RESUMO

Objective To investigate the dosimetry differences of target and OARs of an integrated design of fields in IMRT and the mainstream IMRT technique for post-radical mastectomy.Methods A total of 41 patients with post-radical mastectomy who received IMRT were eligible,the conventional fixing two-degrade collimator and the integrated IMRT fields were designed respectively.The dosimetry parameters of target and OARs,monitor units and delivery time of both plans were compared.Results The dose distribution for targets and OARs of both plans met clinical requirements.The dosimetry parameters of target of both plans showed no statistically significant difference (P>0.05).Compared with the conventional technique,the integrated IMRT plans showed significant advantages,the ipsilateral lung V5 decreased by9.7% (t=2.407,P<0.05),V10 11.2% (t=2.160,P<0.05),V20 17.3% (t=2.465,P<0.05),V30 13.4% (t=2.119,P<0.05),D 13.8% (t=2.258,P<0.05).And the heart V30 decreased by 28.4% (t =2.589,P<0.05),D 23.2% (t=2.409,P<0.05).The dosimetric differences of other OARS were not statistically significant (P>0.05).Conclusions The new method can effectively reduce exposed volume and exposed dose of ipsilateral lung and heart without affecting the target dose coverage.The method has universal applicability to patients with post-radical mastectomy who received IMRT,with important clinical significance.

RESUMO

Breast cancer is the second largest cancer in Indonesia. Patients with advanced-stage breast cancer in particular, are generally filled with anger and depression at the thought of her illness. Family support is essential in patient care, one example is the situation after breast removal surgery (Radical Mastectomy). This study is a qualitative study with phenomenological approach that aims to explore the role of family in providing support to the achievement of self integrity breast cancer patients post Radical Mastectomy. Participants were selected by purposive sampling method which is a family of post Radical Mastectomy patients in Prof.Dr.Margono Soekarjo hospital. There are six partisipants join this research. Data collection techniques using in-depth interviews twice a further analysis of data using methods Collaizz's.The research results show the role of family experience in providing support to the achievement of the integrity of the patient post Radical Mastectomy, covers six main themes: first, various assistance provided by the family, the less the support of the family as indicated by the husband who pays little attention and dishonesty diagnosis. Third, the family response to the condition and Fourth, the need at this moment perceived as a need for information about cancer and quality health services. Fifth, the various barriers in memendapatkan health services, and the sixth, the various changes that occur in the family.The study provides implications of information regarding the role of family in providing social support to women post Radical Mastectomy and increase knowledge in providing nursing care to patients Breast Cancer Post Radical Mastectomy in Prof.Dr.Margono Soekarjo hospital, and help meet the diverse needs would social support to improve the quality of life of patients with breast cancer post radical mastectomy.
