RESUMO

Coronavirus disease emerged in Wuhan, China in December 2019 and led to worldwide pandemic in March 2020. Due to early diagnose, treatment and prevent transmission of COVID-19 disease, we need rapid laboratory tests and develop them. This paper focuses on serum Iron level in patients with COVID-19. We assess the serum Iron level due to the following reasons: I. Iron is a key part of hemoglobin structure in which is essential for providing Oxygen to the body organs, particularly for the patients with pulmonary involvement such as COVID-19. II. Iron deficiency can contribute to muscle weakness and reduction of respiratory capacity, at which increases the risk of deterioration of COVID-19 patients. III. Elevation in Hepcidin level (due to the increase in IL-6 level) in COVID-19 patients inhibits Iron absorption from intestinal lumen and blocks the Iron release from macrophages. IV. Iron is an essential element in infectious suppress and inflammatory process. We noticed that most of the patients, especially admitted to hospital due to the respiratory symptoms, have lower serum Iron level.

RESUMO

Background: One of the important factors associated with maternal and foetal complications during pregnancy is Anaemia. Various oral preparations of iron are available, and each has different bioavailability, efficacy and adverse effects. Lactoferrin is a naturally existing iron-binding multifunctional glycoprotein, and a member of a transferrin family, thus belonging to those proteins capable of binding and transferring iron. Lactoferrin has considerably less gastrointestinal side effects than ferrous sulfate and is very useful as well as promising alternative to ferrous sulphate. Methods: Prospective randomized controlled study. Total 100 females with 24 to 36 weeks of pregnancy with haemoglobin between 8 to 10 grams were included out of which 50 patients were given ferrous sulphate 200 mg BD and 50 patients were given lactoferrin 250 mg BD daily for 8 weeks. Various haematological parameters and the adverse effects of both the drugs were studied at registration, 4 weeks and 8 weeks and compared. Results: Thus, after this study authors can say that the rise in haemoglobin with lactoferrin was 1.58 g/dl while with ferrous sulphate it was 1.67 g/dl at 8 weeks. Adverse effects were much lesser in Group A taking lactoferrin compared to Group B.Conclusions: Thus, lactoferrin has the advantage over ferrous sulphate in having   less side effects and increasing the compliance and thus the efficacy of the drug compared to ferrous sulphate.

RESUMO

Introduction: Gestational Diabetes Mellitus is thedevelopment of carbohydrate intolerance of variable severitywith onset or first recognition during pregnancy. Many studieshad suggested that an elevated serum ferritin level in maternalblood in early as well as mid-pregnancy is an independent riskfactor for development of GDM. In the present study we aimto find the association of serum ferritin levels with serum ironand Hb% in the GDM patients at the time of delivery and alsocorrelate it with cord blood Hb% and iron levels of the newborn.Material and Methods: The study group was composed of50 diagnosed cases of GDM and the control group comprisedof age matched 50 cases of normal pregnancy. Maternal bloodwas used to measure mother’s hemoglobin, iron levels, serumferritin and hsCRP. Cord blood sample was used to estimatehemoglobin and iron levels of the newborns. Results: Our study shows that in the GDM cases the levelof serum ferritin was significantly higher (p <0.001) than inthe non GDM controls at the time of delivery. Cord bloodhemoglobin is negatively correlated with maternal serumferritin levels in GDM.Conclusion: Elevated serum ferritin level in GDM is a markerof inflammation due to increased ROS production caused byiron overload. This oxidative stress might affect the placentaliron transfer to the fetus and fetal Hb synthesis.

