RESUMO

@#Diabetic retinopathy(DR)is a common chronic complication of diabetes mellitus, which cause irreversible damage of microvessels in the retina. DR is a leading blindness eye disease among diabetes mellitus. The pathogenesis of DR is mainly related to oxidative stress, inflammation and neovascularization. DR patients are treated with laser photocoagulation, vitrectomy and medicine in clinical trials. The traditional Chinese medicine and Chinese herb monomers have unique efficacy in the treatment of DR, especially in retinal protection, which provides valuable supplement. The paper summarized application practice and mechanism of representative Chinese herbal formulas and Chinese herb monomers in the treatment of DR, which would provide references for the clinical treatment and new drug development of DR.

RESUMEN Introducción: Las lesiones cervicales por arma de fuego en pediatría tienen baja incidencia, aproximadamente a 5 a 10%, con una mortalidad de 0 a 11%. Objetivo: Describir un caso de politraumatismo por arma de fuego, su manejo multidisciplinario y las consecuencias de la imprudencia a la exposición de niños a las mismas. Caso Clínico: Paciente de sexo masculino, de 13 años de edad, con traumatismo de cara, cuello y tórax por herida accidental por arma de fuego (escopeta) con diagnósticos: Trauma Facial: fractura de maxilar superior e inferior; Trauma de cuello: lesión en cuerpo vertebral C4/C7 con perdigones incrustados; Trauma de tórax: contusión pulmonar derecha; Trauma de partes Blandas: con perdigones incrustados en celular subcutáneo. Ingresa al hospital de Trauma en choque hipovolémico presentando casi inmediatamente paro cardiorespiratorio, respondiendo a reanimación cardiopulmonar avanzada. TAC de cráneo al ingreso normal; al 8vo día se evidencia esquirla en sistema venoso de la base; TAC de cuello: se evidencia desplazamiento de tráquea y aire peri tráquea; AgioTAC: pseudoaneurisma en carótida interna izquierda. Fibrobroncospcopía: contusiones a nivel de tráquea; EDA: esófago y estómago con erosiones cubiertas por fibrina sin evidencia de fístula o perforación de víscera hueca. Tratamiento: en Asistencia Mecánica Respiratoria (ARM) por 10 días; inotrópicos por 72 hs, se realiza reparación de arteria carótida interna; colocación de arco de Erich por cirujanos maxilofaciales. Alta a sala de pediatría con buena evolución.


ABSTRACT Introduction: Pediatric cervical gun injuries have a low incidence, approximately 5 to 10%, with a mortality of 0 to 11%. Clinical Case: a 13-year-old male patient who presented with trauma to the face, neck and thorax due to accidental firearm injury (shotgun) with the following diagnoses: Facial trauma: fracture of the upper and lower jaw; Neck trauma: C4 / C7 vertebral body injury with embedded pellets; Chest trauma: right pulmonary contusion; Soft tissue trauma: embedded pellets in subcutaneous soft tissue. He was admitted to the Trauma hospital in hypovolemic shock, presenting almost immediate cardiorespiratory arrest, which responded to advanced cardiopulmonary resuscitation. CT scan of the skull was normal at admission; on the 8th day it showed evident splintering in the basal venous system; CT of the neck: tracheal displacement and evident peri-tracheal air; Angio CT: pseudoaneurysm in left internal carotid artery. Fibrobronchoscopy: bruises at the tracheal level; EDA: esophagus and stomach with fibrin-covered erosions without evidence of fistula or hollow viscera perforation. Treatment: on Mechanical Respiratory Assistance (MRA) for 10 days; inotropes for 72 hours, internal carotid artery repair was performed, Erich arch placement by maxillofacial surgeons. He was discharged to the pediatric ward with favorable evolution.

