RESUMO

Objective:To investigate the effect of ultrasonic debridement mediated by 0.9% sodium chloride solution and 0.5% iodophor volt combined with eddy current washing and high pressure pulse washing on the removal of colonized bacteria on the wound surface of diabetic foot and wound healing.Methods:From March to November 2020, a total of 60 patients using ultrasonic therapy for debridement were divided into control group, experimental group 1, experimental group 2 and experimental group 3 by random digit table in the fourth People′s Hospital of Dalian. The final effective data collected was 15 cases in each group. The control group was given ultrasonic debridement mediated by 0.9% sodium chloride solution and eddy current washing.Experimental group 1 was given ultrasonic debridement mediated by 0.9% sodium chloride solution and high pressure pulse washing. Experimental group 2 received 0.5% iodophor mediated ultrasonic debridement and eddy current washing. Experimental group 3 0.5% iodophor mediated ultrasonic debridement and high pressure pulse washing. The size of the wound was measured, sampled and bacterial cultured before and after the first, fifth and 10th intervention. The wound bacterial clearance rate and wound area reduction rate were calculated and compared.Results:Before and after 3 interventions, the bacterial clearance rate and the total reduction of wound surface in 4 groups were increased ( P<0.01), the total bacterial clearance rate of experimental group 3 was the highest, which was (93.85 ± 9.87)%.The total reduction rate of wound in experimental group 2 was the highest, which was (20.831 4 ± 9.379 8)%. Conclusions:0.5% iodophor mediated ultrasonic debridement combined with high pressure pulse washing is the most effective way in the removal of diabetic foot wounds, and 0.5% iodophor solution mediated ultrasonic debridement combined with eddy current washing is the most effective in reducing diabetic foot wounds.

RESUMO

Objective:To investigate the effect of ultrasonic debridement machine for postoperative cleaning of perianal abscess.Methods:77 patients with perianal abscess were enrolled in this study.They were divided into control group (38 cases) and observation group (39 cases) by random sampling.The control group was implemented routine treatment post operation and the observation group was implemented routine dressing combined with ultrasonic cleaning machine to clean the crissum.And the visual analogue scale (VAS) was adopted to analyze the pain level of postoperative wound.And the postoperative clearance rate of bacteria, wound healing time, controlled time of infection and cleaning effect of wound between two groups were compared.Results:The VAS score of the observation group on the 1st, 3rd, 5th and 9th day after operation were significantly lower than those of control group (t=8.031, ■on wound of observation group was significantly higher than that of control group while the length stay in hospital of■healing time and controlled time of infection of observation group were significantly lower than those of control group■machine to clean wounds after perianal abscess surgery can effectively remove bacteria of wound, and improve cleaning effect, alleviate pain level of wound, and promote the rapid repair of wounds.

Objective To examine the effect of ultrasonic debridement combined with continuous skin stretching to repair refractory infective wound bed.Methods From January 2016 to July 2016,we treated 15 cases of chronic,refractory infective wound beds using ultrasonic debridement combined with skin stretching technique.Results All the 15 (100%) cases were cured without necrosis of the stretched skin edges.Conclusions Ultrasonic debridement combined with continuous skin stretching is an appropriate technique for repairing the refractory wound bed in the patients who are older,in poor general condition,or poor condition of local skin and soft tissue,or have poor postoperative effect,or surgical contraindication or at higher risk of surgery.

RESUMO

OBJECTIVE:To observe the clinical efficacy and ADR of ultrasound debridement combined with Huangma tinc-ture in the treatment of diabetic foot ulcer. METHODS:90 diabetic foot ulcer patients were randomly divided into combination treatment group(ultrasound debridement combined with Huangma tincture wet compress),Huangma tincture group(Huangma tinc-ture wet compress) and control group (vaseline) with 30 cases in each group. 3 months after treatment,Ulcer healing rate,cure time,effective time,recovery time,cure rate and ADR were observed in each group. RESULTS:The ulcer healing rate of combi-nation treatment group was significantly higher than that of Huangma tincture group and control group,with statistical significance (P<0.05);there was statistical significance between Huangma tincture group and control group(P<0.05). Mean cure time,effec-tive time and recovery time of combination treatment group were all significantly lower than Huangma tincture group and control group,with statistical significance(P<0.05);there was statistical significance between Huangma tincture group and control group (P<0.05). Cure rate of combination treatment group was higher than that of Huangma tincture group and control group,with statis-tical significance (P<0.05);there was statistical significance between Huangma tincture group and control group (P<0.05). No ADR was found in combination treatment group and Huangma tincture group. CONCLUSIONS:The ultrasound debridement com-bined with Huangma tincture in the treatment of diabetic foot ulcer improve healing rate and shorten healing time significantly with-out obvious ADR. It has good clinical efficacy.

Objective To evaluate the treatment effect of negative pressure wound therapy ( NPWT) followed by ultrasonic debridement surgical system ( UDSS) in patients with diabetic foot. Methods Forty-seven diabetic foot ulcer patients( WagnerⅡ~Ⅲ) were divided into two groups,of which 26 patients treated with UDSS combined NPWT,and 21 patients treated with traditional surgical debridement combined NPWT were control group. Cure rate, markedly effective rate and adverse reaction and complication were compared between two groups. Results Compared with control group,the experiment group had higher markedly effective rate in the first week(P<0. 05),and healing rates were higher in the 2th,4th,6th,12th weeks (P<0. 05). There were no differences in the adverse reactions and complications between the two groups. Conclusion UDSS combined with NPWT can accelerate wound cure,may play an important role for the treatment of diabetic foot ulcer.

Objective To evaluate the efficiency and safety of ultrasonic debridement therapy (UDT) and intelligence negative-pressure wound therapy(INPWT) for diabetic foot ulcers with Wagner 2-3degree.Methods Eighty patients with diabetic foot ulcers with Wagner 2-3 degree were divided into two groups according to the treatment method.The UDT and INPWT group (INPWT group) had 53 cases.Routine treatment group(routine group) had 27 cases.Both groups were given blood sugar and blood pressure control,anticoagulation,anti-infection and supportive treatment.The efficiency and complications were compared between two groups.Results In INPWT group,29 cases were healed,14 cases showed obvious effectiveness,7 cases were effective and 3 cases were inefficacy.The total effective rate was 94.34%(50/53).For routine group,they were 7,8,6,6 cases and 77.78%(21/27) respectively.The total effective rate between two groups had significant difference (P <0.05).Pain was found in 9 cases and hemoglobin decreased in 1 case in INPWT group.Conclusion INPWT after UDT for diabetic foot ulcers with Wagner 2-3 degree shows a good efficiency and safety.
