RESUMO

This article aimed to discuss the protection of trans - nerolidol on vascular endothelial cells (ECs) injured by lipopolysac charides. ECs were divided into four groups: normal, model, low and high dose trans - nerolidol treatment groups. The cell survival rate and the contents of NO in the cell culture supernatant were determined. The protein expression and transcript level of pe roxisome proliferator - activated receptor - γ (PPARγ), endothelial nitric oxide synthase (eNOS), and inducible nitric oxide synthase (iNOS) were determined by western blotting and RT - PCR respectively. Compared with the normal group, cell livability, protein e xpression and mRNA transcript level of PPARγ and eNOS decreased, NO contents, protein expression and mRNA transcript tlevel of iNOS increased in model group significantly. Compared with model group, all the changes recovered in different degree in treatmen t groups. Hence, it was concluded that trans - nerolidol can alleviate the ECs injuryby the regulation of iNOS/eNOS through activating PPARγ in a dose - dependent manner


Este artículo tiene como objetivo discutir la protección del trans - nerolidol en las células endoteliales vasculares (CE) dañadas por lipopolisacáridos. Las CE se di vidieron en cuatro grupos: normal, modelo, grupos de tratamiento con trans - nerolidol de baja y alta dosis. Se determinó la tasa de supervivencia de las células y los contenidos de óxido nítrico (NO) en el sobrenadante del cultivo celular. La expresión de p roteínas y el nivel de transcripción del receptor activado por proliferadores de peroxisomas - γ (PPARγ), el óxido nítrico sint et asa endotelial (eNOS) y el óxido nítrico sint et asa inducible (iNOS) se determinaron mediante western blot y RT - PCR, respectivamen te. En comparación con el grupo normal, la viabilidad celular, la expresión de proteínas y el nivel de transcripción de PPARγ y eNOS disminuyeron, los contenidos de NO, la expresión de proteínas y el nivel de transcripción de iNOS aumentaron significativam ente en el grupo modelo. En comparación con el grupo modelo, todos los cambios se recuperaron en diferentes grados en los grupos de tratamiento. Por lo tanto, se concluyó que el trans - nerolidol puede aliviar el daño en las CE regulando iNOS/eNOS a través d e la activación de PPARγ de manera dependiente de la dosis.

Due to the successful application of all-trans retinoic acid (ATRA) and arsenic, the treatment of acute promyelocytic leukemia (APL) with PML::RARA fusion gene has achieved great success. However, some patients are presented with APL phenotype in cellular morphology, immunophenotype, and gene expression profile, while PML::RARA is negative, which is known as atypical APL (aAPL). In aAPL patients, more than 20 fusion genes related to retinoic acid receptors have been reported. It has been discovered that all evaluable patients with RARG fusion genes and approximately half of those with rare RARA fusion genes are resistant to ATRA, however, the molecular mechanisms of this resistance remain poorly studied. Combining with the reports in the 65th American Society of Hematology Annual Meeting, this paper reports great progresses of the key pathogenesis of aAPL and ATRA resistance mechanisms.

Objective:To establish an animal model of trans-sutural distraction osteogenesis in SD rats.Methods:A self-designed V-shaped distraction device(distractor)was fabricated with the traction force(N)of 0,1.3,2.2,3.0,4.3 and 5.0 corresponding to the distraction length(mm)of 5,4,3,2,1 and 0 respectively,meeting the trans-sutural distraction osteogenesis requirements in skull of 5-week-old SD rats.The distractor was plased into the sagittal suture of 12 SD rats.Continuous sampling was conducted 1,3,5 and 7 days respectively(n=3)after operation.The tissue changes in the trans-sutural distraction area were observed by HE and Masson's trichrome staining.Inflammation levels were determined using Arg-1 immunofluorescence staining.The early angiogenesis was clarified through co-staining with CD31 and EMCN.Results:A stable trans-sutural distraction osteogenesis model was estab-lished,5 mm distraction osteogenesis width was observed completely within 7 days of distraction.Significant new bone formation was observed at 7 days after operation.Arg-1 expression increased and was concentrated at the bone margins,overlapping with the areas of new bone formation.EMCN expression gradually decreased,and by day 7 CD31 was predominant,indicating the basic maturation of blood vessels.Conclusion:This study successfully constructed a stable and effective trans-sutural distraction osteogenesis animal model,and provides an experimental basis for the investigation of its early continuous histological changes.