RESUMO

Background: Non-Alcoholic fatty liver disease (NAFLD) has become a worldwide health concern with increase in the global incidence of obesity and it is now considered the hepatic component of the metabolic syndrome. Aims and Objective: The study’s aim was to compare the indices of the Liver Function tests in compensated chronic liver disease patients with and without NAFLD. Materials and Methods: A total of 100 consecutive patients with compensated chronic liver disease were recruited into the study. A structured questionnaire was administered to obtain relevant socio-demographic data. NAFLD was diagnosed based on clinical, biochemical, ultrasonographic and in a few histological features. The Adult Treatment Panel III criteria were used to identify patients with the metabolic syndrome. Results: In our study, 100 participants were recruited into study. 40 out of 60 patients (67%) showed grade-1 fatty liver findings and 20 out of 60 patients (33%) showed grade-2 fatty liver. Approximately 19% had fatty liver finding. The mean (SD) age of persons with NAFLD was 45.12 (±8.07) years compared to 47.49(±11.79) years for persons without NAFLD. The difference was not statistically significant (p=0.2). Body mass index (BMI), central obesity (waist circumference), fasting blood sugar, blood pressure, total cholesterol and triglycerides were significantly higher in the NAFLD group (p= <0.05) respectively. Conclusion: Indices of the deranged Liver functions were more prevalent in persons with NAFLD. It is recommended that patients with NAFLD be screened for metabolic syndrome and appropriate therapy instituted to decrease the risk of both hepatic and cardiovascular complications.

RESUMO

Gangetic plains are known to have a very high incidence of cholelithiasis, the eiopathogenesis of which cannot be ascertained. Aim: to conduct a study to find a correlation, if any between the incidence of gallstone disease and the serum Iron and serum Calcium levels of patients and compare it with controls. Methods: 50 patients and 50 controls were taken and serum Iron and serum Calcium of both were evaluated. The distribution of age, sex and gender were taken as factors. Results and conclusion: The number of female patients was higher than the males but no strong association could be proven in the study. Among the symptoms dyspepsia had the highest incidence but could not be taken as a reliable marker for the diagnosis of cholelithiasis clinically. Hence, ultrasonography remains a very important tool for the diagnosis of the diasease .The role of serum Iron and serum Calcium in the etiopathogenesis could not be established. There was however a strong correlation of previous surgery in the formation of new gallstones .The family history was a strong factor for the formation of new gallstone disease however, the association was not established in this study. Gallstone disease had stronger association with anaemia than with serum Serum Iron. Calcium levels were comparable between the cases and the controls.

RESUMO

Worldwide, iron deficiency anaemia (IDA) is one the most significant nutritional maladies, especially in low and middle-income countries. This is mainly due to the associated morbidity, economic implications on the health system and the welfare state. One of the most prevalent causes of IDA in children is insufficient food intake, which has an obvious connection with the social environment. The main strategy in the management of this condition is counteracting such deficiency through the improvement of nutrition and family upbringing. Aiming to explore affordable, novel and straightforward approaches to treat this nutritional condition, we developed a descriptive study associated with the incorporation of Moringa Oleifera in homemade fruit beverages delivered to 32 children from low-income families, in order to assess the evolution of IDA, evaluating specific clinical parameters such as red blood cells volume, as well as hemoglobin, ferritin and serum iron levels, within a two months lapse. At the end of this period, we observed a significant statistical rise in the levels of all the assessed parameters, finding about a 1.3-fold increase in hemoglobin and ferritin levels and in red blood cells volume; serum iron was 1.1 times higher. These findings are demonstrative of the wide spectrum of the medicinal features of Moringa and establish a promising and accessible strategy easy to incorporate into the diet of families of children suffering this pathology(AU)

Background: In patients with CKD and diabetes combined, anemia may be relative or absolute. If the serum ferritin is more than or equal to 100ng/ml associated with reduced iron saturation, then it is defined as functional iron deficiency anemia. This type of anemia is very common in patients with CKD. To compare serum iron, TIBC, transferrin saturation and serum ferritin in anemia of chronic renal diseases with healthy controls. Methods: A hospital based comparative study was carried out among 30 known cases of chronic kidney disease with anemia. They were compared with 20 age and sex matched healthy control who were free from chronic kidney disease and anemia. The parameters like serum iron, TIBC, transferrin saturation and serum ferritin were compared between the two groups. Student’s t test and a two tailed p value were calculated and if the p value was less than 0.05, it was taken as statistically significant. Results: It was seen that the mean hemoglobin value was significantly less among CKD patients compared to healthy controls (p<0.05). Serum iron was also significantly less among CKD patients compared to healthy controls (p<0.05). TIBC as significantly high among CKD patients compared to healthy controls (p<0.05). This is because of low hemoglobin and low serum iron in CKD patients but again the transferrin saturation was significantly low among CKD patients compared to healthy controls (p<0.05).Conclusions: Anemia prevalence was very high in CKD patients. Hemoglobin, serum iron and transferrin saturation were significantly low and TIBC was significantly high.

