RESUMO

Objective@#To explore the application effect of self-made nursing strategy of device in the severe diarrhea or fecal incontinence.@*Methods@#A total of 60 patients with severe diarrhea or fecal incontinence were randomly divided into study group and control group according to the order of admission, each with 30 patients. The control group received traditional nursing strategy, while the study group was applied self-made perianal skin nursing strategy of device. The change of perianal skin injury, nursing workload and patients′ pain index of two groups were compared.@*Results@#The study group perianal skin injury, nursing workload and patients pain index were (0.40±0.11) points, (2.14±0.22) times/day, (1.36±0.40) points, which were better than (0.84±0.14) points, (3.58±0.29) times/day, (3.28±0.60) points in the control group (t=2.475, 3.934, 2.652, P<0.05).@*Conclusion@#self-made perianal skin nursing strategy of device could reduce the incidence of perianal skin injury, liberation of nursing work, reduce the pain of patients. Which is worthy of clinical application for its convenience.

Objective Incontinence-associated dermatitis(IAD) is a common skin disease, but there is still lack of evidence on which kind of skin proctectant can provide effective prevention and cost reduction. The article aimed to explore the clinical effect and cost-effectiveness of two skin protectants based on structured skin care regimens in the nursing care of incontinent patients and provide evidence for effective prevention of IAD.Methods A total of 124 patients with incontinence were randomly selected and divided into observation group (62 cases) and control group (62 cases). On the basis of the structured skin care program which focused on incontinence management, cleansing and protection, the patients in the observation group were treated with the composite zinc oxide ointment for 7 days, while patients in control group were treated with ostomy powder combined with skin barrier film for 7 days. The main observation indicators were the incidence of IAD and the cost of nursing materials.Results All 62 patients in the observation group completed the study, while in the control group 2 patients fell off because of death and discharge from hospital and 60 patients completed the study. The per-protocol analysis showed that the incidences of IAD in two groups were 6.45% and 20.00%, the intention-to-treat analysis showed 6.45% and 22.58%, and there was significant difference between groups (P<0.05). The nursing cost of the observation group (239.71±48.55) was lower than that of control group (264.35±61.55), and the difference was statistically significant (P<0.05).Conclusion The use of the composite zinc oxide ointment based on a structured skin care regimen can improve the preventive effect of IAD and reducing costs.

RESUMO

Objective To understand the etiology and clinical manifestation of malignant atrophic papulosis,to report the psychological care of one case with malignant atrophic papules,and patients to maintain a good psychological state.Methods Nursing measures such as strengthening skin care,reducing discomfort,avoiding aggravating the injury; attention to abdominal signs,detection of changes in disease condition,prevention of intestinal perforation,related medication education to increase medication compliance were given to this patients.Results The abdominal pain disappeared,lower limb muscle force recovered,and the patient was discharged after the disease condition was steady.Conclusions To patients with malignant atrophic papulosis,early detection and early treatment should be given to them.
