RESUMEN Introducción. La leishmaniasis es una enfermedad zoonótica endémica con amplia distribución en Perú. Objetivo. Identificar los escenarios de transmisión de leishmaniasis y la población que reside en estos, e identificar las características de las poblaciones afectadas durante el periodo 2010 a 2022. Métodos. Estudio descriptivo, transversal, ecológico con unidad de análisis el nivel distrital. Se utilizó datos de fuentes secundarias de acceso público. Resultados. En el periodo de estudio se reportaron 85 117 casos, 7 374 007(22,08%) habitantes residían en 543 distritos con transmisión continua, 11 467 420 (34,33%) habitantes que residían en 454 distritos con reporte esporádico de casos y 14 558 983(43,59%) residían en 893 distritos libres de trasmisión. El 98,16% de los casos ocurrieron en distritos con trasmisión continua, el 1,84%, en distritos con reporte esporádico de casos. La incidencia acumulada media anual fue 3,48 casos/10 000 habitantes. 18 departamentos reportaban trasmisión continua, Madre de Dios (42,45 casos/10 000 habitantes) y Cusco (15,78 casos/10 000 habitantes) tuvieron las tasas más altas. Las poblaciones de mayor riesgo son: hombres, adolescentes y jóvenes; residentes de distritos: de selva, del quintil de mayor pobreza monetaria, del quintil con mayor porcentaje con al menos 1 NBI, y del quintil de menor índice de desarrollo humano. Conclusión. La leishmaniasis tiene amplia distribución en Perú, 56,41% de la población habita en distritos que reportan casos. Existen grupos poblacionales con mayor riesgo absoluto, los cuales pueden ser blanco de intervenciones diferenciadas de prevención y control.


ABSTRACT Introduction. Leishmaniasis is an endemic zoonotic disease widely distributed in Peru. Objectives. Identify transmission scenarios and the population residing in them and identify the characteristics of the affected populations during the period 2010-2022. Methods. Descriptive, cross-sectional, ecological study with district as analysis units. Publicly accessible secondary data sources were used. Results. During the study period, 85,117 cases were reported; 7,374,007 (22.08%) inhabitants live in 543 districts with continuous transmission; 1,146,7420 (34.33%) inhabitants reside in 454 districts with sporadic reports of cases; and 1,455,8983 (43.59%) reside in 893 transmission-free districts. 98.16% of the cases occurred in districts with continuous transmission; 1.84% occurred in districts with sporadic case reports. The mean annual incidence rate was 3.48 cases/10,000 inhabitants. 18 departments report continuous transmission; Madre de Dios (42.45 cases/10,000 inhabitants) and Cusco (15.78 cases/10,000 inhabitants) report the highest rates. The populations with higher absolute risk are men, adolescents, and young people; residents: from jungle districts; from the quintile with the highest monetary poverty; from the quintile with the highest percentage with at least 1 NBI; and from the quintile with the lowest human development index. Conclusion. Leishmaniasis is widely distributed in Peru, with 56.41% of the population living in districts reporting cases. There are population groups with a higher absolute risk, which can be the target of differentiated prevention and control interventions.

Resumo Objetivou-se analisar a associação entre o reconhecimento de uma fonte usual do cuidado de Atenção Primária à Saúde (APS) e o acesso aos serviços de APS, entre adolescentes brasileiros. Estudo transversal, a partir da Pesquisa Nacional de Saúde do Escolar realizada com 68.968 adolescentes brasileiros, através de amostragem por conglomerados. Foram realizadas análises descritivas através do χ2 de Pearson e a razão de prevalência (RP) através dos modelos de regressão logística entre acesso aos serviços de APS e o reconhecimento da FUC APS. Dos adolescentes que procuraram os serviços de APS, 74,6% referiram acesso, sendo a maior do sexo feminino (79,3%). Na análise multivariada, observa-se associação positiva (RP: 1,25; IC95%: 1,24-1,26), e na estratificado por sexo, observou-se associações positivas para ambos os sexos, (RP: 1,30; IC95%: 1,28-1,31) masculino e (RP: 1,21; IC95%: 1,20-1,23) feminino. Verifica-se que a maioria dos adolescentes brasileiros que têm a APS como sua FUC conseguiram acessar os serviços de APS, apesar de que, a falta de acesso foram mais frequentes entre os mais vulneráveis economicamente e devido a comportamentos de risco, indicando iniquidades potencialmente evitáveis por meio de uma APS mais efetiva e longitudinal.


Abstract We analyzed the association between the recognition of a usual source of care (USC) of Primary Health Care (PHC) and access to services among Brazilian adolescents. This is a cross-sectional study using data from the National Adolescent School-based Health Survey with 68,968 Brazilian adolescents and cluster sampling. Descriptive analyses were carried out with Pearson's χ2 and prevalence ratios (PR) using logistic regression models between access and recognition of USC. It was observed that 74.6% reported access, and this was higher among females (79.3%). In the multivariate analysis, there was a positive association (PR: 1.25; 95%CI: 1.24-1.26); and, when stratified by sex, positive associations for both sexes, (PR: 1.30; 95%CI: 1.28-1.31) male and (PR: 1.21; 95%CI: 1.20-1.23) female. The majority of Brazilian adolescents demonstrated PHC as a USC and were able to access services, but lack of access was more frequent among the most economically vulnerable and those with risk behaviors, indicating potentially avoidable inequities with more equitable and longitudinal PHC services.