Objective To investigate the safety and feasibility of modified gasless trans-subclavian approach endoscopic total thyroidectomy for papillary thyroid carcinoma(PTC).Methods We retrospectively analyzed the clinical data of consecutive 82 patients with PTC who underwent the modified gasless trans-subclavian approach endoscopic total thyroidectomy in the Department of Head and Neck Surgery,Sir Run Run Shaw Hospital,Affiliated with the Zhejiang University School of Medicine,from March 2022 to June 2023.Results All operations were successfully completed under endoscopy approach without transfer to open surgery.The maximum diameter of PTC was(6.6±5.4)mm.The operation duration was(156.5±32.7)min,and the number of lymph nodes in the central compartment was(12.3±6.4).The postoperative hospital stay was(3.3±1.2)d.Postoperative complications included two cases of transient hypoparathyroidism and six cases of transient recurrent laryngeal nerve injury.All the patients recovered in one month.One case of postoperative bleeding was observed.No permanent laryngeal nerve injury,permanent hypoparathyroidism or incision infection occurred.Conclusion The gasless trans-subclavian approach endoscopic total thyroidectomy is a feasible and safety approach in selected PTC patients.The lymph nodes in the central compartment are thoroughly dissected.This approach has obvious advantages in functional protection of the anterior cervical region,and has clinical application value.

Colorectal cancer is a major digestive disease in China, with mid-low rectal cancer as the predominant cause. Over the years, Chinese colorectal surgery has made considerable strides, attaining certain successes in clinical diagnosis and treatment, scientific research, and data platform construction. Nevertheless, there is still room for improvement in terms of the accuracy of diagnosis and standardization of treatment. Furthermore, immune checkpoint therapy represented by programmed death-1 have demonstrated initial efficacy in the treatment of mid-low rectal cancer. Further research is required to better understand the relationship and role between immune checkpoint inhibitors and traditional radiotherapy and chemotherapy, and to apply them to precise clinical practice. The authors elaborate on current status of diagnosis and treatment for mid-low rectal cancer in China, in order to provide references for its precission diagnosis and standardized treatment.

Objective To explore the effect of ropivacaine combined with dexmedetomidine in transversus abdominis plane block(TAPB)on postoperative stress hormones and cognitive function in patients undergoing laparoscopic radical gastrectomy.Methods A total of 80 patients undergoing laparoscopic radical gastrectomy at the First Affiliated Hospital of Xinxiang Medical University from April to October 2023 were selected as the research subjects.According to different anesthesia methods,the patients were divided into the observation group and the control group,with 40 patients in each group.Patients in the observation group were injected bilaterally with 2.5 g·L-1 ropivacaine and 0.5 μg·kg-1 dexmedetomidine for TAPB,with 20 mL injection on each side.Patients in the control group were injected bilaterally with 2.5 g·L-1 ropivacaine for TAPB,with 20 mL injection on each side.Mean arterial pressure(MAP)and heart rate(HR)were recorded at the time of admission to the operating room(T1),immediately after endotracheal intubation(T2),40 minutes after pneumoperitoneum(T3),and 15 minutes after extubation(T4).Radioimmunoprecipitation was used to detect serum cortisol(COR)level,and enzyme-linked immunosorbent assay was used to measure serum norepinephrine(NE)and epinephrine(E)levels at 1,6,12,and 24 hours after surgery.Visual analog scale(VAS)was used to assess pain at rest,and Ramsay sedation scale(RSS)was used to evaluate sedation depth.The doses of propofol and sufentanil were compared between the two groups.Serum β-amyloid(Aβ)and S100β protein levels at 1 day before surgery,1 and 3 days after surgery were detected by using the enzyme-linked immunosorbent assay,and cognitive function was assessed at the same time points by using the mini-mental state examination(MMSE).Results At T,and T2,there was no significant difference in MAP and HR between the control group and the observation group(P>0.05).At T3 and T4,MAP and HR in the observation group were significantly lower than those in the control group(P<0.05).At 1,6,and 12 hours postoperatively,VAS score in the observation group was significantly lower than that in the control group(P<0.05).At 24 hours postoperatively,there was no significant difference in VAS score between the control group and observation group(P>0.05).At 1 and 6 hours postoperatively,RSS score in the observation group was significantly higher than that in the control group(P<0.05).At 12 and 24 hours postoperatively,there was no significant difference in RSS score between the control group and observation group(P>0.05).At 1,6,and 12 hours postoperatively,COR,NE,and E levels in the observation group were significantly lower than those in the control group(P<0.05).At 24 hours postoperatively,there was no significant difference in COR,NE,and E levels between the control group and observation group(P>0.05).The doses of propofol and sufentanil in the observation group were significantly lower than those in the control group(P<0.05).One day before surgery,there was no significant difference in MMSE score between the control group and observation group(P>0.05).At 1 and 3 days postoperatively,MMSE score in the observation group was significantly higher than that in the control group(P<0.05).One day before surgery,there was no significant difference in serum Aβ and S100β protein levels between the control group and observation group(P>0.05).At 1 and 3 days postoperatively,serum Aβ and S100β protein levels in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Ropivacaine combined with dexmedetomidine in TAPB in radical gastrectomy can significantly reduce postoperative pain,increase sedative effect,prolong the duration of TAPB,and benefit patients'postoperative recovery with reduced cognitive impairment.

