Objectives: This study was aimed to measure and evaluate the morphological changes of the mandible, related to age and gender in partially edentulous and completely dentate subjects using panoramic radiographs. Methods: This study included a sample of 150 partially edentulous patients and 150 completely dentate patients enrolled in the College of Dentistry, King Saud Bin Abdulaziz University for Health Sciences between January 2015 and December 2018. Five mandibular morphological and anatomical parameters were measured using a Romexis software including; Condylar height. Gonial Angle, Ramus height, Antegonial notch depth and Ramal notch depth. Results: There was a significant difference (p=0.00) seen in the mean values of GA when comparing the partially edentulous (130.01 ± 7.79°) and completely dentate group (126.95 ± 6.64°). The mean values of GA were significantly larger among the younger age group (20-40) in both partially edentulous patients (131.58 ± 8.27°) and completely dentate group (127.17 ± 6.9°) (p<0.05). Conclusion: There was no relationship between the changes in the morphological measurements related to age and gender except for GA where younger subjects have higher GA values. Also, the mean values of GA were significantly higher in the partially edentulous group compared to completely dentate.

Introducción: El edentulismo parcial es una condición final a la presencia de caries dental relacionado con diversos factores sociales, demográficos y culturales. Actualmente, existen escasos estudios que valoren el estado de edentulismo parcial y su asociación con el nivel socioeconómico-cultural. Objetivo: Determinar la frecuencia del tipo de edentulismo parcial bimaxilar según la Clase de Kennedy y su posible asociación con el nivel socioeconómico-cultural en los pacientes de la clínica de la Facultad de Odontología de la Universidad Nacional Mayor de San Marcos del año 2016. Material y Métodos: Estudio descriptivo, observacional y transversal. La muestra estuvo conformada por 100 pacientes adultos edéntulos parciales seleccionados aleatoriamente que fueron evaluados mediante una encuesta y un examen clínico oral. La encuesta estuvo conformada por los datos sobre el nivel socioeconómico-cultural. El examen clínico se evaluó con un odontograma para determinar el tipo de edentulismo según la clasificación de Kennedy. Resultados: El tipo de edentulismo parcial más frecuente fue la Clase III de Kennedy en la arcada maxilar (50 por ciento) y en la mandibular (49 por ciento), los portadores pertenecían en gran medida al nivel socioeconómico-cultural medio (83 por ciento). No se evidenció una asociación estadísticamente significativa entre el nivel socioeconómico-cultural y el tipo de edentulismo parcial según la Clase de Kennedy en ambas arcadas dentarias. Conclusiones: En la población estudiada existe una mayor frecuencia de edentulismo parcial bimaxilar Clase III de Kennedy y una preponderancia del nivel socioeconómico-cultural medio. Sin embargo, no existe asociación entre el tipo de edentulismo parcial según la Clase de Kennedy y el nivel socioeconómico-cultural(AU)


Introduction: Partial edentulism is a final condition to the presence of dental caries related to various social, demographic and cultural factors. Currently, there are few studies that assess the state of partial edentulism and its association with the socioeconomic cultural level. Objective: To determine the frequency of the type of bimaxillary partial edentulism according to Kennedy´s Class and its possible association with the socioeconomic cultural level in patients treated in the clinic of the Faculty of Dentistry of the National University of San Marcos in 2016. Material and methods: A descriptive, observational and cross-sectional study was conducted. The sample consisted of 100 randomly selected partial edentulous adult patients who were evaluated by means of a survey and an oral clinical examination. The survey was made up of the data that relate socioeconomic cultural level. The clinical examination was evaluated with an odontogram to determine the type of edentulism according to Kennedy´s classification. Results: The most frequent type of partial edentulism was Kennedy's Class III in the maxillary arch (50 percent) and in the mandible (49 percent), which was mainly present in patients that belonged to the average socioeconomic cultural level (83 percent). There was no statistically significant association between the socioeconomic cultural level and the type of partial edentulism according to Kennedy´s Class in both dental arches. Conclusions: In the studied population, there is a greater frequency of partial bimaxilar edentulism according to Kennedy's Class III and a preponderance of the socioeconomic cultural level. However, there is no association between the type of partial edentulism according to Kennedy´s Class and the socioeconomic cultural level(AU)

RESUMO

The collapse of the posterior occlusion destroys the normal occlusal plane and causes excessive wear reducing the vertical dimension. Reduced vertical dimension of occlusion causes not only aesthetic and functional problems but also overloading on the temporomandibular joints and abnormalities of muscle nerve system. In order to improve the collapsed occlusal relationship, it is necessary to consider the change of the vertical dimension. It is necessary to make a precise diagnosis and analysis before the treatment and to evaluate the adaption of patient to the new vertical dimension of occlusion. A patient with excessive overbite often has occlusal problems of tooth wear and tooth eruption. Considering these considerations, overall prosthodontic restoration is required to solve the problem. A patient of 68 year old man in this case who suffered major tooth wear and maxillary posterior teeth loss was treated with elevation of vertical dimension of occlusion by maxillary removable dental prosthesis and mandibular fixed prosthesis.

Objective@# To provide a clinical reference by evaluating the restorative results of an accurate impression technique for distal-extension removable partial dentures.@*Methods@#Forty-two patients with free-end dentition defects who visited the Prosthodontic Department of Nanjing Stomatological Hospital, Medical School of Nanjing University, between April 2017 and January 2018 were selected. The final impression was made by an accurate impression technique to fabricate fifty-two removable partial dentures with casting frameworks. The restorative result was judged by clinical examination and patient feedback immediately after wearing dentures and at 1 week, 3 months and 6 months.@*Results@#All fifty-two dentures for forty-two patients were seated successfully, with good retention and stability. Complications occurred throughout the 6-month follow-up, including 4 cases of mucous pain, 4 cases of retention force attenuation and 1 case of abutment fracture. There were no periodontics problems. The dentures garnered high patient satisfaction.@*Conclusion@#The accurate impression technique for distal-extension removable partial dentures showed good clinical results in this follow-up investigation.

