RESUMO

ObjectiveTo investigate the use of contraceptive methods, and to evaluate maternal healthcare services utilization among women within 2 years in Pudong New Area of Shanghai. MethodsA cross-sectional survey was conducted using a questionnaire for women at different postpartum periods who visited a community health clinic with their children from June to November 2021. Data were analyzed using χ2 test and binary logistic regression. ResultsAmong the 1 946 postpartum women surveyed, 1 934 were either married or cohabiting, and1 430 had resumed their sexual life. Among women at 4, 6, 12, 18 and 24 months postpartum, the contraceptive prevalence rates (CPR) were 92.34% (193/209), 92.16% (235/255), 87.31% (282/323), 91.95% (297/323) and 90.00% (288/320), respectively. The modern contraceptive prevalence rates (mCPR) were 87.08% (182/209), 87.06% (222/255), 82.04% (265/323), 83.09% (271/323) and 85.31% (273/320), respectively, while the unmet contraceptive needs (UMNs) were 7.66% (16/209), 7.84% (20/255), 11.46% (37/323), 6.81% (22/323) and 10.00% (32/320), respectively. The use rates of long-acting reversible contraceptive (LARC) methods among women at 6, 12, 18 and 24 months postpartum period were 0.39% (1/255), 2.17% (7/323), 0.31% (1/323), and 2.81% (9/320), respectively. Among all surveyed subjects, 32.17% (626/1 946) had received postpartum contraceptive services only once, while 27.85% (542/1 946) had not received any postpartum contraceptive services. Binary logistic regression analysis indicated that the use of contraceptive methods among postpartum women was associated with whether relevant educational services were received after childbirth and during postpartum home visits (all P<0.05). ConclusionPostpartum women have unmet needs for contraception. Contraceptive guidance at the 42-day postnatal healthcare visit needs to be further strengthened and postpartum contraceptive education could be integrated into the pregnancy care. The quality and effectiveness of contraceptive education during delivery and postpartum home visits, and even at the 42-day postnatal healthcare visit need to be further explored.

Background & objectives: Postpartum intrauterine contraceptive device (PPIUCD) is well accepted and recommended for contraception. However, anxiety at the time of delivery may restrict the acceptance of a PPIUCD for its immediate insertion. So far there is limited evidence to conclude anything concrete on the association between the expulsion rates and the timing of insertion following a vaginal delivery. Thus, this study was undertaken to compare the expulsion rates in immediate and early insertions and their safety and complications. Methods: This prospective comparative study was carried out over 17 months on women delivering vaginally in a tertiary care teaching hospital in South India. A copper device (CuT380A) was inserted using Kelly’s placental forceps either within 10 min of placental delivery (immediate group, n=160) or between 10 min upto 48 h postpartum (early group, n=160). Ultrasound was done before discharge from the hospital. The expulsion rates and any other complications at six-week and three-month follow up were studied. Chi-square test was used to compare the difference in expulsion rates. Results: The expulsion rate was five per cent in the immediate compared to 3.7 per cent in the early group (no significant difference). In ten cases, the device was found to be in the lower uterus upon ultrasound before discharge. These were repositioned. There was no case with perforation, irregular bleeding or infection up to the three-month follow up. Higher age, higher parity, lack of satisfaction and motivation to continue were predictors of expulsion. Interpretation & conclusions: In the present study PPIUCD was found to be safe with overall expulsion in 4.3 per cent. It was marginally, though not significantly, higher in the immediate group.