【Objective】 A new type of testicular hydrocele reversal resection is described under the concept of trans-interfascial plane surgery, in order to improves the understanding of the anatomical level of testicular hydrocele surgery and to optimize the surgical approach. 【Methods】 During Jan. and June 2021, 15 patients with primary testicular hydrocele were treated with hydrocelectomy and gubernaculum preservation.Demographic information, indications of treatment, success rate and complications were collected.The anatomical structures were analyzed using intraoperative findings and photographs. 【Results】 All operations of 15 patients were successful, with the surgery time of 25-48 minutes, with an average of (34.0±6.2) minutes.No patients experienced scrotal hematoma or incision infection.There were no relapses during the 3-month follow-up after surgery.The anatomical points observed during surgery were as follows: we further confirmed that the internal spermatic fascia completely surrounded the testis, epididymis, and the spermatic cord; this layer was an avascular plane, the cremaster muscle and fascial layer between the internal and external spermatic fasciae were absent; intraoperative preservation of the gubernaculum helped to fix the testicles in its natural position. 【Conclusion】 Our novel technique of hydrocelectomy is reliable, and the precise anatomical description of the concept of trans-interfascial plane surgery can help to improve the related surgical techniques.

Aim To investigate the role of metabolites of eicosapentaenoic acid (EPA) in promoting the transdifferentiation of pancreatic α cells to β cells. Methods Male C57BL/6J mice were injected intraperitoneally with 60 mg/kg streptozocin (STZ) for five consecutive days to establish a type 1 diabetes (T1DM) mouse model. After two weeks, they were randomly divided into model groups and 97% EPA diet intervention group, 75% fish oil (50% EPA +25% DHA) diet intervention group, and random blood glucose was detected every week; after the model expired, the regeneration of pancreatic β cells in mouse pancreas was observed by immunofluorescence staining. Human immunodeficiency virus type 1(HIV-1)is the causative reagent responsible for AIDS development. Even though the highly active anti-retroviral therapy(HAART,or the cocktail therapy)that has been widely applied could effectively suppress the infection and replication of HIV-1,the infected people suffer from other related diseases,such as the HIV-associated neurocognitive disorder(HAND). Objective To explore the role and potential mechanisms of all-trans retinoic acid (ATRA) on activation and oxidative stress of hepatic stellate cell (HSC). Methods Platelet-derived growth factor (PDGF-bb, 10 ng/ml) was applied to induce the activation of HSCs, which was then treated with ATRA at a dosage of 5 μmol/L for 48 h. The effects of ATRA on HSC activation were evaluated by detecting changes in cell growth viability and phenotypic marker expression. The effects of ATRA on HSC oxidative stress were evaluated by detecting changes in intracellular reactive oxygen species (ROS), reduced glutathione (GSH) and malondialdehyde (MDA), and the expression of antioxidant genes. The effects of ATRA on HSC autophagic activity were evaluated by detecting changes in autophagy marker expression and autophagic flow. Results Compared with the PDGF-bb group, the cell viability was significantly reduced in ATRA-treated HSCs (P<0.01), as well as the expression of α-SMA and Collagen I. The intracellular levels of ROS and MDA were significantly reduced in ATRA-treated HSCs (P<0.01), whereas the GSH level was significantly increased (P<0.01). The expression levels of antioxidant genes (NRF2, HO-1, and ATF4), were significantly higher in ATRA-treated HSCs than those in the normal ones under PDGF-bb condition (P<0.01). Meanwhile, the expression of autophagy markers Beclin 1 and LC3 Ⅱ/I, and signal of autophagy flow in ATRA-treated HSCs were found to be significantly reduced (P<0.01). Conclusion ATRA significantly inhibited PDGF-bb-induced HSC activation and reduced the level of oxidative stress and autophagic activity of HSCs, which had potential applications in the prevention and treatment of liver fibrosis.

Resumen La Sociedad Argentina de Osteoporosis convocó a especialistas reconocidos en la atención de personas transgénero para la elaboración del primer posiciona miento local sobre la evaluación de la salud ósea en esta población. La ley 26.743 de "Identidad de género" reco noce todas las identidades y garantiza su atención de manera gratuita en el sistema de salud. El impacto de los diferentes tratamientos de afirmación de género sobre la masa ósea ha sido tópico de debate internacional. Hasta la fecha la evidencia sigue siendo limitada y diferentes sociedades han emitido sugerencias y recomendaciones. Por tal motivo, creemos relevante mencionar nuestra experiencia plasmando mediante este documento una serie de sugerencias para ser utilizadas en la atención médica.


Abstract The Argentine Osteoporosis Society convened renowned specialists in the care of transgender people to prepare the first local position on the evaluation of bone health in this population. Law 26.743 on "Gender Identity" recognize all identities and guarantees free care throughout the health system. The impact of different gender affirmation treatments on bone mass has been topic of international debate. To date the evidence remains limited and different societies have issued suggestions and recommendations. For this reason, we believe it is relevant to mention our experience, capturing through this document a series of suggestions to be used in medical care.

