RESUMO

Resumen Los procesos que se llevaron a cabo luego del contacto de los pueblos originarios con el conquistador español, en términos de saberes médicos en el territorio conocido como Virreinato de Nueva Granada, influyeron en las relaciones de conquistador-conquistado en procesos como la estigmatización y persecución de saberes ancestrales, las diversas formas de transculturación y el proceso de expropiación de algunos elementos de la cultura prehispánica.


Abstract The processes that took place after the contact of the original peoples with the Spa nish conqueror, in terms of medical knowledge, in the territory known as the Viceroyalty of New Granada, influenced the relations of conqueror-conquered in processes, such as the stigmatization and persecution of ancestral knowledge, the various forms of transculturation, and the expropriation process of some elements of pre-His panic culture.

@#Dental quackery has been a problem for decades and is becoming a major concern in many countries, including Malaysia. Recent development of a new service offered by quacks in Malaysia is “fake braces”, which alarmed dental professionals. The fake braces appear similar to the professionally fitted orthodontic appliances comprising of archwires that are secured on brackets by coloured ligatures except they are fitted by unqualified individuals who have no formal clinical training. In addition, the orthodontic materials and dental equipment used for this illegal service were substandard and unregulated. Therefore, such fitted appliances are harmful to the teeth and oral health. Efforts to record the extent of fake braces practice and its oral health consequences have been challenging as they are marketed through the social media, and the victims were either reluctant to come forward or did not know the appropriate channel to file a complaint to the health authority. This is an expert opinion paper with the aims to highlight typical presentation of fake braces, modus operandi of fake braces providers, the harmful effects of fake braces on the patient’s oral health, the role of social media advertising in promoting fake braces, and the impacts to the illegal providers.

“Yong-yi” means “quack” in English, which generally refers to a doctor who does not have good medical skills. In the Ming and Qing dynasties in China, various criticism about “Yong-yi” became popularized, and by the late Qing period, “quacks” had become a serious social issue. The theory of traditional Chinese medicine was developed during the Ming and Qing dynasties, and local medical resources also increased. Moreover, the prevalence of medical book publishing led to the openness and generalization of medical knowledge. As a result, not only the number of doctors increased, but also the number of doctors who lack medical knowledge and clinical experience increased. However, at the outset, “Yong-yi” did not only mean doctors with poor medical skills. “Yong-yi” also reflected conflicts and contradictions between doctors. Doctors consistently criticized quacks in an attempt to maintain their identity as a “good” doctor or a Confucian doctor. In this sense, “Yong-yi” was used among physicians as an expression of discrimination and exclusion. The concept of “quackery” was also determined by the relationship between patients and doctors. In general, itinerant doctors, midwives and shaman doctors were regarded as “Yong-yi”; however, they served the medical needs of various patients. Thus, to some extent, “Yong-yi” were also useful medical resources. On the contrary, in certain situations, “shiyi,” physicians who serviced a family for generations and were generally believed to be reliable and as trustworthy doctors, were also labelled as quacks, especially when the patient did not trust them or was not satisfied with the treatment. Therefore, doctors' thoughts about “Yong-yi” did not always coincide with patients' thoughts about “Yong-yi.” However, by the late Qing period, the description of quacks in media reports found a singular connotation, and the divergent social image of quacks disappeared. By this time, quacks were uniformly described as ignorant and irresponsible Chinese medicine practitioners. Specifically, in one murder case in which a “Yong-yi” was accused as the murderer, the report unilaterally reported the patient's claims. Consequently, Chinese medicine practitioners who failed in their treatment of patients became labeled as “quack” doctors. In newspaper reports, “Yong-yi” no longer simply referred to individual cases of “quacks” but had come to represent the entirety of the Chinese medicine practitioner community. On the contrary, Western medical doctors who replaced the status of traditional doctors were positively portrayed. Pictorials also had similar perspectives with newspapers, supporting the narrative of the news with ironic drawings and articles. Overall, media reports regarding “Yong-yi” did not focus on reporting facts, but they had the purpose of making quacks a serious social problem.

The practice of quack dentistry worldwide is a challenge to the profession of dentistry as it undermines the trust of the general public and places the patients at unnecessary risks through their illegal practices. Although the ratio of patients to dentists is rapidly decreasing in Malaysia due to the increased output of qualified dentists from both local and foreign institutions, quack dentistry is still practiced in many areas. This report intends to highlight a popular treatment prescribed by these quack dentists which is removable partial dentures that is fixed to the adjacent teeth by cold cure acrylic. We also aim to discuss the design of the denture and the means employed to remove this ’fixed partial denture’. It is hoped that this case report will highlight the prevalence of the practice of quack dentistry especially in geographical area of this case report and allow the authorities to devise means to stop this illegal practice.
