In recent years, health disparity has become more obvious and people from diverse backgrounds suffer from various kinds of problems. It is crucial to foster doctors who can respond to social needs. I participated in a five-week elective course during his 3rd year of Juntendo University School of Medicine (2015). The course focused on health inequity and social determinants of health (SDH). As a student, I found it extremely important for medical students to learn about SDH. In this report, I will share my experiences as a junior doctor to reflect on what I learned then and how that affects my practice.

The rapidly aging society gives rise to sizable demands for chronic disease nursing care. With IT development as backup, the hospital has developed a platform for regional collaborative nursing informationization within a medical alliance and a health management platform for home care of the elderlies. This practice can build an extended nursing care model, featuring the three main service lines of " hospital-family, hospital-medical alliance and hospital-community hospital-family". The model provides diversified, personalized, disease specific, informationization and multi-disciplinary coverage for nursing care, providing the primary level with high quality nursing resources.Medical alliances and corresponding communities share information, for a full-range, full region and full timeframe coverage of nursing health supervision and scientific guidance, thus effectively minimizing the rate of revisit to upper-level hospitals by patients and implementing the hierarchical medical system.

Objective To explore the differences between the employment quality of graduates ma-joring in Chinese medicine and the demand of employers for providing theoretical reference for improving the employment rate of students in Chinese medicine colleges and universities. Methods The questionnaire on the employment quality of graduates majoring in traditional Chinese medicine and the needs of employers was designed based on the interviews and literature review. The questionnaire contains the dimensions of knowledge, ability and professional accomplishment, with several observation points in each dimension. Each observation point is measured by Likert 5 points, with "5" being the most important and "1" the least important. During the 2017 campus recruitment period of Heilongjiang University of Traditional Chinese Medicine, 350 questionnaires were issued to the graduates and 312 valid questionnaires were collected through convenient sampling, while 228 questionnaires were issued to the employers and 195 valid were collected. The database was established by using Excel software and the independent sample t test was conducted by using SPSS 22.0. Results The differences between graduates and employers were statistically significant in academic performance, knowledge structure, internship experience, professional conformity, scientific crea-tivity, experience of student cadres, CET-4 & CET-6 certificate, physical and mental health, honesty and trustworthiness (t=9.016, P=0.022; t=9.498, P=0.000; t=2.303, P=0.031; t=11.000, P=0.000; t=6.815, P=0.000; t=2.208, P=0.035; t=3.565, P=0.041; t=3.066, P=0.005; t=4.441, P=0.036). Conclusion There is a gap between the employment quality of graduates majoring in traditional Chinese medicine and the demand of employers. Traditional Chinese medicine colleges and universities can improve the employment quality of graduates and promote their successful employment by accelerating the course integration reform, and strengthening the construction of school spirit and credit evaluation system.

OBJECTIVE:To provide reference for cultivating high-level comprehensive medical talents with high quality. METHODS:Based on the market and social needs,the teaching reform was conducted from the concepts of educational ideas, modes,curriculum system,contents,management and other aspects. RESULTS:In order to adapt to the needs of society and era, it should establish large education educational ideas to cultivate student's independent ability to obtain knowledge and self-develop-ment to make people change into community basic services and service and dedicate society with modes of cultivating inter-disci-plinary talents,including combination of prduction and study,foundation strengthening,professional expanding,penetration of sci-ence and technology and combination of arts and science;generally optimize curriculum system with theprinciple of precise defini-tion,clear classification,and optimized combination based on market and social needs;and update contents timely and teaching materials;the teaching management should be changed from closed off management of the past to the external management which dominated by the market assessment. CONCLUSIONS:Reform of medical higher education based on the market and social needs can cultivate the talents with high quality and comprehensive medical knowledge for the social needs,which is conductive to the de-velopment of medical industry.

BACKGROUND: It is important for the health care provider, particularly primary care physicians as gatekeepers of health care, to understand the social interests and needs toward health care. This study was done to find ways to deal with public opinion by analyzing the contributions of health care in the newspapers. METHODS: Two hundred twenty four contributions about health care were sorted out in the three national newspapers during one calendar year from January to December, 1996. These contributions were coded by themes and subjects according the content analysis and qualitative text interpretation. RESULTS: The contributions were classified into three categories, medical insurance, and hospital services, and health care policies. Forty four contributions of medical insurance category were coded into 5 themes and 18 subject. Fifty eight contributions of hospital services category were coded into 3 themes and 31 subjects. One hundred twenty two contributions of medical insurance category were coded into 8 themes and 32 subjects. The themes and subjects were listed and findings were described qualitatively. The characteristics of public opinions about health care were diversity, conflicting interests, plentifulness of complaints and discontent, and finally, infrequency of alternative proposals for problem solving. How to deal with public opinions were discussed. CONCLUSIONS: We can recognize the public attitude of health care and complaints of patients and consumers of health care by analyzing the contributions in the newspapers. These data can be used to develop ways of primary care physicians to deal with patients' needs.

This is a report of the activities of the committee on medical student selection 1994-1996, particularly focused on the 15th Conference on Medical-Student Admission held 1996/8/31 with the subjects of social needs and influences upon high school education for the purpose of improving student selection system in Japan. We must consider how largely admission tests have being influenced high school students at the time of decision making, what medical schools they submit their applications to, and what ability the society or community requires physicians, for creating better system of evaluation for admission in Japan.
