RESUMO

Las dermatosis plasmocitarias comprenden un conjunto de enfermedades inflamatorias de causa desconocida y evolución incierta. Se han descrito para ellas múltiples tratamientos con resultados dispares y éxitos terapéuticos anecdóticos, con recaídas posteriores en la mayoría de los casos. Presentamos en este trabajo, además de una actualización y revisión del tema, cuatro dermatosis plasmocitarias que tuvimos oportunidad de ver en nuestro Servicio: la conocida balanitis de Zoon, las poco descritas vulvitis de Zoon y queilitis plasmocitaria y la infrecuente plasmocitosis cutánea de la que hay muy pocos reportes a nivel mundial. Queremos con este trabajo integrar didácticamente las dermatosis benignas, producidas directamente por la infiltración de plasmocitos, sin causa subyacente demostrable.


Cutaneous plasmocitosys is a group of skin's inflammatories disorders with unknown cause and uncertain evolution. In all treatments cases they describe different and uneven results with a few therapeutic success, most of them with reopen symptoms. In the following work, besides an updates and reviews, we present four cases that we received in our Service. The known balanitis of Zoon, the almost unknown vulvitis of Zoon, plasma cell cheilitis and finally the very unfrequently cutaneous plasmocytosis, which is very worldwide unreported. A purpose of this work have a didactic will to include whole skin disorders directly caused for the plasma cells infiltration with no causes in background.

Plasma cell cheilitis, an idiopathic benign inflammatory disorder of the lip, is characterized by dense plasma cell infiltration in the upper dermis. It usually presents as circumscribed erosive patches or plaques on the lower lip of an elderly person. The cause of plasma cell cheilitis is unknown, and the treatment is often disappointing. Tacrolimus is one of the newer immunomodulators which inhibit T-cell activation and inflammatory cytokine release. It is approved for the treatment of atopic dermatitis, and several reports have found topical tacrolimus to also be effective and well tolerated in other skin disorders, including other types of eczema, papulosquamous disorders, vesiculobullous diseases, vitiligo and connective tissue diseases. Herein, we report 3 cases of plasma cell cheilitis treated by topical tacrolimus ointment.

Plasma cell cheilitis is a rare, idiopathic, benign inflammatory disease of lips, characterized by dense plasma cell infiltrates in the upper dermis. It presents as erythematous patches with erosion usually on the lower lip of elderly people. Although, various therapeutic options, including topical steroids, intralesional injection of glucocorticoids, oral nystatin and topical antibiotics have been tried, none of them is promising. A 68-year-old woman presented with a 4-month history of erosive patches with crusts on her lower lip, which was resistant to topical corticosteroids. A histopathologic examination revealed dense inflammatory cell infiltration mainly consisting of plasma cells in the dermis, consistent with plasma cell cheilitis. She was treated with topical pimecrolimus and after 2 weeks of application, the lesion started to get better. After 7 weeks it had almost disappeared. Herein, we present a case of plasma cell cheilitis treated with topical application of pimecrolimus.

RESUMO

Plasma cell cheilitis is a rare inflammatory disorder which shows a dense, band-like infiltrate of plasma cells in the upper dermis. Although the histological picture is shared with other diseases of the lips, plasma cell cheilitis is not associated with any known dermatoses. Some authors have shown the effectiveness of topical or intralesional corticosteroids; however, there have been many reports describing poor therapeutic response to topical steroids. We de-scribe two patients with plasma cell chielitis whose clinical condition responded rapidly to the intralesional injection of corticosteroids.

BACKGROUND: Plasma cell cheilitis is an idiopathic, benign, inflammatory mucosal condition characterized by a dense plasma cell infiltrate in the mucosa. OBJECTIVE: The aim of this study was to examine the clinical & histopathologic findings of plasma cell cheilitis and review the literatures. METHODS: A total of 8 patients confirmed as plasma cell cheilitis at the Catholic University Hospital from 1999 to 2001 were retrospectively analyzed with hospital charts and histopathologic slides to determine the clinical manifestations, associated diseases, etiology, course, and prognosis and histopathologic findings. RESULTS: The results are summarized as follows; 1. The male and female ratio was 3: 1. 2. The mean age of patients was 68.5 years. 3. The site of plasma cell cheilitis was the lower lip on all patients. 4. The lesion accompanied erosion or ulcer in 75% of patients and easily bleeding in 50% of patients. 5. There was no specific symptom in 50% of patients and pain accompanied in 37.5% of patients. 6. There were good responses on topically applicated clobetasone propionate. 7. Histopathologic features revealed band-like dense infiltration of plasma cells in the upper dermis and there are no atypical nucleus and mitosis. CONCLUSION: Characteristically, plasma cell cheilitis affects the low lip of an elderly person. The histological features are diagnostic. We suggest that plasma cell cheilitis exists much more than diagnosed and that physicians must pay attention to it.

Plasma cell cheilitis is an idiopathic, benign, inflammatory mucosal condition characterized histologically by a dense band-like infiltrate of plasma cells in the upper dermis of the mucosa. In addition to the lips, the disease can affect the penis, vulva, buccal mucosa, palate, gingiva, tongue, epiglottis, and larynx. Plasma cell cheilitis of the lip appears as a circumscribed, flat to slightly elevated, eroded area, usually on the lower lip of an elderly person. We report a 70-year-old man who had a well-demarcated, erythematous, erosive lesion on his lower lip for 1 year. A biopsy specimen showed an upper dermal infiltrate that consisted mainly of plasma cells. Oral griseofulvin, 500mg/day, was begun, and 8 weeks later the lesion showed marked improvement.

RESUMO

Plasma-cell cheilitis, a benign, inflammatory condition, is chacterized by polyclonal plasma cell infiltration in the dermis. It presents as circumscribed flat or elevated patches of erythema, usually on the lower lip in an elderly person. Plasma-cell cheilitis is the counterpart of Zoon's plasma-cell balanitis. A 63-year-old Korean man had an erythematous, erosive patch on his lower lip. By skin biopsy and immunoperoxidase staining to anti-kappa and lamda light chain, he was diagnosed as plasma cell cheilitis. He showed positive result in photopatch test to 6-methyl coumarine. His skin lesion improved after avoiding sun exposure, chewing gum and candy, and by intralesional injections of triamcinolone acetonide.