INTRODUCTION: Vascular trauma is a low frequency event and is related to a high burden of morbidity and mortality. Vascular trauma of the upper limb is of different etiology. More frequent, secondary to closed trauma. It is usually associated with other lesions, soft tissue and nerves. OBJECTIVES: To present a case of complex vascular trauma of the upper limb and its multidisciplinary management. METHOD: Description of the clinical case and literature review. The information was obtained from the patient's clinical record, review and analysis of the published literature was performed using search engines. CASE REPORT: Young man, fall with upper limb in extension, results in elbow dislocation associated with acute ischemia. AngioTAC confirms stop in brachial. Emergency surgery: brachio-radial saphenous bridge. It evolves with absence of pulses. It is reexplored finding incomplete fasciotomy, brachial artery bridge to cephalic vein with inverted saphenous. Elbow dislocated, unstable. Arterial bridge, external fixation and fasciotomy is completed. Coming out with radial pulse. He undergoes multiple surgeries and surgical toilets, achieving skin closure at 45 days. Then traumatology removes external tutors. Electromyography confirms incomplete lesion of median, radial and ulnar. Currently in the process of rehabilitation. DISCUSSION AND CONCLUSION: Vascular trauma of the upper limb is of low incidence. However, they are associated with a high burden of morbidity and mortality, generally occurring in young men. The diagnostic pillar is through the medical history and physical examination. its surgical management.

Introdução: As lesões vasculares da cavidade bucal são representadas por hemangiomas ou más-formações vasculares, varizes e menos comumente flebotromboses. Os sítios de maior acometimento são: lábios, mucosa jugal e língua. Os aspectos clínicos são de lesões nodulares ou papulares, pequenas e bem circunscritas, de coloração avermelhada, consistência resiliente, superfície lisa ou moriforme. A diascopia é uma manobra semiotécnica utilizada na diferenciação entre lesões vasculares e máculas. As principais modalidades de tratamento para as lesões vasculares são: excisão cirúrgica, crioterapia, uso de corticóides, escleroterapia e eletrocoagulação. Objetivo: Relatar três casos clínicos de lesões vasculares pequenas na cavidade bucal, tratadas através da excisão cirúrgica das lesões. Resultados e Discussão: Todos os casos evoluíram com um ótimo trans e pós-operatório. O exame anatomopatológico das mesmas revelou dois casos de hemangiomas e um de flebotrombose. A decisão de abordar cirurgicamente as lesões vasculares foi considerada frente aos aspectos clínicos de lesões bem circunscritas, pequenas, palpáveis, boa localização, sem comprometer estruturas nobres anatômicas. Conclusão: A excisão cirúrgica, abordada através de biópsia excisional de lesões pequenas vasculares, é uma opção viável de tratamento, sem complicações trans e pós-operatórias quando corretamente indicadas e executadas.


Introduction: Vascular lesions of the buccal cavity are represented by hemangiomas or vascular malformations, varicose veins and, less commonly, phlebotrombosis. The sites of major involvement are: lips, jugal mucosa and tongue. Clinical features are small, well circumscribed nodular or papular lesions of reddish color, resilient consistency, smooth or moriform surface. Diascopy is a semi-technical maneuver used in the differentiation between vascular lesions and macules. The main treatment modalities for vascular lesions are: surgical excision, cryotherapy, use of corticoids, sclerotherapy, electrocoagulation. Objective: of this present study was to report three clinical cases of small vascular lesions in the oral cavity, which after a well done clinical examination, the team planned the surgical excision of the lesions and the anatomopathological examination revealed two cases of hemangiomas and one of phlebotrombosis. Method: The decision to surgically approach the vascular lesions of this study was considered against the clinical aspects of well circumscribed and small, palpable lesions, good location, without compromising anatomical noble structures. Results: All cases evolved with a good postoperative period. Conclusion: that surgical excision, treated as an excisional biopsy of small vascular lesions, is a viable treatment option, without trans and postoperative complications when correctly indicated and performed.

Many factors can cause liver fibrosis and cirrhosis in clinical practice, and liver cirrhosis caused by liver vascular lesions often has insidious progression. Due to a lack of knowledge in clinicians and limited diagnostic and treatment techniques, patients often have poor prognosis. The liver vascular system has a unique structure, and patients with different locations and extents of lesion have different clinical manifestations and outcomes. Correct diagnosis and differentiation of liver cirrhosis caused by different types of liver vascular lesions is an important and difficult issue and needs careful attention in clinical practice. This article summarizes the association of portal vein lesion, hepatic vein lesion, hepatic sinusoid lesion, and hepatic artery lesion with liver cirrhosis. A deep understanding of the clinical manifestations, pathological features, and key points in differential diagnosis of these lesions helps with correct diagnosis and treatment of such disease.