Resumo As pessoas trans estão expostas a diferentes situações de vulnerabilidade que se entrecruzam, causando impactos negativos no processo saúde-doença-cuidado. Para melhor compreender essa questão, buscamos conhecer as experiências de sete estudantes trans de uma universidade pública de Minas Gerais, a partir de entrevistas norteadas por roteiro semiestruturado. As falas foram analisadas pela perspectiva da análise de conteúdo, e as categorias finais foram tensionadas com estudos das ciências sociais e da saúde coletiva. De forma geral, as narrativas se diferenciaram daquelas que compõem a maioria das pesquisas sobre o tema no Brasil, sobretudo em relação à manutenção de vínculos familiares e ao acesso à universidade pública. Nesse sentido, formulamos a hipótese de que a interação desses elementos com outros marcadores de diferenças pode contribuir positivamente para o estabelecimento de alianças e estratégias, a fim de lidar com o regime de inteligibilidade das identidades de gênero e com as dificuldades de acesso à educação, ao trabalho e à saúde. Compreender essa dinâmica e suas limitações, em uma perspectiva em que o individual se enlaça ao coletivo, possibilitou evidenciar o funcionamento da (hétero)normatividade nos processos de segregação de pessoas trans, assim como refletir sobre ações políticas que possam transformar essa realidade social efetivamente.


Abstract Transgender people are exposed to different situations of vulnerability that intersect, having negative impacts on the health-illness-care process. To better understand this issue, we sought to know the experiences of seven transgender undergraduates at a public university in Minas Gerais, from semi-structured interviews. The narratives were analyzed on the perspective of content analysis, and the final categories juxtaposed with studies of social sciences and public health. In general, the narratives are different from those which account for most of the research on the theme in Brazil, especially regarding maintaining family bonds and the access to public universities. Hence, we formulated the hypothesis that the interaction between these elements and other markers of differences can positively contribute to establishing alliances and strategies to deal with the regime of gender identity intelligibility and the constraints of access to education, work, and health. Understanding these dynamics and their limitations, under the perspective in which the individual and the collective are intertwined, enabled us to highlight the role of (hetero)normativity in the segregation processes of transgender people, as well as reflecting on the political actions that might effectively transform this social reality.

ABSTRACT Objective: To understand the narratives of transgender women and travestis (TGW) from four Brazilian cities regarding access to and use of health services. Methods: Qualitative study carried out within the scope of the TransOdara project, cross-sectional multicenter mixed methods research conducted between 2019-2021. Fifty-two in-depth interviews with TGW in Manaus, Campo Grande, Porto Alegre and São Paulo were analyzed. The analysis was guided by philosophical hermeneutics. Results: Reports of discrimination, stigmatization and pathologization reiterate the difficulties faced by TGW in seeking healthcare. The recurrence of disrespect for the social/corrected name reveals obstacles to the recognition of transgender identities and, in some cases, the intention of inhibiting transsexuality-travestilidade. Other difficulties arise from actions that disregard the health specificities of TGW or the precarious social conditions that affect some of them. On the other hand, based on experiences of respect and adequate care, participants identify an ongoing change, which is expressed in greater availability of services and improved assistance. There is an expectation of continued expansion of services, technologies and training of health professionals. Conclusions: The identified change has been undertaken at the interface of public health policies with LGBT+ activism and the production of knowledge about TGW health needs. Although the identified advances are insufficient to change the scenario of the historical exclusion experienced by TGW in health services, they point to promising ways to improve their health conditions.


RESUMO Objetivo: Compreender as narrativas de Mulheres Trans e Travestis (MTT) de quatro cidades brasileiras acerca do acesso e do uso de serviços de saúde. Métodos: Estudo qualitativo realizado no âmbito do projeto TransOdara, pesquisa transversal multicêntrica de métodos mistos, conduzida entre 2019 e 2021. São analisadas 52 entrevistas em profundidade com MTT em Manaus, Campo Grande, Porto Alegre e São Paulo. O tratamento analítico foi orientado pela hermenêutica filosófica. Resultados: Relatos de discriminação, estigmatização e patologização reafirmam as dificuldades enfrentadas por MTT na busca por cuidado com a saúde. A recorrência do desrespeito ao nome social/retificado revela obstáculos ao reconhecimento das identidades trans e, em alguns casos, a intenção de inibir a transexualidade-travestilidade. Outras dificuldades decorrem de ações que desconsideram as especificidades de saúde das MTT ou as condições sociais precárias que afetam algumas delas. No entanto, a partir de experiências de respeito e atendimento adequado, as participantes identificam uma mudança em curso, que se expressa em uma maior disponibilidade de serviços e na melhoria da assistência. Há uma expectativa de continuidade da ampliação de serviços, tecnologias e capacitação dos/as profissionais de saúde. Conclusão: A mudança identificada tem sido empreendida na interface das políticas públicas de saúde com o ativismo LGBT+ e a produção de conhecimento acerca das questões de saúde das MTT. Os avanços empreendidos, ainda que insuficientes para mudar o cenário da histórica exclusão vivida por elas nos serviços de saúde, apontam caminhos promissores na melhoria de suas condições de saúde.

