RESUMO

BackgroundChronic superficial gastritis (CSG) is a common clinical disease in children. The emotional behavior of CSG children is susceptible due to them suffering from such disease at young age. ObjectiveTo explore the impact of coping strategies on emotional behavior and the effect of family function in children with CSG, and to provide references for clinical intervention in CSG children with emotional behavior problems. MethodsA total of 177 children with CSG admitted to Anhui Children's Hospital from June 2019 to January 2023 were selected as the research subjects. Investigation on family function, emotional and behavioral problems and coping strategies of children was conducted by employing the Family APGAR index (APGAR), the Strengths and Difficulties Questionnaire (SDQ) and Coping Strategies Questionnaire (CSQ). The structural equation model was used to test the mediating effect of family function between coping strategies and emotional behaviors. ResultsThe APGAR score was negatively correlated with both SDQ score and negative coping strategies score (r=-0.507, -0.551, P<0.01), but was positively correlated with positive coping strategy score (r=0.579, P<0.01). The positive coping strategy score was negatively correlated with SDQ score (r=-0.539, P<0.01), while the negative coping strategy score was positively correlated with SDQ score (r=0.543, P<0.01). The result showed that family function played a partial mediating role between positive coping strategies and emotional behavior [indirect effect was -0.133 (95% CI: -0.256~-0.079, P<0.01), accounting for 29.40% of the total effect]. The same mediating effect happened between negative coping strategies and emotional behavior [indirect effect was 0.093 (95% CI: 0.198~0.045, P<0.01), accounting for 28.50% of the total effect]. ConclusionCoping strategies of CSG children can affect emotional behavior directly and indirectly with family function playing a partial intermediary effect.

RESUMO

Chronic superficial gastritis(CSG)is a common disease of the digestive system that possesses a serious pathogenesis.Jinhong tablet(JHT),a traditional Chinese medicine(TCM)prescription, exerts therapeutic effects against CSG.However, the molecular basis of its therapeutic effect has not been clarified.Herein,we employed ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry(UPLC-Q/TOF-MS)based chemical profile identification to determine the chemical components in JHT.Further,we applied network pharmacology to illustrate its molecular mechanisms.A total of 96 chemical constituents were identified in JHT,31 of which were confirmed using reference standards.Based on the bioinformatics analysis using the symptom-guided pharmacological networks of"chi,""blood,""pain,"and"inflammation,"and target screening through the interaction probabilities between compounds and targets, matrix metalloproteinase 2(MMP2),dopamine d2 receptor(DRD2),and Aldo-keto reductase family 1 member B1(AKR1B1)were identified as key targets in the therapeutic effect exhibited by JHT against CSG.Moreover, according to the inhibitory activities presented in the literature and binding mode analysis, the structural types of alkaloids, flavonoids, organic acids, including chlorogenic acid(10),caffeic acid(13),(-)-corydalmine(33),(-)-isocorypalmine(36),isochlorogenic acid C(38),isochlorogenic acid A(41),quercetin-3-O-α-L-rhamnoside(42),isochlorogenic acid B(47),quercetin(63),and kaempferol(70)tended to show remarkable activities against CSG.Owing to the above findings,we systematically identified the chemical components of JHT and revealed its molecular mechanisms based on the symptoms associated with CSG.

RESUMO

Objective To evaluate the gastric microbiome in patients with chronic superficial gastritis (CSG) and intestinal metaplasia (IM) and investigate the influence of Helicobacter pylori (H. pylori) on the gastric microbiome. Methods Gastric mucosa tissue samples were collected from 54 patients with CSG and IM, and the patients were classified into the following four groups based on the state of H. pylori infection and histology: H. pylori-negative CSG (n=24), H. pylori-positive CSG (n=14), H. pylori-negative IM (n=11), and H. pylori-positive IM (n=5). The gastric microbiome was analyzed by 16S rRNA gene sequencing. Results H. pylori strongly influenced the bacterial abundance and diversity regardless of CSG and IM. In H. pylori-positive subjects, the bacterial abundance and diversity were significantly lower than in H. pylori-negative subjects. The H. pylori-negative groups had similar bacterial composition and bacterial abundance. The H. pylori-positive groups also had similar bacterial composition but different bacterial relative abundance. The relative abundance of Neisseria, Streptococcus, Rothia, and Veillonella were richer in the I-HP group than in G-HP group, especially Neisseria (t=175.1, P<0.001). Conclusions The gastric microbial abundance and diversity are lower in H. pylori- infected patients regardless of CSG and IM. Compared to H. pylori-positive CSG group and H. pylori-positive IM, the relative abundance of Neisseria, Streptococcus, Rothia, and Veillonella is higher in H. pylori-positive patients with IM than in H. pylori-positive patients with CSG, especially Neisseria.

