RESUMO

Introduction: as the coronavirus disease 2019 (COVID-19) vaccines are distributed and administered globally, hesitancy towards the vaccine hinders the immunisation of a significant number of vulnerable populations, such as people living with HIV/AIDS. Hence, this study aims to assess COVID-19 vaccine hesitancy among people living with HIV/AIDS (PLHIV) attending clinicaloutpatient follow-up at State Specialist Hospital Maiduguri (SSHM), Borno State, Nigeria. Methods: a hospital-based cross-sectional study design was conducted to assess COVID-19 vaccine hesitancy among 344 PLHIV receiving antiretroviral therapy (ART) at the United States President´s Emergency Plan for AIDS (PEPFAR) clinic in SSHM from 4 th January to 25th February 2022. Data were collected using a structured and pretested interviewer-administered questionnaire. The results were presented using frequencies and percentages. The factors that are associated with COVID-19 vaccine hesitancy were identified using the Chi-square statistical test. Results: among the 344 respondents of the study, only 88 (26.6%) received the COVID-19 vaccine. Out of the 256 respondents that did not receive the vaccine, only 10.5% (27/256) are willing to be vaccinated, while the majority; 57.8% (148/256) are not willing to be vaccinated and 31.7% (81/256) of the respondents are uncertain, thus resulting in a hesitation rate of 89.45%. There was no statistically significant association between COVID-19 vaccine hesitancy and the study´s independent variables; where p-value is greater than 0.05. Conclusion: hesitancy towards COVID-19 vaccine is high among PLHIV and there is no any statistically significant association between COVID-19 vaccine hesitancy and the independent variables of the study where p-value is greater than 0.05. Hence, it is necessary to develop targeted strategies to boost vaccine uptake among this vulnerable population.

RESUMO

Background: Stabbing with kitchen knife is a common method of homicide in Nigeria; and embalming of decomposing stabbed cadavers has been a challenge for morticians in Nigeria, because of the impairment of embalming fluid through the damaged arteries. The aim of this study is to investigate the effectiveness of an embalming mixture containing formalin, methanol and water on stabbed early decomposing carcasses using porcine analogues. Methods: Two infant pigs were used for this study. Animals were sacrificed and allowed to reach the early stage of decomposition before embalming. Cervical arterial and hypodermic embalming techniques were employed. Post-embalming changes were observed and scored. A Pearson correlation analysis was used to determine the relationship between the outcome of embalmment and the embalming mixture. Results: There was a statistically significant moderate positive correlation (r = .576, n = 42, p = .001) between outcome of embalming, and duration of embalming; a statistically significant strong negative correlation (r = -.623, n = 42, p = .001) with volume of embalming fluid; and a statistically significant moderate negative correlation (r = -.528, n = 42, p = .001) with room temperature. There was a statistically insignificant very weak positive correlation (r = .186, n = 42, p = .239) between outcome of embalming and humidity; and a statistically insignificant very weak negative correlation (r = -.141, n = 42, p = .375) with atmospheric temperature. Conclusions: This study validated the embalming methodology and fluid suitable for arresting early decomposition process of stabbed infant carcasses in Nigeria. More so, an embalming mixture containing formalin, methanol and water in equal proportion is suitable for arresting early decomposition of stabbed cadavers.

Aerobic endurance in soccer is considered a determining capacity, for which its enhancement is an indispensable part of the content of sports training, being useful to design specialized models for its improvement, which must first be submitted to theoretical validation. In this sense, the objective of the research is to theoretically validate, through national and foreign experts, a proposal on a group of exercises to develop aerobic endurance in soccer players of the U-14 category. The research is descriptive-explanatory of correlational order, an intervention proposal is validated with a group of aerobic endurance exercises for U-14 soccer players in two moments of the design. Five evaluative indicators were consulted to 25 specialists (Nationals: 14, Foreigners: 11), and the future intervention proposal was theoretically validated. Between independent groups, there were no significantly different evaluations in any of the indicators5. Between related groups, significant differences in favor of the posttest were evidenced, both in the national specialists (Comprehensiveness: r=0.001; Affordability: r=0.002; Progression: p=0.001; Variety: p=0.001; Progressive Specialization: p=0.001), and in the foreign specialists (Comprehensiveness: r=0.006; Affordability: r=0.004; Progression: p=0.002; Variety: p=0.003; Progressive Specialization: p=0.004). The future intervention proposal presented adequate Comprehensiveness, Affordability, Progression, Variety and Progressive Specialization, significantly improving the evaluations of the proposal in its second moment of design. On the other hand, there is an adequate degree of agreement regardless of the theoretical and methodological conceptions of each independent group, an additional value of the proposal presented by the author.