Objective: To explore the changes of incidence of major vascular lesions after intensive intervention of multiple controllable risk factors in the type 2 diabetes mellitus (T2DM) patients in Han population in Northeast China, and to establish an effective, standardized and cost-effective intervention scheme for the primary prevention of major vascular lesions of the T2DM patients. Methods: A total of 300 T2DM patients newly diagnosed or diagnosed in one year without major vascular lesions were randomly divided into conventional treatment group (n= 150) and intensive intervention group (n=150). The clinical study method of parallel control was used to intervene in the some controllable risk factors for 2 years to varying degrees. The body mass index (BMD, waist/hip ratio (WHR), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FPG), 2 h postprandial blood glucose (2 h PBG), glycosylated hemoglobin (HbAlc), serum total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-c), high density lipoprotein (HDL-c), intima media thickness (IMT) and incidence of majoy vascular lesions of atherosclerosis (AS) of the patients in two groups were measured, and the medical expenses of the patients were analyzed statistically. Background@#Dermoscopy, a non-invasive diagnostic tool, has been proven to improve the diagnostic accuracy of vascular tumors since it can aid in identifying vascular structures as well as morphology of vessels. Tumor depth and precise vascular structures can also be evaluated through dermoscopy.@*Objective@#The study aimed to describe the dermoscopic features of red tumors in Filipino patients seen at the Outpatient Dermatology Department of two centers from January 2013 to July 2014.@*Methods@#Patients with red tumors were recruited to the study. Dermoscopic features were described for each tumor, and histopathology was done.@*Results@#A total of 44 patients were included in the study, and 45 red tumors were evaluated. The tumors were located on the face (31%), trunk (36%), and extremities (33%). The majority of the red tumors were confirmed to be vascular tumors (78%) on histopathology. Dermoscopic features that were found in the majority of vascular tumors seen were lacunae and reddish homogenous areas. For non-vascular tumors, vessels were identifiable in 90% of the tumors seen.@*Conclusion@#In this study, the use of dermoscopy in the diagnosis of red tumors proved to be a useful preliminary tool that aided in the observation of vascular pattern, albeit red lacunae, red to dark red homogenous areas, and different types of vessels. The identification of these structures may aid in arriving at a more precise diagnosis and help differentiate vascular from non-vascular tumors.

ABSTRACT. Cognitive impairment includes mild cognitive decline and dementia, such as Alzheimer's disease (AD) and cerebrovascular-related pathologies. Objective: To investigate the profile of AD-related CSF biomarkers in a sample of cognitively impaired and unimpaired older adults with concomitant subcortical cerebrovascular burden. Methods: Seventy-eight older adults attending an outpatient psychogeriatric clinic were enrolled. Diagnoses were based on clinical, neuropsychological, laboratory, and neuroimaging data. Participants were classified into: cognitively normal (controls, n = 30), mild cognitive impairment (MCI, n = 34), and dementia (AD, n = 14). All subjects were submitted to CSF analyses for determination of amyloid-beta (Aß1-42), total tau (t-tau), phosphorylated tau (p-tau) and Aß1-42/p-tau ratio according to the Luminex method. MRI was performed in all individuals, and was scored independently by two experts according to Fazekas scale. Statistical analyses were conducted with the aid of general linear model procedures, and the Chi-squared test. Results: T-tau levels were significantly associated with subcortical lesion pattern when Fazekas was considered as a group factor. CSF biomarkers were not associated with MCI, AD, or controls when considered separately. There was a tendency for reduction in CSF Aß1-42 together with increasing Fazekas scores, but without statistical significance. Comparisons of Aß1-42 and t-tau with each clinical group or with each neuroimaging pattern did not reach statistical differences. Likewise, Fazekas scores had no impact on CAMCOG scores. Conclusion: We found a significant association between t-tau levels and subcortical lesions when all Fazekas classifications were considered as a single group; comparisons of Fazekas subgroups and CSF biomarkers did not reach significance.


