RESUMO

Objective: To build the rabbit model of VX2 hepatic carcinoma, examine the tumor body using the ultrasonic contrast and study the correlation between the blood circulation grading and angiogenesis. Methods: The VX2 tumor strain was prepared in the lateral muscle of the hind legs of 40 male New Zealand rabbits (which were purchased from Nanjing Senbao Biotech Co., Ltd.). The tumor block was embedded in the center of left liver lobe directly to build the rabbit model of VX2 hepatic carcinoma. The ultrasonic contrast was performed 14 d after implanting the tumor body. The semi-quantitative classification (0-IV level) was taken according to the blood flow of tumor vessel. Animals were executed using the air embolism method. The liver was separated to extract RNA and total protein respectively. The real-time PCR and western blotting method were employed to detect the expression of angiogenesis-related factors of VEGF, bFGF and TNF-α, while the ultrasonic contrast to detect the correlation with blood circulation grading. The Pearson product moment correlation coefficient was used to measure the linear relationship between these two variables and analyze the correlation between the blood circulation grading and angiogenesis using the ultrasonic contrast. Results: Thirty-three rabbits had the successful model of VX2 hepatic carcinoma. The blood circulation grading by ultrasonic contrast was: 2 cases at level 0 (6.60%), 5 cases at level I (16.7%), 12 cases at level II (40.0%), 6 cases at level III (20.0%) (local dense or clustered blood flow) and 5 cases at level IV (16.7%). The results showed that there was positive correlation between three angiogenesis-related factors and the blood circulation grading. The correlation coefficient between three angiogenesis-related factors and the blood circulation grading was over 0.9, which indicated the relatively high correlation. Conclusions: The ultrasound blood circulation grading for the hepatic carcinoma can clearly reflect the changes of blood vessel, which will be of critical significance for the early diagnosis of hepatic carcinoma and clinical evaluation of angiogenesis indicators.

Objective To investigate the imaging features and preoperative staging value of ultrasonic endoscopy and gastrointestinal ultrasonic contrast in the gastric carcinoma examination.Methods One hundred and sixty eight patients with gastric carcinoma verified by operation and pathology in our hospital from October 2010 to October 2015 werecollected.Then their imageological data including ultrasonic endoscopy and gastrointestinal ultrasonic contrast examinations were retrospectively analyzed and compared with the postoperative pathological results.Results The accuracy rate of preoperative T staging in ultrasonic endoscopy was significantly higher than that of gastrointestinal ultrasonic contrast,the difference had statistical significance(P<0.05);the accuracy rate of preoperative N/M staging in gastrointestinal ultrasonic contrast was s1gnificantly higher than that in ultrasonic endoscopy,the difference had statistical significance(P<0.05).Conclusion Ultrsonic endoscopy and gastrointestinal ultrasonic contrast have different advantages and values in preoperative staging:ultrasonic endoscopy has higher accuracy rate in the diagnosis of primary gastric carcinoma;gastrointestinal ultrasonic contrast is better in diagnosing metastasis of gastric carcinoma,moreover which shows the consistency with postoperative pathological staging.

Objective: To evaluate the renal ischemia reperfusion injury (IRI) in rabbits using the ultrasonic contrast technique and discuss the clinical value of ultrasonic contrast technique in the diagnosis of renal IRI by comparing the time-intensity curve of renal cortex and the expression of vascular endothelial growth factor (VEGF) of renal tissue. Methods: Twenty 3-month-old New Zealand rabbits were randomly divided into 4 groups, namely Ctrl group, IRI-12 h, IRI-24 h and IRI-48 h groups. The two dimensional gray-scale ultrasonography was employed to determine and mark the position of rabbit kidney. Rabbits were given the intraperitoneal anesthesia with 20% urethane with the dosage of 5 mL/kg. The aseptic operation was performed after the local skin disinfection in the area of both kidneys. The right kidney of animals in the control group was excised without any treatment for the left kidney. After excising the right kidney of animals in groups of IRI-12 h, IRI-24 h and IRI-48 h, the aneurysm clip was used to clip the renal pedicle vessel of left kidney, in order to simulate the ischemia. Because of the tissue ischemia, it could be seen that the color of kidney was changed from bright red to dark red, which indicated the successful modeling of ischemia. The aneurysm clip was released after one hour of maintaining the ischemia. Then the kidney turned out to be bright red from dark red, which indicated that the reperfusion was completed. Taking this moment as the time of ischemia reperfusion, the wound was stitched up. A total of 12, 24 and 36 h after the operation, the two-dimensional and color Doppler flow imaging and ultrasonic contrast were employed for the examination. The dynamic changes of ultrasonic contrast were recorded. The quantitative analysis software (QontraXt) was adopted to analyze the time-intensity curve of echo at different positions of renal cortex. After the ultrasonic contrast testing, rabbits were put to death. The renal cortex tissue was isolated and the tissue RNA and total protein were extracted respectively. Real-time PCR and western blotting were used to detect the VEGF and the Pearson product moment correlation coefficient was used to measure the linear relationship between these two variables. Results: The ultrasonic contrast could clearly reflect the process of IRI. The results of testing at mRNA and protein level indicated that the expression of VEGF in IRI groups was significantly increased (P < 0.05) and the expression of VEGF was also increased by the time of reperfusion. Conclusions: There is the certain correlation between the expression of VEGF and process of IRI. The correlation coefficient between the ultrasonic contrast parameters of AT and TTP and the relative expression of VEGF is over 0.9, which indicates the relatively high correlation. But there is no significant difference in the change of perfusion peak intensity between groups, which has no correlation with the expression of VEGF.