RESUMO

In removable partial denture with crossed occlusion, it is difficult to meet the satisfaction of patient due to variable movement of denture and unfavorable stability under loading. In a case with few unilaterally teeth remaining, additional treatments such as implantation or residual root are required to assure bilaterally stable support. However, due to the medical condition of the patient with hypertension, angina and old age etc., removable partial denture was planned as treatment in this case. Proper diagnosis, accurate analysis and full understanding of the adaptation for neuromuscular system are necessary to recover the vertical dimension of patient from a decreased vertical dimension due to crossed occlusion.

RESUMO

In removable dental prostheses, it is important to minimize impairment of residual tissue caused by wearing dentures. There are two factors that harm residual tissue. The first is functional load bearing of remaining teeth and alveolar ridges and the second is the effect of poor oral hygiene. Double crown retained removable dental prostheses provide rigid support, and it may reduce impairment caused by load bearing of alveolar ridges. Also, dental plaque and oral deposits, which are attached to outer crowns and dentures, can be easily managed extra-orally. In addition, it is beneficial to the health of the marginal gingiva because inner crowns have easy access for oral hygiene. In this case, double crown retained removable dental prostheses were used for the partially edentulous patient with severe residual alveolar bone resorption and poor oral hygiene, and the result was clinically satisfactory in terms of functional, esthetical, and oral hygiene aspects.

Se realizó un estudio descriptivo y transversal de 374 desdentados parciales con disfunción mandibular, quienes acudieron al servicio de prótesis Estomatológica de la Clínica Estomatológica Provincial Docente "Mártires del Moncada" de Santiago de Cuba desde octubre de 2013 hasta igual mes de 2014, para determinar la presencia de ruidos articulares en su articulación temporomandibular. Entre los principales resultados prevalecieron el sexo femenino, el grupo etario de 20-29 años y como manifestaciones clínicas de disfunción articular: el sonido articular en forma de chasquido al inicio y final de la apertura bucal (51,6 y 38,7 %, respectivamente), así como la desviación mandibular (42,4 %)...


A descriptive and cross-sectional study of 374 partially edentulous patients with mandibular dysfunction who assisted the Stomatological Prosthesis Service from "Mártires del Moncada" Teaching Provincial Stomatological Clinic in Santiago de Cuba was carried out from October, 2013 to the same month of 2014, to determine the presence of articular sounds in their temporomandibular joints. Objective To investigate the impact of the modified cast impression technique on distal-extension alve-olar ridge morphology of partially edentulous casts and the restoration outcome of removable partial dentures (PRDs). Meth-ods Fifty-four patients with mandibular free-end dentition defect were selected. Impressions were taken from each patient using both modified cast technique and conventional technique. Coronary cross-sections of the casts were scanned from dis-tal to mesial extension;the images were compared to reveal morphological differences. And the relationship between these differences and location of the cross-sections were analyzed. After the patients were evenly divided into two groups per paired design, the impressions were taken using the respective modified cast and conventional impression techniques to pre-pare RPDs. The subsequent comparisons between the two groups included frequency of adjustments during the adaptive phase, masticatory efficiency of the denture at ending of the adaptive phase and visible movement of the extension base dur-ing mastication. SPSS 16.0 software was used for statistical analysis. Results The number of pixels in the same cross-sec-tion was less in the impression taken with modified cast technique than that with conventional technique. And their differ-ence is of statistical significance (P < 0.01). The difference in the number of pixels (D) was negatively correlated with the number of cutting and grinding, and N (location of the cross-sections). In addition, there were statistically significant differ-ences in frequency of adjustments during the adaptive phase (times:1.12 ± 0.77 vs 3.41 ± 0.82), masticatory efficiency of the denture (0 vs 18.5%) and visible movement of the extension base during mastication (96.3%vs 74.1%) between the modified cast group and conventional group. Conclusion Morphology of distal-extension alveolar ridge of the casts differs signifi-cantly between the impressions obtained with modified cast technique and the impressions obtained with the conventional technique;the difference increases with greater distance to the distal surface of the terminal abutments. The restoration out-come of RPDs is significantly better with the modified cast impression technique than with the conventional impression tech-nique.

STATEMENT OF PROBLEM: Many dental clinicians are concerned about immediate loading of inserted implants. However, there have been few clinical studies surveying the success rates of immediate loading, based on Korean implant systems. PURPOSE: The aim of this study was to evaluate the outcome of immediate functional loading of the implant (SinusQuick(TM) EB, Neobiotech Co., Seoul, Korea) in partially edentulous maxilla or mandible. MATERIAL AND METHODS: Total 15 implants were placed. Within 2 weeks after implant insertion, provisional implant-supported fixed partial dentures were delivered to the patients. Quantitatively, marginal bone loss was measured at the time of immediate loading, after 3-months of continued loading and at the last follow-up. The mean follow-up period was 4.8 months. RESULTS: Mean marginal bone loss from implant surgery to early loading, 3-months follow-up and last follow-up was 0.03 +/- 0.07 mm, 0.16 +/- 0.17 mm and 0.29 +/- 0.19 mm. No implant failed up to 6 months after insertion, resulting in a 100% survival rate. CONCLUSION: Immediate loading exhibited high success rate in partial edentulism for up to 6 months. Well-controlled long term clinical studies with large sample size are necessary to confirm this finding.
