Telepsiquiatría en niños y adolescentes: aplicación en contexto de pandemia / Telepsychiatry in children and adolescents: application in the context of a pandemic

Telemedicina, se refiere a las prestaciones de salud a distancia. Existen muchos modelos de telemedicina descritos e implementados en distintos lugares del mundo. La telepsiquiatría (y telemedicina) que se está implementando hoy en Chile y el mundo, ya sea desde un marco institucional o privado, es directa hacia el paciente, esto implica que no es una "inter -o tele -consulta" donde hay un profesional presencial que se hace responsable de la indicación, en este caso la atención completa y directa médico-paciente se lleva a cabo a distancia, quebrando, de alguna manera, la regla de oro de la atención clínica que es la consulta presencial entre el médico y el paciente. Se revisan algunas sugerencias descritas en la literatura y otras que ha dado la experiencia. La idea de este artículo es discutir los desafíos clínicos, éticos y legales, junto con los beneficios y oportunidades de la telepsiquiatría en el contexto actual.Palabras claves: Telemedicina, telepsiquiatría, teleconsulta, médico-paciente, covid-19.

Summary. Telemedicine refers to remote health care. There are many telemedicine models described and implemented in different parts of the world. The telepsychiatry (and telemedicine) that is being implemented today in Chile and the world, either from an institutional or private framework, is directed towards the patient, this implies that it is not an "tele-consultation" where there is a another face-to-face professional who is responsible for the prescription of treatment. In our case, the complete and direct doctor-patient care is carried out at a distance, breaking in some way the golden rule of clinical care that is face-to-face consultation between the doctor and the patient. Some suggestions described in the literature are reviewed and we make other suggestions based on our experience. The idea of this article is to discuss clinical, ethical and legal challenges, along with the benefits and opportunities of telepsychiatry in the current context.Key words: Telemecine, telepsychiatry, tele-consultation, physician-patient, covid-19.

Application analysis of radiological imaging tele-consultation / 中华医院管理杂志

Objective To investigate radiological imaging tele-consultation in clinical application. Methods All cases of the tele-consultation analyzed retrospectively came from the Hospital′s telemedicine center from January to December of 2015. The statistical analysis covered the level of applying hospital, modality type of imaging,inspection area,as well as the timing and purpose of the consultation.Results A total of 179 medical institutions required image consultation in 2015,most of which secondary hospitals,up to 58.66%. A total of 2 454 cases were consulted,including X-ray of 278 cases (11.33%),CT of 1 353 cases (55.13%), and MRI of 823 cases (33.54%). Purposes of tele-consultation vary from differential diagnosis of 1 602 cases(65.28%),definitive diagnosis of 766 cases(31.21%),and confirmative review of 86 cases (3.51%). 90.59% of the consultation cases were finished within 24 hours. Conclusions Radiological imaging tele-consultation could offer local hospitals with diagnosis efficiently effectively. On one hand,this service saves patients'costs and gains time for treatment. On the other,it makes primary hospitals more efficient in radiological imaging diagnosis.

The clinical application and comparative analysis of mobile tele-consultation terminal based on cell phone / 中国医学装备

Objective:To explore the current situation of obtaining high quality and efficient medical service in clinical application of mobile tele-consultation terminal about cell phone. Methods: The technical basis of mobile teleconsultation terminal of cell phone and its application status in domestic and abroad were analyzed. This method was compared with other tele-consultation method at service, mode and cost and other aspects.Results: In the clinical application, mobile tele-consultation terminal about cell phone has many positive roles and advantages, for example, it could enhance the level of diagnosis and treatment for doctor who work in district and community, and resolve medical burden for patients, and promote grading diagnosis and treatment.Conclusion:The mobile tele-consultation terminal can obtain high quality and efficient medical service by investing lower capital. And with the continuous development of the network tele-consultation technique, satellite consultation technique and mobile terminal consultation of cell phone in future, they will further enhance the hospital medical service ability.

Implementation of Digital Field Medical Clinic Under Wireless Network Based on HIS / 医疗卫生装备

Objective To improve the informatization of field clinic for quicker and more effective cure of the wounded and the sick.Methods Making the use of the experiences from many years' application of No.1 Military Medical Project,the digital field clinic was set up based on HIS under the circumstances of wireless network.Results The digital field clinic provided convenience for doctors and nurses,optimized the curing procedure,standardized the material management,improved the curing efficiency,and enlarged the curing scope.Conclusion The digital field clinic facilitates the data transmission of medical nursing and assistant diagnosing photos.During the implementing process,the digital field clinic also provides new ways and ideas for the informatization of medical support of other types.

Localization of tele-consultation by integration of military medical tele-information resources / 医疗卫生装备

ST-TELEMED Perfect Information Tele-Consultation system is developed based on No.1 & No.2 Military Medical Project,which is the first PLA's consultation system integrating HIS,PACS,RIS and LIS.Thus information share in PLA's medical system and the treatment capability of primary medical teams can be improved.