Background Polycyclic aromatic hydrocarbons (PAHs), one of the main components of fine particulate matter (PM2.5), have a certain impact on ambient air quality, and long-term exposure to PAHs may pose potential health risks to human beings. Objective To identify the distribution characteristics and sources of PAHs in atmospheric PM2.5 in a district of Taizhou City from 2019 to 2021, and to evaluate the health risks of PAHs to the population in the area through the inhalation pathway. Methods From 2019 to 2021, air PM2.5 sampling was carried out at a state-controlled surveillance point in a district of Taizhou City for 7 consecutive days on the 10th-16th of each month, the sampling time was 24 h·d−1, and the sampling flow rate was 100 L·min−1. PM2.5 mass concentration was calculated by gravimetric method. A total of 16 PAHs were determined by ultrasonic extraction-liquid chromatography. Kruskal-Wallis H test was used to compare the distribution charac teristics of PAHs concentrations by years and seasons, characteristic ratio and principal component analysis (PCA) was used to analyze their sources, and a lifetime carcinogenic risk (ILCR) model was used to assess the health risk of PAHs. Results From 2019 to 2021, the annual average concentrations [M (P25, P75)] of ∑PAHs in atmospheric PM2.5 in the selected district of Taizhou City were 6.52 (2.46, 10.59), 8.52 (4.56, 12.29), and 3.72 (1.51, 7.11) ng·m−3, respectively, and the annual benzo[a]pyrene (BaP) excess rates (national limit: 1 ng·m−3) were 27.38% (23/84), 47.62% (40/84), and 19.04% (16/84), respectively, both presenting 2020> 2019 > 2021 (P<0.001, P<0.05). The ∑PAHs concentration distribution showed a seasonal variation, with the highest value in winter and the lowest value in summer (P<0.05). Among the atmospheric PM2.5 samples, the proportion of 5-ring PAHs was the highest, the proportion of 2-3-ring PAHs was the lowest; the proportion of 2-4-ring PAHs showed a yearly upward trend, and the proportion of 5-6-ring PAHs showed yearly downward trend (P<0.05). The characteristic ratio and PCA results suggested that the sources of sampled PAHs were mainly mixed sources such as dust, fossil fuel (natural gas), coal combustion, industrial emissions, and motor vehicle exhaust emissions. The ILCR (RILCR) of PAHs by inhalation for men, women, and children were 1.83×10−6, 2.35×10−6, and 2.04×10−6, respectively, and the annual average RILCR was 2.07×10−6, all greater than 1×10−6. Conclusion For the sampled time period, the main sources of PAHs pollution in atmospheric PM2.5 in the target district of Taizhou City are dust, fossil fuel (natural gas), coal combustion, industrial emissions, motor vehicle emissions, etc., and PAHs may have a potential carcinogenic risk to local residents.

The correct pairing of disulfide bonds maintains the correct folding mode and high-level structure formation of peptides and protein drugs, which is crucial for the quality control of products. In order to ensure that the disulfide bonds are correctly paired, disulfide bond analysis is an essential part of peptides and protein drug characterization. Mass spectrometry can be used to analyze disulfide bonds. However, insulin and its analogues have two pairs of disulfide bonds without restriction enzyme cutting site. Conventional collision-induced dissociation (CID) and high-energy induced cleavage (HCD) cannot accurately locate the complex disulfide bond. In our study, three methods were used to localize the complex disulfide, including enzyme digestion combined with key peptide fragment in source decay (ISD) fragmentation method, enzyme digestion combined with partial reduction alkylation method, intact protein source ISD and electron transfer dissociation (ETD) cleavage method, The applicability of insulin aspart, insulin lispro and insulin glargine were also investigated. This study provides a new way for the quality control of disulfide bonding mode of insulin and its analogues, and also provides a reference for the disulfide bond localization of peptides or proteins containing this complex disulfide bond.

RESUMO

Swept-source optical coherence tomography angiography(SS-OCTA)is a new vascular imaging technique that was recently proposed. It has the advantages of being non-invasive, quick, high-resolution, and automated vascular stratification imaging. It is extremely helpful in the early diagnosis of ophthalmology-related diseases, as well as in the evaluation of treatment effectiveness and the tracking of disease progression. Based on the foundation of OCTA, SS-OCTA utilizes a fast-tuning laser with a wavelength of 1 050 nm for deeper penetration and non-invasive depth-resolved imaging of the retinal and choroidal microvascular systems, deepening the understanding of the characteristics of a wide range of ophthalmic diseases(fundus lesions, glaucoma, neurodegenerative diseases, etc.). The structures of the anterior segment of the eye can also be studied using SS-OCTA, including changes in the depth and density of corneal neovascularization as well as changes in iris neovascularization before and after therapy. This approach provides a novel tool for ophthalmic clinical practice. The development of the clinical use of SS-OCTA technology in ophthalmology is reviewed in this article.

RESUMO

Needle-free injection technology (NFIT) refers to the drug delivery systems in which drugs are propelled as high-speed jet streams using any of the pressure source to penetrate the skin to the required depth. NFIT is a promising drug delivery system as it enables the injection of liquids, powders, and depot/projectiles, and has the advantages of preventing needle stick accidents, improving drug bioavailability, eliminating needle-phobia, increasing vaccine immunity, simplifying operations and is convenient for patients to use. NFIT and its research background, the structure and classification of needle-free jet injectors (NFJI), drugs that can be delivered using NFJI and the factors affecting the injection effect are comprehensively reviewed in this paper. The limitations and potential development directions are summarized to provide a theoretical basis for the application and development of NFIT.