RESUMO. O comprometimento cognitivo inclui alterações leves da cognição e demência, como doença de Alzheimer (DA) e patologias vasculares associadas. Objetivo: Investigar o perfil de biomarcadores da DA no líquor e doença cerebrovascular concomitante em idosos com e sem alterações cognitivas. Métodos: Foram incluídos 78 sujeitos de um ambulatório de psicogeriatria. Efetuaram-se os diagnósticos com base em dados clínicos, neuropsicológicos, laboratoriais e neuroimagem. Os participantes foram classificados em: cognitivamente normais (controles, n = 30), comprometimento cognitivo leve (CCL, n = 34) e demência (DA, n = 14). Todos foram submetidos ao exame liquórico para determinação de ß-amiloide (Aß1-42), tau total (t-tau), tau fosforilada (p-tau) e razão Aß1-42/p-tau, segundo o método de Luminex. RM foi efetuada em todos os indivíduos. Dois especialistas independentes avaliaram as imagens segundo a escala de Fazekas. As análises estatísticas basearam-se em modelo linear geral e teste qui-quadrado. Resultados: T-tau foi significantemente associada ao padrão de lesão subcortical quando o grau de Fazekas foi considerado como fator grupal. Não houve associação entre biomarcadores e diagnóstico clínico de CCL, DA e grupo controle, considerados individualmente. Observou-se uma tendência de redução de Aß1-42 concomitante com elevação dos escores de Fazekas, sem correlação significante. Comparações entre Aß1-42 e tau e diagnóstico clínico ou neuroimagem não foram significantes. Os resultados de Fazekas não influenciaram os escores do CAMCOG. Conclusão: Como principal resultado, observou-se associação significante entre os níveis de t-tau e lesões subcorticais quando as classificações de Fazekas foram incluídas em um único grupo. As comparações dos subgrupos de Fazekas e biomarcadores liquóricos não foram significantes.

Objective To compare the degree of coronary lesions in different types of heart failure with ischemic heart disease. Methods This retrospective analysis include 282 cases diagnosed as heart failure with ischemic heart disease in Zhengzhou People′s Hospital from January 2015 to May 2016. Based on LVEF and the level of BNP and NT-proBNP examination results ,282 cases were divided into HFmrEF(89 cases),HFrEF(109 cases) and HFpEF (84 cases). The clinical basic data ,cardiac color Doppler ultrasound and the difference of coronary artery lesions were compared. Results (1) HFpEF,HFmrEF and HFrEF groups gensini score were 30.45 ± 33.18,62.12 ± 41.25,86.72 ± 38.80. The differences are significant(P < 0.05). Pairwise comparison showed significant difference.(2)HFpEF,HFmrEF and HFrEF groups SYNTAX score were 11.77 ± 8.39,19.08 ± 8.39,26.41 ± 10.31. Compared with HFpEF group,the SYNTAX score of other groups are higher(P < 0.05). (3)The duration of disease ,LVEDD ,LVESD ,LVMI ,the number of vascular lesions was significantly higher in HFrEF group than those in other groups. (4) Multivariate correlation analysis suggested that SYNTAX scores were related to cardiac function classification,BNP,NT-proBNP and LVEF. Gensini integral is related to cardiac function classification ,BNP and NT-proBNP. Conclusions With the severity and complexity of coronary artery disease,the degree of heart failure is more serious. The complexity and severity of coronary artery disease is an important factor in heart failure in patients with ischemic heart disease.

Objective To investigate the expression of miR-155 ,nuclear factor kappa B (NF-κB) and soluble intercellular adhesion molecule -1 (sICAM-1 ) in peripheral blood in patients with type 2 diabetes mellitus (T2DM ) ,and to explore the role of miR-155 in vascular lesions of T2DM. Methods A total of 165 T2DM patients and 60 health subjects from health examinations were enrolled in this study. All the subjects were divided into two groups :subjects without vascular lesions group and vascular disease group. Vascular disease group was further divided into microangiopathy group ,macroangiopathy group , microangiopathy+ macroangiopathy group ;based on 24 hUAlb level normal urinary albumin (NAU ) group ,microalbuminuria (MAU) group ,macroalbuminuria (MAAU) group. MiR-155 ,NF-κB and sICAM-1 in peripheral blood were tested by RT-PCR. Single factor analysis of variance was used for comparison among groups. Stepwise regression analysis was used for correlation factors analysis. Results The expression of miR-155 ,NF-κB and sICAM-1 were significantly higher in T2DM group than in NC group [(1.82 ± 0.71) vs (1.00 ± 0.12) ,(2.28 ± 0.66) vs (1.04 ± 0.33) ,(1.88 ± 0.80) vs (1.03 ± 0.30) ,P<0.05]. The level of miR-155 ,NF-κB ,sICAM-1 were significantly higher in T2DM vascular lesions group than in subjects without vascular lesions group [(1.95 ± 0.73) vs (1.34 ± 0.29) ,(2.40 ± 0.65) vs (1.65 ± 0.16 ) ,(2.01 ± 0.77 ) vs (1.07 ± 0.41 ) ,P < 0.05 ]. The expression of miR-155 was higher in microangiopathy+ macroangiopathy group than in macroangiopathy group [(2.36 ± 0.61 ) vs (1.77 ± 0.59) ,P < 0.05 ].The expression of NF-κB was also significantly higher in microangiopathy +macroangiopathy group than in microvascular disease group and macroangiopathy group (P<0.05). The level of miR-155 was significantly higher in group MAU group and MAAU group than in NAU group [(1.41 ± 0.49) ,(2.64 ± 0.52) vs (1.04 ± 0.20) ,P<0.05] ,and NF-κB and sICAM-1 were also higher than NAU group (P<0.05). Multiple stepwise regression analysis showed that miR-155 was positively correlated with NF-κB and sICAM-1(t=4.235 ,9.728 ,P<0.01). Conclusion The expression of miR-155 increases in T2DM patients with vascular complications ,and this trend is the same as NF-κB and sICAM-1. It suggests that miR-155 maybe involves in the pathogenesis of diabetic chronic vascular disease.

