RESUMO

@#AIM: To investigate the effect of silicone hydrogel soft contact lens in the treatment of persistent corneal epithelial defect. <p>METHODS: The clinical data of 56 patients(56 eyes)with persistent corneal epithelial defect treated in our hospital from January 2014 to January 2017 were retrospectively analyzed; 21 patients in the control group received hydrogel contact lenses, and 35 patients in the observation group received silicone hydrogel soft contact lens. Then the clinical effect, the classification of corneal opacity, the improvement of visual acuity, the subjective comfort and complications of the patients were recorded. <p>RESULTS: Wilcoxon rank sum test showed that there were significant differences in the clinical efficacy between the two groups(<i>P</i>=0.042). The total effective rate of the observation group was significantly lower than that of the control group(94% <i>vs</i> 76%). There was no significant difference in Haze classification between the groups after 3d of treatment(<i>P</i>>0.05); after 1wk and 1mo of treatment, there were significant differences in Haze classification between groups(<i>P</i><0.05); after 1mo of treatment, the proportion of Haze grading 0 in the observation group was significantly higher than that in the control group(49% <i>vs</i> 19%), there was significant difference between the two groups(<i>χ</i><sup>2</sup>=4.481, <i>P</i>=0.027). There was a significant difference in visual acuity between the two groups(<i>Z</i>=-3.347, <i>P</i><0.05); the visual acuity of 91.43% in the observation group were improved, which was significantly lower than that of the control group(52.38%, <i>P</i>=0.002). There was no significant difference in the comfort scores between the two groups before treatment(<i>P</i>>0.05). After the first day of treatment, the comfort scores of both groups increased. The comfort scores of the observation group after treatment 1d, 3d and 1wk were significantly lower than those of the control group(<i>t</i>=-17.422, -15.827, -16.588; <i>P</i><0.01). The average healing time of corneal epithelium in the observation group was 4.25±1.05d, and the control group was 5.96±2.16d(<i>t</i>=-3.395, <i>P</i><0.05). The incidence of adverse reactions in the observation group was lower than that in the control group(<i>P</i><0.05). <p>CONCLUSION: Silicon hydrogel soft corneal contact lens can be used in the treatment of persistent corneal epithelial defect.

AIM: To investigate the clinical safety and effectiveness of the two kind of bandage contact lenses: Senofilcon A (Johnson & Johnson Acuvue Oasys) and Balafilcon A ( Bausch& Lomb pure vision ) after laser - assisted subepithelial keratectomy (LASEK). METHODS: Thirty - eight patients (76 eyes) who had undergone the LASEK were divided into two groups. One group of patients wore Balafilcon A, the other group of patients fitted with Senofilcon A. The lenses were worn continuously for 7d. This was a seven - day experience and the patients scored for the symptom of the eyes (sore eyes, foreign body sensation and tearing) on the third day and the seventh day. Both of the two groups of patients taken off the soft contact lens on the seventh day and let their vision and corneal staining checked. RESULTS: The symptoms of eye sore and tearing of the two groups patients were different. The patients who wore the Senofilcon A were better. The pain of eyes were also different at 3 and 7d after surgeries(Z = - 4. 146, P =0. 000; Z= - 2. 814, P = 0. 005). The difference on tearing between the two groups at 3 and 7d after surgeries were significant ( Z = -2. 309, P = 0. 021; Z= -3. 276, P= 0. 001). There was no difference on sensation of dryness between the two groups at 3 and 7d after surgeries (Z= -0. 447, P=0. 655; Z= -0. 966, P = 0. 334). After the lenses were taken off, the visual acuity of patients wearing Senofilcon A was better ( t = 3. 800, P = 0. 001 ); corneal staining showed limited spots in 1- 2 quadrants with significant difference (Z= -2. 384,P= 0. 017). CONCLUSION: The Senofilcon A ( Johnson &Johnson Acuvue Oasys ) and Balafilcon A ( Bausch& Lomb pure vision) bandage contact lenses are safe and effective after LASEK, and the former is better than the latter in epithelial regeneration.

Aim: Detection of keratometric differences induced by daily wear of second generation silicone hydrogel (SiH) contact lens (CL) with hydraclear. Study Design: Prospective cross-sectional study. Methodology: The researchers investigated the difference between the keratometric values at equivalent spherical and cylindrical diopters of daily wearers of second generation SiH CL and those in an age-matched control group who wear only spectacles because of SiH CL’s stiffer moduli. The study also examined the effect of second generation SiH CLs on the corneal topography. Seventy asymptomatic, biomicroscopically normal eyes of SiH CL wearers and 70 eyes of non-lens wearer control subjects have been included in this study. The SiH CL wearers have been wearing lenses in the range of less than 6 months to 3 years. General Linear Models (GLM) with type III sum of squares was used (SPSS 15.0 for Windows). Results: Statistical analysis showed no significant difference between two groups regarding keratometric values (p > 0.05). Conclusion: Results suggest that daily wear of second generation SiH CL with hydraclear, which havemoderate modulus and higher oxygen permeability, does not affect the central keratometric readings.

RESUMO

Desde sua concepção, por Leonardo da Vinci há mais de 500 anos, as lentes de contato têm apresentado relevantes avanços tecnológicos, principalmente a partir do início do século passado. A permanente introdução de novos materiais e desenhos produziu, ao longo do tempo, a designação de ?gerações? de lentes de contato, de forma a facilitar sua classificação para o oftalmologista. Este artigo discute a real relevância e utilidade existentes neste tipo de classificação quanto à escolha da melhor opção terapêutica para a correção dos diversos vícios de refração na prática clínica.

RESUMO

Os mecanismos envolvidos na estabilização das lentes de contato gelatinosas para a correção do astigmatismo representam fator importante na adaptação e manutenção a longo prazo pelos pacientes. A despeito dos avanços recentes observados em relação aos materiais, em especial com a introdução das lentes de silicone hidrogel, durante praticamente 30 anos nenhuma novidade relevante foi apresentada em relação aos desenhos de estabilização das lentes, prevalecendo o uso dos desenhos à base de prisma de lastro e dupla zona fina. Recentemente foi introduzida em nosso meio uma lente de contato com um mecanismo de estabilização nomeado ?desenho de estabilização acelerada?. Este artigo se propõe a rever este tema e analisar comparativamente os dados disponíveis sobre esta recente tecnologia.