@#Objective To prepare murine and rabbit polyclonal antibodies against rabies virus(RV) matrix(M) protein and compare their reactivity.Methods The prokaryotic expression vector pET-28a-M was constructed by using the cDNA of cells infected with RV CVS-11 strain as template,then transformed into E.coli BL21(DE3),and the induced by IPTG to express M protein.After nickel column affinity chromatography and dialysis renaturation,female BALB/c mice and New Zealand white rabbits were immunized with the M protein,and the whole blood was taken to separate the serum.The titers of the murine and rabbit polyclonal antibodies were detected by ELISA,and the reactivity was measured by Western blot,indirect immunofluorescence assay(IFA) and immunoprecipitation(IP).Results The plasmid pET-28a-M was constructed correctly as identified by sequencing.The titers of murine and rabbit polyclonal antibodies were 1:100 and 1:256 000respectively,and the polyclonal antibodies had reactivity with different RV strains.Conclusion The murine and rabbit polyclonal antibodies against M protein were successfully prepared,which provides important biological tools for exploring the interaction between M protein and host protein as well as studying the pathogenesis of RV.

Objective To provide a reference for avoiding the harm to critical target organs following considerable inhalation exposure to the transuranium element americium (Am) as well as post-accident decorporation or other radiation protection measures. Methods We established calculation programs based on the generic criteria for internal radiation emergency preparedness and response in the IAEA Safety Guide No.GSG-2 and current new ICRP biokinetic models and parameters, taking an inhalation of 241Am (activity mean aerodynamic diameter of 5 μm, σ = 2.5) by an adult worker as an example; and determined that the critical target organs were the lung AI region, red bone marrow, and the main source organs leading to acute doses to the critical target organs were the lung AI region, blood, and trabecular bone surface. Results The retention fractions in the main source organs over time after 241Am inhalation were calculated. Conclusion After being absorbed into blood, Am moves quickly to other parts, and Am of different absorption types shows similar early changes in retention fractions in blood: the retention fractions of Am of S, M, and F types in blood peak around 0.03 d, and then halve around 1.7 d. Inhaled Am shows different changes over time in retention fractions in the lung AI region and trabecular bone surface in the early stage: the retention fractions of S- and M-type Am in the lung AI region change little with time, while F-type Am transfers quickly from the lung to blood; In trabecular bone surface, S-type Am increases quickly in the first 7 d, M-type Am gradually increases mainly in the first 2 weeks, and F-type Am increases quickly in the first 2 d.

ObjectiveTo elucidate the principles and methods of the Bayesian probabilistic linkage model, and to demonstrate the effect of applying the model in linking birth and death data. MethodsThrough the Shanghai birth and death registration system, data of 199 025 infants born in 2017 and 1 512 infants who died in 2017 and 2018 were collected. After cleaning the data, the data were divided into monthly blocks and fully linked. The Jaro-Winkler algorithm and Euclidean distance were employed to measure the similarity of fields for matching. A Bayesian probabilistic linkage model was constructed and the linking effect was evaluated using a confusion matrix. ResultsUsing the Bayesian probabilistic linkage model, the birth and death data of infants were effectively linked, revealing that 36.71% of infants who died in Shanghai were born outside the city, and the probability of infant death was 2.6‰. The confusion matrix of the test set showed a recall rate of 0.86, precision of 0.76, and an F-score of 0.81. ConclusionThe practical application of Bayesian probabilistic linkage demonstrates a good model performance, enabling the establishment of birth-death cohorts that more accurately reflect the true levels of infant mortality. Utilizing this technique to integrate data from different departments can effectively improve research efficiency in the field of public health.

El campo de la radiología ha experimentado avances notables en las últimas décadas, con desarrollos que van desde la mejora de la calidad y digitalización de las imágenes hasta la detección asistida por computadora. Particularmente, la aparición de técnicas de Inteligencia Artificial basadas en Deep Learning y Visión Computacional han promovido soluciones innovadoras en el diagnóstico y el análisis radiológico. Se explora la relevancia de los desarrollos y modelos open source en el progreso de estas técnicas, resaltando el impacto que la colaboración y el acceso abierto han tenido en el avance científico del campo. La investigación tiene un enfoque cualitativo, con alcance descriptivo y retrospectivo, de corte longitudinal. Se realizó un análisis documental de la evolución y el impacto del open source en la Radiología, poniendo de relieve la colaboración multidisciplinar. Se examinaron casos de uso, ventajas, desafíos y consideraciones éticas en relación con la implementación de soluciones basadas en Inteligencia Artificial en Radiología. El enfoque open source ha mostrado ser una influencia positiva en la Radiología, con potencial para influir en la atención médica, ofreciendo soluciones más precisas y accesibles. No obstante, se presentan desafíos éticos y técnicos que requieren atención.


The field of radiology has seen notable advances in recent decades, with developments ranging from image quality improvement and digitization to computer-aided detection. Particularly, the emergence of Artificial Intelligence techniques based on Deep Learning and Computer Vision have promoted innovative solutions in diagnosis and radiological analysis. This article explores the relevance of open source developments and models in the progress of these techniques, highlighting the impact that collaboration and open access have had on the scientific advancement in this field. This research has a qualitative approach, with a descriptive, retrospective, longitudinal scope. A documentary analysis of the evolution and impact of open source in Radiology was carried out, highlighting multidisciplinary collaboration. Use cases, advantages, challenges and ethical considerations were also examined in relation to the implementation of AI-based solutions in Radiology. The Open Source approach has been shown to be a positive influence in Radiology, with the potential to influence medical care, offering more precise and accessible solutions. However, there are ethical and technical challenges that require attention.

