RESUMO

The purpose of this case report and review is to present the successful management of complicated crown-root fracture of maxillary central incisor with a multidisciplinary approach. A 57-year-old male patient reported with a complaint of unpleasant appearance due to a broken 21. Clinical and radiological examination revealed a complicated oblique crown-root fracture extending below the gingival margin. The tooth was endodontically treated. Surgical crown lengthening with osseous recontouring was performed. After 7 days prefabricated temporary crown was cemented, 6 weeks after surgery fiber post was placed followed by composite core build up. The tooth was restored with a permanent restoration of porcelain fused to the metal crown with gingival porcelain. Examination after 3 months revealed the good periodontal health, esthetic, normal function and patient satisfaction. The key factor in the successful functional and esthetic rehabilitation of a complicated crown-root fracture is weighing of all the available options with a multidisciplinary approach.

OBJECTIVES: Crown-root fracture and cervical caries in maxillary premolars constitute a challenge in cases of subgingival placement of restoration margins. Surgical extrusion has been practiced successfully in permanent anterior teeth. The aim of the present retrospective study was to assess the clinical outcome of surgical extrusion after orthodontic extrusion in maxillary premolars. MATERIALS AND METHODS: Twenty-one single, tapered root maxillary premolars with subgingival crown-root fracture or caries were included. Presurgical orthodontic extrusion was performed on all teeth to prevent root resorption. Extent of extrusion and rotation was determined based on crown/root ratio. The postoperative splinting period was 7 to 14 days. Clinical and radiographic examination was performed at an interval of 1, 2, and 3 months. RESULTS: After the mean follow-up of 41.9±15.2 months, failure was observed as increased mobility in 3 of 21 cases. No significant difference was observed in the outcome of surgical extrusion based on tooth type, age, sex, 180° rotation, or time for extraction. Furthermore, marginal bone loss was not observed. CONCLUSION: Surgical extrusion of maxillary premolars can be a possible therapeutic option in cases of subgingival crown-root fracture.

RESUMO

Clinical crown lengthening procedure would be an effective method for overcoming adverse clinical condition such as short abutment length. There are three kinds of methods in clinical crown lengthening, those are, surgical crown lengthening, orthodontic extrusion and surgical extrusion. Clinicians have to try their best to choose a proper method among those for favorable results. This report aims to review the considerations in each method with various cases and to suggest a decision flow for appropriate selection.

A cirurgia plástica periodontal tem sido indicada para pacientes que apresentam descontentamento com seu sorriso ou desarmonia em relação a sua anatomia gengival. O procedimento tradicional tem sido realizado através da elevação de um retalho para expor o tecido ósseo e subsequente, osteotomia/osteoplastia. Este procedimento exige sutura e um maior tempo de cicatrização, tornando o pós-operatório mais desfavorável em relação às técnicas menos invasivas. Com o avanço das técnicas minimamente invasivas, tem sido proposta a técnica Cirúrgica Periodontal "Flapless", um procedimento minimamente invasivo e inovador, com resultados clínicos extremamente favoráveis. Tem como principal indicação pacientes com o biótipo periodontal fino e intermediário e o planejamento cirúrgico consiste na realização de uma tomografia computadorizada do paciente utilizando um afastador bucal tipo expandex para afastar os tecidos moles e facilitar a visualização da crista óssea vestibular e, desta forma,visualizar e calcular a quantidade de tecido ósseo a ser removido. Após a excisão do tecido gengival, é realizada a osteotomia via sulco gengival, sem abertura de retalho, com utilização de microcinzéis. Um dos principais benefícios é a finalização do procedimento sem a necessidade de suturas, menor desconforto pós-operatório, melhor reparação tecidual e harmonia estética. As evidências clínicas e científicas mostram que o aumento de coroa clínica com a técnica "Flapless" atinge resultados satisfatórios em menor tempo, melhor pós-operatório e previsibilidade (AU)


The increase in clinical crown "Flapless" is a common procedure employed at periodontal plastic to patients with discontent with its gummy smile or with the in harmonious of its gumlines hape. The traditional procedure is performed it the flap lifting to expose the bone and then, osteotomy/ osteoplasty. As a resultof the flap, it requires suturing, making it uncomfortable postoperative. However, with the advancement in a esthetic and constant demands for searching improved results, it was proposed the "Flapless" Technique, which consist of minimally invasive and innov ative procedure, with very favorable clinical out comes. It is mainly indicated to treat patients with thin and intermediate periodontal biotype. Aided by computerized tomography with Expandex, we have a predictability of the amount of bone to be removed. After excision of gingival tissue, gingival sulcus via osteotomy is performed with out opening flap, using micro-chisels. One of the main benef its is the completion of the procedure without sutures, less postoperative discomfort, optimizedt issue repair and perfect as the tic harmony. Clinical and scientific evidence shows that te increase in clinical crown flapless reache satisfactory results in a shorter time, best post operative and predictability in clinical out come. (AU)

BACKGROUND: In order to restore severely attrited teeth properly, surgical intervention in the form of a crown-lengthening procedure may be required. And also, proper diagnosis and treatment sequencing is critical to obtain a successful results. Adequate diagnostic wax-up ensures good esthetics and healthy periodontal tissue. CASE DESCRIPTION: This clinical case report describes a diagnostically based protocol for restoration on mandibular anterior teeth with crown lengthening procedure and the treatment of partially edentulous mandible combined with an edentulous maxilla. In addition, the effort to prevent the combination syndrome was described. CLINICAL IMPLICATION: An interdisciplinary diagnosis and examination through visualization of the desired results ensure conservative and more predictable outcome.
