RESUMO

Xiaoyaowan has the effects of soothing the liver, invigorating the spleen, regulating menstruation, and nourishing blood. It is often used to treat the distending pain of the chest and hypochondriac, loss of appetite, dizziness, and irregular menstruation caused by liver depression and spleen deficiency. The formula originated from the Sinisan of Treatise on Cold Damage(《伤寒论》) in the Han dynasty and was officially formulated in the Song dynasty's Taiping Royal Prescriptions(《太平惠民合计局方》). In the Ming and Qing dynasties, it was developed into more comprehensive formulas such as modified Bazhentang, modified Xiaoyaowan, and other formulas. In recent years, Xiaoyaowan has become a classic formula for treating many symptoms of liver depression. The Pharmacopoeia of the People's Republic of China(《中华人民共和国药典》), 2020 edition, records Xiaoyaowan and modified Xiaoyaowan. Modern clinical research has further expanded the therapeutic range of this formula. Through literature research, it is found that there are certain reports on the clinical application and quality analysis of Xiaoyaowan, but the relevant literature lacks collation so far. Therefore, relevant literature was consulted and sorted out. The paper summarized the treatment of mammary hyperplasia, depression, irregular menstruation, melasma, menopausal syndrome, and polycystic ovary syndrome with Xiaoyaowan, as well as the quality analysis of paeoniflorin, ferulic acid, and glycyrrhizinic acid of Xiaoyaowan. It is expected to lay a foundation for further research on clinical application, pharmacodynamic mechanism, and quality control of this classic formula.

RESUMO

Objective:To evaluate the clinical effect of treatment of activating blood and removing blood stasis, invigorating the spleen and soothing the liver for the patients with gastric collateral stasis syndrome and chronic atrophic gastritis (CAG).Methods:Randomized controlled trial. A total of 68 CAG patients admitted to the Huairou Hospital of Traditional Chinese Medicine from January 2018 to January 2021 who met the selection criteria were divided into 2 groups according to the random number table method, with 34 in each group. The control group received conventional western medicine treatment, such as inhibition of acid, protecting the gastric mucosa, and the observation group was treated with Traditional Chinese Medicine (TCM) herbal prescription of activating blood and removing blood stasis, invigorating the spleen and soothing the liver. Both groups were treated for 12 weeks. TCM symptom scores were performed before and after treatment. The serum level of pepsinogen Ⅰ(PG Ⅰ), pepsinogen Ⅱ (PGⅡ) were detected by ELISA, and the PG Ⅰ/PG Ⅱ ratio was calculated. Gastroscopic biopsy was performed to observe the changes of intestinal metaplasia of gastric mucosa and glandular atrophy, and to evaluate the clinical efficacy. Results:The total responsive rate was 85.3% (29/34) in the study group and 58.8% (20/34) in the control group. There was significant difference between the two groups ( χ2=9.35, P=0.030). After treatment, the scores of stomachache, fullness of feeling in the observation group were significantly lower than those in the control group ( t=2.97, 3.80, P<0.05). After treatment, the level of serum PG Ⅰ[(76.21 ± 17.35) mg/L vs. (66.8 ± 18.77) mg/L, t=2.15] and PG Ⅰ/PG Ⅱ [(4.67 ± 0.99) vs. (3.90± 1.25), t=2.81] in the study group were significantly higher than those in the control group ( P<0.05), and PG Ⅱ [(16.36 ± 1.85) mg/L vs. (17.42 ± 2.05) mg/L, t=2.24] was significantly lower than that of the control group ( P<0.05). After treatment, intestinal metaplasia and glandular atrophy was significantly more improved or reversed than those in the control group ( χ2=20.67,9.33, P<0.05). Conclusion:The methods of activating blood and removing blood stasis, invigorating the spleen and soothing the liver can reverse the precancerous lesions of patients with gastric collateral stasis syndrome of chronic atrophic gastritis and have a good prognosis.

