RESUMO

As implant can be covered by National Health Insurance Service (NHIS), it was increased the interest in the removable partial denture (RPD) with the surveyed fixed prosthesis supported by implant fixture. To achieve predictable result, it needs the prudent implant planning by basic principles of RPD and patient's residual ridge. This 67 years old age male had a few unilateral remaining teeth, and hoped the treatment covered by NHIS. As using CAD software, the position of implant is planned with regarding to occlusal table of provisional denture, basic principles of RPD, and resorbed residual ridge. The definitive prostheses can ensure the stability and retention of removable prosthesis. When planning implant fixed prostheses, the digital technique was utilized to consider basic principle of RPD and resorbed residual ridge. As a result, it provided satisfactory prostheses.

RESUMO

Double crown prostheses can be used in patients who have a few remaining teeth and poor periodontal condition because of secondary splinting of abutments, vertical loading, decrease of the length of lever arm due to fulcrum line located on margin of inner and outer crown. Successful results of treatments using double crown prostheses for the partially edentulous patients who have a few remaining teeth and implant overdenture using a small number of implants have been reported. In this case, there were a few remaining teeth with a very poor periodontal condition in maxilla, and there were a failed implant with severe alveolar bone resorption and shrinkage in the mandible. The main objective of this report is to introduce our case because a double crown partial denture showed satisfactory results in functional and esthetical aspects during more than one-year follow-up period.

RESUMO

A few authors have reported good clinical results using double crown removable partial denture (RPD) with a few remaining teeth. Hybrid telescopic double crown is a good indication for a patient with poor periodontal condition and/or few remaining teeth after extraction, especially located in cross-arch position. In this case, there was a poor periodontal condition with teeth mobility. Several teeth with poor prognosis were extracted. Remaining anterior teeth was restored with fixed prostheses and edentulism was restored with Kennedy class I removable partial denture in maxilla. In mandible, it was hard to restore with clasp removable partial prostheses because of bilaterally isolated remaining teeth so that hybrid double crown removable partial denture with friction pin was suitable for this case. The objective of this report is to discuss the characteristics and the utility of hybrid double crown prostheses using a few remaining teeth in mandible.

OBJECTIVES: To understand the relationship between monthly household income and number of remaining teeth as an indicator of oral health, and to find the differences between middle-aged and older people in this respect. METHODS: This study was conducted on data extracted from the Korean National Health & Nutrition Examination Survey, which surveyed all Korean citizens in 2012. From these data, data of 4502 people about 40 years of age who completed the questionnaires and examination survey were selected and analyzed. This study investigated the main factors affecting the remaining teeth, and analyzed the moderating effect of age on the relationship between income level and remaining teeth. RESULTS: The average number of remaining teeth was 25.17 for middle-aged people and 15.95 for older people, which was significantly different. The number of remaining teeth for middle-aged people showed a significant relationship with income level, education level, and smoking habit. The number of remaining teeth in older people was related to the same factors, along with an additional factor of whether or not they underwent oral examinations. From the interaction analysis conducted to find the moderating effect of age on the relationship between the number of remaining teeth and income level, the number of remaining teeth was more affected by income level in older people than in middle-aged people. CONCLUSIONS: Income level was strongly related to oral health, particularly in older people. Therefore, it can be concluded that an oral health promotion policy is necessary for low-income people, particularly those in the older age group.

OBJECTIVES: The purpose of this study was to assess the associations between oral health, activity of daily living, and cognitive impairment among elderly people who live at home in a rural area. METHODS: A total of 183 participants older than 70 years of age were recruited from a public health center in a rural Korean city. Trained examiners conducted questionnaire surveys and at-home oral exams to assess oral health, geriatric function, and cognitive impairment. Oral health was assessed by counting remaining teeth and examining salivary flow. Geriatric function and cognitive impairment were assessed using the Mini-Nutritional Assessment (MNA), the Mini-Mental State Examination for Dementia Screening (MMSE-DS), and an activity of daily living (ADL) questionnaire. RESULTS: Mean age (SD) of the participants was 83.77 (5.99) years and 68.9% of them were women. The participants who had a lower number of remaining teeth were significantly older, had severe cognitive impairment, and poor ADL and MNA results. The participants with severe cognitive impairment were significantly older and had fewer remaining teeth and low salivary flow. Simple linear regression analysis showed an association between remaining teeth and cognitive impairment with a P-value of 0.000, which disappeared after adjusting for sex and age or sex, age, denture use, and salivary flow, respectively. CONCLUSIONS: Possible associations between remaining teeth and salivary flow, activity of daily living, and cognitive impairment were discovered among some elderly people living in a rural area.

RESUMO

Chronic periodontitis involves subsequent loss of teeth, and if left untreated, can lead to adjacent teeth drifting and supraeruption of the rest dentition. Careful consideration has to be given when deciding extraction of remaining teeth in treatment of periodontally compromised dentitions. For tooth-supported fixed partial dentures or removable partial dentures, periodontally compromised teeth are extracted due to possible early failure from functional overload, but for implant restoration, the teeth could be used as supports for fixed partial dentures because implants can reduce overload on teeth. The remaining natural teeth can help clinicians restoring vertical dimension and normal occlusal plane in full mouth rehabilitation because it conserves patients' proprioceptive response. This clinical report describes treatment of a patient who has a few remaining teeth and supraeruption of the rest dentition from severe chronic periodontitis. Satisfactory clinical result was achieved with full mouth rehabilitation using a few teeth and implants.

RESUMO

Successful results of treatments using double crown prostheses for the partially edentulous patients who have a few remaining teeth have been reported in several journals. A double crown removable partial denture can be an alternative treatment for the patients with a poor periodontal condition of remaining teeth. Since a double crown removable partial denture can be applied without the risk of surgical operation to the medically compromised patients with a poor periodontal condition which is inadequate for dental implants, it has psychological and economical advantages. In this case, there were sufficient remaining teeth to be restored with fixed prostheses in maxilla, while there were a few remaining teeth with a very poor periodontal condition so that it was almost impossible to restore with a clasp removable partial denture using these remaining teeth in mandible. In addition, the patient had the medical history of surgical operation due to osteomyelitis in the mandibular anterior areas a year ago, thus difficult to conduct an implant placement. The main objective of this report is to introduce our case because a double crown partial denture using a few mandibular remaining teeth showed satisfactory results in functional and esthetical aspects during more than two years follow-up period in this unfavorable condition.
