RESUMO

Objective To compare vacuum-assisted excision with periareola incision surgery for benign breast cancer.Methods One hundred and twenty one patients with benign breast nodules treated in our hospital from October 2014 to October 2015 were enrolled in the study,including 58 cases received vacuum-assisted excision (study group) and 63 cases received periareola incision surgery (control group).There were no differences in age[(30.2 ±5.4) vs.(32.1 ±6.2) years] and tumor diameter[(1.3 ±0.4) vs.(1.5 ± 0.3) cm] between two groups.Results There were no severe complications in both groups.Compared with the control group,the operation time was shorter] (20.4 ±4.7) vs.(48.2 ± 15.4)min,t =3.165,P =0.006],intraoperative blood loss was less [(18.4 ± 5.7) vs.(17.2 ± 3.7) ml,t =-0.297,P =0.672],malformation rate was lower [5.2% (3/58)vs.22.2% (14/63),x2 =7.27,P =0.007],incidence of postoperative hematoma was higher [20.7% (10/58) vs.6.3% (4/63),x2 =3.502,P =0.041],and the cost of surgery was more expensive(5 100 ± 300) vs.(3 400 ± 1 200) Yuan,t =2.181,P =0.023].Conclusion Comparing with conventional periareolar incision surgery,the vacuum-assisted excision has its advantages and disadvantages for treatment benign breast tumors,and two surgical modalities can be chosen accordingly.

RESUMO

Prior to the formation of new metastases,a large number of cancer cells released from the primary tumor enter the dormancy period.Once the dorruancy is broken,tumor cells will recover the capability of proliferation.In recent years,various hypotheses and mechanisms of tumor metastasis have been studied.Primary tumor resection is considered to be an important factor to break tumor dormancy state.It is recognized as an important reason to promote tumor metastasis.The improvement of surgical technique and further study on the mechanism of dormancy may provide new ideas for the treatment of metastatic tumor.

RESUMO

Objective To evaluate autologous buccal mucosal patch grafting for correcting penile deformity in the surgical treatment of Peyronie's disease. Methods 14 patients with symptoms of Peyronie's disease for more than three months suffered from persistent pain ,severe curvature of the penile shaft and resistant to conservative treatment underwent plaque incision and autologous buccal mucosa patch grafting. Results Postoperative follow up has been 0.5 to 2years.The penile shaft became straight with no narrowing or indentation in 12 patients (86%) . In 14% of cases (2/14) some curvature of the penile shaft persisted and there was pain on erection.In all the patients there was no change in penile length. 86% of patients had overall satisfaction after surgery with improvement in psychological and social well-being. Conclusions The autologous buccal mucosal patch graft provides an anatomical and functional tunied substitute resulting in correction of the penile deformity in Peyronie's disease.

RESUMO

Objective To improve the surgical technique for the removal of huge staghorn stones. Methods 202 cases treated with an incision of the intrarenal sinus supplemented by a postrenal lower pole segmental incision for the removal of huge kidney stones were studied. Results The biggest stone was 9.8 cm?5.0 cm?3.2 cm and in one case the stones amouned to 74. The average postoperative hospitalization days was 12.8. Postoperative examination with KUB+IVU or B ultrasound revealed residual stone in 9 cases, the diameters being less than 0.8 cm. Conclusions The procedure needs no interruption of renal pedical and with the advantages of simple,less bleeding,complete removal of calculus,and preservation of renal function.

RESUMO

Objective To evaluate wedge resection of the back labium renalis and intrasinusal pyelolithotomy for staghorn calculi of kidney in patients with intrarenal pelvis and renal malrotation. Methods From August 1998 to February 2001, 18 cases of staghorn stones of kidney were removed via the wedge resection of the back labium of hilum renalis and incision of the intrarenal sinus. Results The mean operative time was 110 min and the mean blood loss 300 ml.The biggest stone was 6.5 cm?5.0 cm?3.0 cm and in one case the number of stones amounted to 1 400. Postoperative KUB showed no residual stones and postoperative IVU indicated normal renal function.All patients have been followed up for 1~3 years and have been free of stone. Conclusions The procedure has the advantages of no need of pedicle renalis occlusion,less bleeding,a clear operation field,easy staghorn calculi removal and the avoidance of renal hilum outlet obstruction.