ABSTRACT Objective: The study aimed to estimate the prevalence of acquired syphilis and associated factors in a national survey. Methods: TransOdara was a cross-sectional study comprising transgender women and travestis (TGW) in five major cities in Brazil during December of 2019 and July of 2021. The sample was recruited using the respondent-driven sampling (RDS) method. The outcome "active syphilis" was defined as a positive treponemal test and Venereal-Disease-Research-Laboratory (VDRL) title greater than∕ equal to ⅛. Sociodemographic variables were described. Bivariate and multiple logistic regression were performed, and odds ratios (OR) and 95% confidence intervals (95%CI) were estimated. All analyses were performed in R, 4.3.1. Results: A total of 1,317 TGW were recruited, with 1,291 being tested for syphilis, and 294 (22.8%) meeting the criteria for active syphilis. In bivariate analysis, black/mixed race (OR=1.41, 95%CI 1.01-1.97), basic level of education (OR=2.44, 95%CI 1.17-5.06), no name change in documents (OR=1.39, 95%CI 1.00-1.91) and sex work (past only OR= 2.22, 95%CI 1.47-3.32; partial OR=2.75, 95%CI 1.78-4.25; full time OR=3.62, 95%CI 2.36-5.53) were associated with active syphilis. In the multivariate analysis, sex work was the only associated factor, 2.07 (95%CI 1.37-3.13) past sex work, 2.59 (95%CI 1.66-4.05) part-time sex work and 3.16 (95%CI 2.04-4.92) sex work as the main source of income. Conclusion: The prevalence of active syphilis in this study was elevated compared with other countries in Latin America. Sex work was an important associated factor with active syphilis, highlighting the impact that this condition of vulnerability may have in the health of TGW, as members of a key, marginalized population.


RESUMO Objetivo: O estudo teve como objetivo estimar a prevalência de sífilis adquirida e fatores associados em uma pesquisa nacional. Métodos: "TransOdara" foi um estudo transversal compreendendo mulheres trans e travestis (MTT) em cinco grandes cidades do Brasil durante dezembro-2019 e julho-2021. A amostra foi recrutada usando o método respondente-driven sampling (RDS). O desfecho "sífilis ativa" foi definido como um teste treponêmico positivo e título do Venereal-Disease-Research-Laboratory (VDRL) maior ou igual a ⅛. Variáveis sociodemográficas foram descritas. Análises bi- e multivariadas foram realizadas, e odds ratio (OR) e IC95% foram estimados. Todas as análises foram realizadas no R,4.3.1. Resultados: Um total de 1.317 MTT foram recrutadas, com 1.291 sendo testadas para sífilis, das quais 294 (22,8%) preencheram os critérios para sífilis ativa. Na análise bivariada, raça negra/parda (OR=1,41; IC95% 1,01-1,97), nível básico de educação (OR=2,44; IC95% 1,17-5,06), não alteração do nome nos documentos (OR=1,39; IC95% 1,00-1,91) e trabalho sexual (pregresso OR=2,22; IC95% 1,47-3,32; parcial OR=2,75; IC95% 1,78-4,25; período integral OR=3,62; IC95%: 2,36-5,53) foram associados à sífilis ativa. Na análise multivariada, o trabalho sexual foi o único fator associado, 2,07 (IC95%: 1,37-3,13) trabalho sexual passado, 2,59 (IC95% 1,66-4,05) trabalho sexual em tempo parcial e 3,16 (IC95% 2,04-4,92) trabalho sexual como principal fonte de renda. Conclusão: A prevalência de sífilis ativa neste estudo foi elevada em comparação com outros países da América Latina. O trabalho sexual foi um fator associado importante com sífilis ativa, destacando o impacto que essa condição de vulnerabilidade pode ter na saúde das MTT, como membros de uma população-chave marginalizada.

