RESUMO Objetivo Verificar a influência das variáveis demográficas e socioeconômicas no limiar de reconhecimento de fala no ruído (LRF) obtidos no teste de dígitos no ruído (TDR) no Português Brasileiro em normo-ouvintes. Método Estudo transversal e prospectivo. A amostra de conveniência foi composta por 151 sujeitos normo-ouvintes com idade entre 12 e 79 anos (média =34,66) que realizaram audiometria tonal liminar e teste de dígitos no ruído branco com sequência de trios numéricos em estímulo diótico (inphase) no mesmo dia. O TDR foi realizado com um smartphone Motorola Z3 play com acesso à internet com fones de ouvido intra-auriculares. Os limiares de reconhecimento dos dígitos no ruído (LRF) foram analisados em função do sexo, idade, escolaridade e nível socioeconômico. Foi utilizado para comparar as amostras independentes, o teste não-paramétrico Kruskal-Wallis e Mann-Whitney, adotando-se o nível de significância de 5%. Resultados A média do LRF foi de -8,47 dBNA (dp -3,89), com mediana de -9,6 dBNA. O LRF foi proporcionalmente inverso à escolaridade e nível socioeconômico e mais negativo (isto é, melhor) com menor faixa-etária. Não houve evidência de influência do sexo no LRF do TDR. Conclusão Idade, escolaridade e nível socioeconômico mostraram influenciar o limiar no TDR; essas variáveis devem ser consideradas na análise de desempenho do TDR no Português Brasileiro em sujeitos normo-ouvintes.


ABSTRACT Purpose Verify how demographic and socioeconomic variables on the in-noise speech recognition threshold (SRT) from the digits-in-noise test (DIN) in Brazilian Portuguese influence normal-hearing subjects. Methods Cross-sectional, prospective study. The convenience sample had 151 normal-hearing subjects between 12 and 79 years (mean=34.66) who underwent pure tone audiometry and digits-in-noise test with white noise using a sequence of three numbers in diotic stimulus (in-phase) on the same day. The DIN was performed using a Motorola Z3 Play smartphone with internet access and in-ear headphones. In-noise digit speech recognition threshold (SRT) was analyzed for gender, age, educational levels, and socioeconomic status. We used the non-parametric version of the Kruskal-Wallis and Mann-Whitney U tests to compare independent samples adopting a significance level of 5%. Results The mean SRT was -8.47 dBNA (SD -3.89) with a median of -9.6 dBNA. The SRT was proportionally inverse to educational levels and socioeconomic status and more negative (better) with lower age groups. Gender did not influence the DIN SRT. Conclusion Age, educational levels, and socioeconomic status influenced the DIN threshold. These variables must be considered when analyzing DIN performance in Brazilian Portuguese in normal-hearing subjects.

Resumen La separación y el cierre monoaural son mecanismos del procesamiento auditivo que permiten mantener interacciones comunicativas significativas en ambientes reales, con condiciones acústicas adversas, ruido competitivo e interlocutores no ideales. Estas habilidades dependen de la redundancia intrínseca, que está determinada por las estructuras y fisiología del sistema nervioso auditivo central, y de la redundancia extrínseca, que está determinada por las pistas acústicas, lingüísticas y contextuales de la señal auditiva. Estos procesos se han evaluado a través de distintas formas de degradación de la redundancia extrínseca, dando origen a tres categorías de prueba: pruebas de habla filtrada de pasa baja, pruebas de habla en ruido y pruebas de habla de tiempo comprimido. Últimamente, se han popularizado las pruebas de habla en ruido, sin embargo, las tres categorías cuentan con documentación que avalan su utilidad, se encuentran disponi-bles en español y es posible aplicarlas tanto en niños como en adultos. A pesar de que algunas cuentan con valores normativos establecidos, es necesario interpretar los resultados considerando su validez ecológica y algunas variables como escolaridad, nivel socio-económico y otros determinantes sociales de la salud que podrían influenciar el rendimiento.


Abstract Monaural separation and closure are auditory processing mechanisms that allow to maintain significant communicative interactions in real environments, with adverse acoustic conditions, competitive noise and non-ideal speakers. These skills depend on intrinsic redundancy, which is determined by the structures and physiology of the central auditory nervous system, and extrinsic redundancy, which is determined by the acoustic, linguistic and contextual cues of the auditory signal. These processes have been assessed through different forms of degradation of extrinsic redundancy, giving rise to three test categories: low-pass filtered speech tests, speech in noise tests and time-compressed speech tests. Speech in noise tests have become popular in recent years, however, all three categories have documentation to support their usefulness, are available in Spanish and can be applied to both children and adults. Although some have established normative values, it is necessary to interpret the results considering their ecological validity and some variables such as schooling, socioeconomic status and other social determinants of health that could influence performance.