RESUMO

Purpose: Iron deficiency anemia is the most occurring formof anemia in India. Hemoglobin A1c (HbA1c) is used in diabeticpatients as an maker to reflect glucose levels of the last 3months. Like blood sugar levelsHbA1c levels are also affectedby presence of variant hemoglobin, hemolytic anemia, nutritional anemias, uremia, pregnancy and acute blood loss. However, reports on the effects of iron deficiency anemia onHbA1clevels are inconsistent. We conducted this study with anaim to analyze the effects of iron deficiency anemia onHbA1clevels among non-diabetics.Methods: 200 patients with Hb<10g/dl underwent peripheralblood film testing. All patients with microcytic hypochromicanemia underwent iron studies .Out of these, 100 non diabeticpatients diagnosed with iron deficiency anemia underwentHbA1c test and data was statically analysed. Results: We found that incidence of iron deficiency anemiawas 54%. Serum iron and ferritin concentrations are inverselyassociated with HbA1c concentration and that HbA1cconcentration tended to be higher in the presence of irondeficiency anemia. Conclusion: This study found a positive correlation betweeniron deficiency anemia and increased HbA1c levels. Hence,Iron deficiency anemia has to be kept in mind before using theHbA1c to diagnose diabetes.

RESUMO

Background: Iron deficiency anaemia still remains the most common cause of anaemia not only in India but also world over. According to world heath report, there are 1,788,600 people in this world suffering from Iron deficiency anaemia. Iron deficiency anaemia is foremost prevalent disease-causing morbidity in world and therefore it is always absolutely necessary to detect this particular condition in early stages before the eventual development of various dreadful complications like Heart failure and Myocardial infarction. The aim of the study is to find incidence of iron deficiency anaemia in patients with hypo proliferative anaemia presentation, with a possible iron deficient state, by analyzing the haematological and biochemical parameters. Methods: The study was conducted from November 2017 to May 2018 for a period of 6 months which included 50 subjects from both sex groups, aged 20-80 years with the diagnosis of hypo proliferative anaemia. Results: The study results indicate that females (60%) were significantly overrepresented compared to males (40%). Of the 50 subjects 38% were in stage of negative iron balance (stage1) and 32% were in stage of iron deficient erythropoiesis(stage2) and 30% were in normal stage. Conclusions: This Observational study showed a majority of patients with hypo-proliferative anaemia presenting at early stages of negative iron balance and iron deficient erythropoiesis thereby indicating the importance of initiating iron therapy at an early stage even without correlative iron studies.