ABSTRACT Objective: To estimate the prevalence and factors associated with hepatitis A, B, and C in transgender women and travestis's networks, in 5 regions of Brazil. Methods: This cross-sectional study includedtransgender women and travestis in five Brazilian capitals (Campo Grande, Manaus, Porto Alegre, Salvador, and São Paulo), between December/2019 and July/2021. All samples were subjected to detection of serological markers of hepatitis virus A (HAV), B (HBV), and C (HCV) infections through rapid tests and chemiluminescent microparticle immunoassays. Positive samples in the screening tests were submitted to detect HBV DNA and HCV-RNA by real-time PCR and genotyped by Sanger sequencing. Results: Analysis of 1,317 samples showed network prevalence rates of 69.1%, 25.1%, and 1.5% for HAV, HBV, and HCV exposure, respectively. A high susceptibility rate to HBV infection (35.7%) and low prevalence of vaccine response markers (40%) were also observed. Age greater than 26 years, self-declared black/brown skin color, having only primary education, history of incarceration, and use of a condom in the last sexual intercourse with a casual partner were associated with total anti-HAV. Exposure to HBV was associated with age greater than 26 years, self-declared black/brown, history of being a sex worker, and incarceration. Age > 37 years, history of sexual abuse, and frequent alcohol consumption were associated with hepatitis C infection. Conclusion: The highest prevalence of HAV in this population was found in the North and Northeast regions, and the prevalence found was higher than that in the general population, suggesting greater vulnerability. The prevalence of HCV infection in our study was similar to that observed in the general population.


RESUMO Objetivo: Estimar as prevalências e fatores associados com as hepatites A, B e C em mulheres trans e travestis em cinco regiões do Brasil. Métodos: Estudo transversal com mulheres trans e travestis em cinco capitais brasileiras (Campo Grande, Manaus, Porto Alegre, Salvador e São Paulo), entre dezembro/2019 e julho/2021. As amostras foram submetidas à detecção de marcadores das infecções pelos vírus das hepatites A (HAV), B (HBV) e C (HCV), utilizando-se testes rápidos e quimioluminescência. Amostras positivas foram submetidas à detecção de HBV-DNA e HCV-RNA por PCR em tempo real e genotipadas por sequenciamento de Sanger. Resultados: As análises de 1.317 amostras indicaram taxas de prevalências nas mulheres trans e travestis recrutadas de 69,1%, 24,4% e 1,5% para exposição ao HAV, HBV e HCV, respectivamente. Elevada taxa de suscetibilidade ao HBV (35,7%) e baixa prevalência do marcador vacinal (40,0%) foram observadas. Mostraram-se associadas à presença de anti-HAV: idade maior que 26 anos, autodeclarar-se preta-parda, ter apenas educação básica, história de encarceramento e uso de preservativo na última relação sexual com parceiro casual. Quanto à exposição ao HBV, foi associada a idade maior que 26 anos, cor da pele preto-parda, ter sido profissional do sexo e história de encarceramento. Idade maior de 37 anos, história de abuso sexual e consumo frequente de álcool foram associadas ao HCV. Conclusão: As maiores prevalências de HAV nessa população encontram-se nas regiões Norte e Nordeste. Com relação ao HBV, a prevalência encontrada foi superior à encontrada na população geral, sugerindo maior vulnerabilidade. A prevalência do HCV foi semelhante à encontrada na população geral.

ABSTRACT Objective To identify groups of transgender women and travestis (TGW) with specific patterns of gender-based discrimination (GBD) and analyze the factors associated with GBD. Methods A cross-sectional study was conducted with TGW recruited through respondent-driven sampling in five Brazilian cities (2019-2021). Latent class analysis was used to characterize GBD (low, medium, and high) using 14 observable variables. Descriptive analysis was performed, and associations between predictor variables and GBD were estimated by adjusted odds ratios (aOR) using ordinal logistic regression. Results Out of a total of 1,317 TGW, 906 (68.8%) answered questions about GBD. Most were under 34 years old, single, and had a Brown race/skin color. GBD was classified as "low," "medium," and "high," with estimates of 41.7, 44.5, and 13.8%, respectively. Variables positively associated with higher intensity of GBD included living in Manaus compared to São Paulo, being ≤34 years old compared to >34, being homeless compared to living in one's own house or rented apartment, not having legally changed one's name compared to those who had, and reporting physical or sexual violence compared to those who did not report. Variables negatively associated with higher intensity of GBD included having a Brown or Asian race/skin color compared to White and a monthly income ≥1 minimum wage compared to ³1. Conclusion A high proportion of GBD was observed in Brazilian TGW, with this outcome associated with more vulnerable sociodemographic characteristics and a history of violence.


