SUMMARY: The present study aimed to evaluate wrist (lunate) anatomy in terms of the incidence of lunatum morphology on plain-radiographs among the Anatolian (Turkey) population, accompanied by demographic analysis. We obtained all the patients' data regarding demographical features, diagnosis, and posteroanterior (PA) X-ray imaging. Two radiograph-reviewers repeated the analysis twice, one month later, blinded to their findings before the previous review. The lunatum structure was determined as Type-1 (n:293) and Type-2 (n:207) for each radiograph. Most of the 500 wrists' radiographs [n:293 (58.6 %)] were type-I lunate. The mean age was 36.7±13.3 (range:18-90) years. Sex distribution was as follows: 185 (63.1 %) males to 108 (36.9 %) females. Type-2 lunate was seen in 207 participants (41.4 %). The mean age for type-2 was 41.6±15.2 (18-88) years. 142 (68.6 %) participants were male sex, while 65 (31.4 %) were females. The mean age of subjects with type-I showed a difference with type-II (p=0.007). There was no relationship in terms of sex (p=0.206) between the groups. In the Anatolian region, type-1 lunate was dominant compared to type-2. The incidence rate of lunate type in Anatolian population was similar to the Arab population.


El presente estudio tuvo como objetivo evaluar la anatomía de la articulación radiocarpiana en términos de la incidencia de la morfología del hueso semilunar en radiografías simples entre la población de Anatolia (Turquía), acompañado de un análisis demográfico. Obtuvimos todos los datos de los pacientes con respecto a las características demográficas, el diagnóstico y las imágenes de rayos X posteroanteriores (PA). Dos revisores de radiografías repitieron el análisis dos veces, un mes después, sin conocer sus hallazgos antes de la revisión anterior. La estructura del lunatum se determinó como Tipo-1 (n:293) y Tipo-2 (n:207) para cada radiografía. La mayoría de las 500 radiografías de muñecas [n:293 (58,6 %)] fueron semilunar tipo I. La edad media fue de 36,7±13,3 (rango: 18-90) años. La distribución por sexos fue la siguiente: 185 (63,1 %) hombres y 108 (36,9 %) mujeres. El semilunar tipo 2 se observó en 207 participantes (41,4 %). La edad media para el tipo 2 fue de 41,6±15,2 (18-88) años. 142 (68,6 %) participantes eran del sexo masculino, mientras que 65 (31,4 %) eran del sexo femenino. La edad media de los sujetos con tipo I mostró una diferencia con el tipo II (p = 0,007). No hubo relación en cuanto al sexo (p=0,206) entre los grupos. En la región de Anatolia, el semilunar tipo 1 era dominante en comparación con el tipo 2. La tasa de incidencia del tipo semilunar en la población de Anatolia fue similar a la de la población árabe.

Background: This study was conducted to compare the plain radiography with the abdominal ultrasonography in the detection of pneumoperitoneum in suspected cases of hollow viscus perforation. Methods: A total number of 60 patients with suspected hollow viscus perforation were studied. All the patients had undergone plain radiography (Erect x-ray abdomen and left lateral decubitus views), ultrasonography and exploratory laparotomy. The investigational findings were compared with that of laparotomy findings. They were compared in terms of their sensitivity, specificity, predictive value of a positive and negative results and their percentage of false positive and false negative results. Results: Of the 60 patients, who underwent laparotomy, 57 had hollow viscus perforation. Out of 3 non-hollow viscus perforated cases 2 had appendicular perforation and 1 had mesenteric lymphadenitis. In the diagnosis, ultrasonography vs. radiography, their respective parameters were sensitivity (73.7% vs. 80.7%), specificity (66.7% each), predictive value of a positive test (97.7% vs. 97.9%), predictive value of a negative test (11.8% vs. 15.4%), percentage of false negative (26.3% vs. 19.3%) and percentage of false positive (33.3% each). Conclusion: In detection of pneumoperitoneum plain radiography appears to be more sensitive than ultrasonography with comparable specificity. Ultrasonographic finding of pneumoperitoneum is considered as an added finding.