RESUMO

AIM:To investigate the inhibitory effect of the"decoction for soothing liver and removing stasis and toxicity(SGQYJDF)"on hepatocellular carcinoma(HCC)proliferation in nude mice by inducing ferroptosis via the tumor protein 53(p53)/solute carrier family 7 member 11(SLC7A11/xCT)/glutathione peroxidase 4(GPX4)pathway.METHODS:An ectopic subcutaneous tumor model was established by injecting SK-Hep-1 cells subcutaneously into the right axilla of nude mice.Upon formation of tumor, the mice were randomly divided into five groups(i.e.,control group,low-,medium-and high-dose SGQYJDF groups and medium-dose SGQYJDF plus Sorafenib group). Each group of mice was orally administered with the corresponding therapy for 14 consecutive days, during which the tumor size was observed regularly. At the end of treatment, the tumor growth inhibition rate was calculated based on tumor mass, and histopatho-logical changes were observed by HE staining. Then, the levels of malondialdehyde(MDA),glutathione(GSH)and fer-rous ions(Fe2+)were detected by colorimetric assays. The expression of the proliferation markers Ki-67 and GPX4 was de-tected by immunohistochemistry(IHC).The expression of p53 and xCT was detected by Immunofluorescence(IF).And the expression of p53,xCT and GPX4 was determined by Western blot.RESULTS:(1)SGQYJDF was found to dose-de-pendently decrease tumor volume(P<0.01)and inhibit tumor mass growth(P<0.01),and meanwhile, reduce the per-centage of Ki-67-positive cells(P<0.01)and their proliferation ability in tumor tissues,as compared to the control group.(2)In terms of Ferroptosis-related indicators,SGQYJDF was found to dose-dependently increase the levels of Fe2+ and MDA but decrease the level of GSH in tumor tissues(P<0.01),as compared to the control group.(3)In terms of protein expression,SGQYJDF was found to dose-dependently upregulate the expression of p53(P<0.05)but inhibit the expres-sion of xCT(P<0.05)and GPX4(P<0.01).Notably,medium-dose SGQYJDF plus sorafenib showed a stronger effect than high-dose SGQYJDF.CONCLUSION:Our findings suggest that SGQYJDF can induce Ferroptosis to inhibit the proliferation of subcutaneously transplanted tumor tissues in nude mice by upregulating the expression of p53,and inhibit-ing the expression of xCT and GPX4.

RESUMO

ObjectiveTo explore the effects of different treatment methods of "soothing liver, invigorating spleen, soothing liver and invigorating spleen, soothing liver first and then soothing liver and invigorating spleen, as well as invigorating spleen first and then soothing liver and invigorating spleen" on liver depression combined with liver injury in rats and their action mechanisms. MethodA six-week rat model of liver depression combined with liver injury was established by restraint stress and subcutaneous injection of carbon tetrachloride (CCl4, 5.89 g·kg-1, once every three days). At the same time, the drugs were given by gavage. Forty-eight male SD rats of clean grade were randomly divided into eight groups, namely the normal group, model group, bicyclol (0.2 g·kg-1) group, Sinisan (4.32 g·kg-1) group, Liu Junzitang (9.26 g·kg-1) group, Chaishao Liu Junzitang A (Chai A, soothing liver and invigorating spleen,13.57 g·kg-1) group, Chaishao Liu Junzitang B (Chai B, soothing liver first and then soothing liver and invigorating spleen, 13.57 g·kg-1) group, and Chaishao Liu Junzitang C (Chai C, invigorating spleen first and then soothing liver and invigorating spleen, 13.57 g·kg-1) group, with six rats in each group. The pathological changes in liver and colon tissues of each group were observed under light microscope and electron microscope. The serum biochemical indexes of the liver were detected using an automatic biochemical analyzer. The relative mRNA expression levels of Takeda G protein-coupled receptor 5 (TGR5) and intestinal mucosal zona occluden-1 (ZO-1), Occludin, and Claudin-1 in the liver and colon were detected by reverse-transcription polymerase chain reaction (RT-PCR). The positive expression rate of proliferating cell nuclear antigen (PCNA) in the colon was detected by immunohistochemistry. ResultCompared with normal group, the model group exhibited significantly elevated serum alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), and direct bilirubin (DBIL) (P<0.01), lowered TGR5 mRNA expression in liver tissue, up-regulated TGR5 mRNA expression in the colon tissue (P<0.05,P<0.01), and down-regulated ZO-1, Occludin, and tight junction protein-1 (Claudin-1) mRNA expression and PCNA in the colon tissue (P<0.01). Compared with the model group, bicyclol and Chai C remarkably decreased the levels of serum ALP, ALT, AST, TBIL, and DBIL (P<0.05,P<0.01), while Liu Junzitang, Chai A, Chai B, and Chai C significantly up-regulated the TGR5 mRNA expression in the liver and down-regulated its expression in the colon (P<0.01). Bicyclol, Chai A, Chai B, and Chai C enhanced the ZO-1 and Claudin-1 mRNA expression in the colon (P<0.05,P<0.01). Bicyclol, Sinisan, and Chai C increased PCNA expression (P<0.01). The comparison with the Chai C group showed that the TGR5 mRNA expression in the liver and ZO-1 mRNA expression in the colon of the bicyclol and Sinisan groups were lower, whereas the TGR5 mRNA expression in the colon was higher (P<0.01). However, the PCNA expression in the colon of the Liu Junzitang and Chai B groups declined significantly (P<0.05). ConclusionIn the presence of liver injury, invigorating spleen first helps to relieve the liver injury, and the efficacy of "spleen-invigorating" therapy in increasing the intestinal mucosal tight junction proteins and improving the gastrointestinal function is related to its activation of TGR5 to improve the intestinal mucosal barrier function, promote the renewal of intestinal stem cells, and drive the regeneration after injury.