Abstract Objective To evaluate the expulsion and continuation rates of the copper intrauterine device (IUD) inserted in the immediate postpartum period in a Brazilian public university hospital. Materials and Methods In the present cohort study, we included women who received immediate postpartum IUD at vaginal delivery or cesarean s March 2018 to December 2019. Clinical data and the findings of transvaginal ultrasound (US) scans performed 6-weeks postpartum were collected. The expulsion and continuation rates were assessed 6-months postpartum using data from the electronic medical records or by telephone contact. The primary outcome was the proportion of IUDs expelled at 6 months. For the statistical analysis, we used the Student t-test, the Poisson distribution, and the Chi-squared test. Results There were 3,728 births in the period, and 352 IUD insertions were performed, totaling a rate of 9.4%. At 6 weeks postpartum, the IUD was properly positioned in 65.1% of the cases, in 10.8% there was partial expulsion, and in 8.5% it had been completely expelled. At 6 months postpartum, information was obtained from 234 women, 74.4% of whom used IUD, with an overall expulsion rate of 25.6%. The expulsion rate was higher after vaginal delivery when compared with cesarean section (68.4% versus 31.6% respectively; p = 0.031). There were no differences in terms of age, parity, gestational age, final body mass index, and newborn weight. Conclusion Despite the low insertion rate of copper IUDs in the postpartum period and a higher expulsion rate, the rate of long-term continuation of intrauterine contraception was high, indicating that it is a useful intervention to prevent unwanted pregnancies and to reduce short-interval birth.


Resumo Objetivo Avaliar as taxas de expulsão e continuação do dispositivo intrauterino (DIU) de cobre inserido no pós-parto imediato em um hospital universitário brasileiro. Materiais e Métodos Neste estudo de corte transversal, foram incluídas parturientes submetidas à inserção de DIU de cobre no pós-parto imediato entre março de 2018 e dezembro de 2019. Foram coletados dados clínicos e da ultrassonografia (US) transvaginal realizada após seis semanas. As taxas de expulsão e de continuação foram avaliadas após seis meses por meio de dados do prontuário ou por contato telefônico. O resultado principal foi a proporção de DIUs expelidos em seis meses. Para análise estatística, utilizaram-se o teste t de Student, a distribuição de Poisson, e o teste do Qui quadrado. Resultados Houve 3,728 nascimentos no período, e foram inseridos 352 DIUs, em uma taxa de 9,4%. Com 6 semanas, o DIU estava bem posicionado em 65,1% dos casos, em 10,8%, houve expulsão parcial, e, em 8,5%, fora totalmente expelido. Aos 6 meses de pós-parto, foram obtidas informações de 234 mulheres, 74,4% das quais usavam DIU, com uma taxa de expulsão geral de 25,6%. A taxa de expulsão foi maior após o parto vaginal do que após cesariana (68,4% versus 31,6%, respectivamente; p = 0,031). Não houve diferenças quanto à idade, paridade, idade gestacional, índice de massa corpórea final, e peso do recém-nascido. Conclusão Apesar da baixa taxa de inserção e alta taxa de expulsão, a taxa de continuação em longo prazo da contracepção intrauterina com DIU de cobre foi elevada, o que indica que se trata de intervenção útil para prevenir gestações indesejadas em curto intervalo de tempo.

Background: India is the second most populated country in the world after China with more than a billion people and has highest number of maternal deaths in the world. The aim of the study was to find out the complications after insertion of post placental Cu T 380 A in women undergoing caesarean delivery.Methods: A prospective observational study was carried out in the department of obstetrics and gynecology, Dr RPGMC Kangra (Rural Medical College) after taking approval of Protocol Review and Institutional Ethics Committee of the Institution. A total of 104 women delivering by caesarean section and wanting post-placental intra-caesarean Cu T 380 A insertion and who were meeting WHO standard medical criteria for PPIUCD insertion and were willing to comply with the study protocol was recruited for the study.Results: There were no major complications and only minor side effects like pain and fever were observed in 6.25 and 6.90 and 9.37 and 11.11% of women who were admitted electively and in emergency, respectively during follow-up visit up to 6 months. String became visible in 72.12% of women at the 6 weeks follow-up visit and their visibilities increased with time and at 6 months follow-up in 90.81% of the cases. Continuation rate was 100% at 6 weeks post-partum follow-up. After that spontaneous expulsion occurred in 4 cases (3.84%) and another 4 women (3.84%) requested removal for various reasons leading to continuation rate of 92.30% at six months post-partum follow-up. There was no case of pregnancy with Cu T in situ with no failure at the end of study at six months post-partum.Conclusions: Post placental insertion of Cu T 380 A in women undergoing caesarean section was safe and effective method of postpartum family planning, the complaints and complications initially increased but decreased at 6 months follow-up.

