RESUMO

A common clinical problem encountered by colorectal surgeons is the secondary tumors of the ovary (STO), particularly in young female patients. Most STO are from the digestive tract, and the known possible metastatic mechanisms include lymphatic, hematogenous, and intraperitoneal spreading. The molecular and histopathological characteristics of STO from different sites are diverse. It is particularly important to correctly identify the origin and feature of STO, which should be clarified by combining medical history, histopathology, immunohistochemistry, molecular biology, imaging and other means. The prognosis of patients with STO is poor in general. Comprehensive therapies based on surgical resection can benefit some patients. There is no specific treatment for STO at present, but not giving up easily on these patients is the right choice that every surgeon should understand.

RESUMO

Objective To study the feasibility of non-invasive spatially-resolved near-infrared spectroscopy (SR-NIRS) in clinical bedside monitoring of shock.Methods The central venous blood samples of 25 patients with shock were collected and the central internal jugular central vein oxygenation (ScvO2) level was measured.The self-developed non-invasive SR-NIRS device was used to measure tissue blood oxygen saturation (StO2) surrounding the region of jugular central vein.In addition,the artery oxygen saturation (SaO2) and partial pressure of oxygen (PO2) were also measured using conventional methods.The correlation between StO2 between ScvO2,SaO2 and PO2 was analyzed.Results StO2 levels in shock patients were highly correlated with ScvO2 levels (r=0.84,P<0.001) and the concordance coefficient of 0.80 was high.Conclusion StO2 value collected from the surrounding region of jugular central vein by SR-NIRS device can be used as an indicator of shock suggesting the potential of noninvasive SR-NIRS for bedside shock monitoring.

RESUMO

Objective To test the effectiveness, reliability and acceptability of the European Organization for Research and Treatment of Cancer (EORTC) QLQ-STO22 scale in gastric cancer patients in China. Methods One hundred and twenty-eight cases were collected in the Affiliated Cancer Hospital of Xiangya School of Medicine of Central South University from September 2014 to April 2015. All the patients completed the EORTC QLQ-STO22 and EORTC QLQ-C30 scales and given the Zubrod-ECOG-WHO (ZPS) score. Karen Bach coefficient and Pearson correlation test were used for statistical analysis while using ZPS score to detect EORTC QLQ-STO22 in validity. After score was standardized, P<0.05 represented the difference had statistical significance. Results The Karen Bach coefficient was 0.607-0.830, confirming that the EORTC QLQ-STO22 scale had good reliability. A number of enhanced analysis showed that the scale had good convergent validity and divergent validity. In the same or similar dimension, EORTC QLQ-C30 and EORTC QLQ-STO22 scales had good correlation and the correlation scores were higher than 0.400. The patients were divided into four groups according to ZPS score, with ZPS score increase, the overall quality of life scores were decreasing and entries associated with symptoms were increasing, showing difference between different groups(P<0.05). Conclusion The EORTC QLQ-STO22 scale shows high reliability and validity that can be used for assessing the quality of life of patients with advanced gastric cancer in China.

OBJECTIVE: To evaluate the discrepancies between initial STO and final STO in Class III malocclusions and to find which factors are related to the discrepancies. METHODS: Twenty patients were selected for the extraction group and 20 patients for the non-extraction group. They were diagnosed as skeletal Class III and received presurgical orthodontic treatment and mandibular set-back surgery at Pusan National University Hospital. The lateral cephalograms were analyzed for initial STO (T1s) at pretreatment and final STO (T2s) after presurgical orthodontic treatment, and specified the landmarks as coordinates of the X and Y axes. RESULTS: Differences in hard tissue points (T1s-T2s) in the X coordinates of upper central incisor edge, upper first molar mesial end surface, lower central incisor apex, lower first molar mesial end surface and mesio-buccal cusp and Y coordinates of upper central incisor edge, upper central incisor apex, upper first molar mesio-buccal cusp were statistically significant in the extraction group. Differences in hard tissue points (T1s-T2s) in the X coordinates of upper central incisor edge, lower central incisor apex, lower first molar mesial end surface and Y coordinates of lower central incisor apex were statistically significant in the non-extraction group. In the extraction group, the upper arch length discrepancy (UALD) had a statistically significant effect on maxillary incisor and first molar estimation. Lower arch length discrepancy and IMPA had statistically significant effects on mandibular incisor estimation in both groups. CONCLUSIONS: Discrepancies between initial STO and final STO and factors contributing to the accuracy of initial STO must be considered in treatment planning of Class III surgical patients to increase the accuracy of prediction.

