RESUMO

Introduction: An endodontic-periodontic lesion becomes a challenge for a clinician to diagnose and manage. when it comes to combined endodontic-periodontic lesion the most common treatment alternative is extraction and replacement with prosthesis. With numerous complications associated with prosthesis as well as patient's desire to retain natural teeth make it most important task for clinicians to conserve such teeth. Method: Four patients were included in this case series. Cases were treated with endodontic and periodontic treatment sequentially as per the guidelines. Results: After a follow-up period ranging from 6 months to 2 years, it was observed that all teeth remain asymptomatic and in normal function. Conclusions: This case series shows that it is possible to change the prognosis of teeth affected by combined endodontic-periodontal lesions, provides that correct diagnosis and proper oral hygiene maintenance is implemented

RESUMO

Objective:To explore a new method of whole-process digital esthetic prosthodontic rehabilitation combined with periodontic surgery for complicated anterior teeth esthetic defects accompanied by soft tissue morphology,to provide an alternative choice for solving this problem under the guidance of three-dimensional (3 D) printing digital dental model and surgical guide,thus completing periodontic surgery and digital esthetic rehabilitation of anterior teeth.Methods:In this study,12 patients with complicated esthetic problems accompanied by soft tissue morphology in their anterior teeth were included.The dentition and facial images were obtained by intra-oral scanning and three-dimensional (3D) facial scanning and then calibrated.Two esthetic designs and prosthodontic outcome predictions were created by computer aided design/computer aided manufacturing (CAD/CAM) software combined with digital photography,including consideration of white esthetics and comprehensive consideration of pink-white esthetics.The predictive design of prostheses and the facial appearances of the two designs were evaluated by the patients.If the patients chose the design of comprehensive consideration of pink-white esthetics,they would choose whether they would receive periodontic surgery before esthetic rehabilitation.The dentition design cast of those who chose periodontic surgery would be 3D printed for the guide of periodontic surgery accordingly.Results:In light of the two digital designs based on intra-oral scanning,facing scanning and digital photography,the satisfaction rate of the patients was significantly higher for the comprehensive consideration of pink-white esthetic design (P < 0.05) and more patients tended to choose priodontic surgery before esthetic rehabilitation.The 3 D printed digital dental model and surgical guide provided significant instructions for periodontic surgery,and achieved success transfer from digital design to clinical application.The prostheses were fabricated by CAD/CAM,thus realizing the whole-process digital esthetic rehabilitation.Conclusion:The new method for esthetic rehabilitation of complicated anterior teeth esthetic defects accompanied by soft tissue morphology,including patient-involved digital esthetic analysis,design,esthetic outcome prediction,3D printing surgical guide for periodontic surgery and digital fabrication is a practical technology.This method is useful for improvement of clinical communication efficiency between doctor-patient,doctor-technician and doctors from different departments,and is conducive to multidisciplinary treatment of this complicated anterior teeth esthetic problem.

RESUMO

Objective To observe the clinical curative effect of the joint application of Vitapex paste and AH plus,which cured the combined periodontic -endodontic lesion .Methods 72 patients of 113 teeth with the com-bined periodontic-endodontic lesion were chosen ,which were randomly divided into the two groups ,with all complete periodontal initial therapy and routine root canal preparation and dressing at the same time .The observation group included 36 cases of 58 teeth,when the first time to see a doctor ,injected Vitapex paste to the periapical tissue and periodontal tissue sparse area for 1 -2weeks.The sceond time,the tap root canal was closely filled withc AH plus paste and gutta-percha.The control group concludes 36 cases of 55 teeth,with the traditional method,the tap root canal was filled with AH plus paste and gutta -percha directly .Then observation of postoperative emergency response within 1 week,of periodontal clinical indicators (including the PD,BOP,PLI) in 1 month,3 months,6 months after periodontal initial therapy ,and of the long-term curative effect after 1 year.Results 1 week after root canal filling , there were 48 cases no reaction , 9 cases mild reaction and 1 case severe reaction in the observation group , while 47cases no reaction ,7 cases mild reaction and 1 case severe reaction in the control group .There was no statistical difference of evaluation of emergency response between the two groups (χ2 =0.002,P>0.05).Compared with the situations before treatment ,PD,BOP and PLI were all reduced significantly in 1,3 and 6 months after periodontal ther-apy (P0.05),while 1 month (t=2.581,5.984,1.870,all P<0.05),3 months (t=4.806,7.640,1.034,all P<0.05) and 6 months (t=4.612,8.043,1.167,all P<0.05) later,there was statistical difference of decreased value between the two groups .The 1 year follow up showed that significant efficiency in the observation group was 46.55%,and total efficiency was 91.38%,while significant efficiency 27.27% and total effi-ciency 70.91%in the control group ,and there were statistical differences of significant efficiency and total efficiency between the two groups (χ2 =16.761,21.061,all P<0.05).Conclusion With the joint application of Vitapex paste and AH plus ,and two method curing the combined periodontic -endodontic lesion ,there was no evident differ-ence compared with the traditional short -term emergency response .By long-term observation ,found that this method could effectively promote the healing of periapical tissue and periodontal tissue ,and that curative effect is remarkable compared with the traditional method .

