RESUMO

@#Objective    To verify the feasibility of a self-designed magnetic anchoring and traction device (MATD) for assisting two-port video-assisted thoracoscopic esophagectomy. Methods    Three Beagle dogs were selected as animal models with age ranging from 1-6 years and weight ranging from 8-12 kg, and they underwent two-port video-assisted thoracoscopic esophagectomy after general anesthesia. We used the MATD to retract the esophagus to different directions, which assisted mobilizing esophagus, detecting the nerves along esophagus and dissecting paraesophagus lymph nodes. The operation time, blood loss and feasibility of the MATD were recorded. Results    With the aid of the MATD, we successfully retracted and mobilized the esophagus, detected the nerves and dissected the lymph nodes in three Beagle dog models. During the operation, the MATD provided sufficient and steady traction of esophagus to achieve a good exposure of the operative field, effectively decreasing the interference between working instruments. The MATD worked well. The mean operation time was 30 min, and the mean intraoperative blood loss was about 10 mL. Conclusion    It is effective to use the MATD to assist retracting esophagus during video-assisted thoracoscopic esophagectomy. The magnetic anchoring and traction technique can assist to expose the surgical field, decrease the interference between the working instruments and have the potential clinical application.

RESUMO

Objective To observe the clinical efficacy of abdominal acupuncture plus cervical curvature traction in treating cervical spondylosis of vertebral artery type. Method Eighty patients with cervical spondylosis of vertebral artery type were divided by the random number table into two groups, with 40 cases in each group. The treatment group was intervened by abdominal acupuncture plus cervical curvature traction, and the control group was given oral administration of flunarizine. The two groups were compared before and after treatment in terms of clinical symptom score, cervical vertigo symptom and function assessment scale, and hemodynamics, etc. Result Both groups attained significant efficacy, and the total effective rate was 92.5% in the treatment group, markedly higher than 77.5% in the control group (P<0.05); the score of cervical spondylosis symptom scale in the treatment group was higher than that in the control group after treatment, and the difference was statistically significant (P<0.05); after treatment, the evaluations of vertigo, neck-shoulder pain, headache, daily living and work, mental and social adaptation were significantly improved in both groups (P<0.01). Conclusion Abdominal acupuncture plus cervical curvature traction can obviously improve symptoms such as vertigo, neck-shoulder pain, headache, daily living and work, and mental and social adaptation, and boost the blood flow velocity of vertebrobasilar artery in patients with cervical spondylosis of vertebral artery type. The achievement of therapeutic efficacy is associated with the improvement of vertebrobasilar blood supply.

Objective To observe the surgical effect of nipple retractor to inverted nipples.Methods Between January 2001 and January 2014,this nipple retractor to inverted nipples was used to correct 604 inverted nipples of 322 patients.The follow-up period ranged from 6 to 24 months.The clinical results and complications were evaluated.Results Excellent nipple morphology and patient satisfaction were achieved in 211 cases (93.8 %),basically normal nipple morphology and patient satisfaction were achieved in 12 cases (5.3%),and recurrent nipple inversion occurred in three cases (1.3%).The latter three patients had experienced failed open correction for their inverted nipples,and some degree of nipple inversion occurred after removal of the frame.In this study,28 patients (12.4 %) experienced successful pregnancy and breast-feeding.No loss of nipple sensation occurred in 225 cases.Nipple erection was observed after stimulation in 215 cases (95.6%).Conclusions The described technique is minimally invasive,simple,safe,and reliable.Satisfying aesthetic results can be achieved without destroying breast feeding function and nipple sensation.