Objective:To evaluate the clinical efficacy of modified Liqi-Hewei Decoction combined with conventional western medicine therapy in the treatment of Hp-positive chronic superficial gastritis (CSG). Methods:A total of 96 patients with Hp-positive CSG in the First People’s Hospital of Lianyungang and Changshu No.1 People’s Hospital who met the inclusion criteria between January 2017 and January 2019 were divided into two groups according to the random number table method, with 48 in each group. The control group was treated with conventional western medicine, and study group was additionally given modified Liqi-Hewei Decoction on the basis of control group. Both groups were treated for 8 weeks and followed up for 12 weeks. The TCM symptom score, gastric mucosa and pathological grading were scored before and after treatment. Levels of motilin, gastrin and somatostatin were detected by immunoturbidimetry, and levels of CD3 +, CD4 + and CD8 + were detected by automatic flow cytometry and the ratio of CD4 +/CD8 + was calculated. The Hp negative conversion rate was observed, the adverse reactions were recorded, the clinical efficacy was evaluated and the recurrence rate was statistically analyzed. Results:The Hp negative conversion rate was 89.6% (43/48) in study group and was 72.9% (35/48) in control group, where the difference was statistically significant ( χ2=4.376, P=0.036). The total effective rate was 95.8% (46/48) in study group and was 83.3% (40/48) in control group ( χ2=5.031, P=0.025). After treatment, the scores of epigastric pain, upper abdominal fullness, acid reflux, belching and poor appetite in the study group were significantly lower than those in control group ( t=8.919, 3.971, 7.949, 8.171, 9.865, all Ps<0.01). The scores of gastric mucosa and pathological grading were significantly lower than those in control group ( t=13.705, 15.495, all Ps<0.001). After treatment, the levels of gastrin [(126.15 ± 14.36) ng/L vs. (152.38 ± 17.51) ng/L, t=8.025], motilin [(93.59 ± 11.87) ng/L vs. (102.48 ± 14.68) ng/L, t=3.263] and somatostatin [(36.76 ± 8.97) ng/L vs. (40.84 ± 10.68) ng/L, t=2.027] in the study group were significantly lower than those in the control group ( P<0.01 or P<0.05). The levels of CD3 +, CD4 + and CD4 +/CD8 + were significantly higher than those in the control group ( t=6.883, 6.720, 4.306, all Ps<0.001). The recurrence rate was 4.3% (2/46) in the study group and was 17.5% (7/40) in the control group ( χ2=3.950, P=0.046). During treatment, the incidence rate of adverse reactions was 8.3% (4/48) in the study group and was 12.5% (6/48) in the control group ( χ2=0.446, P=0.504). Conclusion:Modified Liqi-Hewei Decoction combined with conventional western medicine therapy can effectively improve the levels of gastrointestinal hormones and clinical symptoms, enhance the immunity and Hp negative conversion rate, and reduce the recurrence rate of patients with Hp-positive CSG.

RESUMO

PURPOSE: Telomeres are specialized structures containing unique (TTAGGGG) repeats at the ends of eukaryotic chromosomes that are thought to be important in the protection and replication of chro mosomes. Lagging strand DNA synthesis at the end of linear chromosomes cannot be complieted (referred to as the end-replication problem), and this results in the progressive shortening of telomeric repeats with each cell division. This shortening of telomeres has been proposed as the mitotic clock, and when telomeres are sufficiently short, may contribute to replicative cellular senescence in cells. The enzyme telomerase permits the de novo synthesis of telomeric DNA into chromosomal ends. Apoptosis is physiologic cell death and prevent cancerous transformation of cell. METHODS: We measured the telome rase activity with the use of a PCR-based telomerase assay and apoptosis, in 10 superficial gastritis and 13 gastric cancer patients. RESULTS: Telomerase activity was present in 70% of the gastric cancer patient and in 92.7% of the superficial gastritis patients (P=0.171). Apoptosis was present in 0% of the gastric cancer patients, and in 80% of the superficial gastritis patients (P=0.001). CONCLUSION: Telomerase activity was detected in and cancer and inflammatory tissue. Although there is progressive cellular division due to telomerase activity, cancerous transformations are prevented by apoptosis.

