RESUMO

【Objective】 To investigate the variation of hematological parameters in male plateletpheresis donors. 【Methods】 A total of 194 male plateletpheresis donors from Fujian Blood Center were divided into two groups according to the frequency of blood donation: Group 1 (n=107), with the number of plateletpheresis donation less than or equal to 12 per year; Group 2 (n=87), with the number of plateletpheresis donation more than 12 per year. Serum ferritin (SF) and related iron metabolism indexes, red blood cell count (RBC), hemoglobin (Hb), hematocrit(Hct), platelet count (Plt) and other blood routine indexes, as well as percentage of reticulocyte counts (RET%), immature reticulocyte fraction(IRF) and other reticulocyte indexes were measured before blood donation and analyzed by statistical methods. 【Results】 Compared with Group 2, the RBC, Hb, Hct, SF in Group 1 were significantly higher, while Plt, RET%and IRF were significantly lower(P<0.05), and the probability of ferritin decrease in Group 1 was lower, with significant difference(P<0.05). 【Conclusion】 As the number of donation increased, male plateletpheresis donors were prone to iron deficiency, and the bone marrow hematopoiesis were obviously enhanced. We should be more concerned about male plateletpheresis donors who donated more than 12 times per year, further more, SF monitoring should be conducted and the blood donation interval should be appropriately extended.

Objective To observe and study the diagnostic value of related serum collagen indexes ,liver function indexes and re-ticulocyte parameters in patients with cirrhosis .Methods 54 patients with cirrhosis in our hospital from February 2015 to May 2016 were selected as the observation group ,54 healthy persons at the same time were selected as the control group ,and then the related serum collagen indexes ,liver function and reticulocyte parameters of two groups were detected and compared .The serum collagen related indexes ,liver function indexes and reticulocyte parameters of observation group with different Child-Pugh classifica-tions and different types of cirrhosis were analyzed and compared .Results The related serum collagen indexes including PⅢP ,PⅠCP and CⅣof observation group were(10 .25 ± 0 .95)ng/L ,(145 .87 ± 16 .34)μg/L and(85 .95 ± 7 .93)μg/L respectively ,the liver function indexes including AST ,ALT and GGT were(32 .52 ± 4 .15)U/L ,(33 .23 ± 3 .56)U/L and(26 .86 ± 2 .53)U/L respective-ly ,and the reticulocyte parameters including RET ,IRF and RPI were(1 .72 ± 0 .17)% ,(20 .87 ± 2 .23)% and(1 .79 ± 0 .20)respec-tively ,and they were all higher than those of control group .The related serum collagen indexes ,liver function and reticulocyte pa-rameters of observation group with different Child-Pugh classifications of cirrhosis had significant differences (P<0 .05) ,while the detection results of observation group with different types of cirrhosis had no significant differences (P>0 .05) .Conclusion The re-lated serum collagen indexes ,liver function indexes and reticulocyte parameters have a relatively high diagnostic value for the pa-tients with cirrhosis .

Objective:To investigate clinical value of the blood analyzer reticulocyte parameter in the diagnosis of anemia.Methods: 271 cases of anemia patients were divided into iron deficiency anemia(126 cases) and hemorrhagic anemia group(145 cases), namely the observation group; and other 50 healthy physical examinees were selected as the control group. SysmexXE-5000 automatic blood analyzer were used to detect the reticulocyte parameters, including reticulocytes(RET) ratio, high fluorescence reticulocytes(HFR) ratio of middle fluorescence reticulocytes(MFR) ratio and the immature reticulocytes(IRF) ratio.Results: Compared with the control group, the RET, HFR, MFR and IRF were significantly higher in the observation group. The sensitivity of iron deficiency anemia and blood loss anemia was 96.2% and 63.5%; the specificity of anemia was 96.0%.Conclusion: Detection of reticulocyte parameters can reflect the marrow hyperplasia patients with anemia. It has a high sensitivity and specificity in the diagnosis of various types of anemia, which has a high practical value in clinic.

Objective To understand the relationship between the changes of coagulation function ,platelet parameters and retic‐ulocyte parameters with the Child‐Pugh grade in the patients with liver cirrhosis .Methods The coagulation function index (PT , APTT ,Fig ,TT) ,platelet parameters(PLT ,MPV ,PDW ,PCT) and reticulocyte parameters(RET# ,RET% ,IRF) were detected in 163 patients with liver cirrhosis and 50 healthy controls by using the automatic hematological analyzer and the automatic blood co‐agulation analyzer .Then the changes of coagulation function ,platelet parameters and reticulocyte parameters were compared be‐tween the liver cirrhotic group and the control groups ,and among different Child‐Pugh grade groups .Results Compared with the control group ,Fig in the liver cirrhosis group was declined ,PT ,APTT and TT were prolonged ,PLT and PCT was decreased ,while MPV and PDW were elevated ,RET count ,RET% and IRF were increased ,the difference between the two groups were statistically significant(P<0 .05);Fig was gradually declined with the Child‐Pugh grade elevation ,PT ,APTT and TT were prolonged ,PLT and PCT was decreased ,while MPV and PDW were increased with the Child‐Pugh grade elevation ,and RET count ,RET% and IRF were increased (P<0 .05) .Conclusion The blood coagulation disturbance and abnormality of platelet parameters and reticulocyte parameters exist in the patients with liver cirrhosis .The changes of coagulation function ,platelet parameters and reticulocyte param‐eters are closely related to the Child‐Pugh classificatione ,which are the important indicators for judging the liver damage ,bleeding tendency and bone marrow hematopoietic function in the patients with liver cirrhosis .

Se revisan los antecedentes históricos del conteo de reticulocitos como determinación indispensable en el laboratorio de hematología para la evaluación de la actividad eritropoyética durante la clasificación, diagnóstico y monitoreo de la respuesta terapéutica en distintos trastornos y situaciones clínicas, principalmente en casos de anemias. Se describe el tránsito del tradicional método de conteo de reticulocitos manual al método automatizado y la integración de los parámetros reticulocitarios al hemograma automatizado actual; además, se analizan las desventajas del método de recuento manual y las ventajas del método automatizado, así como los principios de detección en que se basa el conteo electrónico de reticulocitos. Con relación a los parámetros reticulocitarios, se describe su medición, cálculo y unidades de medida; también se resalta la importancia de la fracción de reticulocitos inmaduros y del contenido de hemoglobina reticulocitaria como variables de mayor uso clínico e investigativo en la evaluación de la respuesta medular ante diversos trastornos clínicos y protocolos terapéuticos. Por último, se alude a la necesidad del conocimiento y empleo de las variables reticulocitarias en la práctica clínica de rutina por parte de los clínicos y especialistas en hematología.


The historical background of reticulocyte count is reviewed as an essential determination in the laboratory of hematology for the evaluation of erythropoietic activity during classification, diagnosis and monitoring of therapeutic response of different conditions and clinical situations are also reviewed, especially in anemia The transition from traditional manual reticulocyte counting method to automated method and integration of the reticulocyte parameters to current automated complete blood count are described. The disadvantages of manual method and the advantages of automated methods are cited, as well as detection principles in which electronic reticulocyte count is based. Regarding reticulocyte parameters, measurement, calculation and units are described. The importance of immature reticulocyte fraction and reticulocyte hemoglobin content as variables most clinical and research use in evaluating bone marrow response to various clinical disorders and therapeutic protocols are highlighted. Finally, the need for knowledge and use of reticulocyte variables in routine clinical practice by clinicians and hematologist is referred.
