RESUMO

El síndrome de atrapamiento del nervio supraescapular es una causa infrecuente de dolor en el hombro y de difícil diagnóstico clínico. Puede ser consecuencia de traumatismo, infección, compresión extrínseca o estiramiento secundario a movimientos repetitivos. Los quistes supraglenoideos deben considerarse en el diagnóstico diferencial pues son una causa frecuente de compresión del nervio supraescapular que ocurre casi exclusivamente en hombres. Se evaluaron con resonancia magnética (RM) y en forma retrospectiva siete pacientes con diagnóstico de atrapamiento del nervio supraescapular por quiste supraglenoideo. La RM mostró la causa y localización de la compresión nerviosa así como la distribución del edema muscular en todos los casos. Puede evaluar el grado de severidad de la enfermedad y realizar el diagnóstico diferencial con otras causas que provocarían atrapamiento del nervio supraescapular. (AU)


Suprascapular nerve entrapment has been regarded as a relatively uncommon condition that is easily overlooked in the differential diagnosis of shoulder discomfort. The common feature is nerve damage, caused either by trauma, extrinsic compression or by stretching secondary to repeated movements. Ganglion cysts account for the majority of the masses causing entrapment and seem to occur exclusively in men. Seven cases of suprascapular nerve entrapment caused by supraglenoid cyst compression were reviewed with MRI. MRI showed the cause and location of nerve compression as well as the distribution of muscular edema in all cases. MR imaging also allows an evaluation of the severity of the disease. It is also very efficient in excluding several differential diagnoses. (AU)

Although, arthroscopic cyst and labral debridement is generally accepted as the effective treatment, there is limited literature available regarding treatment options for paralabral cysts in the hip. We present a case of paralabral cyst compressing left common femoral vein in the hip that was treated with sono-guided cyst aspiration followed by arthroscopic labral debridement.

Paralabral cysts of the shoulder are rare, and there are few reports available that describe anteroinferior paralabral cysts arising from a detached antero-inferior glenoid labral tear without shoulder instability. We report an antero-inferior labral tear without shoulder instability in adolescent overhead athlete associated with paralabral cyst that leads to axillary nerve neurapraxia. Although nonoperative management of such labral lesions may provide symptoms of relief, it may not be enough for the athlete to return to the game. However, surgical treatment in this case provides successful recovery and rapid return to playing baseball play without having to worry about the progression of muscle denervation.

RESUMO

PURPOSE: To evaluate the correlation between MRI findings and hip joint lesions evaluated by arthroscopy in patients affected by paralabral cyst. MATERIALS AND METHODS: Of patients treated by hip arthroscopy from Jan 1996 to Mar 2010, 17 cases symptomatic of paralabral cyst as observed by preoperative MRI were analyzed. The presence of an acetabular labrum tear, and the location and size of the cyst were evaluated using both preoperative MRI and intraoperative arthroscopy. Visual analogue scale (VAS), modified Harris hip scale, and Hip outcome score results were compared as determined by the two assessment methods. RESULTS: According to the MRI findings, the locations of the cysts included 5 cases in the anterosuperior, 2 anterior, 4 anteroinferior, 2 posterosuperior and 4 posteroinferior. The mean size of all cysts was 25.1x12.5x13.8 mm3. Postoperatively, the mean VAS score for all patients improved from 7.7 to 1.5, their modified Harris hip score improved from 58.8 to 90.7, and their Hip outcome score improved from 54 to 93.5. CONCLUSION: Painful paralabral cyst was mainly associated with an acetabular labral tear and we concluded that observation of paralabral cyst upon preoperative MRI provided indirect evidence of proximal acetabular labral pathology.