Objetivo. Determinar los niveles plasmáticos de vitamina D y su relación con la ocupación, procedencia y los valores de calcio, leucocitos, hemoglobina y plaquetas en adultos mayores de Lima Metropolitana. Método. Estudio observacional descriptivo de corte transversal, participaron 100 adultos mayores de uno u otro sexo de Lima Metropolitana, ellos fueron reclutados durante las estaciones de invierno-primavera del 2022, los niveles de vitamina D se categorizó como suficiente, insuficiente y deficiente, las concentraciones séricas de vitamina D se midieron por radioinmunoensayo. La relación de variables se realizó con el coeficiente de correlación de Spearman y regresión logística. Resultados. La edad media fue 69.6 años, el 71%, fueron del sexo femenino, la concentración media de vitamina D fue 36.56 ng/ml, el 13% tuvieron niveles de deficiente, y el 32% de insuficiente y el 53% suficientes. Según sexo, el 77,7% de los que tuvieron niveles de deficiente/insuficiente fueron mujeres. En el análisis bivariado no hubo correlación entre los valores de vitamina D con calcio, hemoglobina leucocitos y plaquetas, los que procedieron de distritos de menor temperatura tuvieron 2,25 veces más riesgo de tener niveles insuficientes/deficiente de vitamina D. Conclusiones. El 45% de los adultos mayores tuvieron niveles deficientes/insuficiente de vitamina D, siendo más frecuente en las mujeres y de los procedentes de distritos de menor promedio de temperatura.


Objective. To determine the plasmatic levels of vitamin D and its relationship with the occupation, origin and the values of calcium, leukocytes, hemoglobin and platelets in older adults of Metropolitan Lima. Methods Observational descriptive cross-sectional study, 100 older adults of either sex from Metropolitan Lima participated, they were recruited during the winter-spring seasons of 2022, vitamin D levels were categorised as sufficient, insufficient, and deficient, serum vitamin D concentrations were measured by radioimmunoassay. The relationship of variables was established with the Spearman correlation coefficient and logistic regression. Results. The mean age was 69.6 years, 71% were women, the mean vitamin D concentration was 36.56 ng/ml, 13% had deficient levels, 32% were insufficient and 53% sufficient. By sex, 77.7% of those with deficient/insufficient levels were women. In the bivariate analysis, there was no correlation between vitamin D values with calcium, hemoglobin, leukocytes and platelets; those who came from districts with lower temperatures had a 2.25 times greater risk of having insufficient/deficient levels of vitamin D. Conclusions. 45% of older adults had deficient/insufficient levels of vitamin D, more frequent in women and those coming from districts with lower average temperatures.

RESUMEN Objetivo: Identificar los factores asociados al delirio de emergencia en niños en la Unidad de Recuperación Posanestésica de un Instituto Especializado en Salud Pediátrica del Perú. Métodos: Se realizó un estudio transversal en niños de 2 a 6 años, clasificación del estado físico I y II, en procedimientos electivos bajo anestesia entre septiembre y diciembre 2022. La variable principal fue la presencia de delirio de emergencia evaluada con la escala Pediatric Anesthesia Emergence Delirium mayor o igual a 10 puntos. Para identificar los factores asociados con el delirio, se aplicó el modelo de regresión de Poisson y se consideró significativo un valor p < 0,05. Resultados: De los 150 niños, se encontró que la incidencia de delirio de emergencia se presentó en el 10,6%. De ellos, el 81,4% fueron menores de 5 años, el dolor se asoció significativamente en el 37,5% con RP = 3,63, IC95% [1,20, 10,1] y se observó que el 68,8% necesitó cuidados paliativos. Conclusiones: El dolor se asoció con el delirio de emergencia posanestésico en niños. La prevención y el tratamiento del delirio de emergencia deben centrarse en el control del dolor posoperatorio y en la vigilancia cuidadosa de los pacientes después de la cirugía.


ABSTRACT Objective: Identify associated factors with emergence delirium in children in the Post-Anesthetic Recovery Unit of a Pediatric Health Specialized Institute in Peru. Methods: A cross-sectional study was carried out in children from 2 to 6 years old, physical status classification I and II, in elective procedures under anesthesia. The main variable was the presence of emergence delirium evaluated with the Pediatric Anesthesia Emergence Delirium scale greater than or equal to 10 points. To identify the factors associated with delirium, the Poisson regression model was applied and a p value < 0.05 was considered significant. Results: Of the 150 children, it was found that the incidence of emergence delirium occurred in 10.6%. Of these, 81.4% were under 5 years of age, pain was significantly associated in 37.5% with PR = 3.63, 95%CI [1.20, 10.1] and it was observed that 68, 8% required palliative care. Conclusion: Pain was associated with delirium of postanesthetic emergence in children. Prevention and treatment of emergence delirium should focus on postoperative pain control and careful monitoring of patients after surgery.