RESUMO Objetivo Identificar grupos de mulheres trans e travestis (MTT) com padrões específicos de discriminação por identidade de gênero (DIG) e analisar os fatores associados à DIG. Métodos Estudo transversal com MTT recrutadas por respondent-driven sampling em cinco capitais brasileiras (2019-2021). Análise de classes latentes foi usada para caracterizar a DIG (em baixa, média e alta) usando 14 variáveis observáveis. Análise descritiva foi realizada e as associações entre variáveis preditoras e DIG foram estimadas por odds ratio ajustados (ORaj), usando regressão logística ordinal. Resultados Do total de 1.317 MTT, 906 (68,8%) responderam perguntas sobre DIG. A maioria apresentava idade ≤34 anos, solteiras e com raça/cor de pele parda. DIG foi classificada em "baixa", "média" e "alta", com estimativas de 41,7, 44,5, 13,8%, respectivamente. As variáveis positivamente associadas à maior intensidade de DIG foram: morar em Manaus em comparação com São Paulo; possuir idade £34 anos em comparação com idade >34; estar em situação de rua em comparação com quem mora em casa ou apartamento próprio ou alugado; não ter retificado o nome em documento em comparação com quem retificou; e relato de violência física ou sexual em comparação com quem não relatou. As variáveis negativamente associadas à maior intensidade de DIG foram: raça/cor de pele parda ou amarela em comparação com branca; e renda média mensal <1 salário mínimo em comparação com ≥1 salários. Conclusão Observaram-se alta proporção de DIG em MTT brasileiras e associação desse desfecho com características sociodemográficas mais vulneráveis e histórico de violência.

Resumo Este artigo de opinião consiste em uma entrevista realizada com Leonardo Peçanha, um homem negro trans, ativista no campo dos direitos humanos e dos direitos das pessoas trans e pesquisador do campo da Saúde Coletiva. Na conversa que se segue Leonardo Peçanha, de forma interseccional, reflete sobre o lugar da beleza, modificações corporais e atividade física no processo de transição de gênero de sujeitos homens trans e transmasculinos, como também toca no tema das (im)possibilidades das pessoas trans no campo dos esportes de alto rendimento e nos conta sobre o concurso de beleza Mister Trans Brasil.


Abstract This opinion article consists of an interview with Leonardo Peçanha, a Black trans man, a human rights and trans people rights activist and a researcher in Collective Health. In this interview, he reflects intersectionally on the place of beauty, body changes, and physical activity in the gender transition process of trans and transmasculine men. Este estudo objetivou identificar, analisar e integrar resultados de estudos que examinaram o impacto da pandemia de COVID-19 na saúde mental de pessoas transexuais e travestis. Foi realizada uma revisão integrativa da literatura, em conformidade com as diretrizes PRISMA. Conduziu-se uma busca sistemática dos estudos primários publicados em três bases de dados: PubMed/MEDLINE, LILACS e PsycINFO. Foram incluídos oito estudos que atenderam aos critérios de inclusão. A análise temática do material gerou seis categorias temáticas. Os estudos analisados apontaram que a situação de vulnerabilidade social ao qual as pessoas trans encontram-se submetidas no cotidiano foi ampliada durante a pandemia. A necessidade do distanciamento físico resultou no aumento de sintomas de ansiedade e depressão, agravamento das condições econômicas, dificuldades para acessar procedimentos e medicamentos que compõem o processo de afirmação de gênero, além de exacerbar conflitos familiares e manifestações de transfobia. Há necessidade de implementar políticas públicas e uma rede de proteção social para diminuir a vulnerabilidade e sofrimento psicossocial de pessoas trans. (AU)


This study aimed to identify, analyze, and integrate findings from studies that examined the impact of the COVID-19 pandemic on the mental health of transgender and transvestite individuals. An integrative literature review was conducted according to PRISMA guidelines. A systematic search of primary studies was conducted across three databases: PubMed/MEDLINE, LILACS, and PsycINFO. Eight studies that met the inclusion criteria were included. Thematic analysis of the material generated six thematic categories. The studies analyzed pointed out that the situation of social vulnerability faced by trans people in their daily lives intensified during the pandemic. The need for physical distancing led to increased symptoms of anxiety and depression, worsening economic conditions, and difficulties in accessing procedures and medications that make up the gender affirmation process, in addition to exacerbating family conflicts and manifestations of transphobia. Therefore, there is a need to implement public policies and a social protection network to reduce the vulnerability and psychosocial suffering of the transgender and transvestite community. (AU)


El objetivo de este estudio es identificar, analizar e integrar los resultados de los estudios que examinaban el impacto de la pandemia de COVID-19 en la salud mental de las personas transexuales y travestis. Abstract Existing meta-analyses on the effect of online psychological interventions (OPIs) have found small to medium effect sizes for the treatment of anxiety and depression. On the other hand, third-generation trans-diagnostic OPIs are very rare and, due to the large variability among disorders, symptoms or target populations, it is difficult to assess their overall effect. Other systematic reviews and meta-analyses have overly broad inclusion criteria that make the understanding of the findings more difficult. The current study aims to analyze the empirical evidence for third-wave trans-diagnostic OPIs designed to decrease symptoms and promote psychological flexibility, including studies that compare a OPI to some control condition (e. g., waiting list, treatment as usual or other that should not have any effect) and include a general symptomatology scale as dependent variable. A search without filters or timeframe was performed on Scopus and 1 408 articles were found, among which 21 were reviewed in depth and 6 were included for meta-analysis. Risk of bias was assessed by a quality and heterogeneity assessment. Separate meta-analyses were performed for general distress and psychological flexibility at post-treatment and last follow-up. Risk of bias analysis suggest low risk of threats to validity and attribute heterogeneity to between-study attrition rates. Additionally, meta-regression models for duration, attrition rate, and mean age are proposed for each time point. The results show significantly large effect sizes for both variables at both time points. According to the meta-regression models attrition rates are a mediating variable for the effect on general distress both at completion and at the last follow-up. On the other hand, duration, age and attrition rate are all mediating variables for the effect on psychological flexibility at the end of treatment. The findings suggest that the high attrition rates observed on tele-psychology need to be mitigated; if this is not possible, intention-to-treat approaches should be adopted for data analysis.