Abstract Introduction: Deficient auditory processing can cause problems with speech perception and affect the development and evolution of reading skills. The efferent auditory pathway has an important role in normal auditory system functions like speech-in-noise perception, but there is still no general agreement on this. Objective: To study the performance of the efferent auditory system in a group of children with reading impairment in comparison with normal reading and evaluation of its relationship with speech-in-noise perception. Methods: A total of 53 children between the ages of 8-12 years were selected for the study of which 27 were with reading impairment and 26 were normal reading children. Transient evoked otoacoustic emissions suppression and auditory recognition of words-in-noise test were performed for all the children. Results: The average amplitude of transient evoked otoacoustic emissions suppression showed a significant difference between the two groups in the right (p = 0.004) and in the left ear (p = 0.028). Assessment of the relationship between transient evoked otoacoustic emissions suppression and monaural auditory recognition of words-in-noise scores showed a significant moderate negative relationship only in the right ear (p = 0.034, r = −0.41) of the normal reading children. Binaural auditory recognition of words-in-noise scores were significantly correlated with the amplitude of transient evoked otoacoustic emissions suppression in the right ear (p < 0.001, r = −0.75) and in the left ear (p < 0.001, r = −0.64) of normal reading children. In the reading impaired group, ?a weaker correlation was observed between binaural auditory recognition of words-in-noise scores and transient evoked otoacoustic emissions suppression in the right (p = 0.003, r = −0.55) and in the left ear (p = 0.012, r = −0.47). Conclusions: Transient evoked otoacoustic emissions suppression pattern in the reading impaired group was different compared with normal reading children, and this difference could be related to efferent system performance. Words-in-noise scores in children with impaired reading were lower than in normal reading children. In addition, a relationship was found between transient evoked otoacoustic emissions suppression and words-in-noise scores in both normal and impaired reading children.


Resumo Introdução: O processamento auditivo deficiente pode causar problemas na percepção da fala e afetar o desenvolvimento e a evolução das habilidades de leitura. A via auditiva eferente tem um papel importante nas funções do sistema auditivo normal, como a percepção da fala no ruído, mas ainda não há um consenso sobre isso. Objetivo: Estudar o desempenho do sistema auditivo eferente em um grupo de crianças com dificuldade de leitura em comparação às com leitura normal e avaliação de sua relação com a percepção da fala no ruído. Método: Foram selecionadas para o estudo 53 crianças entre oito e 12 anos, das quais 27 tinham dificuldade de leitura e 26 apresentavam leitura normal. A avaliação por emissões otoacústicas evocadas transientes e o teste auditory recognition of words-in-noise foram feitos em todas as crianças. Resultados: A amplitude média da supressão das emissões otoacústicas evocadas transientes mostrou diferença significante entre os dois grupos na orelha direita (p = 0,004) e esquerda (p = 0,028). A avaliação da relação entre a supressão das emissões otoacústicas evocadas transientes e os escores monoaurais do teste auditory recognition of words-in-noise mostrou uma relação negativa moderadamente significante apenas na orelha direita (p = 0,034, r = -0,41) das crianças com leitura normal. Os escores binaurais do auditory recognition of words-in-noise foram significantemente correlacionados com a amplitude de supressão das emissões otoacústicas evocadas transientes na orelha direita (p < 0,001, r = -0,75) e na orelha esquerda (p < 0,001, r = -0,64) das crianças com leitura normal. No grupo com dificuldade de leitura, uma correlação mais fraca foi observada entre os escores binaurais do auditory recognition of words-in-noise e supressão das emissões otoacústicas evocadas transientes, na orelha direita (p = 0,003, r = -0,55) e na esquerda (p = 0,012, r = -0,47). Conclusões: O padrão de supressão das emissões otoacústicas evocadas transientes no grupo com dificuldade de leitura foi diferente em comparação com as crianças com leitura normal e essa diferença pode estar relacionada ao desempenho do sistema eferente. Os escores de palavras no ruído em crianças com dificuldade de leitura foram menores do que nas crianças com leitura normal. Além disso, foi encontrada uma relação entre a supressão das emissões otoacústicas evocadas transientes e os escores de palavras no ruído tanto em crianças com leitura normal quanto nas com dificuldade de leitura.