RESUMEN Introducción: La pérdida auditiva o hipoacusia en el adulto mayor se produce, generalmente, por cambios degenerativos propios del envejecimiento y puede influir en el deterioro cognitivo de la persona, que se manifiesta por distintos cambios en la memoria a medida que se incrementa la edad. Objetivo: Determinar la relación entre la pérdida auditiva y el deterioro cognitivo en el adulto mayor. Métodos: Estudio descriptivo, correlacional y transversal. Se realizó otoscopía, audiometría y se aplicó el cuestionario de Pfeiffer en 104 adultos mayores de 65 años, que acudieron al servicio de Otorrinolaringología del Hospital Nacional Arzobispo Loayza. Se empleó como instrumento el otoscopio, el audiómetro y el cuestionario de Pfeiffer. Se analizó la relación mediante el coeficiente de correlación de Rho de Spearman. Resultados: Se encontró pérdida auditiva en 58 casos (55.8%) y normoacusia en 46 (44.2%). Al aplicar el cuestionario de Pfeiffer para determinar el estado cognitivo, se obtuvo una puntuación global promedio de 8.5, determinándose que los valores menores a 8 se consideran como deterioro cognitivo leve. Se evidenció deterioro cognitivo leve en el 7% de normoacúsicos y en el 26% de hipoacúsicos. Al realizar la correlación de Rho de Spearman, se encontró correlación positiva alta entre la pérdida auditiva y deterioro cognitivo (0.864), significativa (0,000<0,05). Conclusión: Existe relación positiva alta entre la pérdida auditiva y el deterioro cognitivo en el adulto mayor.


ABSTRACT Introduction: Hearing loss or hearing loss in the elderly is generally caused by degenerative changes typical of aging and can influence the cognitive deterioration of the person, which is manifested by different changes in memory as age increases. Objective: To determine the relationship between hearing loss and cognitive impairment in the elderly. Methods: Descriptive, correlational and cross-sectional study. Otoscopy, audiometry and the Pfeiffer questionnaire were performed in 104 adults over 65 years of age, who attended the Otorhinolaryngology service of the Arzobispo Loayza National Hospital. The otoscope, the audiometer and the Pfeiffer questionnaire were used as instruments. Analyzing the relationship using Spearman's Rho correlation coefficient. Results: Hearing loss was found in 58 cases (55.8%) and normal hearing in 46 cases (44.2%). When applying the Pfeiffer questionnaire to determine cognitive status, an average global score of 8.5 was obtained, determining that values less than 8 are considered mild cognitive impairment. Evidencing mild cognitive impairment in 7% of normoacusics and in 26% of hypoacusics. When performing Spearman's Rho correlation, a high positive correlation was found between hearing loss and cognitive impairment (0.864), significant (0.000<0.05). Conclusion: There is a high positive relationship between hearing loss and cognitive impairment in the elderly.

RESUMEN Introducción: La diabetes continúa siendo una de las principales causas de discapacidad y muerte en la población mundial. Alrededor del 25% de las personas con diabetes desarrollarán una úlcera en alguno de sus miembros pélvicos inferiores. Objetivo: El presente estudio evalúa los aspectos clínicos relacionados con la amputación del miembro inferior pélvico en una cohorte de pacientes con diabetes mellitus. Métodos: Estudio retrospectivo, transversal, realizado en colaboración entre el Instituto Mexicano del Seguro Social y la Facultad de Farmacia de la Universidad Autónoma del Estado de Morelos, implicó una revisión de expedientes de pacientes con diabetes mellitus tipo 2 en el Hospital General Regional "Ignacio García Téllez". Se seleccionaron 100 expedientes clínicos y Electrónicos basados en criterios de inclusión, que incluían edad mayor de 18 años, afiliación en el sitio del estudio, evolución de la diabetes de al menos 10 años, tratamiento farmacológico para la diabetes y diagnóstico de pie diabético con curación completa o amputación como resultado. Los análisis estadísticos se realizaron mediante STATA y se obtuvo aprobación ética. Resultados: Los pacientes con un control glucémico óptimo cuantificando sus niveles de glucosa en ayunas (<130 mg/dl) así como sus valores de hemoglobina glicosilada (< 7%) tuvieron una menor frecuencia de amputaciones (p˂0,001; Chi2) en comparación con aquellos pacientes sin un control glucémico adecuado. Conclusión: Se encontró que ser hombre, valores de hemoglobina glucosilada superiores al 7% y valores promedio de glucosa en ayunas superiores a 130 mg/L aumentan la probabilidad de presentar una amputación de extremidad inferior.


ABSTRACT Introduction: Diabetes continues to be a leading cause of disability and death in the world's population. About 25% of people with diabetes will develop an ulcer in one of their lower pelvic limbs. Objective: The present study evaluates the clinical aspects related to lower pelvic limb amputation in a cohort of patients with diabetes mellitus. Lazarte Echegaray Hospital during the period 2017-2020. Methods: Retrospective, cross-sectional study, conducted in collaboration between the Mexican Institute of Social Security and the School of Pharmacy of the Autonomous University of Morelos State, involved a review of records of patients with type 2 diabetes mellitus at the Regional General Hospital "Ignacio García Téllez". One hundred clinical and electronic records were selected based on inclusion criteria, which included age over 18 years, affiliation at the study site, diabetes evolution of at least 10 years, pharmacological treatment for diabetes and diagnosis of diabetic foot with complete healing or amputation as an outcome. Statistical analyses were performed using STATA and ethical approval was obtained. Results: Patients with optimal glycemic control by quantifying their fasting glucose levels (<130 mg/dl) as well as their glycated hemoglobin values (< 7%) had a lower frequency of amputations (p˂0.001; Chi2) compared to those patients without adequate glycemic control. Conclusion: Being male, glycosylated hemoglobin values greater than 7% and mean fasting glucose values greater than 130 mg/L were found to increase the likelihood of having a lower extremity amputation.

