RESUMO

Cancer-related fatigue (CRF) belongs to the category of "consumptive disease" in TCM, and its occurrence is based on "internal deficiency" of the body causing by the tumor. Its nature is intermingled deficiency and excess. Its pathogenesis is the deficiency of qi, blood, yin and yang and zang-fu viscera dysfunction caused by disorders of "rise and fall of middle qi" and kidney origin depletion. The theory of "treating overstrain syndrome with warming methods" originates from Huang Di Nei Jing, which proposes that warming methods are the basic methods of treating consumptive disease. Therefore, starting from the cause and pathogenesis of CRF, this article sorted out the theoretical origin of "treating overstrain syndrome with warming methods", and discussed the clinical application of warming methods for the treatment of CRF combining with modern clinical research, with the purpose to provide references for clinical practice.

Objective To investigate the clinical effect of traditional Chinese medicine syndrome differentiation-based treatment combined with entecavir in the treatment of chronic hepatitis B. Methods A total of 80 outpatients with chronic hepatitis B were randomly divided into the observation group (n=40) and control group (n=40).The patients in the control group were treated with entecavir tablets. The patients in the observation group were given TCM syndrome differentiation-based treatment in addition to the treatment received by the control group. Clinical symptoms relief, improvement of liver function indexes, serological conversion and HBV-DNA negative rate were compared between the two groups after 48 weeks of treatment. Results The clinical symptoms of abdominal distension, fatigue, pain and anorexia were relieved better in observation group than in control groups. The difference was significant between the two groups (P<0.05). The observation group had significant therapeutic advantages over the ALT recurrence rate compared to the control group, especially 24 weeks ago (P<0.05). There was no statistically significant difference between the combined group and the control group after 12, 24, 48 weeks after treatment, and the HBV-DNA (both greater than 0.05) were clinically modified. Conclusion TCM syndrome differentiation-based treatment combined with entecavir had significant therapeutic advantages in the treatment of chronic hepatitis B, which could relieve clinical symptoms, improve liver function indexes, and converse serological changes and be worthy of clinical popularization.

Objective@#To explore the clinical effect of traditional Chinese medicine (TCM) syndrome differentiation in the treatment of pneumoconiosis.@*Methods@#We selected 160 pneumoconiosis patients who were hospitalized in our hospital from July 2015 to July 2016. 102 patients were divided into experimental groups, and the other 58 patients were assigned to the control group. These patients were randomly assigned. The control group was given routine treatment, while the experimental group received conventional treatment combined with TCM syndrome differentiation. We rated patients' quality of life using the SF-36 Quality of Life Scale. C-reactive protein levels in both groups before and after treatment were detected by ELISA. Furthermore, we detected the expression levels of IL-6 mRNA and TGF-β mRNA in serum before and after treatment by RT-PCR.The analysis was performed with SPSS19.0 software. The normal distribution measurement data was expressed by Mean±SD, and the t-test was used for statistical analysis. Non-normal distribution measurement data were expressed as median(M) and interquartile range (P25, P75), and non-parametric test was used for statistical analysis.@*Results@#Compared with the control group, the readmission rate in the experimental group decreased within 3 months; The SF-36 quality of life score results of the two groups showed a statistically significant difference in BP, VT and RE (P<0.05);The levelofCRPdecreasedsignificantlyafter treatment in bothgroups,and thedifferencewasstatistically significant (P<0.05).Comparedwith thecontrolgroup, the levelsofIL-6mRNAandTGF-β1mRNA alsodecreased (P<0.05).@*Conclusion@#TCM syndromedifferentiation and treatmentofpneumoconiosis can effectively improve the patient's condition and life quality. We predict that it may inhibit the progression of pulmonary fibrosis in pneumoconiosis.

Objective To assess the effectiveness of traditional Chinese Medicine (TCM ) syndrome differentiation treatment for diabetic nephropathy in elderly patients. Methods A total of 96 elderly patients with diabetic nephropathy admitted to our hospital from January 2015 to December 2017 were recruited and divided into a regular care group (n= 48)and a TCM group(n= 48).Both groups received conventional treatment ,with TCM syndrome differentiation treatment added to the TCM group. The effectiveness of the treatment modalities was compared. Results In the regular care group ,significant effectiveness was observed in 12 patients ,effectiveness in 24 patients ,and ineffectiveness in 12 patients ,with a total effectiveness rate of 75% .In the TCM group ,significant effectiveness was seen in 18 patients ,effectiveness in 27 ,and ineffectiveness 3 ,with a total effectiveness rate of 93%. The total effectiveness rate of the TCM group was higher than that of the regular care group(χ2 =6.400 ,P=0.011).After treatment ,fasting blood glucose ,2 h postprandial blood glucose , urea nitrogen ,creatinine ,and 24 h urine protein in the TCM group were significantly lower than in the regular care group and than pre-treatment levels(P<0.05). Conclusions TCM differentiation can not only significantly improve the treatment effectiveness in elderly patients with diabetic nephropathy ,but also relieve the clinical symptoms.