Self-reported difficulties in speech-in-noise (SiN) recognition are common among tinnitus patients. Whereas hearing impairment that usually co-occurs with tinnitus can explain such difficulties, recent studies suggest that tinnitus patients with normal hearing sensitivity still show decreased SiN understanding, indicating that SiN difficulties cannot be solely attributed to changes in hearing sensitivity. In fact, cognitive control, which refers to a variety of top-down processes that human beings use to complete their daily tasks, has been shown to be critical for SiN recognition, as well as the key to understand cognitive inefficiencies caused by tinnitus. In this article, we review studies investigating the association between tinnitus and cognitive control using behavioral and brain imaging assessments, as well as those examining the effect of tinnitus on SiN recognition. In addition, three factors that can affect cognitive control in tinnitus patients, including hearing sensitivity, age, and severity of tinnitus, are discussed to elucidate the association among tinnitus, cognitive control, and SiN recognition. Although a possible central or cognitive involvement has always been postulated in the observed SiN impairments in tinnitus patients, there is as yet no direct evidence to underpin this assumption, as few studies have addressed both SiN performance and cognitive control in one tinnitus cohort. Future studies should aim at incorporating SiN tests with various subjective and objective methods that evaluate cognitive performance to better understand the relationship between SiN difficulties and cognitive control in tinnitus patients.

BACKGROUND AND OBJECTIVES: Numerous studies have indicated deterioration of speech perception in noisy conditions among the elderly even those with normal hearing capabilities. The aim of this study was to investigate the effects of age on the speech-in-noise identification by speech-in-noise (SIN) test, subjective ratings of hearing difficulties by speech, spatial, and qualities of hearing scale (SSQ) questionnaire and encoding of fundamental frequency (F0) by Speech auditory brainstem response (ABR) in the elderly and comparing the results with young people. SUBJECTS AND METHODS: The present study was conducted on 32 elderly people aged over 60 years old (17 male and 15 female) with the mean age of 68.9 (standard deviation=6.33) possessing normal peripheral hearing and 32 young subjects (16 male and 16 female) aged 18-25 years old. RESULTS: Findings showed that the score of SIN test is lower among the elderly people as compared with young people in signal-to-noise ratios of 0 and -10 based on Iranian version of SSQ questionnaire (p < 0.001). The range of F0 amplitude in the elderly people is also lower than young people (p < 0.001) in Speech ABR. CONCLUSIONS: It seems that speech processing in older people is deteriorated comparing with young people regardless of their normal peripheral auditory thresholds. This decrease will result in weaker perception and improper segregation of speech from other competing sources.

BACKGROUND AND PURPOSE: Speech-in-noise perception deficits have been demonstrated in patients with mild cognitive impairment (MCI). However, it remains unclear whether the impairment of speech perception varies between MCI subtypes. The purpose of this study was twofold: 1) to compare speech perception performance among MCI subgroups, and 2) to identify the cognitive domains specifically related to speech-in-noise perception. METHODS: We studied 46 patients with MCI and 39 hearing-threshold-matched cognitively normal elderly (CNE) subjects. Two different patient classifications were used: 1) patients with amnestic mild cognitive impairment (aMCI) (n=21) or nonamnestic mild cognitive impairment (naMCI) (n=25), and 2) patients with frontal-executive dysfunction (FED) (n=16) or without FED (n=30). All of the subjects underwent audiometric, neuropsychological, and speech perception assessments. Speech-in-noise perception was measured using sentence recognition tests in the presence of two types of background noise at four levels. RESULTS: First, as the level of background noise increased, the MCI with FED group scored lower than both the MCI without FED and CNE groups under both types of noise. Second, both the naMCI and aMCI groups scored lower than the CNE group, but there were no differences between the naMCI and aMCI groups in sentence recognition under any noise conditions. Third, significant correlations were found between sentence recognition and executive function scores both in the MCI groups and in the CNE group. CONCLUSIONS: Our findings suggest that frontal-executive function is strongly related to speech-in-noise perception and that MCI patients with FED have greater deficits in speech-in-noise perception compared to other subgroups of MCI.

@#This paper describes the systematic process followed in the development of culturally appropriate equalized speech-in-noise sentences suitable for use in an adaptive Speech-In-Noise training protocol for adults in Malaysia. The process involved three iterative phases of development. They were (1) analysis, (2) design and (3) development phases. In the analysis phase, important variables that needed to be considered for speech-in-noise materials were identified through literature review and discussion with the experts in the field. Next, in the design phase, the compilation and formation of sentences, evaluation of naturalness and recording of the speech materials were done. The last phase was the development phase which involved the evaluation of performance intensity function and equalization of intelligibility. The final outcome of these phases were 171 sentences with equal intelligibility that can be used interchangeably in a speech-in-noise training protocol for adults in Malaysia