Antecedentes: En las proteínas no se logra siempre su cristalización, de buen tamaño y de buena calidad para someterla a difracción de rayos X. De tal manera que se abre un campo para el desarrollo de estudios teóricos moleculares y proteínicos, que permiten la representación de las moléculas en tres dimensiones, proporcionando una información espacial para estudiar la interacción entre ligandos y receptores macromoleculares. Materialesy Métodos: Estudio In silico, a partir del análisis de secuencias primarias de seis diferentes proteínas LuxS cristalizadas de diversas bacterias, se seleccionó la proteína 1J6X del Helicobacter pylori, por su similaridad con la secuencia de la proteína LuxS en Porphyromonas gingivalis (P. gingivalis) cepa W83, para producir un modelo por homología de esta proteína, utilizando los programas Sybyl y MOE. Se realizó un acoplamiento con el ligando natural para evaluar la reproducibilidad del modelo en un ambiente biológico. Resultados: Se desarrolló el modelado de la proteína LuxS de P. gingivalis cepa W83, que permite el acercamiento a una estructura que se propone, por la interacción entre la proteína y su ligando natural. El modelo generado con recursos computacionales logró una correcta estructura molecular que aceptó la realización de diversos cálculos. El acoplamiento demostró una cavidad donde se logran diversas posiciones del ligando con buenos resultados. Conclusiones: Se obtuvo un modelo 3D para la proteína LuxS en la P. gingivalis cepa W83 validado por diferentes métodos computacionales con una adecuada reproducibilidad biológica por medio del acoplamiento molecular.


Background: Crystallization is not always achieved for all proteins in a good size and a good quality for X-ray diffraction. So that condition opens a field for the development of theoretical molecular and protein studies allowing the representation of the molecules in 3D, providing spatial information to study the interaction between ligands and macromolecular receptors. Materials and Methods: In silico study from primary sequence analysis of six different proteins LuxS crystallized of several bacteria. 1J6X protein of Helicobacter pylori was selected for its similarity with the LuxS protein sequence in Porphyromonas gingivalis (P. gingivalis) strain W83 to produce a homology model of this protein, using the Sybyl and MOE software. A docking was performed to assess the reproducibility of the model in a biological environment. Results: The LuxS protein modelling of P. gingivalis strain W83 was developed, which allows the approach to a proposed structure for the interaction between the protein and its natural ligand. The model generated with computational resources achieved the correct position and biological behavior by means of developed calculations. The docking showed a cavity in which the ligand adopted several positions with good results. Conclusions: A LuxS protein model was obtained, validated by different methods. This generated a 3D model for LuxS protein in P. gingivalis strain W83 with biological reproducibility by means of molecular docking.

RESUMO

This report describes clinical cases of a palato-gingival groove on a maxillary lateral incisor with associated localized periodontal disease and pulp necrosis. The tooth of the first case was extracted because of severe bone destruction. The palato-gingival groove of the second case was eliminated using a round bur, and the resulting defect was filled with synthetic graft and covered by an absorbable membrane. Both diagnosis and treatment of palato-gingival groove were very difficult and usually extraction of the involved tooth is the treatment of choice, but combined endodontic-periodontic treatment allowed the tooth to be saved.