ABSTRACT Objective: To determine the prevalence of burnout syndrome and the associated variables in medical specialists in Mexico. Methods: Observational, descriptive and cross-sectional study, by means of a census of 540 medical specialists from three Regional Hospitals. Using their identification card and self-administered Maslach Burnout Inventory-Human Services questionnaire, descriptive statistics and inferential analysis were performed using SPSS 15.0 and Epi-infoV6.1. Results: There was a 90.0% response in the specialists studied. Burnout was detected in 45.9%. There were significant differences in variables: being female; under 40 years of age; without a stable partner, and less than 15 years together; a working couple; childless; clinical specialty; less than 10 years of professional and current employment, and accumulated work day. A negative correlation was found in burnout with emotional exhaustion, and with depersonalisation. It was positive with a lack of personal fulfilment at work. Conclusions: Burnout is common (45.9%) in specialist physicians. The average levels of the subscales are close to normal. Emotional exhaustion and depersonalisation behave inversely proportional to the total score of the syndrome, and directly proportional to the lack of personal fulfilment in the work with burnout.

RESUMO

Objective:To understand the mental health of residents and specialists during the COVID-19 outbreak and provide references for improving their mental health. Methods:An online questionnaire survey was conducted, using the Generalized Anxiety Disorder Scale and the Patient Health Questionnaire Depression Rating Scale, to evaluate the mental health of 302 residents and specialists in a top general hospital. SPSS 23.0 was used to perform t-test on the relevant data. Results:Among them, 32.5% (98/302) and 49.7% (150/302) of those physicians showed varying degrees of anxiety disorder and depression, of which 6.3% (19/302) and 18.6% (56/302) were moderate or higher anxiety and depression. After they were grouped according to work fatigue scores (boundary as 60 points), risk degree of infection with the COVID-19, frequency of duty, and sleep time (boundary as 7 h), there were statistically differences in the physician self-evaluation rates of anxiety and depression between the two groups ( P<0.05). After controlling the above variables, Logistic regression results showed that the physicians with high risk of infection with the COVID-19 were more likely to show anxiety ( OR=2.142, 95%CI=1.170-3.922) and depression ( OR=2.038, 95%CI=1.185-3.505) than those with low-risk. Conclusion:In the context of the COVID-19 outbreak, residents and specialists, especially those with high risk of infection with the COVID-19, show severe mental health problems such as anxiety disorders and depression, which requires special attention and support.

RESUMO

@#
Objective: To determine the changes in the clinical practice of infertility specialists in the Philippines, specifically, the proportion of specialists who stopped practice, length of break from clinics, the decrease in caseload, changes in consultation platform, screening frequency, number of clinicians that contracted COVI D 19 and safety protocols employed during the COVID - 19 pandemic.

Methods: A cross-sectional study using an online questionnaire given to all members of the Philippine Society for Reproductive Medicine (PSRM) last November 2020. Participants were requested to answer the questionnaire through google spread sheet, with informed consent. All data were collated, summarized and reported in terms of frequencies, and measures of central tendency.

Results: There were 110 active members of the PSRM who participated in the study. There was an overall decrease in the percentage of fertility consultations during the pandemic. There is also a substantial decrease in the caseload from different practices: 45.4% reported more than 50% decrease in !VF-related procedures, 54.5% noted more than 50% decrease in intrauterine insemination procedures, 52.8% reported more than 50% decrease in ovulation induction treatment, and 66.3% reported more than 50% decrease in endoscopic procedures. About 37% of the respondents deferred approximately 5-10 cases for fertility treatment, with 27.3% of the respondents reported deferring embryo transfer to less than 5 couples. More than half of the specialists (51.8%) stopped performing fertility procedures and 68.2% of the respondents completely stopped doing out patient consultations. However, some specialists resumed clinics by May 2020 (30.9%) and June 2020 (30.9%). Upon resumption of clinics, specialists used both face to face consultation and teleconsultation, with the largest proportion of respondents using both platforms. Safety measures employed during face-to-face consultations to prevent disease transmission included hand sanitizers in the clinics, restricted number of people in the waiting room and consultation room, use of personal protective equipment, symptom check prior to face to face consults, installation of air purifiers, and acrylic barriers, and use of health declaration forms. Majority of the specialists are aware of the international and local guidelines regarding infertility care during the pandemic (98.2%) and inform patients regarding the contents of the guidelines (90%).

Conclusion: There is a significant decline in the consultations and fertility procedures during the pandemic. Specialists are well informed of the international and local guidelines regarding fertility care during the pandemic.