Background: The choice of a contraceptive method is a complex decision. Health care providers have an important role in providing information and supporting patients' decision making about contraceptive choices through counselling. Non-use of contraceptive methods, use of less effective methods, and incorrect and inconsistent use of methods underlie the high frequency of unintended pregnancy. Prospective cross-sectional study was undertaken to study the role of peri-partum counselling in improving choice of postpartum contraception at Pravara Rural Hospital Loni, Ahmednagar, Maharashtra, India.Methods: Four hundred and fifty women in third trimester of pregnancy were individually counselled about the postpartum contraception, using educational material and pamphlets. The choice of postpartum contraception before and after the counselling was noted.Results: Among primi gravidas, the most favored choices for postpartum contraception before counselling were condom (25%) and lactational amenorrhea method (12%). In the same group of women, the post counselling choices were PPIUCD (45%), condom (17%) and interval IUCD (7%). Among multi-gravida, the most favored choices for postpartum contraception before counselling were postpartum tubal ligation (26%), condom (20%) and lactational amenorrhea method or calendar method (17%). In the same group of women, the post counselling choices were postpartum tubal ligation (32%) PPIUCD (12%), condom (8%).Conclusions: The study supports the usefulness of good quality counselling both with respect to the interpersonal relationship between the patient and the provider and quality of information that is provided during counselling. Postpartum intrauterine contraceptive device (PPIUCD), a long acting reversible contraceptive, will fulfil the felt need of postpartum contraception in near future.

Background: Postpartum contraception is essential for avoidance of unwanted pregnancy and for adequate spacing between two pregnancies. There are many socio demographic and cultural factors that influence the choice of contraception in rural community in India. Third trimester of pregnancy is ideal time for counselling the women regarding breast feeding and contraception. The objective of the present study was to find out the choice of postpartum contraception among antenatal women and the socio demographic and cultural determinants that influence this choice.Methods: Six hundred pregnant women were interviewed regarding their choice of postpartum contraception during their antenatal visit in third trimester of pregnancy, using a pre-validated and pre-tested brief questionnaire. The choices were compiled and analysed to draw conclusions.Results: Postpartum sterilization was choice of 30% of multiparous women. Primi-parous women either opted for barrier contraception like condom (10%), intrauterine contraception (9%) or oral steroidal pills (8%). The progesterone injectable contraceptives and centchroman each were chosen by 2% respondents. There was strong influence of education, parity, sex of the living children on the choice of contraception. It was observed that 40% of women did not want to use hormonal pills and intrauterine contraceptives due to strong age old misbelieves associated with them.Conclusions: Women in rural area prefer permanent method of contraception in the form of tubectomy operation after having desired number of children. There is insufficient spacing between pregnancies due to either non-use of contraception or inconsistent use of temporary method of contraception. More than 50% women are dependent on the husband regarding the choice and practice of contraception. The level of education of woman, age at marriage, socio economic class, desired sex combination of children are strong determinants of choice of contraception.

Continuation rate was 100% at 6 weeks post-partum follow up. After that spontaneous expulsion occurred in 4 cases (3.84%) and another 4 women (3.84%) requested removal for various reasons leading to continuation rate of 92.3% at six months post-partum follow up. There was no case of pregnancy with Cu T in situ with no failure at the end of study at six months post-partum.Conclusions: Intra caesarean insertion of PPIUCD is practical, convenient, safe, effective and acceptable contraceptive method for spacing of the birth in this rural setting.