TCM is a traditional medicine, and the modernization of TCM is urgent. Whether in the medical management or scientific research, the application of computer technology has penetrated into all levels. The following article described the contribution made by computer technology, pointed out possible problems and provided relevant thinking based on the application of TCM related systems and data mining technology.

Objective To provide references for enriching and innovating patterns of TCM syndrome differentiation by exploring the relationship between pathological changes and TCM syndrome differentiation of delayed graft function (DGF) after renal transplantation. Methods Totally 57 patients with DGF after renal transplantation who were diagnosed by pathological biopsy were also diagnosed by TCM doctors to obtain TCM syndromes. And the pathological diagnoses were compared with the distribution of TCM syndromes to discuss the relationship between them. Results All the patients were classified into four TCM syndromes:spleen-kidney qi deficiency, liver-kidney yin deficiency, qi-stagnation and blood stasis, and deficiency of both qi and yin. The results of pathological biopsy were as follows:acute tubular necrosis, calcineurin inhibitor toxicity and acute rejection. Quantitative corresponding relation was found between the pathological diagnoses and the TCM syndromes. Conclusion There are inner links between pathological diagnosis and TCM syndromes of DGF, which can be references for diagnosis and treatment of TCM syndrome differentiation.

Objective To investigate the relationship between primary TCM syndromes and heart rate variability in hypertension, evaluate the development of disease. Methods 182 hypertension patients were recruited in accordance with the standard of hypertension. According to the Chinese medicine dialectical criteria, these patients were further divided into 5 groups, namely, hyperactivity of liver-fire group, phlegm-dampness retention group, blood stasis group, hyperactivity of Yang due to yin deficiency group and Qi and blood deficiency group. Another 60 healthy people were recruited as a control group. 24h Holter was adopted to monitor heart rate variability. Results Analysis of TCM syndromes and heart rate variability, anger on inflammation group had positive relation with SDNN, RMSSD, LF, HF and TP(r were respectively 0.161,0.372, 0.223, 0.281, 0.301);Phlegmy wet resistance groups had positive relation with SDNN, HRV triangular index, VLF, LF, VLF/HF, LF/HF, LFnorm.(r were respectively 0.175, 0.211, 0.174, 0.262, 0.159, 0.264, 0.263);blood stasis group had negative relation with LF/HF, LFnorm(r were respectively-0.185、-0.018).Yang due to yin deficiency group had negative relation with SDNN, RMSSD, VLF, LF.(r were respectively -0.204, -0.161,-0.184,-0.197);Qi and blood deficiency group had negative relation with SDNN, SDANN, RMSSD and HRV triangular index, VLF, LF, LFnorm, and had positive relation with HFnorm(r were respectively-0.353,-0.348,-0.258, -0.303, -0.268, -0.361, -0.352, 0.267). Conclusion Hypertension patients with different type of TCM syndromes had different characteristics of autonomic nerve functional disorders, which both reflected the important pathological changes of TCM syndromes and had close relation with the course of hypertension, providing objective basis for clinical diagnosis of syndrome differentiation.

Objective To observe the effect of the Chinese medicine on the patients’ thyroid autoantibodies TPOAb and TGAb of Hashimoto's thyroiditis. Methods 100 cases of Hashimoto's thyroiditis were randomly divided into group A and group B, with 50 cases in each group. Group A was given levothyroxine sodium(L-T4) to maintain thyroid function(FT3,FT4,TSH)in the normal range, at the same time Chinese medicines of soothing liver and strengthening spleen, nourishing the liver and kidney, activating blood and removing blood stasis were additionally added;while group B was taken L-T4 to maintain the thyroid function in the normal range. The levels of TPOAb, TGAb were determined before and after treatment in both groups. Results After the treatment, the level of TGAb and TPOAb[respectively(106.3±29.5)IU/ml,(871.5± 209.3)IU/ml] in group A were decreased compared with their previous level [respectively(385.5±76.6)IU/ml, (1621.5±399.2)IU/ml], the difference was statistically significant(t were 48.2、10.6,P<0.01). The level of TGAb and TPOAb [respectively(437.6±135.4)IU/ml,(1798.6±434.6)IU/ml] in group B were slightly increase than their previous level[respectively(383.9±105.8)IU/ml,(1633.2±396.5)IU/ml], with no significant difference. The levels of TPOAb and TGAb in group A had significant difference than those in group B after the treatment(t were 22.3、19.6,P<0.01). Conclusion TCM combined with L-T4 can reduce the level of thyroid autoantibodies of TPOAb and TGAb-in patients.