RESUMEN Introducción: La anemia es un problema de salud pública que afecta principalmente a los niños menores de cinco años. Objetivo: Determinar el impacto del control de crecimiento y desarrollo (CRED) en la reducción de la anemia en niños menores de cinco años en el Perú considerando el análisis de la Encuesta Demográfica y de Salud Familiar (ENDES) 2021. Método: Estudio analítico y transversal. Con un análisis secundario de datos de la encuesta ENDES del año 2021. Resultados: El 31.4% de niños menores de cinco años tenía anemia; el 38.3% corresponde al área rural; 34.0%, a niños varones; 56.9%, a niños menores de un año; 38.5% de madres tienen educación primaria y 40.8% pertenece al índice de los más pobres; como se ve, las diferencias son estadísticamente significativas. En relación con el CRED, se evidenció que 32.4% de niños que sí se les hizo algún CRED y el 30.2% de niños que no, tenían anemia. En cuanto al lugar donde se realizó, se observó que 32.4% de niños que se atendían en un establecimiento de salud y el 32.9% de niños que fueron atendidos por la enfermera tenían anemia. Conclusión: La anemia es un problema de salud pública presente y condicionada a diversos factores y determinantes. Durante la pandemia, el impacto de CRED en la reducción de la anemia fue ambiguo, por lo que se tiene supuestos de si las intervenciones están teniendo el impacto deseado y esperado.


ABSTRACT Introduction: Anemia is a public health problem that mainly affects children under five years of age. Objective: Determine the impact of growth and development control (CRED) in reducing anemia in children under five years of age in Peru considering the analysis of the Demographic and Family Health Survey (ENDES) 2021. Method: Analytical and cross-sectional study with a secondary analysis of data from the 2021 ENDES survey. Results: 31.4% of children under five years of age had anemia, where 38.3% corresponded to rural areas; 34.0%, male children; 56.9%, under one year old; 38.5% were mothers with primary education and 40.8% belonged to the index of the poorest, showing statistically significant differences. In relation to the CRED, it was evident that 32.4% of children who did undergo a CRED and 30.2% of children who did not had anemia. Regarding the place where it was carried out, it was observed that 32.4% of children who were cared for in a health facility and 32.9% of children who were cared for by the nurse had anemia. Conclusion: Anemia is a public health problem that is present and conditioned by various factors and determinants. During the pandemic, the impact of CRED in reducing anemia was ambiguous, so there are assumptions about whether the interventions are having the desired and expected impact.

RESUMEN Objetivo: Describir las actitudes de los médicos hacia el abordaje de la neuropatía diabética. Métodos: Estudio descriptivo, observacional, transversal y prospectivo, muestra 143 médicos, muestreo no probabilístico por conveniencia. Las variables: años de ejercicio profesional, especialidad, proporción de pacientes en la consulta médica con diagnóstico de diabetes mellitus (tipo 1 o 2), proporción de pacientes con diabetes mellitus con diagnóstico de neuropatía diabética, actitudes hacia el abordaje de la neuropatía diabética. Se empleó escala Likert de 5 categorías para evaluar actitudes en 3 dimensiones: priorización, diagnóstico y tratamiento. Se emplearon estadísticos descriptivos. Resultados: En la dimensión priorización el 57.4% prioriza el control metabólico sobre la evaluación de complicaciones. Dimensión diagnostico el 80.5% de médicos encuestados se basan en síntomas y signos referidos por el paciente para hacer el diagnóstico de dicha complicación, el 66,5% reconocen que no usan instrumento para la evaluación de la neuropatía y el 39.9% tampoco lo usarían así lo tuviera. En la dimensión tratamiento el 73,5% reconocen que el tratamiento analgésico de la neuropatía diabética es frustrante y el 50,4% siente "temor" para titular la dosis de la medicación analgésica anti-neuropática por los efectos adversos. Conclusiones: Las actitudes de los médicos comprometen diferentes áreas del abordaje de la neuropatía diabética como la priorización, el diagnóstico y el tratamiento con una tendencia a la priorización del control metabólico y de otras complicaciones microvasculares, al sub-diagnóstico, al "sub-tratamiento" y a la necesidad de referir a los pacientes para manejo especializado.


