RESUMO

A study was conducted in São Paulo, Brazil, to compare azithromycin with thiamphenicol for the single-dose treatment of chancroid. In all, 54 men with chancroid were tested. The etiology was determined by clinical characterization and direct bacterioscopy with Gram staining. None of the patients had positive serology or dark-field examination indicating active infection with Treponema pallidum. Genital infections due to Neisseria gonorrhoeae and herpes simplex virus were excluded by polymerase chain reaction testing. For 54 patients with chancroid, cure rates with single-dose treatment were 73 percent with azithromycin and 89 percent with thiamphenicol. HIV seropositivity was found to be associated with treatment failure (p=0.001). The treatment failed in all HIV positive patients treated with azithromycin (p=0.002) and this drug should be avoided in these co-infected patients. In the view of the authors, thiamphenicol is the most indicated single-dose regimen for chancroid treatment.

RESUMO

Ten patients with donovanosis were treated with thiamphenicol for two weeks. In eight of them, included two HIV infected patients, lesions healed. The safety profile of thiamphenicol makes it a useful and cost-effective agent in the management of donovanosis. Randomized controlled trials should be conducted with these treatment options.

A comparative study of the treatment of non-specific vaginitis was carried out in order to compare the efficacy of the use of tetracycline and thiamphenicol among women attending the sexually tranamitted disease clinic of the Department of Obstetrics and Gynaecology, Siriraj Hospital. The inclusion criteria included women with abnormal vaginal discharge having a pH of 5 or more but in whom the presence of Trichomonas vaginalis, Candina albicans or Neisseria gonorrhoea was not observed. Eighty-one women were enrolled in this study and were randomly categorized into two groups, i.e. the tetracycline treatment group and the thiamphenicol treatment group. Two grams of each drug were given orally for seven consecutive days to the women in the respective groups. In this study, vaginal discharge was cultured for Gardnerella vaginalis, Chlamydia trachomatis and for aerobic bacteria; the growth of G. vaginalis and C. trachomatis was encountered in 71.6 and 12.35 percent of the cultured, respectively. The cure rate achieved with tetracycline was 82.93 percent while that of thiamphenicol was 97.5 percent. In this study equal dosage of thiamphenicol yielded a significantly (P=.028) better result than tetracycline in the treatment of non-specific vaginitis with regard to improvement of signs and symptoms. No adverse side-effect was encountered in either treatment group.

OBJECTIVE:To analyze the problems existing in the use of thiamphenicol for outpatients. METHODS:In this retrospective study,a total of 743 outpatient prescriptions of Thiamphenicol enteric-coated tablets in one hospital during Mar. 2008 were analyzed in respect of the prescription writing,medication suitability and amount of money of prescriptions,etc. RESULTS:The problems in the use of Thiamphenicol manifested as nonstandard in prescription writing,irrational in clinical drug choice,drug combination,administration and dosage,etc. CONCLUSION:The use of Thiamphenicol in the hospital is somewhat irrational thus remains to be standardized further.
