should prevent severe hemodynamic changes and treat them immediately if they occur. However, it seems that glucose administration is not necessary to the liver recipient during stage II because there is no significant hemodynamic depression due to hypoglycemia at this time and hyperglycemia occurs later.
