RESUMO

<p><b>PURPOSE</b>To study the effects of surgical and nonoperative treatment on wrist function in patients with distal radius fracture.</p><p><b>METHODS</b>In total, 97 patients treated for distal radius fracture in the Department of Orthopedic Trauma at the People's Hospital of Peking University from Jan. 2010 to Jun. 2016 were selected for outpatient follow-up, including manipulative reduction and dorsal splint fixation in 24 cases, bivalve cast fixation in 19 cases and open reduction and internal fixation in 54 cases. Evaluation was based on Sartiento's modification of the Gartland and Werley score. Efficacy was assessed with wrist pain as the focus.</p><p><b>RESULTS</b>The wrist function scores of the surgical group were better than nonoperative groups. There was no significant difference in wrist function scores between the dorsal splint group and the bivalve cast group. The ulnar wrist pain incidence had no significant difference in surgical and nonoperative groups. The displace rate in dorsal splint group was higher than other groups.</p><p><b>CONCLUSION</b>The overall effect of surgical treatment of distal radius fracture is better than nonoperative treatment. The ulnar wrist pain incidence has no significant difference in these groups. Dorsal splint fixation is more prone to displace than bivalve cast fixation.</p>

Objective By discussing the Achilles tendon rupture line of two different methods of plaster fixation after operation,embodies the advantages of downward legs plaster fixation method when the new operation.Methods From January 2014 to December in the inpatient department for Achilles tendon repair in 34 cases as the control group,krackow suture after upward leg plaster fixation;from January 2015 to December in the inpatient department for Achilles tendon repair in 32 cases of patients as the observation group, the concept of krackow suture after downward leg plaster cast, 6 week of daily life were compared between the two groups at discharge and after the operation,the function of ankle and knee joint situation ability. Results At the time of discharge and 6 weeks after operation,the self-care ability,ankle function and knee joint function of the observation group were better than those of the control group (all P<0.001).The complication rate in the observation group was lower than that in the control group (P<0.05).Conclusion The new downward Achilles tendon plaster fixation is superior to the traditional plaster fixation method,can improve the patients'self-care ability,reduce complications,promote the recovery of limb function,promote early rehabilitation of patients.

Objective To compare the efficacy of small splint fixation and plaster fixation in treating distal radius fracture. Methods One hundred and four cases of type A2 distal radius fracture admitted from January of 2012 to May of 2013 were randomly divided into small splint fixation group and plaster fixation group. After reduction, patients in the two groups were fixed with small splint and plaster separately. Wrist function scores, 36-item Short Form ( SF-36) scores and roentgenographic scores were evaluated for the two groups at different time points. Results ( 1) Within 3 months of follow up, the improvement of wrist function in small splint fixation group was significantly superior to that in plaster fixation group (P0.05). (2) SF-36 scores of the two groups were gradually increased along with the follow up. After follow-up for one week, 2 weeks, one month and 3 months, the SF-36 scores in small splint fixation group were significantly higher than that in plaster fixation group ( P0.05). ( 3) After follow up for one year, the excellent rate for roentgenographic scores was higher than 80% in both groups, and there was no significant difference between the two groups (P>0.05). Conclusion Both small splint fixation and plaster fixation have satisfacory efficacy in treating type A2 distal radius fracture. But during the therapeutic process, wrist function scores and life quality scores in the small splint fixation group were higher than those in the plaster fixation group.

RESUMO

[Objective]To evaluate the clinical result after treatment on fracture of distal radius with different fixations of the wood splint/plaster splint/"T" model plate.[Method]There were 236 cases with the fracture of distal radius,there were divided three types of A,B,C according to the AO classification,70 cases in 236 cases were treated with close diaplasis and external wood splint fixation,55 in 236 cases with external plaster splint fixation,III in 236 cases with operation and internal "T" model plate fixation.Results were evaluated by replacement and function after treatment,and percentage of excellence was compared with three different fixation.[Result]236 cases were all followed-up,mean follow-up period was 14.6 months(6~20months).The excellence rate of diaplasis was not significantly different in A and B type by comparing three different fixation,the excellence rate of function with wood splint fixation and "T" type plate internal fixation was better than that of plaster splint fixation in A and B type,there were not significantly difference about the excellence rate of function with the external wood splint fixation and the internal "T" model plate fixation.The excellence rate of function and reduction with"T" type plate internal fixation were better than with plaster splint and wood splint in C type.[Conclusion]The author suggests open reduction with "T" type plate internal fixation about C type fracture,close reduction and external wood splint fixation about A and B type distal radius fracture.