RESUMO

BACKGROUND: Helicobacter pylori a worldwide infection in which over 50% of the general population is estimated to be affected. The association of Helicobacter pylori infection with various gastrointestinal disorders is well known. Many dyspeptic patients infected with H. pylori can be treated in primary care. One of the non-invasive means of detecting H. pylori is serology IgG antibody test (GAP). The aim of this study was to investigate the association between the serologic H. pylori IgG antibody response to antibiotic regimen in patients infected with H. pylori. METHODS: A total of 44 patients (21 males, 23 females) who underwent health screening in a university hospital during the period from May 1996 to July 1998 and diagnosed positive by H. pylori serologic IgG antibody test and confirmed to have H. pylori infection on biopsy from endoscope procedure were selected. All the patients had dyspeptic symptoms and were diagnosed as having chronic superficial gastritis on endoscopy. Serological determination of the IgG antibodies against H. pylori was carried out before starting treatment for H. pylori eradication and 6 month after treatment. RESULTS: The number of patients was 44 with 21 (47.7%) males and 23 (52.3%) females. In both males and females. Among those with positive H. pylori antibody who were treated with antimicrobial therapy the rate of seroconversion to negative was 28.6% in males and 39.1% in females. Those who were checked positive for H. pylori antibody and were shown to be still positive but with lower titers after eradication therapy was 57.1% for males and 47.8% for females. Those with positive H. pylori antibody who had shown seroconversion or decreased antibody titers after antimicrobial therapy were 85.7% for males and 86.9% for females. The number of subjects who failed to result in seroconversion or conversely had increased titers for H. pylori antibody after antimicrobial therapy was 14.3% for males and 13.1% for females. Overall, among those treated with BMT regimen, 11 out of 28 (39.3%) seroconverted and 12 out of 28 (42.9%) resulted in lower antibody titers. Among those treated with BMA regimen, 4 out of 16 (25.0%) seroconverted and 11 out of 16 (68.8%) resulted in lower antibody titers. Failure rates of BMT and BMA regimens were 5 out of 28 (17.8%) and 1 out of 16 (6.2%), respectively. CONCLUSION: Based on above results, antimicrobial therapy of patients infected with H. pylori clearely indicate improvement of serologic IgG antibody resulting in seroconversion and/or decrease in titers with high rates. More accurate results may be obtained if the follow-up period is extended with the use of more reliable non-invasive techniques to detect H. pylori infection before and after treatment.

RESUMO

Superficial gastritis has been classified as a type of chronic gastritis, since a report of Schindler in GASTRLTIS, 1947. Howev~er, Benedict reported that superficial gastritis is only acute gastritis or shows normal mucosa histologically. The com mon endopical findings of chronic superficial gastritis were adherent mucus, edema, redness. The common redness which were encounterd are patchy redness and comb-like redness(Kammrotung). We studied the relationship between the redness of superficial gastritis and acute inflammatory changes histologically. Each case of superfieial gastritis was biopsied to redening and non-redening mucosa respectively. We collected 24 cases with 48 biopsy specimens. The results are as follows: Acute inflammatory changes were found only one case in redness and none in non-redness groups. There was no difference in acute inflammatory changes in two groups. The degree of mucosal atrophy were 62.5%, 66.7% in normal mucosa, 12.5%, 16.7~% in mild atrophy, 25%, 12.5% in moderate atrophy, 0%, 4.l% in severe atrophy with respect to redness and non-redness mucosa respectively. There was no difference in degree of mucosal atrophy in two groups. Mucosal atrophies were higher in older ages above 41 years old than below 40 and with increasing age, there was increasing tendency of mucosal atrophy.