ABSTRACT Objective: To describe the attitudes of doctors towards the approach to diabetic neuropathy. Methods: Descriptive, observational, cross-sectional and prospective study, sample of 143 doctors, non-probabilistic convenience sampling. The variables: years of professional practice, specialty, proportion of patients in the medical consultation with a diagnosis of diabetes mellitus (type 1 or 2), proportion of patients with diabetes mellitus with a diagnosis of diabetic neuropathy, attitudes towards the approach to diabetic neuropathy. A 5-category Likert scale was used to evaluate attitudes in 3 dimensions: prioritization, diagnosis and treatment. Descriptive statistics were used. Results: In the prioritization dimension, 57.4% prioritize metabolic control over the evaluation of complications. Diagnostic dimension: 80.5% of doctors surveyed rely on symptoms and signs reported by the patient to make the diagnosis of said complication, 66.5% recognize that they do not use an instrument for the evaluation of neuropathy and 39.9% would not use it either. had it. RESUMEN Objetivo: Determinar la asociación de la adicción a redes sociales y las consecuencias en salud mental en estudiantes de medicina humana de la Universidad Ricardo Palma, año 2021. Materiales y métodos: Estudio observacional transversal, participaron 454 estudiantes de medicina humana de primero a quinto año, elegidos por muestreo aleatorio estratificado proporcional al tamaño. El instrumento fue validado, obteniendo un Alpha de Cronbach (α=0,90), los cuestionarios empleados fueron: Adicción a redes sociales (ARS), escala de depresión, ansiedad, estrés (DASS-21) y índice de la calidad del sueño de Pittsburgh (PSQI). A fin de evaluar la asociación entre variables, utilizamos razón de prevalencia (RP) crudo y ajustado, mediante el modelo de regresiones de Poisson con el enlace logarítmico y con intervalo de confianza del 95%. Resultados: El 65,4% eran mujeres, con edad promedio 21,2 años. Se evidenció asociación significativa entre adicción a redes sociales alto y salud mental (RPc: 2,59; IC95%: 1,92-3,50; p: 0,000), así mismo, los niveles alto y medio de adicción a redes sociales con síntomas depresivos, ansiedad, estrés (p: 0,000) y calidad de sueño (p<0,010), respectivamente. El 41%, 49,1% y 9,9% de los estudiantes presentaron adicción a redes sociales bajo, medio y alto respectivamente. Un 57,5% presentaron síntomas depresivos, 56,4% ansiedad, 70,5% estrés y 77,3% mala calidad de sueño. Conclusiones: Encontramos una fuerte asociación entre ARS y salud mental. Por lo cual se recomienda a los universitarios que sean evaluados por especialistas de salud mental para una correcta identificación, evaluación y manejo de los problemas de la ARS.


ABSTRACT Objective: To determine the association of addiction to social networks and the consequences in mental health in human medicine students of the Ricardo Palma University, year 2021. Materials and methods: Cross-sectional observational study, with the participation of 454 human medicine students from first to fifth year, chosen by stratified random sampling proportional to size. The instrument was validated, obtaining a Cronbach's Alpha (α=0.90), the questionnaires used were: Addiction to Social Networks (ARS), Depression, Anxiety, Stress Scale (DASS-21) and Pittsburgh Sleep Quality Index (PSQI). In order to evaluate the association between variables, we used crude and adjusted prevalence ratio (PR), using the Poisson regression model with the logarithmic link and with the 95% confidence interval. Results: 65.4% were women, with an average age of 21.2 years. A significant association was found between high social network addiction and mental health (PRc: 2.59; 95% CI: 1.92-3.50; p: 0.000) and, likewise, between high and medium levels of social network addiction. social and with depression, anxiety, stress (p: 0.000) and sleep quality (p<0.010), respectively. 41%, 49.1% and 9.9% of the students presented low, medium and high addiction to social networks, respectively. 57.5% had depression, 56.4% anxiety, 70.5% stress and 77.3% poor sleep quality. Conclusions: There was a strong association between ARS and mental health. Therefore, it is recommended that university students visit mental health specialists quarterly for proper identification, evaluation, and management of the problems generated by ARS.

RESUMEN Objetivo: Construir y validar una escala de resiliencia familiar en cuidadores familiares de personas con discapacidad (ERF-PD). Métodos: El diseño de investigación es instrumental. El muestreo fue de tipo no probabilístico intencional, siendo la muestra 301 familiares de personas con discapacidad. Se construyeron inicialmente 40 ítems, los cuales fueron reduciéndose en la medida que se realizaban los análisis estadísticos respectivos. Resultados: Se obtuvo valores de 1,00 en la V de Aiken en todos los ítems. En función al análisis de ítems se depuraron 9 de ellos. Del análisis factorial exploratorio quedaron 17 ítems que explican el 56,0% de varianza total, asimismo, el análisis factorial confirmatorio obtuvo buenos índices de ajuste para esta estructura (Chi2/gl=1,876; CFI=0,982; TLI=0,979; RMSEA =0,037; SRMR=0,064). La consistencia interna evidenció coeficientes por encima de 0,70 en la escala general y en los tres factores. Conclusión: La escala presenta evidencias de validez y fiabilidad para la muestra evaluada, lo que justifica su uso en investigaciones y en la práctica profesional.


ABSTRACT Objective: To create and validate a family resilience in caregivers relatives of people with disabilities (ERF-PD). Methods: The research design is instrumental. The sampling was non probabilistic with a sample of 301 relatives of people with disabilities. Initially, 40 items were created but this number was reduced after the statistics analysis. Results: Aiken's V values of 1.00 were obtained in all items. After the item analysis, nine items were removed. From the exploratory factor analysis, 17 items remained that explain 56.0% of the overall variance; whereas the confirmatory factor analysis obtained good fit indexes for this structure (Chi2/gl=1.876; CFI=0.982; TLI=0.979; RMSEA =0.037; SRMR=0.064). Internal consistency showed coefficients above 0.70 in the general scale and its three factors. Conclusion: The scale shows evidences of validity and reliability for the evaluated sample, justifying its use in research and professional activity.

