RESUMO

The construction of county medical service community (medical community) is an important measure for building a hierarchical and integrated medical and health service system. The article takes Yinzhou District of Ningbo city as an example to elaborate the strengths and weaknesses of the internal environment and the opportunities and challenges of the external conditions using SWOT analysis in the construction of county medical community, also put forward corresponding countermeasures and suggestions to provide reference for the further improvement.

Objective:To investigate the disease spectrum referred from the community health service centers to the district tertiary hospital in the region.Method:The data of the referred patients from the community health service centers to Shanghai Yangpu District Hospital-a tertiary hospital in the region from January 2016 to December 2019 were collected. The composition and characteristics of the disease spectrum of the referred patients were analyzed.Results:The number of patient visits to the community health service centers in the region was increased from 4 684 938 in 2016 to 4 776 921 in 2019. The number of patient referral from the community centers to Yangpu Hospital was increased from 3 125 in 2016 to 6 853 in 2019. There were total 18 308 patients referred in 4 years, including 8 012 males (43.8%) and 10 296 females (56.2%). The age range of referred patients was from 1 to 99 years, including 53 referred patients aged ≤ 18 (0.3%), 927 aged 19-39 (5.1%), 2 831 aged 40-59 (15.5%), 4 424 aged 60-79 (62.2%) and 1 209 aged ≥ 80 (17%). The top five systems of diseases, which accounted more than 60% of total diseases, were circulatory system, endocrine nutrition and metabolic system, urogenital system, eye and appendage, and digestive system. Compared with male patients, more female patients with urogenital diseases were referred.Conclusion:The referral rate of community health service centers were increased from 2016 to 2019. The referred patients cover all age groups, more frequently in age group 60-79. The diseases of referred patients cover all system, and are different between males and females.

RESUMO

Since 2009, a five-day, regional health care, clinical clerkship in medically underserved areas has been available to all fifth-year medical students at the University of Tsukuba. In the program, students listen to and observe health problems in non-clinical settings, wherever these problems actually exist. Students can experience health care lectures and health examinations. They can also experience a clinical work environment. The short stay helps students to understand the living environment of the area they observe. It is also effective for students to consider health problems from the perspective of the local inhabitants. This program aims to directly convey the interest and importance of community medicine, and to contribute to the supply of physicians in these areas. A longitudinal study is needed to evaluate the effect of the program.

RESUMO

By means of literature review,the paper made a comparative study of key development projects of healthcare information in China over the last decade,in terms of their project characteristics,economic environment,development entity,investment and compensation practice,development strategy,coverage,experience and promotional values.Setbacks identified include the following:discrepancy in regional development stages of healthcare information; successful pilots are limited to scientific research samples; lack of regional models; systems separation; poor continuity and coordination; waste and insufficiency of resources in the investment and construction among others.The authors recommend a better exchange of experiences in building the regional healthcare information service platforms,greater investment,better compensation mechanism,better top-tier design,and maximized coverage within reach of supervision and technology.

Regional health planning is a leading and scientific management philosophy for healthcare development in the international community.Information resources planning paves the way for scientific development.In the ongoing health reform in China,planning of regional health resources will play a key role by leveraging information technology.From the viewpoints of health information resources and information resources planning,the paper defines the concept of regional health information resources planning in microcosmic perspective.It summarized the five features of public welfare,specificity,imbalance,small channel and low noise,and non-symmetry.Authors compared the five planning modes,namely the community medical information resources mode,the general hospitals as the center of information resource mode,the regional PACS mode,the regional health data resource mode,and the regional medical coordination resources mode.In the paper,the planning is rounded up as four key points,that is,the basic steps of situation assessment and analysis,routine work of health information standardization,the planning should both comply with general rules of information resource planning and characteristics of the health sector,in addition to guidelines of forward-looking concepts.

To identify the key elements and available tools of regional health information resources plamling,the study introduced an innovative I-beam map,aiming to solve the two key questions of what and how to plan The paper covered such issues of planning as what is the present situation of regional health information,how to divide functional categories of regional health information resources,what are included in the business subclasses,which core data are used in the business process,which data to share and exchange,how to regulate such data,how to manage the regulations and make continuous improvement,and how to evolve towards planning standardization discriminatively.The study also drew a midfield line with such seven fields as medical service,maternity and child care,disease control and Drevention,health administration,emergency management,blood management and primary healtheare.This midfield line is used,along with stakeholders,planning process,information flow and planning output make the four quadrants,describing the whole process in the I-beam form.This practice can provide the methodology ofhow to plan,in the hope of guiding regional health information resource planning from theory to practice.

Healthcare in Japan is approaching a crossroads. Birth rates are lowering, and various problems associated with a rapidly increasing elderly population are erupting. To turn around regional medical care, a complete rethinking on the part of physicians and the citizenry alike is called for. Physicians need to understand not only the latest clinical resources, but also what role medical care plays in everyday life. And we should reassess our regional medical care to gain a better understanding of its value and meaning.<br>In an aging society, we need to build communities able to extend the helping hand of care to the elderly, whatever position they may be in. As science and technology advance, great changes in medical care are coming about. We see home healthcare becoming more high-tech. And it would be best for us to build a healthcare system that links the functionality of various medical institutions from the most advanced hospitals to more common private infirmaries. Answering the question of how the increasing number of women doctors would best provide medical care throughout life will also provide a breakthrough to our shortage of doctors. This is an age in which both patients and healthcare providers can move forward together. And we are counting on the nation to put forth its best efforts.
