RESUMO

Objective:To explore the efficacy of the oncoplastic round block technique in surgical management of idiopathic granulomatous mastitis (IGM).Methods:From January 2014 to December 2019, a total of 18 patients (24 to 38 years old, 32.2 years in average) with IGM underwent excision of the inflammatory breast mass with oncoplastic round block technique, the postoperative clinical efficacy was summarized and analyzed.Results:All 18 patients with IGM underwent excision of the inflammatory breast mass with oncoplastic round block technique, among them 2 patients underwent round-block reduction surgery of contralateral breast at the same time. The median follow-up duration was 16.1 months (from 6 to 36 months). Incision poor healing occurred in two cases which was cured after dressing change. Recurrence occurred in one case at 6 months after operation, and then cured with conservative measures. No other severe complications occurred. All patients were satisfacted with the results.Conclusions:Application of oncoplastic round block technique in surgical management of IGM may remove more tissue in order to reduce the recurrent rate, and get a better cosmetic results.

RESUMO

Objective To clarify the applied anatomy of Chinese female breasts,to improve the breast reduction mammoplasty,and to compare the quality of life in women with symptomatic macromastia before and after reduction mammoplasty with the modified round block technique.Methods The blood supply and innervation of the nipple-areolar complex were dissected in 6 Chinese adult females (12 breast specimens).Special attention was paid to the Würinger horizontal septum and the medial and lateral ligaments.The specific beneficial effects of reduction mammoplasty were evaluated with the modified round block technique,and a general questionnaire the Short Form-36 Health Survey Questionnaire (SF-36) was used in this study,which has good reliability and validity.The scores of the patients before and after the surgery were collected respectively.Results In 12 breast specimens,there existed Würinger horizontal septum and the medial and lateral ligaments.The blood supply and nerve innervation of nipple-areolar complex went along Würinger horizontal septum.In all the patients enrolled,SF-36 showed significant higher quality of life after the operation with regard to 7 of 8 investigated domains.Role of emotion increased most significantly.Increased self-esteem and increased personal and public self-consciousness after surgery were observed.Conclusions The incidence of postoperative complications is reduced after using the modified round block technique.Patients have a higher degree of satisfaction overall.These data are further evidence that breast hypertrophy is not solely an esthetic problem.The reduction mammoplasty with the modified round block technique is an ideal technique.

INTRODUÇÃO: Aumentos mamários exigem planejamento com critérios objetivos e rigorosos, que foram padronizados pelo sistema High Five, de Tebbetts. Existe um grande número de combinações possíveis, entre via de acesso, plano e tipo de implante, o que permite proporcionar o melhor resultado para cada paciente. MÉTODO: 100 pacientes operadas, 30 com implantes anatômicos e 70 com redondos, por via retromuscular ou subglandular, com a realização de roundblock ou não. Todos os casos foram operados pela via periareolar. A avaliação pré-operatória foi realizada conforme a sistematização High Five. RESULTADOS: Roundblock foi mais utilizado no grupo de implantes anatômicos, representando 43% da amostra neste caso. O plano subglandular foi o mais utilizado, representando 76,6% da amostra total. Com relação ao implante anatômico, 86,7% dos implantes foram colocados no plano subglandular. Dos anatômicos, 71,4% foram colocados no plano subglandular. Ocorreram quatro casos de contratura capsular graus III e IV, mais comum com o uso de implante redondo. O índice de rotação do implante anatômico foi de 10%, com três casos no total. CONCLUSÃO: Ocorreu baixo índice de complicações, em conformidade com a incidência da literatura. O acesso periareolar proporcionou bons resultados em todos os casos. O paciente deve ser informado sobre a possibilidade da rotação do implante anatômico.


INTRODUCTION: Breast augmentation requires planning with objective and rigorous criteria, which were standardized by the High Five system of Tebbetts. There are a many possible combinations based on access route, implant plane, and type of implant, enabling the best outcome for each patient. METHOD: Of 100 patients studied, 30 received anatomic implants and 70 received round implants in the retromuscular or subglandular position, with or without round block technique. All cases used periareolar access. Preoperative evaluation was performed according to the High Five system. RESULTS: The round block technique was used more often with anatomic implants (43%). The subglandular plane was most commonly used (76.6%). Of 30 anatomic implants, 86.7% were placed in the subglandular plane. Of 70 round implants, 71.4% were placed in the subglandular plane. There were 4 cases of grade III and IV capsular contracture, which is more common with the use of round implants. The rate of rotation among anatomic implants was 10%, with 3 cases. CONCLUSION: There was a low complication rate, in accordance with published data. Periareolar access provided good results in all cases. The patient should be informed about the possibility of rotation with use of anatomic implants.

RESUMO

The goals of reduction mammoplasty are to remove the appropriate amount of breast tissue and to make the nice cone-shaped breast. For these goals, various methods have been used and newly introduced. Recently, as women's interest in cosmetic attractiveness increases, surgeons make attempts to minimize postoperative scars. Periareolar approach has an advantage of invisible postoperative scar, but has many disadvantages - flattening of breast shape, recurrence of breast ptosis, hypertrophy and widening of periareolar scar, enlargement and distortion of areola caused by tension. After Benelli introduced Round block technique, this approach became one of the most popular methods. The characteristics of Round block technique are periareolar approach, superiorly based dermoglandular pedicle, criss-cross mastopexy, and Round block suture. Authors modified periareolar Round block technique to adjust to Korean women and have operated on 11 patients since 1997. We focused on medializing the lateral portion of inframammary fold and avoiding asymmetry and protrusion of nipple-areolar complex. Round block suture with uniform thickness and length must be applied at the same time to prevent areolar complications. In design, authors referred to the statistical analysis on Korean female. The results are aesthetically and functionally satisfactory and there has been no significant complication. In conclusion, this method is effective for young woman or mild macromastia with moderate ptosis. Besides, an accurate understanding of each step of operative procedure and enough knowledge of anatomy and physiology of the breast are essential.

RESUMO

Breast has been considered as the symbol of the female. Ideal size of the breast was related to the age and culture. The goal of reduction mammaplasty is breast with natural cone shape, minimizing scars, well-placed and sensate nipple-areolar area and maintaining breast physiology. The ideal breast reduction should create beautiful breast with limited scars. We performed 37 cases of reduction mammaplasty. The techniques used were inferior dermal flap technique for 16 cases, vertical reduction mammaplasty for 18 cases, and periareolar round block technique for 3 cases. We compared advantages and disadvantages of the techniques and set up useful guide-lines to match the technique with the problem of each individual. The guidelines are as follow: 1. Inferior dermal pedicle technique is suitable for moderate to severe macromastia with ptosis. 2. Vertical reduction technique is effective for moderate macromastia in young women. 3. Periareolar round block technique is effective for mild to moderate macromastia in young women.