RESUMEN Introducción: La cirugía de Whipple, también conocida como duodenopancreatectomía cefálica (DPC), es un procedimiento muy usado para varios tipos de cáncer peri ampular. Objetivos: Establecer los factores asociados a la evolución con complicaciones post DPC en el Servicio de Cirugía de Páncreas del Hospital Nacional Edgardo Rebagliati Martins en Lima, Perú. Métodos: Se realizó un estudio observacional, cuantitativo, analítico, transversal y retrospectivo en 81 pacientes durante el período comprendido entre enero de 2017 hasta diciembre de 2019, quienes fueron sometidos a una DPC. La variable dependiente fueron las complicaciones postquirúrgicas y las independientes fueron indicadores preoperatorios, perioperatorios y post operatorios. Se revisaron las historias clínicas. Se realizó regresión logística para hallar los OR crudos y ajustados. Resultados: La media de la edad fue 65,97±10,14 y el sexo más frecuente fue el masculino. La complicación más frecuente fue la fístula pancreática (34,6%). Los factores asociados a complicaciones post DPC fueron el sexo masculino (ORa: 4,46, IC95%: 1,35-14,77), ampuloma (ORa: 6,92, IC95%: 1,75-27,48) y la consistencia blanda del páncreas (ORa: 4,52, IC95%: 0,07-0,58). Conclusiones: Los factores asociados a complicaciones en la evolución post DPC fueron el sexo masculino, el ampuloma, y la consistencia blanda del páncreas.


ABSTRACT Introduction: Whipple procedure, also known as pancreatoduodenectomy (PD), is a widely used procedure for several types of peri-ampullary cancer. Objectives: To establish the factors associated with postoperative complications after PD in the Pancreas Surgery Service of the Edgardo Rebagliati Martins National Hospital in Lima, Peru. Methods: An observational, quantitative, analytical, cross-sectional, and retrospective study was carried out on 81 patients during the period from January 2017 to December 2019, who underwent PD. The dependent variable was postoperative complications and the independent variables were preoperative, perioperative, and postoperative variables. The medical records were reviewed. Logistic regression was performed to find the crude and adjusted ORs. Results: The mean age was 65.97±10.14 and the majority of patients were male. The most common postoperative complication was pancreatic fistula (34.6%). Factors associated with postoperative complications after PD were male sex (aOR: 4.46, 95% CI: 1.35-14.77), ampullary cancer (aOR: 6.92, 95% CI: 1.75-27.48), and soft consistency of the pancreas (aOR: 4.52, 95% CI: 0.07-0.58). Conclusions: The factors associated with postoperative complications after PD were male sex, ampulloma, and soft consistency of the pancreas.

RESUMEN Introducción: El bullying, es un comportamiento perjudicial en contextos escolares, afectando la vida de los estudiantes. Objetivo: Identificar y comparar indicadores de comportamiento desajustado en estudiantes con diferente propensión al bullying. Metodología: Este estudio descriptivo y cuantitativo utilizó encuestas y un diseño comparativo seguido de correlacional para evaluar comportamientos desajustados (estrés, baja autoestima, incivismo, desajuste familiar, propensión a la violencia) en estudiantes secundarios de Lima y Callao. La muestra no probabilística incluyó 1345 estudiantes de primer y segundo año, divididos en dos grupos según su propensión al bullying (alto -Grupo E- y bajo -Grupo C-). Se aplicaron escalas de los inventarios INDACPS y la Escala de Propensión al Bullying (EPB), analizando los datos con SPSS v.25 y Jamovi. Las diferencias entre grupos se evaluaron mediante la prueba U de Mann-Whitney, y la correlación de variables se determinó con la correlación de Spearman. Resultados: El Grupo E tuvo niveles más altos de estrés, baja autoestima, incivismo, desajuste familiar y propensión a la violencia, con significancia estadística (p<0,001) en comparación al Grupo C. Se encontraron correlaciones estadísticamente significativas entre la propensión al bullying y estos indicadores, siendo más fuertes en la muestra total (p<0,001). Además, se observaron diferencias significativas por sexo en estrés, baja autoestima, desajuste familiar y propensión a la violencia, pero no en incivismo y propensión al bullying. Conclusión: Los estudiantes con mayor propensión al bullying exhiben mayores niveles de estrés, baja autoestima, incivismo, desajuste familiar y propensión a la violencia. Lo que resalta la relevancia de intervenciones integrales.


ABSTRACT Introduction: Bullying is harmful behavior in school contexts, affecting students' lives. Objective: To identify and compare indicators of maladjusted behavior in students with different propensities to bullying. Methodology: This descriptive and quantitative study used surveys and a comparative followed by correlational design to assess maladjusted behaviors (stress, low self-esteem, incivility, family maladjustment, propensity to violence) in secondary students from Lima and Callao. The non-probabilistic sample included 1345 first and second-year students, divided into two groups based on their bullying propensity (high -Group E- and low -Group C-). Scales from the INDACPS inventories and the Bullying Propensity Scale (BPS) were applied, analyzing data with SPSS v.25 and Jamovi. Differences between groups were assessed using the Mann-Whitney U test, and variable correlation was determined using Spearman's correlation. Results: Group E had higher levels of stress, low self-esteem, incivility, family maladjustment, and propensity to violence, with statistical significance (p<0.001) compared to Group C. Statistically significant correlations were found between the propensity to bullying and these indicators, being stronger in the total sample (p<0.001). Additionally, significant gender differences were observed in stress, low self-esteem, family maladjustment, and propensity to violence, but not in incivility and bullying propensity. Conclusion: Students with a higher propensity to bullying exhibit higher levels of stress, low self-esteem, incivility, family maladjustment, and propensity to violence, underscoring the importance of comprehensive interventions.