RESUMO

Background: Malnutrition is an important factor that influences the course of pregnancy and fetal development. It is a serious international problem which leads to a deficiency of metals, trace elements, vitamins, and proteins. Much attention has been given to the effects associated with protein deficiency. Nowadays the effects due to the deficiency of trace elements have been given much importance. Trace elements are essential for life. Deficient intake- leads to impairment of some functions. The impairment may be corrected by intake of the element in physiological amount. With the deficiency of these elements, the organisms can neither grow nor complete its life cycle. Death of an organism results when there is an absolute deficiency. Aim: To compare serum iron and zinc level in term and preterm babies. Materials and methods: Sixty pregnant women who came from in and around Chidambaram who fulfilled the inclusion criteria during the period from July 2001 to March 2002 were included in this study. Among them, 50 delivered at term (37 to 42 weeks) and the rest delivered prematurely (< 37 weeks). Serum iron and zinc level were estimated by standardized technique. The results are analyzed accordingly. Results: The estimated mean iron value for the term babies was found to be 114.34 μg/dl with the standard deviation of 34 μg/dl. In case of preterm babies, the estimated mean value for iron was found R. Manimozhi Malathi, S. Ramesh, D. Rajkumar. Comparative study of serum iron and zinc levels in term and preterm babies delivered in RMMCH. IAIM, 2018; 5(9): 58-62. Page 59 to be 97.35 μg/dl with the standard deviation of 46 μg/dl. The mean zinc value of term babies was found to be 76.26 μg/dl with the standard deviation of 23 μg/dl. For preterm babies mean zinc value was found to be 56.53 μg/dl with the standard deviation of 29 μg /dl. Conclusion: Preterm birth deprives the fetus of a significant accumulation of iron in storage that occurs beyond 32nd week, total body iron deposits in tissues, Hb and zinc are low in premature babies. Iron deficiency affects perinatal growth, maturation, and function of multiple organ systems including the heart, skeletal muscle, gastrointestinal tract, and brain. Anemia of prematurity has been defined as low hematological constant levels of hemoglobin (Hb), hematocrit (Ht), serum iron, number of erythrocytes, the reticulocytes.

RESUMO

Objective To observe the changes of serum iron in patients with bronchopneumonia (bacterial pneumonia, mycoplasmal pneumonia) before and after treatment, and to discuss whether it can be used as an indicator of infection. Methods Forty cases with bacterial pneumonia and 41 cases with mycoplasmal pneumonia were recruited. The serum iron levels before and after treatment in bacterial pneumonia group and mycoplasmal pneumonia group were compared and analyzed. The correlation between white blood cell count, percentage of neutrophils, serum C-reactive protein (CRP) levels and the serum iron levels before treatment, and changes of the serum iron before and after treatment were analyzed. Results The level of serum iron after treatment in the bacterial pneumonia group [(16.28 ±5.81) μmol/L] was significantly higher than that before treatment [(4.83 ± 2.12) μ mol/L] (t =-11.19,P<0.001).The level of serum iron after treatment in the mycoplasmal pneumonia group [(15.17 ±5.31) μmoL/L] was also significantly higher than that before treatment [(4.77 ± 1.99) μmol/L] (t =-11.29,P <0.001).The serum iron levels between the two groups before and after treatment had no statistically significant differences (t =0.135,0.898,P =0.893,0.373).There was no correlation between white blood cell count, percentage of neutrophils,CRP and serum iron levels before treatment (bacterial pneumonia group:r =-1.87,-0.219,-0.152;mycoplasmal pneumonia group:r =-0.032,-0.302,-0.274) and changes of the serum iron before and after treatment (bacterial pneumonia group:r =0.098,0.062,0.205;mycoplasmal pneumonia group:r =0.01 1,0.171,-0.105,P > 0.05).Conclusion The serum iron level is significantly decreased in children with pulmonary infectious diseases and increased to normal level after anti-infection treatment.Serum iron can not be used as an indicator of infection in children.

The aim of this work was to evaluate the evolution of the analytical performance of serum iron determination by the laboratories participating in the Sub- Program PEEC-Hematology (PEEC-H) EQAS Program Prof. Dr. Daniel Mazziotta of the Argentine Biochemical Foundation. To this end, results of serum iron determinations from July 2010 to July 2015 (surveys #77, 81, 85, 89, 93 and 97) were used. Up to 2011, there were methods both with and without deproteinization, the latter being the most used (94%). In 2015, only one commercial method without deproteinization was used, with colorimetric methods employed in 95% of the cases. In all the surveys analyzed, the acceptable DRP was ±10%. In surveys 77, 81 and 85, 56% of the laboratories had an acceptable performance percentage, and it evolved to a 70% in the surveys 89, 93 and 97, three years later. According to these considerations, there is no need to adjust the acceptable DRP for the iron analyte. In this way, laboratories will continue to work in order to improve their performance.