Uterine arteriovenous fistula (UAVF) is a rare condition,however it can cause life-threatening vaginal bleeding or acute intra-abdominal hemorrhage.Many diagnosis methods may be used,among which angiography is the gold standard of diagnosis.There is no clear treatment guideline so far.Treatments can be made according to patient's age,symptoms,bearing requirement and pathological changes of hemodynamic parameters.

Objective To investigate the relationship between vascular lesion and serological changes in patients with coronary heart disease (CHD) complicated with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods According to the standard, a total of 168 patients of OSAHS complicated with CHD were selected in this study. Those patients were divided into 3 groups according to the apnea hypopnea index (AHI) level:light group (AHI, 5-14/h), moderate group (AHI, 15-30/h) and severe group (AHI,>30/h). Syntax scores were performed on three groups according to coronary angiography results. The data of hemoglobin (Hb), platelet count (PLT), fibrinogen (FIB), D-Dimer (DD), total cholesterol (TC), low density lipoprotein (LDL), high density lipoprotein (HDL), triacylglycerol (TG), alanine aminotransferase (ALT), aspartate transaminase (AST), uric acid (UA), creatinine (Cr) and echocardiographic examination index were collected and analyzed in three groups. Results The Syntax score was significantly higher in severe group than that in mild and moderate groups (P 0.05). Conclusion The serology and cardiac structure can change gradually in severe OSAHS patients, and the coronary artery lesion will be more complex. Therefore, the clinical treatment should pay attention to screening for OSAHS in patients with coronary heart disease.

El angioma en penacho es un raro tumor vascular benigno de comportamiento localmente agresivo que aparece en la infancia temprana; puede complicarse con el fenómeno de Kasabach-Merritt. Presentamos el caso de un masculino de 1 mes de edad con una lesión tegumentaria compatible con esta patología.


The tufted angioma is a rare benign vascular tumor with locally aggressive behavior that appears in early childhood; It may be complicated by the of Kasabach-Merritt phenomenon. We report a male of 1 month of age with the characteristic clinical lesion.

RESUMO

Large vascular lesions have become one of the main causes in the death and disability of type 2 diabetes mellitus (T2DM). The article systematically and in-depthly studied the mechanism of large vascular lesions in T2DM, in order to seek effective drug prevention and treatment. It was of vital importance in the improvement of prognosis of T2DM patients. In traditional Chinese medicine (TCM), the basic pathogenesis of T2DM large vascular lesions was deficiency in origin and excess in superficiality, in whichqi-yin deficiency was the origin and the obstruction of collaterals was the superficiality. In western medicine, the body endothelial dysfunction, oxidative stress reaction and abnormal gene expression and so on could cause T2DM large vascular lesions. This article summarized related research literatures on both TCM and western medicine in recent years. It discussed studies on mechanism of T2DM large vascular lesions.

