RESUMO

Objective To establish a rat model of superior mesenteric vein thrombosis by vein ligation and to simulate the pathological process of the disease, and to provide the basis for studies of its pathogenesis and treatment.Methods Ninety-six SPF male SD rats were randomly divided into three groups: Group A (sham operation group), group B (strangulation group) and group C (simple group), 32 rats in each group.Rats in group A were only opened the abdominal cavity but not blocked the blood supply.The rats were sacrificed at 8, 24, 48 and 72 h after operation.The rats in groups B and C were subjected to establish the strangulation and simple models by superior mesenteric vein thrombosis, respectively, and were sacrificed at 8, 24, 48 and 72 h after modeling.Histological changes (H&E staining) in the rat intestinal tissues were evaluated by a pathological scoring system.The levels of intestinal fatty acid binding protein (IFABP) and α-glutathione S-transferase (α-GST) were detected by ELISA.Results The rat model of mesenteric vein thrombosis was successfully established, with a success rate of 100% (96/96).The pathological analysis revealed that compared with the group A, different degrees of blood stasis and injuries were observed in the intestinal tissues of groups B and C, and the injury were gradually increased in the group B, while gradually reduced in the group C.The degrees of blood stasis and injury were positively correlated with the scope of ligation.The result of ELISA showed that the serum levels of IFABP and α-GST of the rats in groups B and C were significantly higher than those in group A (P < 0.05), and the degree of elevation was positively correlated with the scope of ligation.Conclusions In this study, the rat model of superior mesenteric vein thrombosis is successfully established by vein ligation.This model is simple and easy to operate with a high success rate, and can be used in related research.

Most reported cases of lower respiratory tract infection due to herpes simplex virus type-1 (HSV-1) occurs in immunocompromised patients, however there are few cases of viral infection in inmunocompentes critically ill patients (1-3). Objective: Report the case of fatal VHS-1 pneumonia in an immunocompetent patient. Case: A ten year old girl, immunocompetent, who presents respiratory symptoms and progresses rapidly to severe hypoxemia, instability and death. Lung biopsy reported Cowdry type A inclusions and the polimerasa chain reaction (PCR) was positive for HSV-1. We discuss the clinical features, diagnosis, treatment and prognosis in critically ill immunocompetent patients with HSV-1 infection in lower respiratory tract.


La mayoría de los casos reportados de infección por el virus herpes simplex tipo-1 (VHS-1) en el tracto respiratorio inferior ocurren en pacientes inmunosuprimidos; sin embargo, se han reportado casos de aislamiento del virus en pacientes inmunocompentes en estado crítico. Objetivo: Reportar el caso de neumonía grave y fatal por VHS-1 en un paciente inmunocompetente. Caso Clínico: Paciente de 10 años, sexo femenino, inmu-nocompetente, que inicia síntomas respiratorios y progresa rápidamente a hipoxemia severa, inestabilidad y muerte. La biopsia pulmonar reportó inclusiones de Cowdry tipo A y una reacción en cadena de la polimerasa (PCR) positiva para VHS-1. Discusión: Se comenta el cuadro clínico, diagnóstico, tratamiento y pronóstico en pacientes inmunocompetentes críticamente enfermos con infección por VHS-1 en el tracto respiratorio inferior.

Objective To explore the therapeutic effect of Yupingfeng powder in secondary immunodeficiency caused by simple nephrotic syndrome(SNS) in children.Methods Seventy-eight cases of SNS patients were divided into 2 groups randomly.One was control group which were treated with immunosuppressant only.The other group was Yupingfeng powder group which were treated with both immunosuppressant and Yupingfeng powder.The morbidity of infection,the relapse rate of SNS,and serum immunoglobulin IgA and IgG and the T-lymphocytes subtype were detected before and after treatment in two groups.Results The morbidity with infection was 55.3% in Yupingfeng powder group,which shows a remarkable decrease compared with control group(85%).The relapse rate of SNS was 39.5% in Yupingfeng powder group which was much lower than that in control group(67.5%).The level of serum IgA and IgG before treatment were(3.88?1.22) g/L and(0.83?0.14) g/L and rose up to(10.06?1.89) g/L and(1.38?0.10) g/L after treatment in Yupingfeng powder group.Whereas the control group with(6.23?1.55) g/L and(0.85?0.13) g/L was remarkably lower than those in Yupingfeng powder group.CD4+,CD8+ and the ratio of CD4+/CD8+ were(37.91?3.89)%,(30.69?3.08)% and(1.24?0.49)% respectively before treatment.They rose up to(42.38?4.89)%,(26.01?2.20)% and(1.63?0.41)% respectively in Yupingfeng powder group,while those in control group were(39.87?3.91)%,(29.76?3.55)% and(1.34?0.24)% respectively after treatment.Conclusion It is feasible to treat the Yupingfeng powder to cure the of secondary immunodeficiency caused by SNS in children.