RESUMO

Objective: To investigate the mechanism of lipid metabolism disorders in Kupffer cells (KCs) of non-alcoholic fatty liver disease (NAFLD) rats mediated by LXRα-SREBP-1c pathway and the interference of soothing liver and invigorating spleen recipe (SLISR) on it. Methods: SD male rats were randomly divided into five groups: normal, model, soothing liver recipe (SLR), invigorating spleen recipe (ISR), and soothing liver and invigorating spleen recipe (SLISR) groups. The rats in treatment groups were administered for 8 weeks. The liver tissue was stained with H&E and oil red O. The levels of hepatic lipid and blood lipid were measured by biochemical analyzer. KCs were isolated from the livers of rats to evaluate the expression of LXRα, SREBP-1C, and FAS mRNA by real-time fluorescence quantitative PCR tests; LXRα, SREBP-1C, and FAS proteins were measured by Western blotting. Results: The H&E and oil red O staining results showed that the model rats successfully reproduced typical pathogenetic and histopathological features of NAFLD. The levels of hepatic lipid and blood lipid in the model rats were dramatically increased. Compared with the model group, the values of hepatic lipid and blood lipid in the treatment groups were significantly ameliorated (P < 0.05, 0.01). The yields of purified KCs from each rat were 2×107-3×107. The viability of KCs was higher than 95%, with the purity over 90.18%. Compared with the model group, the expression of LXRα, SREBP-1C, and FAS mRNA and proteins was decreased in all treatment groups, especially in the SLR group (P < 0.05). Conclusion: SLISR may protect liver against injury included by lipid metabolism disorders in KCs through LXRα/SREBP-1c signaling pathway, which may be an important mechanism for the prevention and treatment of NAFLD.

Based on the primary regimen, the patients were additionally given Shugan Qingre Recipe ( mainly composed of Rhizoma Dioscoreae, Radix Bupleuri, Radix Paeoniae Alba, Radix Rehmanniae, Cortex Moutan, Herba Artemisiae Scopariae, Poria, Fructus Corni, Fructus Lycii, Herba Agrimoniae , Radix Rubiae, Radix Glycyrrhizae) for 3 months (12 weeks), 2 doses a day, decocting with water for oral use. Before and after treatment, the changes of Chinese medical syndrome scores and the laboratory indexes were investigated. Results (1) After treatment for 12 weeks, 10 cases were markedly effective, 16 effective , 5 improved, one ineffective, and the total effective rate arrived to 81.25%. ( 2) After treatment for one month, the scores of traditional Chinese medical syndrome were not improved ( P>0.05 compared with those before treatment) . At the end of 2-month and 3-month treatment course, the scores were improved obviously compared with those before treatment, the difference being significant (P<0.05 or P<0.01) . (3) Platelet count rose up from the fourth week of treatment, and the difference was significant compared with that before treatment ( P<0.01) . Conclusion Therapy of soothing liver and clearing heat shows certain ef fect for the treatment of glucocorticoid-dependent adult chronic CITP, and has satisfactory clinical prospect.

RESUMO

Objective To observe the effects of intervention of soothing liver and activating blood Chinese medicine on cardiac function and myocardial pathologic morphology of BMSCs transplanting on myocardial IRI of rats, and investigate its myocardial protection mechanism. Methods Model of myocardial IRI was established by coronary artery ligation in rats. SD rats were randomly divided into sham operation group, IRI group, BMSCs group and combined group. Rats in combined group were filled the stomach with soothing liver and activating blood Chinese medicine, and rats in other groups were filled the stomach with the same dose of normal saline. After 4 weeks, myocardial pathologic morphology was observed with light microscope. Cardiac function was detected with ultrasonic cardiogram. Results Compared with BMSCs group, heart function of the combined group improved, with significant statistical difference (P<0.05,P<0.01). Pathological observation showed that myocardial structure and pathological morphology were obviously promoted in the combined group. Conclusion Soothing liver and activating blood Chinese medicine could improve heart function and myocardial pathological morphology of IRI rats with BMSCs transplantation.