The purpose of this study is to examine reproducibility of operation plan and 3-dimentional jaw movement patterns by comparing jaw position of STO with post-operative jaw position. Twenty patients with class III dental and skeletal malocclusion who were treated with Le-Fort I osteotomy and B-SSRO were reviewed. Lateral cephalometric radiographs were taken within two weeks before operation and two days after operation. Cephalometric radiographs were compared and analyzed with orthognathic computer program 'V-Ceph(TM)'. Post-operative maxillary advancement was insufficient compared to maxillary advancement through STO. Post-operative setback movement was over compared to mandibular setback movement through STO. But statistically this is not significant. Maxillary vertical location is insignificant on the whole. Especially post-operative maxillary clockwise rotation is significant compared to maxillary rotation through STO. Post-operative maxillary clockwise rotation tendency is generally observed in all patients. So surgeons and staffs must consider this tendency when operation plan is established ans operation is being performed. Using intra or extra oral marking points, face bow, and bite plate will make exact surgery possible.

Poor circulation is considered to be a cause of stiff shoulders, but there have been no studies on deep hemodynamics and the subjective estimation/palpation of stiff shoulders. We evaluated the relationship between deep hemodynamics and the degree of the subjective estimation/palpation of stiff shoulders by near-infrared spectrophotometry.<br>The subjects were 146 patients who visited our center and 23 healthy volunteers. Deep hemodynamics (tissue oxygen saturation: StO<sub>2</sub>, total hemoglobin concentration: total Hb) was measured in the scapular region of the bilateral shoulders using a deep hemodynamics measurement system (PSA-IIIN, Biomedical Science), and its relationship with the severity of the subjective estimation of stiff shoulders (5-grade rating) and that of palpation (4-grade rating) was evaluated. Deep hemodynamic values were affected by the body mass index (BMI) that is highly correlated with subcutaneous fat thickness. Therefore, analysis was performed in 70 patients and 8 healthy volunteers with BMI of 20-24 that does not affect hemodynamic values. Compared with the healthy volunteers, patients who reported marked shoulder stiffness showed a significant decrease in total Hb, and that who reported shoulder stiffness showed a significant decrease in StO<sub>2</sub>. On the other hand, compared with the healthy volunteers, patients with marked shoulder stiffness observed by palpation showed significant decreases in both StO<sub>2</sub> and total Hb; the decreases were more marked with more marked stiffness. These results suggested that deep hemodynamics is a diagnostic parameter of stiff shoulders.

Objectives:To translate the English version of Quality of Life Questionnaire of Stomach 22 into Chinese,and to test the reliability and validity of the Chinese Version of QLQ-STO22.Methods:From 1st June to 31st December,2003,140 patients with gastric cancer were sampled as study participants in three hospitals using cluster sampling method.All participants were interviewed with QLQ-STO22 Chinese version by the investigators who were trained in advance.Results:Nearly all ICCs of scales of STO22 were above 0.75;the split-half reliability coefficient is 0.78 and the Cronbach'a coefficient is 0.80.These results proved that the questionnaires had good test-retest reliability,split-half reliability and internal consistency.Three common factors were extracted by factor analysis,which ccould account for more than 60% of total variance and factor loads of the three common factors were above 0.5 in related items.Conclusion:QLQ-STO22 has good reliability and validity,which is available for the study of life quality among Chinese gastric cancer patients.