Abstract Introduction: The difficulty the elderly experience in understanding speech may be related to several factors including cognitive and perceptual performance. Objective: To evaluate the influence of cognitive performance, depressive symptoms, and education on speech perception in noise of elderly hearing aids users. Methods: The sample consisted of 25 elderly hearing aids users in bilateral adaptation, both sexes, mean age 69.7 years. Subjects underwent cognitive assessment using the Mini-Mental State Examination and the Alzheimer's Disease Assessment Scale-cognitive and depressive symptoms evaluation using the Geriatric Depression Scale. The assessment of speech perception in noise (S/N ratio) was performed in free field using the Portuguese Sentence List test. Statistical analysis included the Spearman correlation calculation and multiple linear regression model, with 95% confidence level and 0.05 significance level. Results: In the study of speech perception in noise (S/N ratio), there was statistically significant correlation between education scores (p = 0.018), as well as with the Mini-Mental State Examination (p = 0.002), Alzheimer's Disease Assessment Scale-cognitive (p = 0.003), and Geriatric Depression Scale (p = 0.022) scores. We found that for a one-unit increase in Alzheimer's Disease Assessment Scale-cognitive score, the S/N ratio increased on average 0.15 dB, and for an increase of one year in education, the S/N ratio decreased on average 0.40 dB. Conclusion: Level of education, cognitive performance, and depressive symptoms influence the speech perception in noise of elderly hearing aids users. The better the cognitive level and the higher the education, the better is the elderly communicative performance in noise.


Resumo Introdução: A dificuldade na compreensão de fala dos idosos pode estar relacionada a vários fatores, como o desempenho cognitivo e perceptual. Objetivo: Avaliar a influência do desempenho cognitivo, sintomas depressivos e escolaridade no reconhecimento de fala no ruído de idosos usuários de próteses auditivas. Método: A amostra constituiu-se de 25 idosos usuários de próteses auditivas em adaptação bilateral, de ambos os sexos e média de 69,7 anos. Os indivíduos foram submetidos à avaliação cognitiva por meio do miniexame do estado mental (MEEM) e a escala de avaliação da doença de Alzheimer-cognitiva (ADAS-Cog) e avaliação de sintomatologia depressiva por meio da escala de depressão geriátrica (EDG). Já a pesquisa do reconhecimento de fala no ruído (relação S/R) foi feita, em campo livre, por meio do teste lista de sentenças no português (LSP). A análise estatística incluiu o cálculo de correlação de Spearman e modelo de regressão linear múltiplo, foram adotados coeficiente de confiança de 95% e nível de significância de 0,05. Resultados: No estudo do reconhecimento de sentenças no ruído (relação S/R) houve correlação, com significância estatística, entre a escolaridade (p = 0,018); assim como com os escores dos testes MEEM (p = 0,002), o Adas-Cog (p = 0,003) e o EDG (p = 0,022). Observou-se que, para um aumento de uma unidade no escore do Adas-Cog, a relação S/R aumenta, em média, 0,15 dB e para um aumento de um ano na escolaridade, a relação S/R diminui, em média, 0,40 dB. Conclusão: O nível de escolaridade, o desempenho cognitivo e sintomas depressivos influenciam o reconhecimento de fala no ruído de idosos usuários de prótese auditiva. Quanto melhor o nível cognitivo e maior a escolaridade, melhor é o desempenho comunicativo do idoso no ruído.

BACKGROUND AND OBJECTIVES: Central auditory processing disorder [(C)APD] refers to a deficit in auditory stimuli processing in nervous system that is not due to higher-order language or cognitive factors. One of the problems in children with (C)APD is spatial difficulties which have been overlooked despite their significance. Localization is an auditory ability to detect sound sources in space and can help to differentiate between the desired speech from other simultaneous sound sources. Aim of this research was investigating effects of an auditory lateralization training on speech perception in presence of noise/competing signals in children suspected to (C)APD. SUBJECTS AND METHODS: In this analytical interventional study, 60 children suspected to (C)APD were selected based on multiple auditory processing assessment subtests. They were randomly divided into two groups: control (mean age 9.07) and training groups (mean age 9.00). Training program consisted of detection and pointing to sound sources delivered with interaural time differences under headphones for 12 formal sessions (6 weeks). Spatial word recognition score (WRS) and monaural selective auditory attention test (mSAAT) were used to follow the auditory lateralization training effects. RESULTS: This study showed that in the training group, mSAAT score and spatial WRS in noise (p value≤0.001) improved significantly after the auditory lateralization training. CONCLUSIONS: We used auditory lateralization training for 6 weeks and showed that auditory lateralization can improve speech understanding in noise significantly. The generalization of this results needs further researches.
