RESUMO

Objective:To investigate the turnover intention and its influencing factors in rural general practitioners in southwest Shandong province.Methods:In May 2020, a survey with self-designed questionnaire was conducted among 2 805 rural general practitioners in southwest Shandong province selected by multi-stage sampling method. Pearson chi square test (χ2) and binary logistic regression were used to analyze the factors influencing the turnover intention of rural general practitioners.Results:A total of 2 805 questionnaires were sent out, 2 693 were collected and 2 272 were valid, with an effective rate of 84.4%. Among 2 272 participants, 1 076 (47.4%) had medium to high level turnover intention. Binary logistic regression showed that part-time job ( OR=1.443, 95% CI: 1.105-1.884, P<0.01), average monthly night shifts ≥20 times ( OR=1.340, 95% CI: 1.106-1.623, P<0.01), daily working time ≥13 hours ( OR=1.358, 95% CI: 1.107-1.666, P<0.01), insomnia ( OR=2.075, 95% CI: 1.755-2.454, P<0.01), feeling depressed at work ( OR=2.987, 95% CI: 2.516-3.546, P<0.01), degree of emotional exhaustion ( OR=3.801, 95% CI: 3.188-4.533, P<0.01) and degree of de-personalization tendencies ( OR=2.493, 95% CI: 2.086-2.981, P<0.01) were the significant factors influencing the turnover intention of rural general practitioners. Conclusions:Rural general practitioners in southwest Shandong have a high-level turnover intention, part-time jobs, average number of night shift per month, working time, insomnia, depression and job burnout are the main factors affecting the turnover intention. Necessary measures should be taken by relevant departments to enhance the stability of rural general practitioners.

RESUMO

Based on synergetics, system theory and dissipative structure theory, the connotation and composition of the compound system of rural public health service in country area are described in the paper. The authors proposed that the synergy degree model of the compound system for rural public health service in county areas comprises the order degree model and the synergy degree model of the compound system, which are composed of such subsystems as functionality, service network, demand, and economy. They also probed into the evaluation method for synergy degree and built the measuring index system of synergy degree comprising 59 level-2 evaluation indicators.

Objective To measure and analyze the order degree of the internal subsystems and the synergetic degree of the compound system of rural public health service in county areas based on the synergy model of the compound system. Methods According to the distribution of eastern, central and western regions in China, Xiangshan county in Zhejiang province, Wuzhi county in Henan province and Bin county in Shaanxi province were sampled. Based on the index system of synergetic degree measurement including the 52 level-2 evaluation indicators, a questionnaire survey was used to measure the synergetic degree of the compound system of rural public health service in county areas. Results The highest synergetic degrees of the compound system of rural public health service in Xiangshan county, Wuzhi county and Bin county were only 0. 18153, 0. 18068 and 0. 21312 respectively. This indicated the synergy at a low degree. And the synergy of the supply-demand system was an important influential factor for synergy of this compound system, as their development trends were consistent basically. Conclusions The synergy model of the compound system can play a useful role in rural public health service system in county areas. The demand of rural residents deserves more attention and the difference between the service supply and demand should be reduced.

Objective To analyze the coordination and continuity of service delivery for hypertension patients in rural health network,so as to provide references for improving integrated service utilization.Methods Self-developed questionnaire of service coordination and continuity delivery according to the research literature at home and abroad was used as the tool,and the data was analyzed by descriptive statistics.Results 49.6% patients were accompanied by one or more diseases.About 73.0% had seeking care experience in township health centers.Patients with seeking care to two or more medical institutions accounted for 51.3%.Recommending referral institutions for patients with poor therapeutic effect by grassroots doctors were the biggest proportion with 28.4% and 68.7% respectively.The proportion that superior doctors treating patients according to the related information from subordinate institutions was less than 43.0%,the proportion that junior doctors continuing to treat referral patients according to early diagnosis and treatment information was just over 40%.Easier upward referral but harder downward referral also existed among medical settings of different levels.Conclusion The disease characteristics of rural patients increased the possibility of seeking care among vertical medical institutions,but the level of continuity and coordination service delivery was not high in this network.So all-sided service integration to rural tertiary health institutions should be strengthened.