Resumen Los metaanálisis existentes sobre el efecto de las intervenciones psicológicas en línea (OPIs, por sus siglas en inglés) han encontrado tamaños de efecto pequeños o medianos para el tratamiento de la ansiedad y la depresión. Por otra parte, las OPIs transdiagnósticas de tercera generación son muy escasas y, debido a la gran variabilidad entre los trastornos, los síntomas o las poblaciones objetivo, es difícil evaluar su efecto global. Otras revisiones sistemáticas y metaanálisis tienen criterios de inclusión demasiado amplios que dificultan la comprensión de los hallazgos. El presente estudio tiene como objetivo analizar la evidencia empírica actual para las OPIs transdiagnósticas de tercera generación diseñadas para disminuir los síntomas y promover la flexibilidad psicológica, incluyendo estudios que comparen una OPI con alguna condición de control (por ejemplo lista de espera, tratamiento habitual u otro que no debería tener efecto) y que incluyan una escala de sintomatología general como variable dependiente. Se excluyeron estudios basados en otras terapias y/o diseñados para prevenir o tratar una población, un trastorno o un conjunto de síntomas específicos. También se excluyeron los protocolos de estudio, los diseños pretest-postest y otros en los que era imposible calcular el tamaño del efecto. Se realizó una búsqueda sin filtros ni marco temporal en Scopus y se encontraron 1 408 artículos entre los cuales 21 fueron revisados en profundidad y 6 fueron incluidos en el presente estudio. El riesgo de sesgo se evaluó mediante una evaluación de calidad y heterogeneidad; no fue posible realizar análisis de sesgo de publicación. Se realizaron metaanálisis separados para el malestar general y la flexibilidad psicológica en postratamiento y último seguimiento. El análisis del riesgo de sesgo sugiere un bajo riesgo de amenazas a la validez y atribuye la heterogeneidad principalmente a las tasas de deserción entre los estudios. Además, se proponen modelos de metarregresión para la duración, la tasa de deserción y la edad promedio en cada punto temporal. Los resultados muestran tamaños de efecto significativamente grandes para ambas variables en ambos puntos temporales y se evalúa su heterogeneidad, que se atribuye en gran medida a las tasas de deserción de los estudios incluidos. Según los modelos de metarregresión, las tasas de deserción son una variable mediadora del efecto sobre el malestar general tanto en el momento de la finalización como en el último seguimiento. Por otra parte, la duración, la edad y la tasa de abandono son variables mediadoras del efecto sobre la flexibilidad psicológica al final del tratamiento. Los resultados sugieren que es necesario mitigar las altas tasas de deserción observadas en la telepsicología y, cuando no sea posible, adoptar enfoques de intención de tratar para el análisis de los datos.

ABSTRACT Introduction: Acute promyelocytic leukemia currently presents an excellent chance of cure with protocols based on all-trans-retinoic acid (ATRA) and anthracycline or only differentiation agents. However, high early mortality rates continue to be reported Methods: Between 2000 and 2018, patients were enrolled and retrospectively analyzed by medical records. A modified AIDA protocol, with a 1-year shortening of the treatment duration, reduction in the number of drugs and a strategy to reduce early mortality by the postponement of the initiation of anthracyclines were employed. Overall and event-free survival rates and toxicity were analyzed Results: Thirty-two patients were enrolled, of whom 56% were female, with a median age of 12 years and 34% belonged to the high-risk group. Two patients had the hypogranular variant and three had another cytogenetic alteration, in addition to the t(15;17). The median start of the first anthracycline dose was 7 days. There were two early deaths (6%) due to central nervous system (CNS) bleeding. All patients achieved molecular remission after the consolidation phase. Two children relapsed and were rescued by arsenic trioxide and hematopoietic stem cell transplantation. The presence of disseminated intravascular coagulation (DIC) at diagnosis (p = 0.03) was the only factor with survival impact. The five-year event-free survival (EFS) was 84% and 5-year overall survival (OS) was 90% Conclusion: The survival results were comparable to those found in the AIDA protocol, with a low rate of early mortality in relation to the Brazilian reality.