Objective To investigate the clinical value of clinical application in diagnosis of prostate cancer (PCa) by prostate biopsy guided by rectal ultrasound contrast guided biopsy.Methods One hundred and ninety-eight patients with prostate in Punan Hospital of Pudong New District of Shanghai were investigated.According to different detection methods, the research objects were divided into two groups, the patients were performed with the ultrasound contrast guided biopsy as the imaging group (n =96), the patients with Doppler ultrasound guided biopsy as the ultrasonic group(n =102).The puncture Results were compared with pathological diagnosis.The positive rate of PCa and number of puncture needle were compared with the two puncture methods.The value of the application of the prostate biopsy guided by rectal ultrasound in diagnosis of prostate cancer was evaluated.Result One hundred and ninety-eight patients were all received pathological diagnosis,78 cases benign lesions, 120 cases were diagnosed as PCa.Thirty-six cases of benign lesions were confirmed by pathological biopsy, 60 case PCa.There were 42 cases of benign lesions in ultrasonic group, 60 case PCa.The positive rate of PCa in the imaging groupwas 62.5% (60/96), the ultrasonic group was 58.82% (60/102).There was no difference in Pca positive rate between the ultrasound group and the contrast group(x2 =0.104, P=0.747).The positive number of Pca in the imaging group was 28.50% (17/60), the difference was statistically significant higher than that of the common group(18.80% (11/60), P =0.001).The average of the patients in the imaging group was 8.19 needle,less than the ultrasonic group per capita 11.31 needle less.When f/tPSA less than or equal to 0.15, Pca positive rate of the contrast group was 46.55% (27/55), higher than the ultrasonic group(9.30% (4/43)), the difference was statistically significant (P =0.001);when f/tPSA more than 0.15, the positive rate of Pca in the contrast group was 92.11% (35/38), less than the ultrasound group (94.92 (56/59)), the difference was not statistically significant (P =0.89).Anal pain, hematuria and hematochezia in contrast group (7.29% (7/96), 2.08% (2/96), 10.42% (10/96)) were significantly less than the ultrasound group(22.55% (23/102), 8.82% (9/102), 23.53% (24/102)), the difference was statistically significant (P =0.003,0.039,0.014).Conclusion Under the guidance of ultrasound contrast, rectal biopsy has important diagnostic value for prostate cancer.Under the premise of reducing the number of puncture needle,can improvethe positive rate of Pca, reduce the pain of patients and the occurrence of complications after puncture.When f/tPSAless than or equal to 0.15,puncture positive rate in contrast group is higher than the ultrasonic group, puncture effect is better.

Objective To explore the value of oral ultrasonic contrast agent on preoperative T staging of gastric cancer.Methods 62 patients diagnosed with gastric neoplasms by operation and pathology were analyzed retrospectively.Patients were treated by oral ultrasonic contrast agent and conventional ultrasonography before surgery,two methods were compared with postoperative pathology.Results Oral ultrasonic contrast agent and conventional ultrasound detection rates of gastric neoplasms were 100.0 %(62/62)and 64.5 % (40/62),The difference was significant between the two examination methods(x2 =24.369,P < 0.05).The oral ultrasonic contrast agent and conventional ultrasound with T1,T4 staging accuracy rates were 85.7 %(6/7),0,92.3 %(36/39),59.0 %(23/39),there were significant differences(both P < 0.05).T2,T3 staging accuracy rate were 75.0 %(3/4),0,83.3 %(5/6),33.3 %(2/6),the differences were not significant(P =0.143,P =0.242).Conclusion Oral ultrasonic contrast agent in preoperative T staging diagnosis has higher accuracy rate in gastric neoplasms,it could guide clinical rational therapy.It is worth promoting non-invasive,convenient means of stomach diseases census.

RESUMO

Objective To study the evaluation of ultrasonic gastrography examination(UGE) in general survy.Methods Collected and analyzed the cases of physical examination in outpatient service ,from Jan,2007 to Mar,2007,in which all the patients who stated having upper bellyache underwent UGE. Compared the results of UGE with results of gastroscope examination to find out the accuracy of UGE.Results The image manifestations have a certain degree regularities .The percentage of accurate ultrasonic diagnoses is 89%.Conclusions UGE is an effective method to diagnose gastric diseases.

RESUMO

The focal hepatic lesion caused by local injection of absolute alcohol in rats was evaluated with ultrasonic contrast agent and pathologic examination. Twenty adult Wistar rats weighing about 200 g were injected with absolute alcohol (0.05-0.1 mL each one) on the exterior left lobe of the liver under the monitoring of ultrasound. Pulse inversion harmonic imaging was used to evaluate the focal lesion after bolus injection of ultrasonic contrast agent (0.05 mL/200 g) through caudal vein.Seven days later, the focal lesion was studied again as before. The exterior left lobe of liver with focal lesion was incised and underwent pathologic examination. The results showed that all of the focal lesions could be defined clearly after bolus injection of the ultrasonic contrast agent under the mode of pulse inversion harmonic imaging. There was good correlation between the size of the focal lesion measured by ultrasound on the 7th day after the "ablation" under the mode of pulse inversion harmonic imaging and that gotten by pathologic examination (P=0.39). The focus size measured by ultrasound right after the ablation was larger than that gotten by pathologic examination (P= 0.002). It was concluded that ultrasonic contrast agent plus pulse inversion harmonic imaging could be used to assess the size of the focal hepatic lesion caused by local injection of absolute alcohol in rats.