PURPOSE: For decision making in the management of vertebral fractures such as operation or not, sagittal parameters like Cobb angle and wedge compression ratio are important. Plain radiography had been the only image tool for measuring such parameters until 3D computed tomography (CT) became popular recently. In this study, we investigated the measurement discrepancy between plain radiography and 3D CT. MATERIALS AND METHODS: Plain radiography and 3D CT of 45 thoracolumbar and lumbar fracture patients (male=21, female=24) were evaluated. We measured sagittal angle and vertebral height on lateral radiography and sagittal CT. Sagittal angle was measured between the upper body and lower body of fractured vertebrae. Anterior and posterior heights were measured to assess anteriorposterior (AP) wedge ratio. RESULTS: The sagittal angle of plain radiography (13.1+/-14.3degrees) was significantly larger than that of 3D CT (8.2+/-13.0degrees) by 4.9degrees (p<0.001). AP wedge ratio of plain radiography was on average 65+/-17%, which was significantly lower than the 3D CT (73+/-17%) by 8% (p<0.001). The severer the initial kyphotic deformity, the more discrepancy of results between the two methods was observed. CONCLUSION: Significant discrepancy was observed in sagittal features of fractured vertebra between plain radiography and 3D CT. Measured values of plain radiography showed more kyphotic features of the fractured body.

PURPOSE: We propose to improve the use of Proximal Femoral Nail Anti-rotation in Korea by reporting anatomical measurements of the normal Korean proximal femur. MATERIALS AND METHODS: A total of 230 patients were enrolled who had all experienced a femoral intertrochanteric fracture and undergone the Proximal Femoral Nail Anti-rotation surgical procedure between February 2007 and April 2011. We measured the neck-shaft angle and endosteal width at the isthmus of a normal femur, and the distance between the greater trochanter and the nail tip of the Proximal Femoral Nail Anti-rotation in post-operative plain radiography. We analyzed the average and standard deviations of the measurements. We also investigated correlations with the patient gender. RESULTS: The average neck-shaft angle and endosteal width at the isthmus were 129.7degrees(111.0~138.3degrees) and 14.5 mm (9.7~23.1 mm), respectively. The average protruded nail length was 4.9 mm (1.0~15.0 mm). The femur neck-shaft angle had a correlation ratio with gender (p=0.000). However, the endosteal width at the isthmus level and the protruded nail length had no correlation ratio with gender (p=0.108, 0.573, respectively). CONCLUSION: Until now, in intertrochanteric fracture operations using Proximal Femoral Nail Anti-rotation, the selection of devices has been extremely limited. Through this study we present the average Korean anatomical neck-shaft angle, endosteal width of the femur, and protruding length of the nail tip. We expect that these numerical values can be used in the production of new devices with shorter proximal nails, which would be more appropriates for Koreans.

RESUMO

Endoscopic retrieval of foreign bodies is sometimes a challenging task. We report a case of simultaneous removal of 5 coins from the stomach without causing mucosal injury using a specially-designed devices consisting of retrieval net, endoscopic attachment balloon and a disposable tip attachment. This technique has not been described before.

RESUMO

The fibromatosis is a rare timorous with local invasion, but is not metastasized distantly. This term should not be applied to nonspecific reactive fibrous proliferations that are part of an inflammatory process of are secondary to injury of hemorrhage and have no tendency toward growth or recurrence. It arises principally from the connective tissue of muscle and overlying fascia or aponeurosis (musculoaponeurotic fibromatosis), and chiefly affects the muscle of shoulder, pelvic girdle, and extremity. The term 'aggressive fibromatosis' is also employed to describe this disease, but it is impossible to predict the clinical course in the individual case. The fibromatosis arising in the mediastinum is very rare, and the report about it is nearly absent. The plain radiography shows merely mass with soft tissue density. The CT demonstrates a poorly defined homogenous or heterogeneous mass, isodense with skeletal muscle on precontrast-images, and slightly hyperdense to muscle on postcontrast-scan. Accurate delineation between the tumor & surrounding tissue is vague or frequently impossible. The authors experienced one case of the mediastinal fibromatosis recently and report the case with review of concerned literature.
