RESUMO

Acute retropharyngeal calcific tendinitis or calcific tendinitis of the longus is an under-recognized clinical syndrome first described by Hartley in 1964. This syndrome is caused by calcium hydroxylapatate deposition in the longus colli muscle and tendon. Patients typically present with acute neck pain, neck spasm, a restrained range of motion, odynophagia, dysphagia and sore throat. Clinically, it can be misconceived as acute retropharyngeal abscess because of its rather non-specific presentation and rare occurrence. Diagnosis of retropharyngeal calcific tendinitis can be made radiographically with plain radiograph and CT by detecting calcifications anterior to C1-C3 and swelling prevertebral soft tissue. Recognizing this radiologic imaging findings and features can prevent unnecessary treatment and test. We report three patients with acute prevertebral calcific tendinitis, which was misdiagnosed as acute retropharyngeal abscess.

RESUMO

Intramural esophageal dissection is a rare esophageal disorder characterized by sudden severe retro-sternal pain, hematemesis, odynophagia and dysphagia due to longitudinal separation of submucosa from muscle layer of the esophagus. Even though the etiology of this disorder still remains uncertain, it is usually associated with a rapid increase of intra-esophageal pressure in addition to coagulation disorder. We treated a patient who initially presented with retropharyngeal abscess, but finally progressed to an intramural esophageal dissection, which was successfully treated by endoscopic procedure. This is the first case that recorded disease progression and treatment from a retropharyngeal abscess to esophageal dissection.

RESUMO

Acute calcific prevertebral tendinitis, which is also known as retropharyngeal calcific tendinitis and longus colli tendinitis, is an under-recognized cause of acute cervical pain produced by an inflammation of the longus colli muscle. The typical characteristics of this entity are calcifications at the superior insertion of the longus colli tendons at the C1-C2 level and fluid collection in the retropharyngeal space. The differential diagnosis includes a retropharyngeal abscess, infectious spondylitis or traumatic injury. Knowledge of the clinical and imaging findings can prevent a misdiagnosis and inappropriate attempts at surgical drainage.

RESUMO

A 36-year old man with neck pain developed an acute cervical prevertebral hematoma after acupuncture therapy at an oriental medicine hospital. MR imaging demonstrated a fluid collection, and this suggested a diagnosis of retropharyngeal hematoma, and the patient was managed conservatively. We report here on a case of a retropharyngeal hematoma following acupuncture therapy and we review the relevant literature.

RESUMO

Acute calcific retropharyngeal tendinitis, also known as calcific tendinitis of the longus colli, is an underrecognized inflammatory condition by deposition of calcium hydroxyapatite in the tendon fibers of the longus colli muscles. This disease entity may occur in any part of the body, however, rarely occur in the prevertebral musculature. Symptoms and signs of this disease are neck pain and stiffness associated with odynophagia and retropharyngeal soft tissue swelling. Because of self-limiting disease, the symptoms resolve usually with or without anti-inflammatory medication and analgesics within days or weeks. We report a case of acute calcific retropharyngeal tendinitis with a reviews of the related literatures.