This paper aims to discuss the distribution status of insulin resistance in TCM syndromes of Type 2 diabetes mellitus and to provide objective indicators for TCM syndrome differentiation of Type 2 diabetes mellitus.The research methods of the paper involve:Select 180 cases of Type 2 diabetes mellitus as subjects to be investigated and divide them into three groups of yin-deficiency dryness-heat, dampness-heat distressing spleen and yang-deficiency dampness-stasis.Another 60 healthy cases are selected as comparison group. Indicators such as height,body mass,asting plasma glucose (FPG),fasting insulin (FINS), glycosylated hemoglobin (HbA1C), three acids glyceride(TG),cholesterol (TC) etc,are tested and body mass index (BMI) ,insulin sensitivity index (ISI) are calculated.The comparisons are made between such objective indices and TCM syndromes to discuss the features and distribution rules of TCM syndromes.The results show that the dampness-heat distressing spleen type is the most common; the yin-deficiency dryness-heat type has the shortest course of diseases.The ISI in each group is lower than that of the healthy comparison group (P

@# ObjectiveTo explore the characteristics of traditional Chinese medicine (TCM) syndrome differentiation in chronic complications of type 2 diabetes mellitus (DM).Methods124 patients with chronic complications of type 2 DM were scored by 5 grades according as the severities of their symptoms. There were 5 kinds of patterns such as deficiency of Qi, deficiency of Yin, deficiency of Yang, blood stasis and retention of phlegm and fluid by which the TCM syndrome differentiation was generalized.ResultsThe sequence of TCM patterns was deficiency of Yin, blood stasis, deficiency of Qi, deficiency of Yang and retention of phlegm and fluid, and the syndrome of the two formers were greater than 50%. The proportion of unity of deficient and excessive pattern was 80.5%. Three larger syndrome types were deficiency of both Yin and Yang combined with blood stasis (17.7%), Qi-Yin deficiency with blood stasis ( 16.9 %) and Yin deficiency with blood stasis (16.9%). There was a statistically significant difference in TCM syndromes which were divided into different groups by course of diseases (P<0.05). At onset of DM, the typical symptoms were less observed in the group whose course of disease was less than 5 years, and only 39.1% of patients had the typical symptoms. But at the same time, the prevalences of hypertension and hyperlipidemia were higher in this group than in the others, respectively 63.0% and 87.0%.ConclusionThe primary syndrome is unity of deficient and excessive pattern in chronic complications of type 2 DM and deficiency of Qi and blood stasis are the commonest patterns in course of DM.

Objective:To observe characteristics of TCM syndrome types in the patient of cerebral infarction,and cerebral infarction with respiratory disorder at sleep.Methods:90 in-patients of cerebral infarction were monitored by polychannel sommocinematogram (PSG)within 72 hours of admission,which were used as diagnosis of respiratory disturbance at sleep,and all of the patients with apoplexy involving the channels and collaterals were divided into 3 types,collaterals hollow and pathogenic wind attacking the interior;Yin-deficiency of the liver and kidney,and wind-Yang stirring upper,hot phlegm and excess of Fu-organs,and wind- phlegm stirring the upper.Results:The patient had sleep structure disorder,with large change of blood pressure and lower heart variety,and decrease of degree of blood oxygen saturation.The apnea indexes were the highest for the patient with phlegm-heat hollow-organ excess,wind-phlegm stirring the upper,and baic normal for collateral hollow and pathogenic wind attacking the interior.Conclusion:There is close relation of cerebral infarction,especially in the patient of hot phlegm and excess of Fu-organ, with respiratory disorder at sleep.

Objective:To explore effect of serum leptin level on nutriture in the patient of chronic renal failure(CRF)and the relation with TCM syndrome types of CRF.Methods:110 cases in CRF group were divided into 5 subtypes and 30 normal persons were used as control group.Serum leptin level,body height,body weight,and body mass index(BMI)were determined in both groups;and serum albumin,serum pre-albumin,transferin,serum creatinine,urea nitrogen,C-reaction protein,blood insulin,and nutrition risk index(NRI)were detected in CRF group.Results:Serum leptin level in CRF group was higher than that in the control group,and leptin level and nutritional index level in both the subtype groups of Qi-and Yang-deficiency of spleen and kidney were significantly different with those in other 3 subtype groups.Conclusion:High blood leptin is possibly one of the reasons inducing malnutriture of CRF patient,and serum leptin is possibly a partial basis of substances of Qi-and Yang-deficiency syndrome of CRF.