Las consultas por enfermedades dermatológicas en los servicios de urgencia presentan una baja frecuencia y poseen una pobre descripción de sus características en la literatura. A raíz de lo anterior, confeccionamos un estudio descriptivo y retrospectivo de las consultas dermatológicas realizadas en el Servicio de Urgencia del Hospital de Quellón, entre Abril 2010 y Marzo 2011. Los resultados mostraron que las causas dermatológicas representan un 4.9 por ciento del total de consultas. Esta cifra varía durante el transcurso del año, evidenciándose una mayor frecuencia en los meses de verano. Además, se observó que la proporción de consultas de urgencias atribuibles a una enfermedad de la piel es mayor en pacientes pediátricos y adolescentes que en los pacientes adultos. Finalmente, del total de consultas dermatológicas, las etiologías infecciosas y alérgicas fueron las diagnosticadas con mayor frecuencia. No se evidenció una diferencia estadísticamente significativa entre los promedios de consultas pediátrico adolescentes y de población adulta, entre las distintas estaciones del año.


The dermatological consultations in the emergency services have a low frequency and a poor description of its features in the literature. Therefore, we made a descriptive and retrospective study of dermatology consultations conducted in the hospital emergency service of Quellón, between April 2010 and March 2011. The results showed that skin pathology represent 4.9 percent of all consultations. This number varies throughout the year, showing a higher frequency in the summer months. In addition, we observed that the proportion of emergency visits attributable to a skin disease is higher in pediatric and adolescent patients than in adult patients. Finally, for all dermatological consultations, infectious and allergic etiologies were the more frequently diagnosed. No statistically significant difference was showed between pediatric adolescent and adult consultations, during the different seasons.

PURPOSE: The purpose of this study was to describe the status of the functional health literacy of the elderly living in a rural community and to identify the influencing factors on the functional health literacy. METHODS: Subjects (n=238) aged over 60 years were selected for data collection. Functional health literacy was measured by Korean Health Literacy, which consisted of 15 items including eight numeracy and seven comprehension items. Descriptive statistics, t-test, ANOVA with Scheffe test, and stepwise multiple regression were used to analyze the data. RESULTS: The mean score of total functional health literacy was 6.66+/-3.31, numeracy was 3.52+/-1.76, and comprehension was 3.41+/-2.10. Of the subjects, 39.1% showed a low score below the median split. In the results of stepwise multiple regression, the factors affecting functional health literacy were basic literacy, age, number of chronic diseases, self-reported health status, and gender. These factors accounted for 33.0% of functional health literacy. CONCLUSION: These results suggest that interventions for improving functional health literacy are necessary to enhance elders' ability of self-care management. In addition, it is needed to take into account the individual characteristics of the subjects depending on their gender, age, and health status.

RESUMO

Through the analysis of the problems of health service delivery system in rural areas and the causes of the abnormal operation of the three-tier rural health service network, this article probed into the continuity of health service in rural China. These issues are observed from the aspects of continuity of disciplines, institutions, relationship and health information. Policy recommendations include remolding the institutional relations in the three-tier rural health service network, constructing reasonable supporting environment and improving the health delivery quality.

Objective The survey of continuing education demand of our city' s rural health service members and their feel of job satisfacrion provides the basis for overall quality of the rural health service members. Methods We adopted the method of multistage sampling to conduct a questionnaire survey and condoct interview on rural health workers in our city, and then utilized software to analyze them. Results The survey comprises the first education qualification and highest qualification of 120 health workers and 228 rural doctors from health clinics in localities asoond our city, and most of them had technical secondary school education. 80.7% of rural health workers and 91.7% of village health workers were willing to enter medical college to further their education through the "University Student in Every Village Project" . 93.3% of rural health workers and 84.2% of village health workers were willing to take non - qualification education. Among the rural health workers, 29.2% were satisfied with their jobs, 44.2% were not satisfied with them. Among the village health workers, 12.3% were satisfied with their jobs, 67.5% were not satisfied with them. Conclusion We should increase fonding towards rural health workers' education to satisfy the demand of continuing education, solve the problem of pensioner' s insurance, and improve levels of job satisfaction.