Objective To investigate the relationship between retinal vascular lesions and cerebral microbleeds ( CMBs) . Methods One hundred twenty-three consecutive patients with cerebral infarction who could sit and finish fundus photography at the Department of Neurology, General Hospital of Beijing Military Command from August 2013 to August 2014 were enrolled prospectively. After informed consent, the patients underwent head MRI examination and bilateral eye fundus photography. They were divided into either a cerebral microbleed group ( CMB group, n =52 ) or a non-cerebral microbleed group ( nCMB group,n=71) according to whether they had CMBs or not. The general clinical data and retinal vascular lesions ( microaneurysms,hard exudates,cotton wool spots,bleeding spots,focal retinal arteriolar narrowing, arteriovenous cross sign,and silver thread-like changes) of both groups were compared. Results (1)There were significant differences in age,hypertension,diabetes,systolic blood pressure,diastolic blood pressure, fasting blood-glucose,triglyceride,and C reaction protein between the CMB group and the non-CMB group (all P<0. 05). (2) The incidences of hard exudates,focal retinal arteriolar narrowing,and arteriovenous cross sign of patients with CMB were higher than those with non-CMB,and there were significant differences (χ2 =6.233,7.675,and9.544,respectively;P=0.040,0.031,and0.019,respectively).(3)There were significant differences in the incidences of hard exudates,focal retinal arteriolar narrowing,and arteriovenous cross sign in patients with different severity of CMBs (Z= -2. 317,-2. 294 and -2. 157,respectively;P=0. 029,0. 033,and 0. 039,respectively). In patients with severe CMB,there were more patients with hard exudates,focal retinal arteriolar narrowing,and arteriovenous cross sign. (4) The age (OR,3. 623, 95%CI 2. 631-7. 866),hypertension (OR,4. 348,95%CI 3. 734-10. 563),diabetes (OR,3. 831,95%CI 2.126-9. 245),fasting blood-glucose (OR,3. 329,95%CI 2. 631-11. 012),arteriovenous cross sign (OR,5. 437,95%CI 3. 441-13. 606),and hard exudates (OR,4. 054,95%CI 3. 137-13. 252) were the independent risk factors for the occurrence of CMBs. Conclusion The arteriovenous cross sign, focal retinal artery narrowing,and hard exudates in retinal vascular lesions are associated with the CMBs and different degree of CMBs classification. Both the arteriovenous cross sign and hard exudates are the independent risk factors for CMBs.

Hemangioendotheliomas are rare vascular tumors that can arise from soft tissue or bone. The former comprises hemangioma, epithelioidhemangioma, hemangiomatosis and massiveosteolysis or Gorham’s disease (which mimics malignantneoplasm due to aggressive osteolytic destruction). The rarer malignant counterparts are epithelioidhemangioendothelioma, hemangioendothelioma and angiosarcoma in increasing order of malignancy. The diagnosis of these lesions is diffi cult both clinically and radiologically. Hereby we report an unusual case which was mistaken for metastasis and was diagnosed with the help of histopathology.

Objetivos: Dentro de las malformaciones vasculares de las arterias en el tórax encontramos: 1) doble arco aórtico; 2) arco aórtico derecho y conducto arterioso izquierdo persistente; 3) arteria subclavia derecha aberrante; 4) arteria pulmonar izquierda aberrante, y 5) arteria innominada anómala. Se revisan los pacientes con arteria subclavia derecha aberrante y su manejo. Métodos: Se estudiaron en forma retrospectiva los expedientes de 29 pacientes en edad pediátrica, de enero de 1992 a diciembre 2012, con las siguientes variables: edad de inicio de los síntomas, manifestaciones clínicas, defectos cardiovasculares, método diagnóstico y abordaje quirúrgico. Resultados: El mayor número de pacientes cursó de forma asintomática, únicamente el 31% lo hizo con síntomas durante el primer año de vida, y se llevó a cabo el diagnóstico en un 35% mediante cateterismo. Fue la persistencia del conducto arterioso la cardiopatía más frecuente, con un 13%; el síndrome de Down se encontró en un 21% de los pacientes. El tratamiento más utilizado fue la sección de la arteria subclavia aberrante para la liberación del esófago. Conclusiones: Es importante la sospecha diagnóstica en pacientes con sintomatología durante la alimentación, con trastornos de la deglución con sólidos y, en algunos casos, con disfagia o hasta con dificultad respiratoria. Un número significativo de estos pacientes no son diagnosticados oportunamente, algunos alcanzan la edad adulta sin diagnóstico. El hallazgo de esta malformación ocurre durante los estudios de imagen, cuando se evalúa la aorta o en el estudio del reflujo gastroesofágico, ya que con el trago de bario se aprecia la compresión extrínseca del esófago.