Background: India was the first country in the world to have launched a National Programme for Family Planning.Methods: A cross-sectional descriptive study was carried out in the in Vaccination unit of rural field practice area of Shri Ram Murti Smarak institute of Medical Sciences (SRMSIMS) Bareilly (U.P). Objective of the study was to assess knowledge and practice of postpartum contraception and factors affecting the usage of contraceptives in Rural area. All females who delivered within last one year were included in the study. A pre-structured questionnaire was used. A total of 98 females were included. The appropriate statistical analysis was done to present the results.Results: 28.57% females had adopted one or the other postpartum contraceptive measure. Condom was the most common method used. Usage of postpartum contraception was significantly associated with women’s and husband’s education, type of delivery and availing of antenatal and postnatal visits.Conclusions: Overall usage of postpartum contraception was low there is need to focus at every step to contact of these women with health facility or health workers

BACKGROUND: The number of Latinos in the US is increasing, and they are at higher risk for unintended pregnancies and short interpregnancy intervals. PURPOSE: This study examined immigrant Latino couples' attitudes and perceptions towards contraception in relation to sexual decision-making. METHODS: Pregnant Latinas and their partners (n=40 couples) were recruited from prenatal care clinics in the southeastern US. This was amixed-method study with a concurrent embedded strategy. Descriptive statistics, paired t-tests, interclass correlation, and the Actor-Partner Interdependence Model (APIM) were used to analyze the quantitative data. Data from open ended questions were analyzed with content analysis. RESULTS: On the group-level analysis, men's Contraceptive Attitudes and Perception Scale scores were significantly different from women's in four key dimensions: partner (comfort in communicating with partner and partner's attitude), side effects, hassle, and cost (p<0.002, 0.046, 0.018, and 0.02). On the dyad-level analysis (between individual couples), no significant correlations were identified. In the dyad analysis using the APIM, only men's contraceptive attitudes and perceptions were negatively associated with women's sexual decision-making (ß=−0.158, p = 0.005). Nearly two-thirds of the couples disagreed on postpartum contraceptive methods. Seven overlapping categories emerged as rationales for choosing specific postpartum contraceptive methods among men and women. CONCLUSIONS: Understanding the dyad-level influence on contraceptive attitudes/perceptions, sexual decision-making helps researchers,and practitioners design evidence-based, culturally tailoredinterventions to assist couples in choosing the best postpartum contraceptives for their family. Decreasing men's contraceptive barriers by providing correct information may be the first step in mediating the dyad-level factor.


TRASFONDO: El número de Latinos en EEUU está creciendo, y ellos tienen alto riesgo de embarazo sin intención y corto intervalo. PROPÓSITO: Este estudio examinó actitudes y percepciones sobre anticonceptivos (APSA) de parejas Latinas inmigrantes relacionado con la toma de decisiones sexuales. MÉTODOS: Latinas embarazadas y sus parejas (n=40 parejas) fueron parte del estudio. Se utilizó el método mixto con la estrategia de concurrente empotrado. Se utilizó la estadística descriptiva, examen de t, correlación entre-clase, y el Modelo de Interdependencia Actor-Pareja (MIAP) para analizar datos cuantitativos. Las preguntas abiertas fueron analizadas con el método de análisis contento. RESULTADOS: En el análisis del nivel grupal, marcas de los APSA de los hombres fueron significativamente diferente de los de mujeres en cuatro dimensiones: parejas, efectos secundarios, molestia, y costo (p<0.002, 0.046, 0.018, y 0.02). En el análisis del MIAP, solo APSA de los hombres fueron asociados negativamente con tomas de decisiones de las mujeres (ß=−0.158, p = 0.005). Casi dos tercios de las parejas estaban en desacuerdo con los métodos anticonceptivos después del parto. Siete categorías surgieron como razones para escoger los métodos anticonceptivos después del parto. CONCLUSIONES: Entender la influencia de APSA y toma de decisiones entre parejas ayudará a los investigadores y enfermeros a diseñar intervenciones basados en evidencia y culturalmente diseñados para ayudar a parejas a escoger anticonceptivos post-parto. Reducir las barreras que tienen los hombres a los anticonceptivos proveyendo información correcta será el primero paso en afectar el factor en el nivel entre parejas.