Objectives: Congenital vascular malformations of the major arteries in the chest have been classified into 5 groups: 1) double aortic arch; 2) right aortic arch with left ligament or persistent ductus arteriosus; 3) aberrant subclavian artery; 4) aberrant left pulmonary artery, and 5) anomalous innominate artery. We reviewed the patients with aberrant right subclavian artery and their treatment. Methods: We studied retrospectively the records of 29 patients with aberrant right subclavian artery in childhood, from January 1992 to December 2012, analyzing the following variables: age at onset, clinical manifestations, associated cardiovascular defects, diagnosis and surgical approach method. Results: We found that most patients have an asymptomatic course, only 31% of them course with symptoms during the first year of life, with an incidental diagnosis of 35% during catheterization or other imaging studies. Patent ductus arteriosus was the most frequently associated congenital malformation, with 13%. Down's syndrome was found in 21%. The most common treatment was surgical section of the aberrant subclavian artery to release the esophagus. Conclusions: This vascular abnormality must be suspected in those patients with dysphagia, dyspnea, chest pain during feeding or breathing difficulties. A significant number of patients are not diagnosed in time, some reach adulthood without a diagnosis. This malformation is often found in imaging studies when evaluating the aorta or in a gastroesophageal reflux study, in which the barium bolus reveals the extrinsic compression of the esophagus.

Se reporta un paciente con hemorragia masiva de vías digestivas altas secundaria a lesión vascular de Dieulafoy atendido en la institución. En este caso no se logró el control adecuado del sangrado por métodos endoscópicos y requirió tratamiento con arteriografía y embolización selectiva. Se hace una revisión de esta infrecuente causa de sangrado de vías digestivas y se enfatiza en su manejo actual.


One patient with massive upper GI bleeding due to Dieulafoy vascular lesion treated at our institution is reported. In this case, adequate control of bleeding could not be achieved by endoscopic methods, and selective arteriography and embolization was required. A review of this uncommon cause of gastrointestinal bleeding and its management is presented.


Reporta-se um paciente com hemorragia massiva de vias digestivas altas, secundária à lesão vascular de Dieulafoy atendido na instituição. Neste caso não se conseguiu o controle adequado do sangrado por métodos endoscópicos e requereu tratamento com arteriografia e embolização seletiva. Faz-se uma revisão desta infrequente causa de sangrado de vias digestivas e se enfatiza em seu manejo atual.

Introduction: Lasers are a good therapeutic tool for congenital and acquired vascular lesions. Technological advances in lasers have reduced the adverse effects and increased the efficacy. Machines: Among the various lasers used for treating vascular lesions, pulsed dye laser (PDL) has the best efficacy and safety data. The other machines that are widely available are Nd:YAG laser and intense pulse light (IPL). Rationale and scope of guideline: Much variation exists in different machines and techniques, and therefore, establishing standard guidelines has limitations. The guidelines recommended here indicate minimum standards of care for lasers on vascular lesions based on current evidence. Physician Qualification: Laser may be administered by a dermatologist, who has received adequate background training in lasers during post-graduation or later at a center that provides education and training in lasers, or in focused workshops, which provide such trainings. He/she should have adequate knowledge of the lesions being treated, machines, parameters, cooling systems, and aftercare. Facility: The procedure may be performed in the physician's minor procedure room with adequate laser safety measures. Indications: PWS, hemangioma, facial telangiectasia, rosacea, spider angioma, pyogenic granuloma, venous lakes, leg veins. Contraindications: Absolute: Active local infection, photo-aggravated skin diseases, and medical conditions. Relative: Unstable vitiligo, psoriasis, keloid and keloidal tendencies, patient on isotretinoin, patient who is not cooperative or has unrealistic expectation. Patient Selection: Patient selection should be done after detailed counseling with respect to the course of lesions, different treatment options, possible results, cost, need for multiple treatments, and possible postoperative complications. Treatment Sessions: The number of treatments per lesion varies from 2 to 12 or more at 6-8 week intervals. All lesions may not clear completely even after multiple sessions in many cases. Hence, a realistic expectation and proper counseling is very important. Laser parameters: Laser parameters vary with area, type of lesion, skin color, depth of the lesion, and machine used. A test spot may be performed to determine individual specifications. Complications: Pain, edema, purpura, bleeding, scarring, postinflammatory hyperpigmentation/hypopigmentation, and atrophy changes.
